    Notes: Summary A convenient miniassay for aflatoxin has been developed for cultures ofAspergillus flavus andA. parasiticus grown for 3–10 days in 10 ml of a coconut extract medium. The sensitivity of the assay, as measured by photofluorometry (365 nm maximum excitation; 445 nm maximum emission), is of the order of 0.01 μM (3.12 ng/ml) for aflatoxin B1 dissolved in aqueous iodine (0.26 mM). High performance liquid chromatography, monitored by fluorometric analysis of both an aflatoxin B1 standard and selected culture filtrates, confirmed the sensitivity of the assay and indicated specificity for iodine-enhanced fluorescence of aflatoxin in the coconut extract medium. Thin layer chromatography further confirmed the aflatoxin titers and the specificity for enhancement of aflatoxins B1 and G1 in culture filtrates.

    Notes: Abstract A primal - dual simplex variant is presented that incrementally builds up the optimal LPbasis matrix. For network LPs, the incremental primal - dual algorithm can adopt steepest-edgedirections of movement utilizing standard graph data structures without additionalcomputational overhead. Computational results indicate the resulting implementation isuniformly superior to other direction movement schemes, and achieves order of magnitudespeedups versus a primal network simplex code on medium size NETGEN problems. Thesespeedups carry over to the generalized network problem domain as well. Some comparisonsare also made against the relaxation method on NETGEN uncapacitated transportation andtransshipment problems.

    Description / Table of Contents: Résumé Les cancers de la marge anale sont traités par exérèse locale, la chirurgie radicale étant réservée aux cancers envahissants ou associés à fistules ou condylomes. Les cancers squameux du canal anal furent traités par amputation abdomino-périnéale sans se soucier du type cytologique. L'irradiation isolée a été pronée par quelques auteurs. Pour notre part, nous avons entrepris une méthode thérapeutique faisant appel simultanément à l'irradiation, à la chimiothérapie et à la chirurgie. Cette étude comprend 104 malades qui ont été traités de décembre 1971 à juillet 1983. Nous avons abandonné l'amputation abdomino-périnéale de routine dès 1975 parce que grâce à la chimiothérapie associée à l'irradiation, nous avions pu contrôler la lésion primitive 5 fois chez 6 malades. Dans les séries la tumeur à disparu chez 97 sujets après chimiothérapie et irradiation. L'opération radicale fut pratiquée de routine chez 24 malades, chez 7 pour lésion résiduelle, et chez 7 pour récidive. Vingt et un décès furent à déplorer dont 13 dûs à l'affection néoplasique elle-meême. Les morts dûs au cancer sont le fait de lésions primitives importantes ou de récidives locales. Le taux de survie extrapolé à 5 ans est de 83% avec 65 malades suivis au moins depuis 5 ans. D'après notre méthode thérapeutique, 16 sur 38 sujets seront soumis à une chirurgie radicale. La chimiothérapie associée à l'irradiation est préconisée pour tous les malades porteurs d'un cancer squameux du canal anal ou d'un cancer envahissant de la marge anale. Ceux qui sont atteints d'emblée d'un cancer volumineux (6–8 cm) et ceux qui présentent une récidive doivent être traités par chimiothérapie et irradiation additionnelle et/ou amputation abdomino-périnéale du rectum.
    Abstract: Resumen Los cánceres del margen anal son tratados mediante resección local, reservando la cirugía radical sólo para cánceres invasivos o para aquellos asociados con fístula o condilomas. Los cánceres escamocelulares han sido tratados con resección abdominoperineal no importa cual sea el tipo celular. La irradiación como única modalidad terapéutica ha sido preconizada por algunos autores, y nuestro grupo ha iniciado un estudio utilizando la combinatión de irradiación, quimioterapia y cirugia. El estudio incluye 104 patientes tratados entre diciembre de 1971 y julio de 1983. La resección abdominoperineal rutinaria fue descontinuada por nosotros en 1975 debido a que la quimioirradiación resultó en la eliminatión de la lesión en 5 de los primeras 6 patientes. En nuestra serie, el tumor macroscópico desapareció en 97 patientes después de la quimioirradiación. La operatión radical fue realizada rutinariamente en 24 patientes, para enfermedad residual en 7 y para enfermedad recurrente en otros siete. Se presentaron 21 muertes, de las cuales 13 se debieron a cáncer del canal anal. Las grandes lesiones primarias y la enfermedad local recurrente son responsables de la mayoría de las muertes por cáncer. La tasa de supervivencia a 5 años se proyecta en 83%, con 65 pacientes seguidos por lo menos por 5 años. Bajo nuestro regimen actual de manejo, sólo 16 de 38 pacientes serían sometidos a cirugía radical. La terapia con quimioirradiación es preconizada para todos los pacientes con cáncer escamocelular del cana anal y para cánceres invasivos del márgen anal. Los pacientes con grandes tumores primarios (6–8 cm) y aquellos con enfermedad recurrente son sometidos a terapia adicional con quimioirradiación y/o resección abdominoperineal del recto.
    Notes: Abstract Anal margin cancers are treated by local excision, with radical surgery reserved only for invasive cancers or those associated with fistula or condylomata. Squamous cell cancers of the anal canal have been treated by abdominoperineal resection regardless of cell type. Radiation therapy alone has been advocated by some, and we initiated a study using a combination of radiation, chemotherapy, and surgery. This study includes 104 patients treated from December, 1971, to July, 1983. Routine abdominoperineal resection was discontinued by us in 1975 because chemoradiation therapy eliminated the primary lesion in 5 of the first 6 patients. In the series, gross tumor disappeared in 97 patients after chemoradiation therapy. Radical operation was done routinely in 24 patients, for residual disease in 7, and for recurrent disease in 7 others. There were 21 deaths of which 13 were due to cancer of the anal canal. Large primary lesions and recurrent local disease account for the majority of deaths due to cancer. The 5-year survival rate is projected to be 83%, with 65 patients followed for at least 5 years. Under our current regimen only 16 of the 38 patients would have had radical surgery. Chemoradiation therapy is advocated for all patients with squamous cell cancer of the anal canal and for invasive cancers of the anal margin. Patients with large (6–8 cm) primary cancers and those with recurrent disease should have additional chemoradiation therapy and/or abdominoperineal resection of the rectum.

    Notes: Summary An immunohistochemical investigation of vimentin, an intermediate filament, was carried out on formalin fixed paraffin embedded sections of 44 malignant mesotheliomas of the pleura and 24 pulmonary adenocarcinomas, in order to assess its value in differential diagnosis. Seventy-five percent of the malignant mesotheliomas showed positive staining for vimentin. An unexpected finding was the presence of vimentin tin in 46% of the pulmonary adenocarcinomas. In either case there was no correlation between the presence of vimentin and the histological types or grades of differentiation. The overall level of vimentin staining was significantly greater in the malignant mesotheliomas than in the pulmonary adenocarcinomas but no single antibody dilution was found to provide clear cut separation of the two groups. Vimentin does not appear to be a simple discriminatory marker of malignant mesothelioma.

    Notes: Summary Elderly patients with end-stage renal disease often remain on dialytic therapy because they are at increased risk for mortality and morbidity. We placed 24 cadaver kidney transplants into 24 patients aged 65–74 years between September 1, 1985, and August 31, 1995. Rates of patient and graft survival were compared with those of 404 concurrent first cadaver-kidney transplant recipients between the ages of 20 and 44 years. The 5-year rates of patient and graft survival were not significantly different (86% versus 92% and 77% versus 63%, respectively; study group presented first). Primary cadaver kidney transplantation can be successfully performed in patients older than 65 years when a selection algorithm is applied to exclude active infection, active malignancy, unsuitable anatomy for technical success, high probability of operative mortality, and noncompliance. Pelvic arteriosclerosis and lower urinary tract abnormalities can cause intraoperative technical problems.

    Notes: Summary To evaluate the role of zinc status in immune system dysfunction in diabetic animals, the interleukin-2 production and the lymphocyte mitogenic response to phytohaemagglutinin, concanavalin A and lipopolysaccharide were measured in streptozotocin-induced diabetic rats, diabetic rats treated with insulin and their non-diabetic controls maintained on low zinc, normal zinc and high zinc diets for 3 weeks. Unstimulated lymphocyte proliferation was significantly lower in diabetic rats compared to nondiabetic control rats maintained on normal zinc diet (1505±318 vs 3447±497 cpm) (p〈0.005) or low zinc diet (546±191 vs 4011±628cpm) (p〈0.005). High zinc diet attenuated the difference between the diabetic rats (2404±833 cpm) and control rats (3929±713 cpm). Insulinised diabetic rats were similar to control rats. Phytohaemagglutinin-stimulated lymphocyte proliferation was not significantly altered with dietary zinc changes, but diabetic rats on low zinc diet had significantly lower (p〈0.025) values compared to control rats on the same diet (41470±7874 vs 72308±8895 cpm). Insulinisation did not normalise phytohemaegglutinin-stimulated lymphocyte proliferation (40711±3666 cpm). Similarly, cells from diabetic rats on low zinc diet, unlike their controls, failed to respond to concanavalin A stimulation. Compared to control rats the diabetic rats on either low or normal zinc diets had lower lipopolysaccharide-stimulated lymphocyte proliferation. High zinc diet or insulinisation normalised mitogenic response of lymphocytes to lipopolysaccharide. Unlike the diabetic rats alterations in dietary zinc intake did not significantly affect the lymphocyte proliferation in control rats. Neither the diabetic state nor zinc status had any significant effect on interleukin-2 production. Thus, zinc status of the animal is an important determinant of cell-mediated immunity, but additional factors peculiar to the diabetic state may be involved in the modulation of the immune system in diabetes.
