    Abstract: To clarify and quantify risk factors among local characteristics of the foot for major amputation in diabetic foot patients. Methods Articles published before January 2018 on PubMed and Embase were conducted observational studies about risk factors for major amputation in patients with diabetic foot were retrieved and systematically reviewed by using Stata 12.0 statistical software. Results A total of 4668 major amputees and 65 831 controls were reported in 18 observational studies. Across the studies, the overall odds ratios (ORs) and 95% confidence intervals (CIs) of significant risk factors are ulcer reaching bone (OR, 11.796; 95% CI, 6.905‐20.152), gangrene (OR, 6.487; 95% CI, 4.088‐10.293), hindfoot position (OR, 3.913; 95% CI, 2.254‐6.795), decreased ankle‐brachial index (ABI) (OR, 2.522; 95% CI, 1.805‐3.523), infection (OR, 2.516; 95% CI, 1.708‐3.706), peripheral arterial disease (PAD) (OR, 2.114; 95% CI, 1.326‐3.372). While there is no significant difference in the size of the ulcer, neuropathy, Charcot foot, osteomyelitis and intermittent claudication (OR, 1.15; 95% CI, 0.85‐1.54). Conclusion Factors among local characteristics of the foot associated with major amputation in patients with diabetic foot are the ulcer reaching bone, gangrene, hindfoot position, decreased ABI, infection, and PAD, a negative risk factor for the risk of amputation. Further studies are required to provide more details of foot local characteristics.

    Abstract: This randomized, double‐blind, placebo‐controlled study assessed the necessity of early intervention, safety and efficacy of intravenous zoledronic acid 5 mg/year in East China women with newly diagnosed osteoporosis at high risk of fracture during a 24‐month treatment period. Methods Subjects (57 [52–62] years old) were randomized 3:2 to zoledronic acid versus placebo (randomized at baseline, zoledronic acid [175 cases] , placebo‐zoledronic acid [110 cases]). The bone mineral density of the lumbar spine and total hip was measured every 6 months with the use of dual‐energy X‐ray absorptiometry. Serum procollagen I N ‐terminal pro‐peptide ( PINP ) and serum C‐telopeptide of type I collagen ( CTX ) levels were measured every 6 months. The primary end point was the rate of change in the bone mineral density at the posteroanterior spine. Results For subjects with measurements at 24 months, zoledronic acid significantly increased bone mineral density ( BMD ) at the lumbar spine (mean percent change ± SD , zoledronic acid 5.390% ± 0.854% versus placebo‐zoledronic acid −1.038% ± 0.599%), the total hip (zoledronic acid 1.900% ± 0.262% versus placebo–zoledronic acid −1.631% ± 0.649%). Serum procollagen I N ‐terminal pro‐peptide ( PINP ) and CTX decreased rapidly with zoledronic acid 5 mg treatment ( P 〈 0.001 versus placebo at 6 month and 24 months) and changed from baseline in the zoledronic acid 5 mg and placebo–zoledronic acid 5 mg at 6 months by a mean of −66.348% and −75.375%, respectively ( P 〈 0.001), and at 24 months by −49.950% and −52.325%, respectively ( P 〈 0.001). No cases of serious adverse events were observed in two groups. Headache, pyrexia and myalgia occurred more commonly within the first 3 days after infusion with zoledronic acid 5 mg than with placebo (13.7% versus 2.1%, P = 0.0018; 28.0% versus 3.2%, P 〈 0.001; 21.7% versus 4.2%, P 〈 0.001, respectively). Conclusions These data show that early application of zoledronic acid 5 mg/year was well stimulated and tolerated for bone mass in newly diagnosed east china subjects with osteoporosis in a 24‐month treatment.

    Abstract: The major histocompatibility complex ( MHC ) is most closely associated with nasopharyngeal carcinoma ( NPC ), but the complexity of its genome structure has proven challenging for the discovery of causal MHC loci or genes. We conducted a targeted MHC sequencing in 40 Cantonese NPC patients followed by a two‐stage replication in 1065 NPC cases and 2137 controls of Southern Chinese descendent. Quantitative RT ‐ PCR analysis ( qRT ‐ PCR ) was used to detect gene expression status in 108 NPC and 43 noncancerous nasopharyngeal ( NP ) samples. Luciferase reporter assay and chromatin immunoprecipitation (Ch IP ) were used to assess the transcription factor binding site. We discovered that a novel SNP rs117565607_A at TRIM 26 displayed the strongest association ( OR  = 1.909, Pcombined = 2.750 × 10 −19 ). We also observed that TRIM 26 was significantly downregulated in NPC tissue samples with genotype AA / AT than TT . Immunohistochemistry ( IHC ) test also found the TRIM 26 protein expression in NPC tissue samples with the genotype AA / AT was lower than TT . According to computational prediction, rs117565607 locus was a binding site for the transcription factor Yin Yang 1 ( YY 1). We observed that the luciferase activity of YY 1 which is binding to the A allele of rs117565607 was suppressed. Ch IP data showed that YY 1 was binding with T not A allele. Significance analysis of microarray suggested that TRIM 26 downregulation was related to low immune response in NPC . We have identified a novel gene TRIM 26 and a novel SNP rs117565607_A associated with NPC risk by regulating transcriptional process and established a new functional link between TRIM 26 downregulation and low immune response in NPC .

    Abstract: During the long coevolution of human cytomegalovirus (HCMV) and humans, the host has formed a defense system of multiple layers to eradicate the invader, and the virus has developed various strategies to evade host surveillance programs. The intrinsic immunity primarily orchestrated by promyelocytic leukemia (PML) nuclear bodies (PML‐NBs) represents the first line of defense against HCMV infection. Here, we demonstrate that microrchidia family CW‐type zinc finger 3 (MORC3), a PML‐NBs component, is a restriction factor targeting HCMV infection. We show that depletion of MORC3 through knockdown by RNA interference or knockout by CRISPR‐Cas9 augmented immediate‐early protein 1 (IE1) gene expression and subsequent viral replication, and overexpressing MORC3 inhibited HCMV replication by suppressing IE1 gene expression. To relief the restriction, HCMV induces transient reduction of MORC3 protein level via the ubiquitin‐proteasome pathway during the immediate‐early to early stage. However, MORC3 transcription is upregulated, and the protein level recovers in the late stages. Further analyses with temporal‐controlled MORC3 expression and the major immediate‐early promoter (MIEP)‐based reporters show that MORC3 suppresses MIEP activity and consequent IE1 expression with the assistance of PML. Taken together, our data reveal that HCMV enforces temporary loss of MORC3 to evade its repression against the initiation of immediate‐early gene expression.

    Abstract: Atrial fibrillation (AF) is the most common sustained arrhythmia, and catheter ablation has been shown to be a highly effective treatment for patients with symptomatic AF. Very early recurrence (VER) of AF within 7 days after catheter ablation is common, but the clinical significance of VER remains unclear. We have examined the usefulness of the noninvasive electrocardiography monitor for the detection of VER and the relationship between VER and late recurrence (LR). Methods Eighty‐eight patients with paroxysmal or persistent atrial fibrillation were retrospectively included. All patients underwent primary catheter ablation at a large general hospital between March 2016 and August 2018. All patients were followed up in atrial fibrillation clinic at an interval of every 3 months for late recurrence of AF. VER was evaluated by one‐lead continuous noninvasive electrocardiography monitoring device for 7 days after ablation. The association between VER and LR was analyzed by univariate and multivariate Cox regression model. Results Mean age was 62.9 ± 9.7 years, and 39.8% were female. Thirty‐two patients (36.4%) experienced VER. After a mean follow‐up of 539.36 ± 211.66 days, 17 patients (19.3%) experienced LR. Multivariate Cox regression analysis revealed VER was an independent predictor of LR: HR 3.6 (95% CI, 1.2–10.8), p  = .020. In addition, diabetes was also associated with LR of atrial fibrillation. Conclusions Noninvasive electrocardiography monitoring was a useful tool for detecting VER and VER after catheter ablation was associated with LR.

    Abstract: Although ageing could increase the risk of delayed healing in diabetic foot ulcers (DFUs) patients, data from middle‐aged patients remains greatly limited. The purpose of this study was to explore the clinical phenotypes, outcomes and predictive factors of DFU in middle‐aged patients. Methods A retrospective cohort study conducted with 422 consecutive inpatients with DFUs who visited our hospital between May 2010 and September 2017; participants were recruited and assigned according to age to either the middle‐aged group or the elderly group. The Demographics, ulcer characteristics, comorbidities and diabetes complications, laboratory tests, socioeconomic data and final outcomes were collected. Moreover, predictive factors of adverse outcomes in middle‐aged DFUs patients were assessed. Results Middle‐aged patients were more likely to have worse lifestyle and glucose control, were more likely to have microangiopathy as a complication, and tended to have larger and deeper ulcers; however, these patients also had higher rates of healing and lower rates of mortality and major amputaion than elderly patients. Severe infection, living alone, current smoking cigarettes, and having a high white blood cell count were independent risk factors for adverse outcomes in middle‐aged patients. Conclusions DFUs are relatively common in middle‐aged patients with diabetes, and these patients have unique clinical phenotypes and risk profiles. Nonetheless, further investigation is needed to clarify whether intervention targeting these easily recognizable risk factors can improve healing and survival rates in middle‐aged DFU patients.

    Abstract: The alteration in the mechanical environment of the necrotic area is the primary cause of the collapse observed in osteonecrosis of the femoral head (ONFH). This study aims to evaluate the biomechanical implications of the China‐Japan Friendship Hospital (CJFH) classification system and hip flexion angles on the necrotic area in ONFH using finite element analysis (FEA). The goal is to provide valuable guidance for hip preservation treatments and serve as a reference for clinical diagnosis and therapeutic interventions. Methods Hip tomography CT scan data from a healthy volunteer was used to create a 3D model of the left hip. The model was preprocessed and imported into Solidworks 2018, based on the CJFH classification. Material parameters and boundary conditions were applied to each fractal model in ANSYS 21.0. Von Mises stresses were calculated, and maximum deformation values were obtained to evaluate the biomechanical effects of the load on the necrotic area and post‐necrotic femur, as well as assess each fractal model's collapse risk. Results (1) At the same hip flexion angle, maximum deformation followed this order: M Type 〈 C Type 〈 L Type. The L3 type necrotic area experienced the most significant deformation at 0, 60, and 110° angles (1.121, 1.7913, and 1.8239 mm respectively). (2) Under the same CJFH classification, maximum deformation values increased with hip flexion angle (0  〈  60  〈  110°), suggesting a higher risk of collapse at larger angles. (3) Von Mises stress results showed that the maximum stress was not located in the necrotic area but near the inner and outer edge of the femoral neck, indicating decreased stiffness and strength of the subchondral bone after osteonecrosis. Conclusion The study found that femoral head collapse risk was higher when the necrotic area was located in the lateral column under the same stress load and flexion angle. Mechanical properties of the necrotic area changed, resulting in decreased bone strength and stiffness. Large‐angle hip flexion is more likely to cause excessive deformation of the necrotic area; thus, ONFH patients should reduce or avoid large‐angle hip flexion during weight‐bearing training in rehabilitation activities.

    Abstract: A novel polymer‐stabilized blue phase liquid crystal display (PS‐BPLCD) using an optimized BPLC for new wall‐electrode cell structure and also new driving method has been developed. Those technologies enable BPLCDs to apply conventional source driver IC in AMLCD applications. This novel PS‐BPLCDs successfully achieved high‐performance LCDs with over 2 times higher optoelectronic properties and a superior reliability.

    Abstract: Graphene's atomic thickness, gapless Dirac–Fermionic band structure, and large thermal conductivity make it a promising element for applications in photonic integrated devices. Importantly, actively tunable graphene‐waveguide‐integrated optoelectronic devices can potentially be utilized for the realization of reconfigurable photonic systems. Since electrical device control is always preferred, researchers have previously demonstrated gate‐variable graphene devices; however, electrical control remains challenging, particularly in all‐fiber systems because of the required complicated configuration and fabrication techniques along with additional signal loss. Here, a graphene‐fiber‐integrated platform is proposed and the manipulation of its nonlinear optical properties is demonstrated by engineering the Fermi–Dirac distribution of graphene based on a convenient electric heating method. For the first time, it is experimentally shown that the nonlinear optical absorption of graphene is correlated to temperature via the thermal relaxation process. In the experiments, the modulation depth variation exceeds 60%. The configuration is simple, cost‐effective, and can be readily extended to other 2D materials. It is believed that this work can contribute to building miniature and compact graphene‐fiber‐integrated devices for actively tunable multifunctional applications.
