    Abstract: Our present study intended to examine the associations of RPEL1 and miR-1307 gene polymorphisms (rs4917385 and rs7911488) with susceptibility, glucocorticoids (GCs) efficacy, anxiety, depression, and health-related quality of life (HRQoL) in Chinese systemic lupus erythematosus (SLE) patients. Methods Initially, 1000 participants (500 SLE cases and 500 controls) were recruited for the case–control study. Then, 429 cases who received GCs were followed through 12 weeks to explore GCs efficacy, depression, anxiety, and HRQoL. We selected the iMLDR technique for genotyping: RPEL1: rs4917385 (G/T) and miR-1307: rs7911488 (A/G). Results The minor G allele of rs7911488 reduced the risk of SLE ( p = .024). Four haplotypes consisting of rs4917385 and rs7911488 were associated with SLE susceptibility ( p 〈 .025). Both rs4917385 and rs7911488 were associated with anxiety symptoms and physical function (PF) in SLE patients ( p 〈 .025). The rs4917385 was associated with depression and its improvement. No statistical significance was found between RPEL1 and miR-1307 gene polymorphisms with GCs efficacy. Meanwhile, additive interaction analysis showed a significant association between RPEL1 and miR-1307 gene polymorphisms with tea consumption in anxiety. Conclusion RPEL1 and miR-1307 gene polymorphisms (rs4917385 and rs7911488) might be related to SLE susceptibility in Chinese population. Additionally, the two polymorphisms were possibly associated with depression, anxiety, and HRQoL in Chinese SLE population.

    Abstract: Hepatic hemangioma is a frequent nonmalignant tumor in the human liver. Although rupture of hepatic hemangioma is a rare complication, it may have serious consequences. In this report, we describe a 56-year-old woman who underwent laparoscopic surgery and open surgery for sudden abdominal pain and peritonitis. Gastrointestinal perforation was prioritized before surgery according to the patient’s symptoms, signs, and radiological report. A giant dark red mass connected to the left liver by a pedicle was unexpectedly found during intraoperative exploration. Hemoperitoneum was also found. Conversely, no gastrointestinal perforation was found during intraoperative exploration. The mass was successfully removed, and the hemoperitoneum was resolved. Postoperative pathological examination showed that the mass was a hepatic hemangioma. Ultimately, the patient was diagnosed with spontaneous rupture of a giant hepatic hemangioma.

    Abstract: Asian American women face disproportionate burden of cervical cancer (CC) than non-Hispanic white women in the U.S. The goal of this study was to assess the feasibility and impact of a culturally tailored intervention to promote Human papillomavirus (HPV) self-sampling test among hard-to-reach Asian American women. Methods We adopted the community-based participatory research (CBPR) approach to conduct this efficacy study. A total of 156 female participants (56 Chinese, 50 Korean, and 50 Vietnamese) were recruited from community-based organizations (CBOs) in the greater Philadelphia metropolitan area. The intervention components included HPV-related education, HPV self-sampling test kit and instructions, group discussions, and patient navigations, all available in Asian languages. We examined several outcomes, including the completion of HPV self-sampling, HPV-related knowledge, perceived social support, self-efficacy, and comfort with the self-sampling test at post-intervention assessment. Results The majority of Asian American women had low annual household income (62.3% earned less than $20,000) and low educational attainment (61.3% without a college degree). We found significant increase in participants’ knowledge on HPV (baseline: 2.83, post: 4.89, P 〈 .001), social support (baseline: 3.91, post: 4.09, P 〈 .001), self-efficacy (baseline: 3.05, post: 3.59, P 〈 .001), and comfortable with HPV self-sample test (baseline: 3.62, post: 4.06, P 〈 .001). Conclusion To the best of our knowledge, this is the first intervention study that promoted HPV self-sampling test among Asian American women. Our findings showed that CBPR culturally tailored intervention of self-sampling was highly effective in empowering low-income Asian American women to conduct HPV self-sampling tests.

    Abstract: We aimed to examine the longitudinal association between frailty and cognitive impairment in the older Chinese population. This prospective cohort study used data from the Chinese Longitudinal Healthy Longevity Study 2011 wave. We calculated the follow-up duration as 3 years from the baseline year. Frailty was measured using the frailty index, and cognitive function was calculated by Mini-Mental State Examination Scale. Participants who were non-frailty and those with normal cognitive function were included in 2011 and followed up in 2014, respectively. Frailty was an independent risk factor for early-onset cognitive impairment. Age, hearing impairment, and a decreased ability to perform daily activities were the main risk factors for cognitive impairment, while affluent economic status was a protective factor. Cognitive impairment was not found to be an independent risk factor for frailty. We concluded that the frailty index is a significant predictor of cognitive impairment among community-dwelling older adults.

    Abstract: Redo coronary artery bypass grafting (redo CABG) is associated with increased mortality and morbidity. The aim of this study was to systematically evaluate the evidence comparing the outcomes of off-pump with on-pump redo CABG. Methods: Studies were systematically searched and identified using PubMed, EMBASE, the Cochrane Library, and the International Clinical Trials Registry Platform (ICTRP) by two researchers independently. The primary outcome was 30-day mortality, and the secondary outcomes were in-hospital mortality, post-operative complications, completeness of revascularization, blood transfusion rate, duration of mechanical ventilation, intensive care unit and hospital stays. Results: The 21 studies including 4,889 patients were enrolled in our meta-analysis. Compared with on-pump, the off-pump technique was associated with significantly reduced 30-day mortality (odds ratio [OR] = 0.43, 95% confidence interval [CI]  = 0.26-0.72, p = 0.001). Moreover, a notably decreased in-hospital mortality (OR = 0.55, 95% CI = 0.39-0.76, p = 0.0004) and incidence of post-operative new-onset atrial fibrillation, myocardial infarction, acute kidney injury, low cardiac output state, blood transfusion rate (OR = 0.46, 95% CI = 0.35-0.60, p  〈  0.00001; OR = 0.54, 95% CI = 0.38-0.78, p = 0.0007; OR = 0.51, 95% CI = 0.37-0.70, p  〈  0.0001; OR = 0.31, 95% CI = 0.20-0.47, p  〈  0.00001; OR = 0.29, 95% CI = 0.14-0.61, p = 0.001) and significantly shortened duration of mechanical ventilation, intensive care unit and hospital stays (mean difference [MD] = −8.21 h, 95% CI = −11.74 to −4.68, p  〈  0.00001; MD = −0.77 d, 95% CI = −0.81 to −0.73, p  〈  0.00001; MD = −2.24 d, 95% CI = −3.17 to −1.32, p  〈  0.00001) could be observed when comparing the outcomes of off-pump with on-pump redo CABG. There was nonsignificant difference between off-pump and on-pump redo CABG in completeness of revascularization. Conclusion: In patients undergoing redo CABG surgery, the off-pump technique was associated with decreased mortality, less post-operative complications when compared to on-pump.

    Abstract: To bring forward an arthroscopic classification of the popliteomeniscal fascicles of the lateral meniscus (PFLM) tears. Design Six fresh frozen knee joint samples of adult males were chosen, and the lateral meniscus at the popliteal hiatus region were measured to analyze their anatomic relationship. Patients who had received magnetic resonance imaging scan at knee joint before the surgery and diagnosed as PFLM tears by arthroscopy from April 2014 to October 2017 were selected. Data regarding the integrity of PFLM were prospectively recorded in a data registry. Tear morphology and treatment received were subsequently extracted by 2 independent reviewers from operative notes and arthroscopic surgical photos. Results The average length and thickness of the popliteal hiatus of the lateral meniscus were 2.09 ± 0.21 cm and 0.43 ± 0.08 cm, respectively. The average length of anterosuperior popliteomeniscal fascicle (APF) was 0.87 ± 0.18 cm, and the posterosuperior popliteomeniscal fascicle (PPF) was 0.72 ± 0.15 cm. A total of 36 PFLM tears in 36 patients were divided as type I (APF tear; n = 5, 13.9%), type II (PPF tear; n = 20, 55.6%), and type III (both APF and PPF tears; n = 11, 30.6%). All patients were treated with arthroscopic all-inside repair using a suture hook for the PFLM tears and follow-up for 21.1 months. All patients have done well with significantly improved Lysholm and International Knee Documentation Committee scores at the last follow-up relative to preoperative scores ( P 〈 0.01). Conclusion This study suggests to possibly classify the PFLM tears for clinical practice.

    Abstract: Dietary restriction (DR) protects against neuronal dysfunction and degeneration, and reduces the risk of ischemic stroke. This study examined the role of silent information regulator T1 (SIRT1) and arterial baroreflex in the beneficial effects of DR against stroke, using two distinct stroke models: stroke-prone spontaneously hypertensive rats (SP-SHRs) and Sprague-Dawley (SD) rats with middle cerebral artery occlusion (MCAO). Sirt1 knockout (KO) mice were used to examine the involvement of sirt1. Sinoaortic denervation was used to inactivate arterial baroreflex. Dietary restriction was defined as 40% reduction of dietary intake. Briefly, DR prolonged the life span of SP-SHRs and reduced the infarct size induced by MCAO. Dietary restriction also improved the function arterial baroreflex, decreased the release of proinflammatory cytokines, and reduced end-organ damage. The beneficial effect of DR on stroke was markedly attenuated by blunting arterial baroreflex. Lastly, the infarct area in sirt1 KO mice was significantly larger than in the wild-type mice. However, the beneficial effect of DR against ischemic injury was still apparent in sirt1 KO mice. Accordingly, arterial baroreflex, but not sirt1, is important in the protective effect of DR against stroke.

    Abstract: Electrospun nanofibrous scaffolds have gained extensive attention in the fields of soft tissue engineering and regenerative medicine. In this study, a series of biodegradable nanofibrous meshes were fabricated by electrospinning poly(ε-caprolactone) (PCL) and poly( p-dioxanone) (PPDO) blends with various mass ratios. All the as-developed PCL/PPDO nanofibrous meshes possessed smooth and highly aligned fiber morphology. The mean fiber diameter was 521.5 ± 76.6 nm for PCL meshes and 485.8 ± 88.9 nm for PPDO meshes, and the mean fiber diameter seemed to present a decreasing tendency with the increasing of the PPDO component. For pure PCL meshes, the contact angle was about 117.5 ± 1.6°, the weight loss ratio was roughly 0.2% after 10 weeks of degradation, and the tensile strength was 41.2 ± 2.3 MPa in the longitudinal direction and 4.2 ± 0.1 MPa in the transverse direction. It was found that the surface hydrophilicity and in vitro degradation properties of PCL/PPDO meshes apparently increased, but the mechanical properties of PCL/PPDO meshes obviously decreased when more PPDO component was introduced. The biological tests showed that 4:1 PCL/PPDO nanofibrous meshes and 1:1 PCL/PPDO nanofibrous meshes could obviously promote the adhesion and proliferation of human adipose derived mesenchymal stem cells more than pure PCL and PPDO meshes and 1:4 PCL/PPDO meshes. The results demonstrated that it is feasible to adjust the surface hydrophilicity, degradation profile, and mechanical properties as well as biological properties of as-obtained nanofibrous meshes by blending PCL and PPDO components. This study provides meaningful reference and guidance for the design and development of PCL/PPDO hybrid nanofibrous scaffolds for soft tissue engineering research and application.

    Abstract: A novel localization approach that seamlessly bridges the macro- and micro-scale models is proposed and used to model the forming-induced residual stresses within a representative volume element of a fiber reinforced composite. The approach uses a prescribed boundary that is theoretically deduced by integrating the asymptotic expansion of a composite and the equal strain transfer, thus rendering the simulation setting to be easier than conventional approaches. When the localization approach is used for the finite element analysis, the temperature and residual stresses within an ideal cubic representative volume element are precisely simulated, given a sandwiched thermoplastic composite is formed under one-side cooling condition. The simulation results, after being validated, show that the temperature gradient has an impact on the local residual stresses, especially on the in-plane normal stress transverse to the fiber, and consequently, influences the structural deformation. This newly designed localization approach demonstrates the advantages of enhanced precision and reduced computational cost owing to the fast modeling of the finely meshed representative volume element. This is beneficial for a detailed understanding of the actual residual stresses at the micro-scale.
