    Abstract: In order to study the stability of power generation in reverse of a pumping station unit, the computational fluid dynamics simulation and finite element analysis of the whole passage has been carried out, to study the pressure pulsation and stress distribution law under the condition of power generation in reverse, and compared with them in pump mode. The simulation results showed that, under the condition of power generation in reverse, the pressure pulsation of each monitoring point in front and at back of the runner were higher than that under pump mode. The most amplitude of pressure pulsation was about twice as large as the pump mode, increased gradually from hub to rim. The flow was seriously affected by the rotating runner. The internal frequency of pressure pulsation was blade frequency. The results of fluid–solid coupling showed that the stress mainly concentrated on the root of the pressure side and the suction side of the blade, and the maximum equal stress appeared on the suction side of the blade. The maximum stress and strain value under the condition of power generation in reverse were 20% higher than that under pump mode, and the maximum stress was about twice.

    Abstract: Conventional cytotoxic chemotherapy offers minor benefit to patients with mucosal melanoma (MM). Although immune checkpoint inhibitors (ICIs) have become the preferred approach in patients with advanced or metastatic cutaneous melanoma, the evidence of their clinical use for MM is still limited. This systematic review aims to summarize the efficacy and safety of ICIs in advanced or metastatic MM. Methods: We searched electronic databases, conference abstracts, clinical trial registers and reference lists for relevant studies. The primary outcomes included the overall response rate (ORR), median progression-free survival (PFS), median overall survival (OS), one-year PFS rate, and one-year OS rate. Results: This review identified 13 studies assessing anti-CTLA-4 monotherapy, 22 studies assessing anti-PD-1 monotherapy, two studies assessing anti-CTLA-4 and anti-PD-1 combination therapy, one study assessing anti-PD-1 antibodies combined with axitinib, and three studies assessing anti-PD-1 antibodies combined with radiotherapy. For most patients who received ipilimumab monotherapy, the ORR ranged from 0% to 17%, the median PFS was less than 5 months, and the median OS was less than 10 months. For patients who received nivolumab or pembrolizumab monotherapy, most studies showed an ORR of more than 15% and a median OS of more than 11 months. The combined administration of anti-CTLA-4 and anti-PD-1 agents showed benefits over single-agent therapy with an ORR of more than 33.3%. In a phase Ib trial of toripalimab in combination with axitinib, approximately half of patients had complete or partial responses. Three retrospective studies that investigated anti-PD-1 antibodies combined with radiotherapy showed an ORR of more than 50%, which was higher than each single modality treatment. Conclusions: Immune checkpoint inhibitors, especially anti-PD-1 monoclonal antibodies alone and in combination with anti-CTLA-4 monoclonal antibodies or other modalities, are promising treatment options for advanced or metastatic MM. However, high-level evidence is still needed to support the clinical application.

    Abstract: The compressive deformation and load of a spacer filament in a warp-knitted spacer fabric was analyzed theoretically in this work. It was found that the compression process of the spacer filament can be divided into four stages, including a Stiff Stage, an Elastic Stage, a Restful Stage, and an Ineffective Stage. Based on the theoretical analysis of the compressive displacement and the radius of the spacer filament at different stages, as well as the bending moment of the spacer filament during compression, the relationship between the critical compressive load at different stages and the structural parameters of the spacer filament in a warp-knitted spacer fabric was found. Furthermore, plane plate compression experiments for spacer filaments were designed and tested to verify the correctness of the theoretical analysis.

    Abstract: Background and Objective: Chuanhong Stroke Capsule (CHSC) has good clinical efficacy in the treatment of cerebral ischemic stroke (CIS) patients. This study aimed to investigate the pharmacological mechanisms of CHSC in treating CIS using bioinformatics. Methods: The active compounds of CHSC were screened by searching Traditional Chinese Medicine System Pharmacological Database and Analysis Platform (TCMSP), Swiss absorption, distribution, metabolism, and excretion (ADME), PubMed, and China National Knowledge Infrastructure (CNKI) databases. Besides, the potential targets of active compounds were obtained through TCMSP and Swiss Target Prediction databases. CIS targets were obtained from GeneCards, Online Mendelian Inheritance in Man (OMIM), and Gene Expression Omnibus (GEO) databases. CHSC-CIS intersection targets were identified by matching the two, and prediction and analysis of biological functions and pathways of intersection targets was used the enrichments of gene ontology (GO) and Kyoto Encyclopedia of Genes and Genomes (KEGG). Furthermore, protein–protein interaction (PPI) network, herb-target, and compound-target network of CHSC-CIS were constructed by Cytoscape3.7.2, and herb-compound-pathway network was drawn with Sankey diagram. Finally, AutoDock was used for molecular docking verification, and identifying the active binding sites in target proteins. Results: A total of 293 putative targets were obtained from 62 active compounds in CHSC. Among them, 209 targets were related to CIS. PPI network showed that the top 16 key targets were RELA, JUN, FOS, MAPK1, AKT1, etc. KEGG pathway enrichment analysis demonstrated that CHSC was enriched in PI3K-Akt, MAPK, and TNF signaling pathways. In addition, GO enrichment analysis showed the significant enrichment of CHSC in the following categories: kinase binding, cellular response to nitrogen compound, etc. Network topology analysis showed that quercetin, luteolin, kaempferol, etc., were the key components in CHSC. Finally, molecular docking studies suggested that the active components in CHSC had a good binding ability with the key targets. Conclusions: Our study demonstrated that CHSC exerted the effect in treating CIS by the characteristics of multi-target and multi-pathway, thereby providing a theoretical basis for further study of the effective components and mechanism of CHSC in the treatment of CIS.

    Abstract: This study was designed to investigate (1) the contour of the distal tibial cutting surface, and (2) the bone mineral density (BMD) of the distal tibial cutting surface used during total ankle arthroplasty (TAA). Methods: Eight-four distal tibial models were created using foot and ankle computerized tomographic (CT) images taken from normal people. The distal tibial cutting surface for TAA was determined to be 10 mm proximal to the tibial plafond. The bony contour and BMD values were determined from the CT image at that level. A bounding box was made on the contour and the width and length of the contour was measured. Regional BMD was evaluated by Hounsfield units (HU) value measurement, with 7 regions of interest (ROI) on 8 different directions for all the 84 CT images. Two different observers made independent measurements and mean HU values for all the 56 ROIs were calculated. Results: Great variations were found among the contours of the cutting surface especially in term of the shape of the anterior and posterior tibial tubercle, and the fibular notch. These variations could be grouped into six categories. For the BMD of the cutting surface, the medial border of the cutting surface did not included cortical bone. The HU value of seven ROIs, which included cortical bone, were significantly greater than all the other ROIs. Few statistical differences were found by multiple comparisons among HU value of all the 49 ROIs without cortex. Conclusions: Great variability existed in the shape and the BMD of the distal tibial cutting surface.

    Abstract: The study is to explore the toxicity of organic extracts and water-soluble fraction of fine particles on human umbilical vein endothelial cells (HUVECs). The exposure doses were 100, 200 and 400 μg/ml, respectively, for two kinds of fractions. Moreover, atorvastatin was used for intervention study. HUVECs were stimulated by 400 μg/ml organic and water soluble extracts, respectively, immediately followed by treatment with atorvastatin in concentrations of 0.1 μmol/L, 1 μmol/L and 10 μmol/L, respectively. Cell viability, malondialdehyde (MDA), nitric oxide (NO), superoxide dismutase (SOD), reactive oxygen species (ROS) and the expression of interleukin-6 beta (IL-6), tumor necrosis factor-α (TNF-α), endothelin-1 and P-selectin were determined in cells. The results showed that MDA and ROS increased in HUVECs after exposed to organic extracts and water-soluble fraction, whereas cell viability, NO and SOD decreased. The mRNA expression of IL-6, TNF-α, endothelin-1 (ET-1) and P-selectin increased after exposed to different fractions. Meanwhile, at the same exposure dose, water-soluble fraction caused more significant increase of MDA, IL-6, TNF-α and P-selectin and decrease of cell viability and NO when compared to organic extracts. Compared to no atorvastatin group, the levels of MDA, ROS and the expression of IL-6, TNF-α, ET-1 and P-selectin decreased in HUVECs in adding atorvastatin group, but cell viability, NO and SOD increased, which indicated that atorvastatin attenuated fine particle-induced inflammatory response, oxidative stress and endothelial damage. The results hinted that the inflammatory response, oxidative stress and endothelial dysfunction might be the mechanisms of cardiovascular injury induced by different fractions of ambient fine particles.

    Abstract: A retrospective multivariate analysis. Objective.: To analyze clinical outcomes of surgical treatment and prognostic factors of local failure after stereotactic body radiation therapy (SBRT) in patients with spinal metastatic non–small-cell lung cancer (NSCLC). Methods.: This study included patients with metastatic spinal cord compression (MSCC) from spinal NSCLC after radiotherapy who received massive spondylectomy for circumferential decompression of spinal cord and reconstruction of spinal stability in our center between May 2006 and February 2017. Neurological function was evaluated using the Frankel score. Overall survival (OS) was estimated by the Kaplan-Meier method. Factors with Pvalues ≤.1 were subjected to multivariate analysis for OS by proportional hazard analysis. Values of P 〈 .05 were considered statistically significant. Results.: The mean age of the 55 included patients (36 male and 19 female) was 57.76 ± 8.94 (median 58, range 36-77) years, with a mean postoperative OS of 14.98 ± 14.81 (median 10.0, range 1-84) months. Neurological function was improved in 46 (83.6%) of the 55 patients after surgery. Prognostic analysis suggested that preoperative frankel score (FS) score, visceral metastasis, D-dimer (D-D) level, and neutrophil/lymphocyte ratio (NLR) were independent prognostic factors for selected patients. Conclusions.: Massive spondylectomy could provide circumferential decompression and improve the neurological function of patients with MSCC from spinal NSCLC after radiotherapy. A preoperative FS score of C/D, no visceral metastasis, D-D 〈 1000 μg/L, and NLR 〈 5 are predictors of better prognosis.

    Abstract: Because of the highly nonlinear and dynamic spatial–temporal correlation of traffic flow, timely and accurate forecasting is very challenging. Existing methods usually use a static adjacency matrix to represent the spatial relationships between different road segments, even though the spatial relationships can change dynamically. In addition, many methods also ignore the dynamic time-dependent relationships between traffic flows. To this end, we propose a new network model to model the spatial–temporal correlation of traffic flow dynamics. Specifically, we design a dynamic graph construction method, which can generate dynamic graphs based on data to represent dynamic spatial relationships between road segments. Then, a dynamic graph convolutional network is proposed to extract dynamic spatial features. We further propose a multi-head temporal attention mechanism to learn the dynamic temporal dependencies between different times and then use temporal convolutional networks to extract the dynamic temporal features. The experimental results on real data show that the model proposed in this paper has a better prediction performance than existing models.

    Abstract: The aim of the present study is to investigate the efficacy of internet-based cognitive behavior therapy (ICBT) for individuals with social anxiety (SA) and different levels of Taijin Kyofusho (TKS) in China. The ICBT program was translated into Chinese with some specific contents adapted for Chinese culture. Participants ( N = 80) with SA were assigned either to a treatment ( n = 55) or control group ( n = 25). Both groups were further divided into subgroups, based on their Taijin Kyofusho Scale (TKSS) scores. Participants in the ICBT treatment group reported significant posttreatment reductions in Social Interaction Anxiety Scale and Social Phobia Scale scores, relative to participants in the control group. In addition, participants in the treatment group with higher pretreatment TKS levels showed significantly greater reductions in TKSS scores. Results suggest that ICBT is a promising approach for the treatment of individuals with SA both with and without features of TKS. Clinical and cross-cultural implications, mechanisms of change, limitations, and future directions are discussed.

    Abstract: A one-dimensional ZnO nanostructure is a versatile and multifunctional n-type semiconductor. In this paper, ZnO nanorods were successfully prepared by the anodic arc plasma method in an oxidizing atmosphere. Methods: The composition, morphology, crystal microstructure, and optical properties of ZnO nanorods were characterized by using X-ray diffraction (XRD), high-resolution transmission electron microscopy (HRTEM) and the corresponding selected-area electron diffraction (SAED), X-ray energy dispersive spectrometry (XEDS), ultraviolet-visible (UV-VIS) spectroscopy, Raman scattering spectrum (Raman), and photoluminescence spectrum (PL). Results: The experiment results show that ZnO nanorods synthesized by this method possess hexagonal wurtzite crystal structure with good crystallization, no other impurity phases are observed, the crystalline size is about 18 nm, and the lattice constant distortion occurs compared to that of bulk ZnO. The morphology of the sample is a rod-like shape, the length ranges from 100 nm to 300 nm, the average diameter is approximately 20 nm, and the aspect ratio is relatively high. The UV-VIS absorption spectrum occurs red shift, The Raman spectrum further demonstrates that the major peaks are assigned to ZnO optical vibrational modes, and the PL spectrum exhibits coexistence properties of ultraviolet (UV) and green emission. Conclusions: The results prove that ZnO nanorods with hexagonal wurtzite crystal structure were successfully prepared by the anodic arc plasma method in an oxidizing atmosphere.
