    Notes: Summary In conscious unrestrained spontaneously hypertensive and normotensive rats, prepared with permanently indwelling abdominal aortic catheters, the effects on blood pressure and heart rate of a 12-day continuous subcutaneous infusion of guanfacine (10 mg/kg/day) and clonidine (500 μg/kg/day) and sudden interruption of these treatments were studied. Both drugs significantly and consistently reduced the mean arterial pressure and heart rate throughout the infusion period in the SH rats, but not in the normotensive animals. The magnitude of the effects of both drugs in the SH rat were similar. Following withdrawal of treatment with guanfacine, a discontinuation syndrome was evoked, much less severe than that observed after suspension of the infusion with clonidine. The withdrawal syndromes were characterized by an overshoot of heart rate and a period of blood pressure lability. In spite of the ineffectiveness of guanfacine and clonidine to reduce blood pressure and heart rate consistently in the normotensive rat, similar withdrawal patterns as those found in the SH rat were observed. These findings are in general agreement with the results previously found in clinical studies in hypertensive patients. The spontaneously hypertensive rat may prove a suitable animal model for pre-clinical studies of discontinuation symptoms after cessation of treatment with antihypertensive drugs.

    Notes: Summary Pithing of anaesthetized normotensive cats significantly lowered the arterial blood pressure and augmented plasma renin activity (PRA). Captopril dose-dependently diminished mean arterial blood pressure in both pithed and intact anaesthetized normotensive cats. The hypotensive effectiveness of captopril was most pronounced in pithed cats. Captopril inhibited the hypertensive response to intravenously administered noradrenaline in pithed cats, but did not attenuate the hypertensive response to noradrenaline in intact cats. The results do not exclude that the hypotensive activity of captopril in intact cats may be causally independent of the attenuating effect of converting enzyme inhibition on the postjunctional α-adrenoceptor mediated vasoconstriction as observed in pithed animals.

    Notes: Summary Animals from a colony of spontaneously diabetic insulin-dependent BB rats were fed from weaning with semisynthetic diets in which natural proteins were replaced by l-amino acids with or without the addition of either milk or wheat proteins. The normal 50% incidence of diabetes in the colony was reduced to 15% in rats fed the basic semi-synthetic diet, while it was 35% and 52% when supplemented with gluten or milk, respectively. Thus, dietary factors might precipitate the expression of the disease.

    Notes: Conclusions Considerations of the structure of a candidate initiating antigen have led us to propose a new model for the susceptibility to Type 1 diabetes. It is quite likely there are other antigens of similar structure of viral or bacterial origin that will provoke a similar response directed against islet beta cells. However, we believe that this hypothesis will provide a framework for the further investigation and research into the mechanism of development of Type 1 diabetes and for the design of therapeutic manoeuvres for its prevention.

    Notes: Abstract Objective: To study the effect of exposure on bronchial responsiveness in pig farmers. Method: A group of 196 pig farmers were tested for lung function and bronchial responsiveness to histamine in the summer of 1992. To achieve sufficient contrast in respiratory morbidity and exposure, 96 of the farmers were selected because they had chronic respiratory symptoms and the remaining 100 because they were free from any respiratory symptoms. Personal exposure to dust, endotoxins and ammonia was measured during 1 working day in the summer of 1991 and 1 day in the winter of 1992. Data on farm characteristics were gathered in the same period. Results: After adjusting for age and smoking behaviour, mild bronchial responsiveness, defined as PC10≤ 16 mg/ml, was associated with the use of quaternary ammonium compounds as disinfectant [prevalence odds ratio (POR) 6.7, 95% confidence interval (CI) 1.4–32.8], use of wood-shavings as bedding (POR 13.3, CI 1.3–136.7), use of automated dry feeding (POR 2.8, CI 1.0–7.8), use of pellets as feeding material (POR 4.8, CI 1.1–21.1) and location of air exhaust via pit or roof in the confinement units (POR 2.7, CI 1.2–6.3). The association with the use of disinfectants other than quaternary ammonium compounds was not significant (POR 2.4, CI 0.7–8.4). No associations between bronchial responsiveness and measured exposure to dust, endotoxins or ammonia were discernible. Conclusion: Protective measures, designed to prevent airway disease in confinement farming, should be based on information about the operational and other characteristics of farms that are related to high exposure and health effects. Specifically, the use of quaternary ammonium compounds as disinfectant, the use of wood-shavings as bedding and the use of automated dry feeding should be discouraged.

    Notes: Summary Between January 1985 and December 1988, 386 patients with multiple myeloma were randomized to receive either MP or combination chemotherapy based on alternating cycles of VCMP and VBAP. The major prognostic parameters did not differ significantly between both treatment groups. A significantly higher proportion of objective responses was observed with combination chemotherapy as compared to MP (47.8 vs 32.2,P = 0.01). The median survival for all patients was 33.5 months. So far no significant differences were found when comparing the survival curves from both groups of patients. However, the median survival of MP-treated patients is 26.8 months, whereas the median survival of patients receiving VCMP/VBAP has not yet been reached. The definitive analysis must await the evaluation of all patients entered into the study and a longer follow-up time.

    Description / Table of Contents: Résumé Le taux d'insuline et d'hormone de croissance à la suite d'une charge en glucose (100 g) par voie orale fut mesuré chez23 acromégaliques qui avaient été traités par radiothérapie. Seize de ces patients étaient considérés cliniquement en phase «inactive» et les sept autres en phase «active». Deux de ces derniers étaient diabétiques et sous traitement. Treize sujets normaux, non obèses, formaient le groupe témoin. Cinq acromégaliques en phase «inactive» ont présenté à la suite d'une charge en glucose des valeurs de la glycémie dépassant les limites supérieures de la normale. Tous les acromégaliques en phase «active» et «inactive» présentaient une hyperinsulinémie à l'état de jeûne et à la suite d'une charge en glucose. Cette observation d'une montée exagérée de l'insulinémie à la suite d'une charge en glucose chez des acromégaliques cliniquement «inactifs» suggère qu'un taux sérique d'hormone de croissance continuellement élevé peut avoir amené des changements permanents de la réponse des îlots du pancréas à la suite d'une stimulation venant du glucose. Les valeurs moyennes du taux sérique de l'hormone de croissance étaient semblables à l'état de jeûne chez le groupe des témoins et chez les acromégaliques en phase «inactive». Ces derniers n'ont pas présenté une baisse du taux sérique d'hormone de croissance à la suite d'une charge en glucose.
    Abstract: Zusammenfassung Die vorliegende Arbeit beschreibt den Verlauf der Seruminsulin- und Wachstumshormonwerte nach oraler Verabreichung von 100 g Glucose bei 23 Patienten mit Akromegalie, die früher mit Bestrahlung behandelt worden waren. Auf Grund der klinischen Befunde zum Zeitpunkt der Untersuchungen wurden sechzehn Patienten als „inaktive“ und sieben Fälle als „aktive“ Akromegale eingestuft. Zwei von den „aktiven“ Akromegalen waren behandelte Diabetiker. Dreizehn gesunde, nicht fettsüchtige Personen bildeten die Kontroll-gruppe. Fünf „inaktive“ Akromegale hatten nach der Glucosebelastung Blutglucosewerte über dem Normbereich. Sowohl die „inaktiven“ als auch die „aktiven“ Akromegalen zeigten Hyperinsulinämie nüchtern und nach Glucosezufuhr. Die gesteigerte Insulinsekretion der klinisch „inaktiven“ Akromegalen nach Glucosebelastung kann bedeuten, daß chronisch erhöhte Spiegel des zirkulierenden Wachstumshormons möglicherweise zu ständigen Veränderungen in der Reaktion derLangerhans'schen Inseln gegenüber Glucosestimulation geführt haben. Der durchschnittliche Serumwachstumshormonwert im Nüchternzustand war ungefähr der gleiche bei der Kontrollgruppe und bei „inaktiven“ Akromegalen; die letzteren wiesen keine Verminderung der Serumwachstumshormonwerte nach Glucoseverabreichung auf.
    Notes: Summary The insulin and growth hormone responses to oral glucose load (100 g) in 23 acronaegalic patients, previously treated by external irradiation, are described. Based on current clinical findings, sixteen acromegalics were considered “inactive” and seven patients “active”. Two of the latter were treated diabetics. Thirteen healthy, non-obese subjects formed the control group. Five “inactive” acromegalics had blood glucose values exceeding the upper limits of normal after the glucose load. Both the “inactive” and “active” acromegalics had hyperinsulinemia in the fasting state and after glucose load. The observation of an exaggerated serum insulin response to glucose load in clinically “inactive” acromegalics suggested that chronically elevated levels of circulating growth hormone may have led to permanent changes in the responsiveness of the pancreatic islets to glucose stimulation. The mean fasting value of serum growth hormone was about the same in the controls and the “inactive” acromegalics; the latter did not show suppression in serum growth hormone levels after glucose load.

    Notes: Summary The effect of bond functions on the basis set superposition error (BSSE) is investigated at both SCF (self consistent field) and correlated levels for a number of basis sets using the pairwise additive function counterpoise (PAFC), the site-site function counterpoise (SSFC), and the newly proposed successive reaction counterpoise method (SRCP). BSSEs using bond functions are shown to be roughly twice those without bond functions, whereas the latter may still be quite sizeable. The addition of f functions dramatically decreases the bond function BSSE. The results obtained support the empirical decision in our earlier papers to neglect BSSE altogether.
