    Abstract: A new lignan, arborlignan A, along with five known substances, neoechinulin A, 4-hydroxy-3,5-dimethoxybenzaldehyde, 3,3′-dimethoxy-4,4′-dihydroxystilbene, β-hydroxypropiovanillone, and coniferyl aldehyde, are isolated from Schefflera arboricola. Their structures are elucidated by mass spectrometry and nuclear magnetic resonance spectroscopy experiments and by comparison with literature data. These compounds are isolated from the genus Schefflera for the first time.

    Abstract: This study investigates the molecular mechanisms by which Alpiniae oxyphyllae fructus (AOF) promotes neuron regeneration. Methods A piece of silicone rubber was guided across a 15 mm gap in the sciatic nerve of a rat. This nerve gap was then filled with different concentrations of AOF extract (0-200 mg/ml). We investigated the role of MAPK (ERK1/2, JNK and p38) pathways for AOF-induced matrix-degrading proteolytic enzyme (PAs and MMP2/9) production in RSC96 Schwann cells. Results The results showed that AOF increased the expressions of uPA, tPA, MMP-9, and MAPKs in vivo. In vitro, our results show that treatment with AOF extract induces ERK1/2, JNK, and p38 phosphorylation to activate the downstream PAs and MMPs signaling expression. AOF-stimulated ERK1/2, JNK, and p38 phosphorylation attenuated by individual pretreatment with siRNAs or inhibitors (U0126, SP600125 and SB203580), resulting in migration and uPA-related signal pathway inhibition. Conclusions Taken together our data suggests the MAPKs (ERK1/2, JNK and p38), PAs (uPA, tPA), MMP (MMP2, MMP9) regenerative and migration signaling pathway of Schwann cells regulated by AOF extract might play a major role in Schwann cell migration and damaged peripheral nerve regeneration.

    Abstract: Hemorrhage is the leading cause of death in trauma settings. Non-compressible torso hemorrhage, which is caused by abdominopelvic and thoracic injuries, is an important cause of subsequent organ dysfunction and poor outcomes in multiple trauma patients. The management of hemodynamically unstable patients with non-compressible torso hemorrhage has changed, and the concept of damage control resuscitation has been developed in the last decades. Currently, resuscitative endovascular balloon occlusion of the aorta (REBOA) as a method of temporary stabilization is the modern evolution of bleeding control, and it is in the middle of a paradigm shift as a treatment for non-compressible torso hemorrhage. Despite its effectiveness in patients with hemorrhagic shock, the application of REBOA remains limited because of lack of experience and troubleshooting guidelines. Objectives: The aim of study was to provide useful tips for the implementing a step-by-step procedure for REBOA in various hospital settings and capabilities. Methods: We introduced REBOA procedures using a REBOA-customized 7 Fr balloon catheter through the animation models or radiography from preparation to access, catheter management, and device removal after procedure completed. Results: We have described REBOA procedures as follows: identification of the common femoral artery, arterial access for placement of a guidewire, precautions during a sheath insertion, guidewire and balloon positioning in the aorta, occlusion zones and adjustment of balloon location, REBOA strategy for extending the occlusion time, balloon deflation and removal, sheath removal, and medical records. Conclusion: We believe that the practical tips mentioned in this article will help in performing the REBOA procedure systematically and developing an effective REBOA framework.

    Abstract: Bronchiectasis is associated with an increased incidence of atherosclerotic cardiovascular disease (ASCaVD) and atherosclerotic cerebrovascular disease (ASCeVD). Its effect on associated mortality is unclear. Objectives: This study investigated the effects of bronchiectasis exacerbation prior to ASCaVD or ASCeVD events on mortality in patients with bronchiectasis using a large population–based database. Methods: A retrospective cohort of patients with bronchiectasis who experienced ASCaVD ( n = 1066) or ASCeVD ( n = 825) was studied for the first time using a nationwide population–based database (National Health Insurance Service-National Sample Cohort, Korea, 2002–2015). We classified each cohort according to the presence of moderate bronchiectasis exacerbation within 1 year before the ASCaVD or ASCeVD event. We evaluated 90-day, 1-year, and all-cause mortalities risk. Results: Within 1 year before the index ASCaVD or ASCeVD event, 149 (13.9%) and 112 (13.6%) patients with bronchiectasis experienced moderate exacerbation(s), respectively. Mild exacerbations did not different in frequency between the survivors and nonsurvivors. In both cohorts, more nonsurvivors experienced moderate exacerbations than survivors. The odds ratios of 90-day and 1-year mortalities and hazard ratios of all-cause mortalities on experiencing moderate exacerbations were 2.27 [95% confidence interval (CI) = 1.26–4.10], 3.30 (95% CI = 2.03–5.38), and 1.78 (95% CI = 1.35–2.34) in the bronchiectasis-ASCaVD cohort and 1.73 (95% CI = 0.94–3.19), 1.79 (95% CI = 1.07–3.00), and 1.47 (95% CI = 1.10–1.95), in the bronchiectasis-ASCeVD cohort. Conclusion: Hospitalization or emergency room visit for bronchiectasis exacerbation within 1 year before ASCaVD or ASCeVD is associated with an increased ASCaVD- or ASCeVD-associated mortality.

    Abstract: We previously reported that endoplasmic reticulum (ER) stress-mediated apoptosis participated in vascular calcification. Importantly, a novel paracrine/autocrine peptide intermedin 1–53 (IMD 1–53 ) in the vasculature inhibited vascular calcification in rats. But the mechanisms needed to be fully elucidated. Vascular smooth muscle cells (VSMCs) calcification was induced by CaCl 2 and β-glycerophosphate. Tunicamycin (Tm) or dithiothreitol (DTT) was used to induce ER stress. We found that IMD 1–53 (10 −7  mol/L) treatment significantly alleviated the protein expression of ER stress hallmarks activating transcription factor 4 (ATF4), ATF6, glucose-regulated protein 78 (GRP78) and GRP94 induced by Tm or DTT. ER stress occurred in early and late calcification of VSMCs but was inhibited by IMD 1–53 . These inhibitory effects of IMD 1–53 were abolished by treatment with the protein kinase A (PKA) inhibitor H89. Pretreatment with IMD 1–53 decreased the number of apoptotic VSMCs and downregulated protein expression of cleaved caspase 12 and C/EBP homologous protein (CHOP) in calcified VSMCs. Concurrently, IMD 1–53 restored the loss of VSMC lineage markers and ameliorated calcium deposition and alkaline phosphatase activity in calcified VSMCs as well. The observation was further verified by Alizarin Red S staining, which showed that IMD 1–53 reduced positive red nodules among calcified VSMCs. In conclusion, IMD 1–53 attenuated VSMC calcification by inhibiting ER stress through cAMP/PKA signalling.

    Abstract: No.253 lymph node is the gateway to systemic metastasis for left-sided colorectal cancer. However, the value of D3 resection is still controversial. This study aimed to identify the incidence rate and prognostic value of 253LN metastasis in patients with left-sided colorectal cancer liver metastasis (CRLM) mainly through blood vessels and thus to provide theoretical basis for 253LN resection. Methods: From February 2012 to February 2019, a total of 281 patients who underwent curative resection for both primary and metastatic tumors were collected retrospectively. The clinicopathological and genetic characteristics were compared between 58 patients with positive 253LN and 223 patients with negative. Relapse-free survival (RFS) and overall survival (OS) were compared with Kaplan–Meier method. Cox regression analysis and a forest plot were conducted for RFS. Results: The incidence of 253LN metastasis in left-sided CRLM was 20.64% (58/281). Those with 253LN positive were T4 stage, N2 stage, and D1/D2 lymph nodes metastatic. About 10.3% (8/78) 253LN positive patients were D1/D2 negative. The 253LN metastasis was an independent risk factor for relapse after curative surgery, but not for OS. Patients with 253LN metastasis had worse RFS, especially in female, adenocarcinoma, poorly differentiated, pT3, preoperative serum CA199  〈  37 U/mL, bilobar liver metastasis, without preoperative chemotherapy, KRAS, NRAS, or BRAF wild type. Conclusion: The incidence of 253LN metastasis in left-sided CRLM is 20.64%, and skip metastasis rate is 10.3%. The 253LN status is an independent prognostic risk factor for RFS but not for OS after curative surgery. Routine resection of 253LN should be applied in curative surgery of left-sided CRLM.

    Abstract: To evaluate the effectiveness and safety of using off-the-shelf “Octopus” technique to treat ruptured or symptomatic thoracoabdominal aortic aneurysm (TAAA) and pararenal abdominal aortic aneurysm (PRAAA). Methods and Results: All cases who underwent “Octopus” technique from May 2016 to May 2019 at our center were retrospectively analyzed. A total of 10 cases (8 males) were included. The mean age was 54.5±14.2 years (range: 31–80 years). Eight cases presented as aneurysm rupture or impending rupture accepted emergency repair. Technical success, defined by placement of all endografts as planned, was achieved in all cases. A total of 30 target visceral branches were successfully cannulated, 9 celiac arteries were covered intentionally. Intraoperative endoleak was observed in 6 patients, all of them were gutter leak. During hospital stay, there was no death, no side branch occlusion or spinal cord ischemia. Median follow-up was 30 months (range: 12–50 months). One patient died of lung cancer at 14-month follow-up. There was no secondary endoleak. The primary endoleak were found spontaneously resolved in 3 cases at 7 days, 3-month, and 1-year imaging. One persistent endoleak totally resolved after sealing of gutter spaces at 4-month follow-up. The other 2 persistent endoleak decreased during follow-up, which are still under observation. The branch patency rate was 90.3% (28/31). All the 3 occluded branches were renal arteries. Branch occlusion occurred in 2 cases at 1-month follow-up and 1 case at 2-year follow-up, but renal insufficiency was not observed in these cases. Obvious aneurysm sac shrinkage (≥5 mm) was observed in all cases. The aneurysm size shrunk from 7.6±1.9 to 5.5±1.4 cm. No spinal cord ischemia occurred during follow-up. Conclusion: Treatment of ruptured TAAA and PRAAA with “Octopus” technique is feasible and safe for high surgical risk patients in the absence of fenestrated and branched devices. The long-term clinical outcomes needed to be investigated.

    Abstract: When a heavy object is cooperatively grasped to move by several fingers of the robot hybrid hand, the inertial properties and the mass distribution of the object must influence largely on the operation precision, grasping stability, and the safety of both the hybrid hand and the object. Hence, it is an important and significant issue to establish and analyze the dynamics model of the moving-object cooperatively grasped by the hybrid hand in order to ensure the safety and grasping stability of the hybrid hand and the object. However, this research has not been conducted. In this paper, a dynamics model of the moving-object grasped by the hybrid hand is established, and its dynamics is studied and analyzed. First, a three-dimensional model of a hybrid hand formed by a novel parallel manipulator and three fingers is designed for cooperatively grasping object. Second, the kinematic formulas for solving the Jacobian matrices, the Hessian matrices, the general velocity/acceleration of the moving platform, and four active limbs of the parallel manipulator are derived. Third, the composite Jacobian matrix and the composite Hessian matrix of the hybrid hand are derived, and the general velocity/acceleration of the moving-object grasped by the hybrid hand is derived. Fourth, dynamics model of the hybrid hand is established, the formulas for solving the dynamic actuation forces of the three fingers and the dynamic actuation forces/torque and constrained forces of the parallel manipulator are derived. Finally, the theoretical solutions of the dynamics model of the moving-object grasped by the hybrid hand are verified by its simulation mechanism.

    Abstract: Wheel squeal is generally attributed to the lateral wheel vibration induced by lateral creepage. It should be noted, however, longitudinal creepage also exists at wheel/rail interface. Given the significance of longitudinal creepage in vehicle dynamics, a test rig that can introduce various longitudinal creepages by changing the teeth number of synchronous pulley is developed in this research, and measurement results show that squeal spectrum has three dominant peaks, whose sound pressure levels decrease with longitudinal creepage. In order to investigate the reason why longitudinal creepage can mitigate wheel squeal, a model of a wheel/rail system is developed in this research to analyse this phenomenon using complex eigenvalue method, and the results show the inner wheel tends to generate unstable vibration at three different vibration modes, correlating well with the dominant peaks of sound spectrum. Further analyses can illustrate the reason why longitudinal creepage can mitigate wheel squeal: the absolute value of effective damping ratio of each mode decreases with longitudinal creepage, so structural damping and frictional damping are less likely to be overcome by negative damping.

    Abstract: Here we presented a case of a 19-month-old boy who developed severe aplastic anemia postacute hepatitis. He was treated successfully with the umbilical cord-derived mesenchymal stromal cells (UC-MSCs) infusion and cyclosporine A (CsA). The boy achieved both hematopoietic recovery and normal lymphocyte proportion. So far, his condition still remains stable. To our knowledge, there is a rare previous report on the utility of MSCs infusion for the treatment of hepatitis-associated aplastic anemia (HAAA). Considering the efficacy, safety, and strong operability, particularly for pediatric patient, the infusion of UC-MSCs combined with CsA could be an effective alternative for the treatment of HAAA.
