Résumé C'est le résultat de l'examen medical clinique et des radiogrammes des dysplasia des hanches qui sont soignées fontionellement: 1. L'antetorsion est plus grand plus plain est le rebord antérieur de l'acetabulum. C'est aussi le résultat des expériences avec des animaux (Idelberger, 1966). 2. Les angles de l'antetorsion et de CCD sont plus grand plus escarpés est le rebord anatomique de l'acetabulum. 3. L'angle CE était choisi comme l'index résumant pour le développement incomplet de l'acetabulum et la latéralisation et c'est pourquoi comme la mesure radiologique pour l'instabilité de l'articulation de la hanche. Les angles AT et CCD sont plus grands plus petit est l'angle CE. 4. Les angles AT et CCD sont conforms. En outre la question était discutée si le traitement par le bandage de Pavlik et par la fixation raide dans la position de Lorenz ou Lange peuvent influencer la torsion de la fin coxale du femur. Les résultats indiquent que l'antetorsion pathologique n'est pas au premier part de l'image pathologique et anatomique de la dysplasia de la hanche mais est une developpement secondaire des conditions changées quant à la statique et dynamique. C'est aussi le résultat des experiences avec des animaux (Idelberger, 1966).
Abstract:
Zusammenfassung Bei der klinischen und röntgenologischen Nachuntersuchung funktionell behandelter Hüftdysplasien ließ sich folgendes zeigen: 1. Die AT ist um so größer, je flacher der vordere Pfannenrand ist. Dies entspricht den Ergebnissen der Tierversuche Idelbergers (1966). 2. Antetorsions- und Schenkelhalswinkel sind um so größer, je steiler das anatomische Pfannendach ist (Idelberger u. Frank, 1952). 3. Der CE-Winkel (Wiberg, 1939, 1944, 1953) wurde als zusammenfassender Index für die mangelnde Pfannendachentwicklung und die Lateralisation und damit als röntgenologisches Maß für die Instabilität des Hüftgelenkes gewählt. Daher sind Antetorsions- und Schenkelhalswinkel in der Regel um so größer, je kleiner der CE-Winkel ist. 4. Antetorsions- und Schenkelhalswinkel verhalten sich bei den untersuchten Dysplasiehüften im wesentlichen gleichsinnig. Weiterhin wurde die Frage erörtert, inwieweit die Pavlik-Bandage und die starre Fixierung in Lorenz- oder Lange-Stellung die Torsion des coxalen Femurendes beeinflussen könnte. Die Ergebnisse sprechen dafür, daß die pathologische Antetorsion nicht primär endogen zum pathologisch-anatomischen Bild der Hüftdysplasie gehört, sondern sekundär Folge der veränderten statisch-dynamischen Verhältnisse des Hüftgelenkes ist.

Summary During the clinical and X-ray examination of functionally treated cases of dislocation of the hip joint we could show the following: 1. The AT is greater, the flatter the front edge of the socket. This is corresponding to the results obtained by Idelberger in animals (1966). 2. The antetorsion angle and the angle subtended by the femoral neck to the shaft are greater, the steeper the anatomical roof of the socket (Idelberger and Frank, 1952). 3. The CE-angle (Wiberg, 1938, 1944, 1953) was chosen as a summarizing measure for the lacking development of the roof of the socket and for the lateralisation, hence as a measure of the instability of the hip joint. Accordingly, the antetorsion angle and the angle subtended by the femoral neck to the shaft are in the rule the greater, the smaller the CE-angle. 4. The antetorsion-angle and the angle subtended by the femoral neck to the shaft have in the examined cases in general the same tendency. It was further discussed which influence the Pavlik-Bandage and the stiff fixation in Lorenz- or Lange-Position have on the torsion of the coxal end of the femur. The results support the thesis that the pathological antetorsion does not belong primarily endogen to the pathologic anatomical picture of the dislocation of the hip joint, but is a socondary consequence of the changed static-dynamic conditions of the hip joint. This is corresponding to the results obtained by Idelberger in animals (1966).
