    Abstract: Early menopause has been found to be associated with higher risk of cardiovascular disease. Our objective was to investigate the impact of early menopause on clinical outcomes for women undergoing percutaneous coronary intervention (PCI). We observed female patients with coronary artery disease (CAD) undergoing PCI and found that women with early menopause (≤46 years old) were more likely to have CAD risk factors and more severe coronary lesions. During the 18-month follow-up, early menopause was associated with similar risk of death and myocardial infarction but higher risk of target lesion revascularization (TLR; 7.8% vs 5.3%, P = .003) and major adverse cardiovascular events (MACEs; 11.3% vs 9.0%, P = .007). After adjustment, early menopause was an independent risk factor for 18-month MACEs (hazard ratio [HR], 1.54; 95% confidence interval [CI] 1.18-2.00) and TLR (HR 1.61; 95% CI 1.21-2.13). In conclusion, for women undergoing PCI, early menopause is associated with higher risk of MACE, which is mainly driven by risk of TLR.

    Abstract: We searched PubMed, EMBASE, Cochrane Library, and Web of Science for studies using fractional flow reserve (FFR) to determine whether revascularization should be performed or deferred for patients with coronary stenosis and grey zone FFR. Meta-analysis was performed using the generic inverse variance method, and hazard ratios (HR) were synthesized with a random-effects model. Of 2766 records, 7 nonrandomized studies including 2683 patients were selected. The pooled results demonstrated, during a median follow-up of 32 months, that revascularization significantly reduced the risk of major adverse cardiac events (MACE; 7 studies: HR [95% confidence interval, CI]: 0.65 [0.45-0.93] , P = .02) and target vessel revascularization (TVR; 4 studies: HR [95% CI]: 0.52 [0.36-0.76] , P 〈 .01). Whereas revascularization was not significantly superior in terms of all-cause death (3 studies: HR [95% CI]: 0.56 [0.26-1.22] , P = .14), cardiac death (2 studies: HR [95% CI]: 0.57 [0.16-2.01] , P = .38), myocardial infarction (MI; 4 studies: HR [95% CI]: 1.03 [0.26-4.03] ), and all-cause death or MI (3 studies: HR [95% CI]: 0.66 [0.20-2.19] , P = .50). Therefore, revascularization appeared to be superior to deferral for patients with grey zone FFR in MACE and TVR, while hard end points did not show such significance.     Abstract: Although the concept of entropy in landscape evolution was proposed over 40 years ago, previous studies of geomorphic entropy paid little attention to the applications of geomorphic entropy in the erosional watershed geomorphic system on the Loess Plateau in China. Therefore, we propose a new concept of entropy called watershed geomorphic entropy (WGE) and its method of calculation based on a digital elevation model and the principles of system theory. To study the geomorphic significances of WGE, we applied the WGE to an artificial rainfall experiment that was originally designed to study erosional processes in a small open watershed geomorphic system on the Loess Plateau. Our study shows that the decrease of WGE in an open watershed geomorphic system means a gradual erosional or erosion-dominated landscape evolutional process and the change of WGE shows a perfectly positive linear correlation with the measured sediment yields of the outlet of the watershed system under our experimental conditions. In addition, to some extent, the decrease of the change of WGE also reflects the reduction of total potential energy of a specific erosional, or erosion-dominated, open watershed geomorphic system.

    Abstract: Recent studies have shown that C-reactive protein (CRP) may be associated with breast cancer. The purpose of this study is to summarize the predictive role of CRP for survival in breast cancer as shown in all available studies worldwide. Methods Related studies were identified and evaluated for quality through multiple search strategies. Data were collected from studies comparing overall, cancer-specific, and disease-free survival (OS, CSS, and DFS) in patients with elevated CRP levels and those having lower levels. Studies were pooled, and combined hazard ratios (HRs) of CRP for survival were calculated. Results A total of 10 studies (n=4,502) were included for this meta-analysis (9 for OS, 3 for CSS, and 3 for DFS). For overall and disease-free survival, the pooled HRs of CRP were significant at 1.62 (95% confidence interval [95% CI], 1.20-2.18) and 1.81 (95% CI, 1.44-2.26), respectively. For cancer-specific survival, the pooled HR in higher CRP expression in breast cancer was 2.08 (95% CI, 1.48-2.94), which could strongly predict poorer survival in breast cancer. Conclusions CRP has a critical prognostic value in patients with breast cancer as an inflammation biomarker.

    Abstract: The behavior of stair/escalator/elevator selection of passengers at the subway platform could directly affect the travel efficiency and even the service level of the station. This paper proposes a stair/escalator/elevator selection model of passengers at the subway platform, where the cellular automata model is used to simulate passenger movement, and the fuzzy logic theory is used to describe selection behavior. The distance to the stair/escalator/elevator and the density of passengers in front of the facility are selected as the main influence factors. The rationality and effectiveness of the selection model are verified by comparing the field data with simulation results of passengers choosing different stairs/escalators/elevators. The relationship between passenger quantity, passenger speed, escalator quantity, escalator location, and the traffic rate is analyzed based on simulation results, which can provide a theoretical reference for the facility layout optimization at the platform floor of the subway station.

    Abstract: To explore the care experiences of spouses as long-term and primary caregivers for disabled older adults in China. Methods A descriptive phenomenological method was used in this study, as well as purposive and convenient sampling. Semi-structured interviews were conducted with 15 spousal caregivers in Guangdong, China, from March to December 2021. Interview audio-recordings were transcribed verbatim and data were analyzed using Colaizzi's phenomenological analysis method. Results We identified four themes from the data: spousal care motivation; sacrifices in caregiving; obstacles in caregiving; spousal caregivers’ positive experiences. Conclusions Spouses took responsibility for providing care for their disabled partners regardless of their willingness. They had positive experiences while providing care, but negative experiences were dominant, especially for spouses of severely disabled older adults. Spouses are always perfect in caregiving roles, although they may also need medical assistance. To prevent a decline in spousal caregivers’ quality of life and relieve their care burdens, health care providers should support them as soon as possible or offer formal care for disabled older adults. It is necessary to intervene considering disabled older adults and their spousal caregivers as a unit to empower their confidence in coping with life together.

    Abstract: Neoadjuvant radiotherapy plays a vital role in the treatment of malignant bone tumors, and non-invasive imaging methods are needed to evaluate the response to treatment. Purpose To assess the value of diffusion kurtosis imaging (DKI) for monitoring early response to radiotherapy in malignant bone tumors. Material and Methods Treatment response was evaluated in a rabbit VX2 bone tumor model (n = 35) using magnetic resonance imaging (MRI), DKI, and histopathologic examinations. Subjects were divided into three groups: pre-treatment, post-treatment, and control groups. The post-treatment group was subclassified into good response and poor response groups according to the results of histopathologic examination. Apparent diffusion coefficient (ADC) and DKI parameters (mean diffusion coefficient [MD] and mean kurtosis [MK]) were recorded. The relationship between ADC, DKI parameters, and histopathologic changes after radiotherapy was determined using Pearson’s correlation coefficient. The diagnostic performance of these parameters was assessed using receiver operating characteristic analysis. Results MD in the good response group was higher after treatment than before treatment ( P  〈  0.001) and higher than that in the poor response group ( P = 0.009). MD was highly correlated with tumor cell density and apoptosis rate (r = −0.771, P  〈  0.001 and r = 0.625, P  〈  0.001, respectively). MD was superior to other parameters for determining the curative effect of radiotherapy, with a sensitivity of 75.0%, specificity of 100.0%, and area under the curve of 0.917 ( P  〈  0.001). Conclusion The correlations between MD, tumor cell density, and apoptosis suggest that MD could be useful for assessing the early response to radiotherapy in rabbit VX2 malignant bone tumors.

    Abstract: The transradial approach (TRA) is commonly applied for coronary catheterization. However, there are few reports on the safety and feasibility of transradial catheterization in patients with prior coronary artery bypass graft (CABG) surgery. We retrospectively evaluated 124 consecutive patients who underwent graft angiography and intervention via the transradial (TRA group, n = 68) or transfemoral approach (TFA group, n = 56). The baseline clinical characteristics between the 2 groups were similar except for prior myocardial infarction. No significant difference ( P 〉 .05)was observed in procedure time, the success rate of puncture, angiography, and intervention procedure between the 2 groups. There was no significant difference in major adverse cardiac and cerebrovascular events during hospitalization. However, the vascular access site complications were significantly lower ( P = .021) and the duration of hospitalization was shorter ( P = .007) in the TRA group. The TRA for coronary bypass graft angiography and intervention was safe and feasible.

    Abstract: Recent evidence has shown that the complete blood count (CBC) is abnormal in patients with Crohn’s disease (CD). We aimed to investigate an effective CBC parameter and explore its impact on disease activity in a large CD cohort. Methods: We performed a retrospective analysis of patients with established CD who underwent clinically indicated endoscopy at four tertiary centres in China between 2016 and 2020. Individual variables of the Simple Endoscopic Score for CD, CBC parameters, C-reactive protein (CRP) levels, erythrocyte sedimentation rate, and faecal calprotectin (FC) were independently reviewed by different investigators. The hold-out method was used to verify the predictive power of the established model. Results: Data from a total of 1388 endoscopic procedures performed for 882 eligible CD patients were available with routine blood parameters and related indicators. The model using platelet-to-lymphocyte percentage ratio (PLpR) had high accuracy for identifying patients in endoscopic remission (ER), with an area under the curve (AUC) of 0.785 [95% confidence interval (CI): 0.784–0.787], which was comparable with that for CRP (AUC: 0.775, 95% CI: 0.774–0.777). Notably, the AUC of PLpR was significantly higher than that of CRP in patients with colonic disease and with a history of surgery. Moreover, after combining the FC with PLpR, the AUC value of FC + PLpR increased up to 0.892 (95% CI: 0.890–0.894) for identifying ER. Conclusions: We explored an index (PLpR) to identify CD patients in ER based on platelet and lymphocyte percentage from the CBC. PLpR helped evaluate the degree of disease activity and monitor the therapeutic response.

    Abstract: Relatively small radial artery may be challenging for cannulation. We investigated whether a distal tourniquet would inflate the proximal radial artery and therefore facilitate cannulation in adults. Methods There were two stages of the study. The first was to measure the characteristics of radial artery by ultrasound imaging before and after tourniquet in volunteers. The second was a prospective, randomized, double blind study. Forty patients (American Society of Anesthesiologists I-III) who needed artery cannulation during operation were enrolled. Patients were assigned into two groups: loosen or tightened tourniquet for proximal radial artery cannulation by traditional palpation technique. The primary endpoints were the success rates and time of first attempt success by traditional palpation technique. The time of success after two to three attempts, failure rates and complications were collected. Results For volunteers, the distal tourniquet significantly expanded the cross-sectional up-forward diameter (p 〈 0.001) and the area (p 〈 0.05), but had no effect on cross-sectional lateral-lateral diameter or circumference of proximal radial artery (p 〉 0.05). The success rate of first attempt was higher in the tighten group (75%) than in the loosen group (15%, p 〈 0.05), but the time for success of first attempt/two to three attempts was similar (19.33 ± 1.12/62.11 ± 37.03 sec loosen group vs. 19.07 ± 12.75/45.55 ± 8.98 sec tightened group, p 〉 0.05). Both groups had same failure rates of 10%. No complication was observed. Conclusions Distal tourniquet could inflate the proximal radial artery and facilitate palpation of radial artery cannulation.
