    Abstract: Phytoremediation is a green technology that removes heavy metal (HM) contamination from the environment by using HM plant accumulators. Among soil microbiota, plant growth promoting bacteria (PGPR) have a role influencing the metal availability and uptake. Methods This current study evaluates the plant growth promoting qualities of microbial flora isolated from rhizosphere, plant roots, and marine aquatic HMs polluted environments in Alexandria through several biochemical and molecular traits. Metal contents in both collected soils and plant tissues were measured. Transcript levels of marker genes ( HMA3 and HMA4 ) were analyzed. Results Three terrestrial and one aquatic site were included in this study based on the ICP-MS identification of four HMs (Zn, Cd, Cu, and Ni) or earlier reports of HMs contamination. Using the VITEK2 bacterial identification system, twenty-two bacteria isolated from these loci were biochemically described. Pseudomonas and Bacillus were the most dominant species. Furthermore, the soil microbiota collected from the most contaminated HMs site with these two were able to enhance the Helianthus annuus L. hyper-accumulation capacity significantly. Specifically, sunflower plants cultivated in soils with HMs adapted bacteria were able to accumulate about 1.7–2.5-folds more Zn and Cd in their shoots, respectively. Conclusion The influence of PGPR to stimulate crop growth under stress is considered an effective strategy. Overall, our findings showed that plants cultivated in HMs contaminated sites in the presence of PGPR were able to accumulate significant amounts of HMs in several plant parts than those cultivated in soils lacking microbiota.

    Abstract: Aflatoxins are considered a severe hazard, contaminate dietary products, and cause malignant alterations in liver tissues. Fermented milk (FM) is prepared using probiotic lactic acid strains. This investigation aimed to produce an integrated milk beverage, inactivating aflatoxins toxicity and biotransformation. The proximate analysis of the investigated materials and biochemical parameter changes of the in-vivo experiment were determined. Results reflected the extract’s valuable content of polysaccharides and antioxidants. Nine phenolics were identified predominantly with catechin (39.67 ± 1.5 µg/g). FM-fortification is reflected by enhancement in protein (49.5 ± 2.97 g/Kg) and fiber content (1.78 ± 0.54 g/Kg) compared to the FM content. Relative rats’ weight gain improved to 34.29% for the fortified-FM group close to the control; it was recorded at 16.47% for the AFM 1 group. Alkaline phosphatase in AFM1 rats was 99.2 ± 1.86 U/L and decreased to 44.2 ± 0.71 U/L in the fortified-FM group (44.2 ± 0.71 U/L) to be close to the control group. Aflatoxin M 1 rats exposure reflects tissue alterations and cell damage, which recorded lesser in rats treated by extract and beverage administrations. The beverage’s corrective action relied on two integrated mechanisms, aflatoxin-binding to bacterial and bioactivity interaction of extract substances. This beverage stopped tissue alterations that occurred due to aflatoxins. The result supports the future production of fortified-milk beverages as a bio-shield against aflatoxin toxicity, besides their nutritional and functional properties.

    Abstract: Egypt has the highest hepatitis C virus prevalence worldwide where about 24% of the people are estimated to carry HCV and more than 50% of blood donors have anti-HCV in some towns. The burden of hepatocellular carcinoma has been increasing in Egypt with a doubling in the incidence rate in the past 10 years. Thus, the aim of the present study was to analyze the interleukin-18 single nucleotide polymorphisms (SNPs) as a diagnostic tumor marker for hepatocellular carcinoma in patients with hepatitis C-related cirrhosis. Results This study included 33 hepatocellular carcinoma (HCC) complicating HCV-related cirrhosis patients, 37 cirrhotic patients without HCC (cirrhosis group), and 20 healthy individuals who were included as a control for 9 months of follow-up. SNPs of the IL-18 gene were genotyped by polymerase chain reaction. There was a statistically significant difference in the GG genotype in the HCC group in comparison with the control group ( P = 0.04). There was a statistically significant difference in the G allele in the cirrhosis and HCC groups in comparison with the control group (p1 〈 0.001 and p2 = 0.03, respectively). Patients with GC genotype have a risk for developing HCC by 6.33-folds more than those with GG genotype while patients with GC genotype have a risk for developing cirrhosis by 5.43-folds more than those with GG genotype, and cirrhotic patients with CC and GC genotype had a risk for developing HCC by 1.17-folds more than those with GG genotype. Conclusion Our findings revealed that the analysis of IL-18 single nucleotide gene polymorphism could be a valuable marker for the prediction of progress towards cirrhosis in chronic HCV patients and also to subsequent development of HCC in HCV cirrhotic patients proved by the results of both GG genotype and its G allele; also, cirrhotic patients with CC and GC genotype have a risk for developing HCC by 1.17-folds more than those with GG genotype.

    Abstract: The presence of bacteremia as a complication of variceal bleeding in patients with liver cirrhosis had been investigated by many studies. The aim of this study was to assess the bacteremia as a risk factor for variceal upper gastrointestinal tract bleeding in cirrhotic patients. A cross-sectional study was conducted on 99 patients with chronic liver disease divided into three groups: group I included 35 patients presented with first attack of variceal bleeding, group II included 35 patients presented with recurrent attacks of variceal bleeding, and group III included 29 patients with no history of previous variceal bleeding as a control group. Routine laboratory tests were done, upper GI endoscopy, blood culture, and measurement of procalcitonin level in blood. Results Patients with recurrent variceal bleeding had statistically ( p 〈 0.05) the highest percentage of positive blood culture followed by patients with first variceal bleeding and the control (60% vs 45.7% vs 24.1%) respectively. In addition to procalcitonin results, patients with recurrent variceal bleeding had statistically the highest values of PCT followed by patients with first variceal bleeding and the control (1.92 vs 0.325 vs 0.22 ng/ml) respectively. Multivariate regression analysis showed that procalcitonin and hemoglobin only was the significant predictors for variceal bleeding. Hemoglobin at cutoff value of ≤ 9.6 and procalcitonin (ng/dl) at cutoff value of 〉 1.76 is the most specific in predicting bleeding 86.21%, 86.21% (CI 95%) respectively. Conclusion Bacteremia and procalcitonin are risk factor for variceal bleeding in cirrhotic patients. Procalcitonin can be used as easily measurable and surrogate biomarker for bacteremia and variceal bleeding.

    Abstract: Sodium glucose co-transporter 2 inhibitors (SGLT2is) are a novel class of oral antidiabetic drugs. To date, there are no pharmacoepidemiologic studies investigating the pattern of use of SGLT2is compared to other oral antidiabetic drugs in the Middle East, including Qatar. Aim This study aimed to explore the trends in the use of SGLT2is compared to other oral antidiabetic drugs in Qatar from 2016 to 2020. Method This is a descriptive, retrospective cross-sectional study where information on all oral antidiabetic drugs dispensed as in- or out-patient prescriptions from 2016 to 2020 in Hamad Medical Corporation hospitals, Qatar were collected. Outcomes included the number and relative frequency of quarterly prescriptions of different oral antidiabetic drug classes [biguanides, sulfonylureas, dipeptidyl peptidase 4 inhibitors, thiazolidinediones, meglitinides, α-glucosidase inhibitors, and SGLT2is] prescribed from 2016 to 2020. Results SGLT2is prescriptions increased from 1045 (2.13%) in 2017 to 8375 (12.39%) in 2020, while sulfonylureas prescriptions declined from 10,436 (21.25%) to 9158 (13.55%) during the same period. Metformin use decreased from 23,926 (48.71%) in 2017 to 30,886 (45.70%) in 2020. The proportions of thiazolidinediones, meglitinides, α-glucosidase inhibitors prescriptions remained stable over the years. Among SGLT2is, empagliflozin prescriptions showed an increase from 537 (10.65%) to 2881 (34.40%) compared to dapagliflozin, which decreased by the end of 2018 from 4505 (89.35%) to 5494 (65.6%). Conclusion SGLT2is have largely replaced sulfonylureas in Qatar. The increasing trend in their use over the years is similar to that reported in other countries. The trend among SGLT2is suggests greater preference for empagliflozin over dapagliflozin.

    Abstract: Though uncommon, complications of endovascular angioplasty and stenting may have devastating outcomes that can threaten limb and life. Adequate awareness of these complications will allow to achieve excellent clinical outcomes. Results Endovascular intervention was successful in treatment of most of distal SFA complications with limited use in flush ostial complications; residual stenosis ≥ 30%, flow-limiting dissection, perforation or rupture; 100% in group (2) vs 50% in group (1) but thrombosis; 75% of distal SFA lesions were treated endovascularly vs 40% of flush ostial lesions; X 2 = 12.7, P = 0.02. Emergency surgery as a bailout was used for most ostial lesion complications: residual stenosis ≥ 30%, flow-limiting dissection, and perforation or rupture; 50% in group (1) vs 0% in group (2); X 2 = 20.1, P = 0.01. Conclusion Bailout procedures are procedures used to save the limb or the patient. Evolving endovascular strategies embrace new technologies in an attempt to improve the safety and efficacy of revascularization procedures for lower extremity arterial occlusive disease.
