    Abstract: To achieve tumor-specific delivery of doxorubicin, TiO 2 @Fe 3 O 4 /PEI/delivery of doxorubicin conjugates were designed and synthesized. Fe 3 O 4 could act as magnetically responsive carriers and enhance the visible light photodynamic activities of TiO 2 . Delivery of doxorubicin was conjugated via coordination bond. The drug release rate at pH 5.2 was much faster than that at pH 7.4, due to pH-sensitive coordination bond. Besides, TiO 2 @Fe 3 O 4 /PEI/delivery of doxorubicin showed high antitumor efficacy combining with phototherapy, good bio-safety, higher cellular uptake with an external magnetic field, and less toxicity in vitro and in vivo. These results suggested that TiO 2 @Fe 3 O 4 /PEI/delivery of doxorubicin may be promising for high tumor treatment efficacy with minimal side effects in future.

    Abstract: Recognizing high-rise building water-use maneuver should be helpful for developing some practical strategies when indoor room space safety becomes a crucial feature in the building operation. This paper presents a data-processing approach for exploring the water-use maneuver of high-rise building. This approach combines the empirical mode decomposition and statistical analysis to process the mezzanine-floor air-pressure data measured in the drainage stack system of Li Ka-Shing building at PolyU of Hong Kong on 28th of April of 2008. Because the mezzanine-floor air-pressure signals in the building drainage system are recorded within about 10 hours with a recording rate of one signal per second, data re-sampling is obviously needed. Otherwise, the direct application of empirical mode decomposition should be unavailable because the mezzanine-floor air-pressure data are too massive. Statistical analysis is further encompassed after the empirical mode decomposition to seek the influences of re-sampling time interval and the empirical mode decomposition index so that the building water-use maneuver can be understood in detail. Practical application: From the viewpoint of Building Services Engineering, any occupied space should be safeguarded, because the depletions of the trap seals 1 and the bathroom floor drain traps 2 can result in cross-contamination via the drainage system. This suggests that it is crucial to investigate building water-use maneuver when indoor room space safety becomes urgent. To explore the water-use maneuver, an indirect way is by analyzing the mezzanine-floor air-pressure in the drainage stack system in terms of an appropriate approach. Since the mezzanine-floor air-pressure in the drainage stack system of an 18-floor building, i.e. Li Ka-Shing building at PolyU of Hong Kong has been recorded, 3 it can be used to propose the water-use maneuver approach which is helpful for developing some practical strategies for indoor room space safety when the safety should be carefully and urgently faced.

    Abstract: The goal of this study is to identify, classify, explore, and understand decision-making complexity elements in major infrastructure projects (MIPs). This article puts forward systematic definitions of decision making, decision-making quality, and decision-making complexity. Based on literature reviews and an in-depth case analysis of Hong Kong-Zhuhai-Macao Bridge (HZMB), a comprehensive framework of decision-making complexity is developed, which divides the elements into six dimensions: technical, social, financial, legal, organizational, and time. The links between different dimensions are also illustrated. This article is expected to deepen our current understanding of decision-making complexity and to provide a fundamental point of reference in the front-end phase of MIPs.

    Abstract: The incidence of tumours found in the patella, including primary and metastatic tumours, is low. Solitary metastasis of oesophageal carcinoma (OC) in the patella is even rarer. A 50-year-old man presented to our clinic because of pain and limited range of motion in the right knee for 4 hours and after a fall. On the basis of the patient’s medical history, he was diagnosed with OC 2 months previously and underwent two cycles of paclitaxel liposome combined with tiggio chemotherapy (oral tiggio, 40 mg, two times/day, with a treatment cycle of 3 weeks). A 99mTc-methylene diphosphonate bone scintigraphy scan showed increased radioactivity in the right patella. A right knee biopsy showed the presence of patellar metastasis from OC. Unfortunately, the patient denied additional treatment and was discharged for personal reasons. At the 1-month follow-up, which was conducted by a telephone survey, we learned that the patient had died of acute pulmonary embolism. X-rays and computed tomography are useful for diagnosing patellar metastases, but 99mTc-methylene diphosphonate bone scintigraphy can help physicians diagnose patellar metastasis of OC more rapidly. Biopsy with pathology is the gold standard for diagnosing patellar metastases. Additionally, timely surgical treatment prolongs the survival time of these patients.

    Abstract: The probability of malignancy in women who are diagnosed with a Breast Imaging Reporting and Data System (BI-RADS) 4A score is low. Application of a second opinion ultrasound (SOUS), which is low in cost and minimally invasive, may lower the biopsy rate for patients who fall into this category. This study aimed to apply SOUS to patients with a BI-RADS score of 4A and predict the pathological results of a biopsy. Methods One hundred seventy-eight patients were analyzed. Univariate and multivariate analyses were performed to screen for predictive factors that are associated with malignancy. Categorical alteration of downgraded, unchanged, or upgraded was made after SOUS results. Changes in category were compared with biopsies to determine their predictive value of benignancy or malignancy. Results Independent factors associated with malignancy were age ( 〉 50 years), tumor size (≥20 mm), margin (not circumscribed), orientation (not parallel), and peripheral location, and an upgraded categorical alteration from SOUS. Downgraded categorical alterations were associated with benignancy. Conclusions In BI-RADS 4A cases, a biopsy is recommended when independent factors are associated with malignancy. A downgraded result from an SOUS examination is a protective factor, supporting the likelihood of benignancy in these patients.

    Abstract: This study is the first attempt to evaluate the use of the Elecsys® HIV combi PT assay in identifying acute and early HIV infection in southwest China. We also analyzed the extent of cutoff ratios overlap between false-positive and true-positive results to aid the identification of HIV infection, using samples from the West China Hospital in Chengdu, Sichuan Province from April 2012 to December 2013. Reactive results from a screening test were retested and all repeatedly reactive samples – if available – were confirmed with Western blot, HIV-1 p24 antigen, or HIV-1 RNA. Of 241,840 samples screened, the Elecsys® HIV combi PT assay identified 54 patients with acute and early HIV infection; 99.8% cases with cutoff index ratios ≥50 were proved to be true-positive HIV infection and 95.6% cases with cutoff index ratios 〈 15 were falsely positive. In conclusion, the Elecsys® HIV combi PT assay can identify acute and early HIV infection, including those who might have been missed by third-generation HIV screening assays and Western blot. However, cutoff index ratios 〈 15 are not always false-reactive results; a definitive result cannot be attained without further confirmation. In resource-poor regions where a HIV-1 nucleic acid test may be unaffordable, detection of HIV-1 p24 antigen can be an alternative strategy to diagnose HIV infection in individuals with a negative or indeterminate Western blot.

    Abstract: Transportation demand management (TDM) is one of the important methods for solving the problem of increasingly severe urban traffic congestion. This paper proposes a bi-level model to optimize urban TDM strategies based on simulation. The upper level is the TDM strategy optimization model, searching for the optimal TDM strategy. The lower level is a traffic assignment model based on the simulation, assigning traffic flow to multimodal transport networks according to the candidate TDM strategies. A heuristic algorithm is also defined and implemented to optimize TDM strategies. Based on VISSIM simulation, the optimization for TDM strategies proposed in this paper is validated with the research area of Harbor Square–San Ba Square–Er Qi Square in Dalian in China. The results show that the optimization of urban TDM strategies can effectively alleviate urban traffic congestion. It provides a scientific decision basis for urban TDM policy.

    Abstract: This study aimed to examine gender differences in smoking and alcohol drinking behaviors in three Asian cities of Hanoi, Shanghai, and Taipei, and to assess the magnitude of gender differences across the three cities. Methods A total of 17,016 adolescents (age: 15–19 years) and young adults (age: 20–24 years) were selected using multi-stage sampling methods and surveyed in face-to-face interviews. A total of 16,554 unmarried respondents were included in this analysis. Results Gender differences were significant for smoking only, drinking only, and both behaviors in each city. Male respondents were 30.66 times more likely to report smoking only than female respondents in Hanoi, followed by Shanghai and Taipei. This pattern was similar for drinking only and both smoking and drinking behaviors. Conclusions The magnitude of gender differences in smoking only, drinking only, and both behaviors widely varies across the three cities. Further research can examine how these differences may be used to prevent and reduce smoking and drinking in the adolescent and young adult population.

    Abstract: This study explores the association of child sexual abuse (CSA) with subsequent health risk behaviors among a cross-section of 4354 adolescents and young adults surveyed in urban and rural Taipei. Descriptive analysis and logistic regressions were employed. The overall proportion of CSA was 5.15%, with more females (6.14%) than males (4.16%) likely to experience CSA. CSA was differently associated with multiple adverse health outcomes, after adjusting other factors, such as age, residence, economic status, education, employment status, and household instability. Both males and females with CSA experience were more likely to report drinking, gambling, and suicidal ideation compared with those who had no history of CSA. However, the significant association between CSA and smoking, fighting, and suicidal attempt was not observed among females. Effective interventions are needed to reduce CSA and its adverse effects on adolescent well-being.

    Abstract: We assessed the effects of upstream administration of the glycoprotein IIb/IIIa inhibitor tirofiban before percutaneous coronary intervention (PCI) on spontaneous reperfusion (SR) of infarct-related artery (IRA) and the clinical outcomes in patients with ST-segment elevation myocardial infarction (STEMI). The incidence of SR of the IRA was significantly higher in the tirofiban group than in the no-tirofiban group (141 [36.5%] vs 21 [17.2%] , P 〈 .001). By multivariate logistic regression analysis, use of tirofiban (odds ratio 2.32, 95% confidence interval 1.25-4.31, P = .008) independently predicted the occurrence of SR. Kaplan-Meier survival analysis demonstrated that major adverse cardiovascular event-free survival was significantly higher in patients treated with tirofiban than in patients without tirofiban at 30-day (log rank = 11.65, P = .001) and 90-day follow-up (log rank = 16.79, P 〈 .001). Upstream administration of tirofiban is significantly associated with increased SR of the IRA and favorable clinical prognosis in patients undergoing PCI for STEMI.
