    Notes: Abstract The spin asymmetryA N for inclusive π0 production by 200-GeV transversely-polarized protons on a liquid hydrogen target has been measured at Fermilab over a wide range ofx F, with 0.5〈p T〈2 GeV/c. Atx F〉0.3, the asymmetry rises with increasingx F and reaches a value ofA N=0.15±0.03 in the region 0.6〈x F〈0.8. This result provides new input regarding the question of the internal spin structure of transversely-polarized protons.

    Notes: Summary An expert system with 60 questions about medical history was developed for 32 rheumatologic diseases: 358 outpatients with joint complaints have been examined. The final diagnosis (result of symptoms, signs, and findings) was compared with the computer diagnoses and with the independently assumed diagnoses of the physician. The only source of information available to the physician was the medical history. Misinterpretation of the computer diagnoses occurred in 25.6% of cases compared with 21.5% of the physician. The final clinical diagnosis remained uncertain in 32.6% of cases. The error frequency of the expert system was influenced by the underlying disease, the certainty of the assumed diagnosis by the physician, the user experience in rheumatology, the number of questions asked, and the time of application before or after the doctor-patient contact. Of the errors 44% were produced because of information deficits of the computer using standardized questions. The information of the physician in the diagnostic process is quite different to that of the computer.

    Notes: Summary A circulating high-molecular-weight glycoprotein called fibronectin plays a part in cell adhesion and migration before phagocytosis and in morphology, differentiation, and metabolism in inflammatory synovial effusions of patients with rheumatic diseases. A technique of nephelometric immunoassay, based on the measurement of an antigen-antibody reaction, was applied to the analysis of fibronectin concentrations in synovial fluids from 20 patients with rheumatoid arthritis (RA) and other diseases (non-RA). RA synovial fluids have a significantly higher concentration than the specimens obtained from Yersinia arthritis patients (n=12). The mean concentration of other synovial fluids, from 12 patients with osteoarthritis of the knees, did not significantly differ from the synovial fluids of control values obtained from patients who underwent meniscectomy. There was a considerably negative correlation between fibronectin levels and overall indices of inflammatory activity, such as Ritchie articular indices or a whole number of painful rheumatoid arthritis joints. However, a particularly distinct correlation was obtained when raised fibronectin levels were compared with the inflammatory activity of the knee joint, from which the specimen was aspirated. Thus, these findings suggest that the measurements of fibronectin in synovial fluid may be of some differential-diagnostic value in rheumatoid variants, but may only serve as an indicator of inflammatory activity if the joint, from which the specimen is obtained, is taken into account.

    Description / Table of Contents: Summary Gangrene of digits is a very rare manifestation of Wegener‘s granulomatosis (WG). We report a case of a 29-year-old woman with nonspecific symptoms like fever, weight loss, arthralgia, arthritis and high systemic inflammatory signs. On the grounds of a presumed infection the patient was treated with antibiotics which showed no effect. Within short she complained of pain and paresthesia of the right foot with a rapid lividity. Angiography demonstrated multiples stenoses and multisegmental occlusions of the lower leg arteries. Together with renal and pulmonal symptoms a WG was suspected, the diagnosis being confirmed by kidney biopsy and positive cANCAs. A few months after starting a combined therapy with cyclophosphamide and corticosteroids the patient showed a partial remission with a residual toe gangrene. Comparing the five worldwide reported cases with digital gangrene and our presentation there is a concordance of all in the occurence of an extremely high disease activity together with a glomerulonephritis. The findings of p-ANCAs in our patient and positive Hbs antigen in another case of WG with digital gangrene suggests a relation to panarteriitis nodosa, where gangren is more common.
    Notes: Zusammenfassung Die periphere Gangrän stellt im Rahmen der Wegener-Granulomatose (WG) eine äußerst seltene Manifestation dar. Wir präsentieren den Fall einer 29jährigen Patientin, die hochakut mit zunächst unspezifischen Beschwerden und Befunden wie Fieber, Gewichtsverlust, Arthralgien bzw. Arthritis und hohen serologischen Entzündungszeichen erkrankte und unter Annahme einer Infektion antibiotisch (ohne Besserung) behandelt wurde. Bald darauf kam es schlagartig zu Schmerzen, Pelzigkeit und livider akraler Verfärbung im re. Vorfuß mit rascher Progredienz. Angiographisch zeigten sich multiple Stenosen und multisegmentale Verschlüsse der Unterschenkelarterien. Gleichzeitiger pulmonaler und renaler Befall führten zur Verdachtsdiagnose einer WG, die nierenbioptisch und bei deutlich positiven c-ANCA bestätigt werden konnte. Die kombinierte Therapie mit Kortikoiden und Cyclophosphamid führte zur Teilremission innerhalb weniger Monate mit Restschädigung einer Zehe. Unserem sowie den weiteren fünf weltweit publizierten Fällen mit dieser Manifestation ist gemeinsam, daß stets eine äußerst hohe Gesamtaktivität der Erkrankung und Nierenbeteiligung vorlag. Der gleichzeitige Nachweis von p-ANCA bei unserer Patientin und des HBs-Antigens in einem weiteren Fall läßt in diesen Fällen an eine Verwandtschaft zur Polyarteriitis nodosa (PAN) denken, wo die Gangrän weniger selten vorkommt. Ob es sich hierbei um das Vorliegen zweier koinzidenter Krankheitsbilder oder um Overlap-Fälle handelt, muß offenbleiben, ein Overlap-Syndrom zwischen WG und PAN ist bisher nicht bekannt.

    Description / Table of Contents: Summary Only during recent years have intensive attempts been made to install a program for more objectively based quality criteria instead of the traditional subjectively and empirically determined therapeutic decisions for the application of DMARDs. The optimized quality management for this treatment should focus on factors such as: diagnosis as early as possible (basic for the introduction of DMARD therapy), reliable assessment of the individual long-term prognosis (as an important aid in the choice of the appropriate drug), prompt determination and establishment of the chosen therapy, and an optimal long-term application of this treatment (including competent control of effectiveness, tolerance, and compliance). Instrumentation for these purposes have been so far only partly available, e.g., diagnostic criteria and control guidelines. Besides the partially missing appropriate measures, maximum quality management is presently opposed by insufficient rheumatological specialist care for many RA patients – caused by the lack of specialists and a dilatory attitude towards involving them, respectively. Nevertheless, there is hope that in the course of new developments the gap between needs and benefits of quality management will grow smaller.
    Notes: Zusammenfassung Erst in den letzten Jahren wird zunehmend ein Instrumentarium dafür erarbeitet, die traditionell subjektiv und empirisch determinierten Therapieentscheidungen zum Einsatz von Basistherapeutika mehr auf den Boden objektiver Qualitätskriterien zu stellen. Ein optimiertes Qualitätsmanagement für diese Therapie sollte Faktoren wie frühestmögliche Diagnosestellung (als Voraussetzung für die Einleitung einer Basistherapie), zuverlässige Einschätzung der individuellen Krankheitsprognose (als wichtige Entscheidungshilfe für die Wahl der geeigneten Substanz), rasche Festlegung und Einleitung der ausgewählten Therapie und eine optimale Langzeit-Durchführung dieser Therapie (mit zuverlässiger Überwachung von Effektivität, Verträglichkeit und Compliance) beinhalten. Die hierfür nötigen Werkzeuge stehen bisher nur teilweise zur Verfügung, so z.B. validierte Diagnosekriterien und Überwachungsrichtlinien. Neben den partiell noch fehlenden geeigneten Instrumenten steht einem optimalen Qualitätsmanagement aber gegenwärtig – bedingt durch einen Mangel an Spezialisten bzw. deren zu zögerliche Miteinbeziehung – auch eine mangelhafte rheumatologische Fachbetreuung vieler Rheumatiker entgegen. Dennoch ist aufgrund der neueren Entwicklungen zu hoffen, daß die Lücke zwischen Bedarf und Möglichkeiten des Qualitätsmanagements immer geringer wird.

    Notes: Abstract In the present study, the immunophenotype of basal cell carcinoma was analysed in comparison with human vellus hair follicular keratinocytes. We also established the lectin binding profile of basal cell carcinoma and human vellus hair follicles (VHF), using several lectins with different sugar specificities. Our findings showed an almost identical immunohistochemical profile for basal cell carcinoma and the suprabulbar region of the outer root sheath of VHF, whereas other follicular compartments such as the bulbar, the isthmus or the supraseboglandular regions did not correlate. In particular, homogeneous and constant expression of the basal differentiation markers CK 5 and CK 14 were found in both specimen, with no expression of the simple epithelium type keratin CK 8 and the suprabasal differentiation markers CK 1 and CK 10. CK 19 showed variable expression in basal cell carcinoma, with constant expression in the outer root sheath and the follicular bulge regions, but was always absent in interfollicular epidermal keratinocytes. In addition, the lectin binding profiles of basal cell carcinoma and the outer root sheath in the suprabulbar region of human VHF were comparable, with the presence of binding sites for PNA, Con A and WGA. These findings provide evidence for a histochemical relationship between basal cell carcinoma and the follicular epithelium of VHF which is closer than that with the epidermis, and suggest its possible origin from or possibly its differentiation pattern towards the cells of the outer root sheath and/or the follicular bulge region of the VHF.

    Notes: Abstract Nucleotide sequence and biochemical analysis of d-β-hydroxybutyrate dehydrogenase (EC 1.1.1.30), isolated from Rhodobacter sp., indicate functional oligomers composed of subunits of 257 amino acids with a calculated M r of 26,800 and a pI of 5.90. Compared to mammalian short-chain alcohol dehydrogenases, the bacterial enzyme lacks a C-terminal lipid anchor domain and was found to be highly active upon expression in Escherichia coli even without lipid supplement. The recombinant enzyme could be highly enriched using a single chromatography step and was shown to be stable over a broad range of pH and temperature.
