    Abstract: Elderly patients are susceptible to postoperative infections with increased mortality. Analyzing with a deep learning model, the perioperative factors that could predict and/or contribute to postoperative infections may improve the outcome in elderly. This was an observational cohort study with 2014 elderly patients who had elective surgery from 28 hospitals in China from April to June 2014. We aimed to develop and validate deep learning-based predictive models for postoperative infections in the elderly. 1510 patients were randomly assigned to be training dataset for establishing deep learning-based models, and 504 patients were used to validate the effectiveness of these models. The conventional model predicted postoperative infections was 0.728 (95% CI 0.688–0.768) with the sensitivity of 66.2% (95% CI 58.2–73.6) and specificity of 66.8% (95% CI 64.6–68.9). The deep learning model including risk factors relevant to baseline clinical characteristics predicted postoperative infections was 0.641 (95% CI 0.545–0.737), and sensitivity and specificity were 34.2% (95% CI 19.6–51.4) and 88.8% (95% CI 85.6–91.6), respectively. Including risk factors relevant to baseline variables and surgery, the deep learning model predicted postoperative infections was 0.763 (95% CI 0.681–0.844) with the sensitivity of 63.2% (95% CI 46–78.2) and specificity of 80.5% (95% CI 76.6–84). Our feasibility study indicated that a deep learning model including risk factors for the prediction of postoperative infections can be achieved in elderly. Further study is needed to assess whether this model can be used to guide clinical practice to improve surgical outcomes in elderly.

    Abstract: As an essential functionality, flexible focusing of surface plasmons (SPs) is of particular interest in nonlinear optics and highly integrated plasmonic circuitry. Here, we developed a versatile plasmonic metalens, a metasurface comprised of coupled subwavelength resonators, whose optical responses exhibit a remarkable feature of electromagnetically induced transparency (EIT). We demonstrate numerically and experimentally how a proper spatial design of the unit elements steers SPs to arbitrary foci based on the holographic principles. More specifically, we show how to control the interaction between the constituent EIT resonators to efficiently manipulate the focusing intensity of SPs. We also demonstrated that the proposed metalens is capable of achieving frequency division multiplexing. The power and simplicity of the proposed design would offer promising opportunities for practical plasmonic devices.

    Abstract: Malaria was once widespread in Guangzhou, China. However, a series of control measures have succeeded in eliminating local malaria infections. Based on the analysis of the characteristics of malaria epidemics in Guangzhou, China, from 1950 to 2022, the changes and effectiveness of malaria control strategies and surveillance management in Guangzhou from 1950 to 2022 are described. Methods Data on malaria prevention and treatment in Guangzhou from 1950 to 2022 were collected, and descriptive epidemiological methods were used to analyse the prevalence of malaria, preventive and control measures taken, and the effectiveness of prevention and treatment in different periods. Data on malaria cases were obtained from the Guangzhou Centre for Disease Control and Prevention (CDC) and the China Communicable Disease Reporting System. Results The development of the malaria control system in Guangzhou has gone through four periods: 1. High malaria prevalence (1950–1979), 2. Intensive prevention and control stage (1980–2000), 3. Consolidating gains in malaria control (2001–2008), and 4. Preventing reestablishment of transmission (2009–2022). During Period 1, only medical institutions at all levels and the local CDCs, the Guangzhou CDC participated in the malaria prevention and control system, establishing a three-tier health system on malaria prevention and control. During Period 2, other types of organizations, including the agricultural sector, schools and village committees, the construction department and street committee, are involved in the malaria control system. During Period 3, more and more organizations are joining forces to prevent and control malaria. A well-established multisectoral malaria control mechanism and an improved post-elimination surveillance management system are in place. Between 1950 and 2022, a total of 420,670 cases of malaria were reported. During Period 1, there was an epidemic of malaria in the early 1950s, with an annual incidence rate of more than 10,000/100,000, including a high rate of 2887.98/100,000 in 1954. In Period 2 malaria was gradually brought under control, with the average annual malaria incidence rate dropping to 3.14/100,000. During Period 3, the incidence rate was kept below 1/100,000, and by 2009 local malaria infections were eliminated. Conclusion For decades, Guangzhou has adopted different malaria control strategies and measures at different epidemic stages. Increased collaboration among civil organizations in Guangzhou in malaria control has led to a significant decline in the number of malaria cases and the elimination of indigenous malaria infections by 2009.The experience of Guangzhou can guide the development of malaria control strategies in other cities experiencing similar malaria epidemics.

    Abstract: By allowing almost arbitrary distributions of amplitude and phase of electromagnetic waves to be generated by a layer of sub-wavelength-size unit cells, metasurfaces have given rise to the field of meta-holography. However, holography with circularly polarized waves remains complicated as the achiral building blocks of existing meta-holograms inevitably contribute to holographic images generated by both left-handed and right-handed waves. Here we demonstrate how planar chirality enables the fully independent realization of high-efficiency meta-holograms for one circular polarization or the other. Such circular-polarization-selective meta-holograms are based on chiral building blocks that reflect either left-handed or right-handed circularly polarized waves with an orientation-dependent phase. Using terahertz waves, we experimentally demonstrate that this allows the straightforward design of reflective phase meta-holograms, where the use of alternating structures of opposite handedness yields independent holographic images for circularly polarized waves of opposite handedness with negligible polarization cross-talk.

    Abstract: Previous studies have reported that maternal smoking during pregnancy and breastfeeding may affect the occurrence of hypertension, but whether early life factors modify the impact of the offspring’s genetic risk on hypertension is still unknown. The aim of this study was to investigate the relationships among maternal smoking and breastfeeding with adult-onset hypertension and the modified impact of offspring genetic susceptibility. Methods This study included 437,185 participants from the UK Biobank who were initially free of hypertension and provided a prospective cohort of individuals aged 40 to 69 years. The association of maternal smoking during pregnancy and breastfeeding with hypertension was examined by using the Cox regression model. Then, a polygenic risk score (PRS) for hypertension was used to test the gene–environmental interaction on hypertension. Results During a median follow-up period of 8.7 years, a total of 68,148 cases of hypertension were identified in this study. The hazard ratios (HRs) and 95% confidence intervals (CIs) of hypertension for maternal smoking and breastfeeding were 1.11 (1.09, 1.13) and 0.96 (0.94, 0.98), respectively. However, no evidence of an interaction between maternal smoking and breastfeeding was observed. Across all levels of genetic risk, including high genetic risk, maternal smoking and nonbreastfeeding had higher hypertension hazards than nonmaternal smoking and breastfeeding, respectively. The adjusted HRs (95% CIs) of hypertension were 1.80 (1.73, 1.87) in those who had high genetic predisposition plus maternal smoking and 1.67 (1.60–1.74) in those with nonbreastfeeding and high genetic risk. There were significant additive interactions between maternal smoking or breastfeeding and genetic factors on the incidence of hypertension. Conclusions Maternal smoking and nonbreastfeeding were associated with a higher risk of hypertension in adulthood and may attenuate the risk of hypertension related to genetic factors. These results suggested that adherence to nonmaternal smoking and breastfeeding was associated with a lower risk of hypertension among participants with all gradients of genetic risk.

    Abstract: Linear polyethylenimine (LPEI) is considered as a desirable gene in vivo delivery system, but whether it could deliver the specific siRNA targeted EGFR to the tumor site to inhibit the growth of NSCLC xenograft in nude mice still needs to be examined. Methods In this study, LPEI/siRNA was made into a complex and SPC-A1-xenografted mice model was established. Then, stable LPEI/siRNA-EGFR complexes were intraperitoneally administrated. Afterwards, tumor growth was measured every 3 days. At the end of the experiment, tumor volume was calculated, and tumors were weighed, and examined for EGFR expression, proliferation, and apoptosis evaluations. By using blood samples, toxicity tests including aspartate aminotransferase (AST), alanine aminotransferase (ALT), urea and creatinine (Cr) were measured for liver and renal function evaluation. Serum concentrations of TNF-α and IFN-γ were also examined. Results Our results demonstrated that LPEI/siRNA-EGFR complexes could downregulate EGFR expression in SPC-A1 xenografted tumor upon single i.p. injection. LPEI/siRNA-EGFR complexes inhibited tumor growth and did not induce organ toxicity in SPC-A1-xenografted mice. At the end of the experiment no significant IFN-α increase was detected in LPEI/siRNA complexes or glucose-treated groups. Conclusions The novel modality of siRNA-based therapy targeting EGFR may be of great potential in NSCLC treatment.

    Abstract: Miniaturized nonvolatile reconfigurable optical components with a subwavelength thickness, extremely compact size, high-speed response, and low power consumption will be the core of next-generation all-optical integrated devices and photonic computing to replace traditional bulky optical devices and integrated circuits, which are reaching physical limitations of Moore’s law. Metasurfaces, as ultrathin planar surfaces, have played a major role in controlling the amplitude, phase, and polarization of electromagnetic waves and can be combined with various active modulation methods to realize a variety of functional devices. However, most existing reconfigurable devices are bounded in volatile nature with constant power to maintain and single functionality, which restricts their further extensive applications. Chalcogenide phase change materials (PCM) have attracted considerable attention due to their unique optical properties in the visible and infrared domains, whereas in the terahertz (THz) regime, research on the reversible phase transition in large-scale areas and applications of Ge 2 Sb 2 Te 5 (GST) are still under exploration. Here, we achieved reversible, repeated, and large-area switching of GST with the help of optical and thermal stimuli. Large-area amorphization with a 1 cm diameter of GST is realized by using a single laser pulse. Then, we incorporate GST into metasurface designs to realize nonvolatile, reconfigurable, multilevel, and broadband terahertz modulators, including the anomalous deflector, metalens, and focusing optical vortex (FOV) generator. Experimental results verify the feasibility of multilevel modulation of THz waves in a broadband frequency range. Moreover, the modulators are reusable and nonvolatile. The proposed approach presents novel avenues of nonvolatile and reconfigurable metasurface designs and can enable wide potential applications in imaging, sensing, and high-speed communications.

    Abstract: Primary cilia extending from mother centrioles are essential for vertebrate development and homeostasis maintenance. Centriolar coiled-coil protein 110 (CP110) has been reported to suppress ciliogenesis initiation by capping the distal ends of mother centrioles. However, the mechanism underlying the specific degradation of mother centriole-capping CP110 to promote cilia initiation remains unknown. Here, we find that autophagy is crucial for CP110 degradation at mother centrioles after serum starvation in MEF cells. We further identify NudC-like protein 2 (NudCL2) as a novel selective autophagy receptor at mother centrioles, which contains an LC3-interacting region (LIR) motif mediating the association of CP110 and the autophagosome marker LC3. Knockout of NudCL2 induces defects in the removal of CP110 from mother centrioles and ciliogenesis, which are rescued by wild-type NudCL2 but not its LIR motif mutant. Knockdown of CP110 significantly attenuates ciliogenesis defects in NudCL2 -deficient cells. In addition, NudCL2 morphants exhibit ciliation-related phenotypes in zebrafish, which are reversed by wild-type NudCL2, but not its LIR motif mutant. Importantly, CP110 depletion significantly reverses these ciliary phenotypes in NudCL2 morphants. Taken together, our data suggest that NudCL2 functions as an autophagy receptor mediating the selective degradation of mother centriole-capping CP110 to promote ciliogenesis, which is indispensable for embryo development in vertebrates.
