    Abstract: Carpal tunnel syndrome (CTS) is a most common peripheral nerve entrapment neuropathy characterized by sensorimotor deficits in median nerve innervated digits. Block-design task-related functional magnetic resonance imaging (fMRI) studies have been used to investigate CTS-related neuroplasticity in the primary somatosensory cortices. However, considering the persistence of digital paresthesia syndrome caused by median nerve entrapment, spontaneous neuronal activity might provide a better understanding of CTS-related neuroplasticity, which remains unexplored. The present study aimed to investigate both local and extensive spontaneous neuronal activities with resting-state fMRI. A total of 28 bilateral CTS patients and 24 normal controls were recruited, and metrics, including amplitude of low-frequency fluctuation (ALFF) and voxel-wise functional connectivity (FC), were used to explore synaptic activity at different spatial scales. Correlations with clinical measures were further investigated by linear regression. Decreased amplitudes of low-frequency fluctuation were observed in the bilateral primary sensory cortex (SI) and secondary sensory cortex (SII) in CTS patients (AlphaSim corrected P 〈 .05). This was found to be negatively related to the sensory thresholds of corresponding median nerve innervated fingers. In the voxel-wise FC analysis, with predefined seed regions of interest in the bilateral SI and primary motor cortex, we observed decreased interhemispheric and increased intrahemispheric FC. Additionally, both interhemispheric and intrahemispheric FC were found to be significantly correlated with the mean ALFF.

    Abstract: Restoration of digital flexion after brachial plexus injury or forearm injury has been a great challenge for hand surgeons. Nerve transfer and forearm donor muscle transfer surgeries are not always feasible. The present study aimed at evaluating the effectiveness of restoring digital flexion by brachialis muscle transfer. Ten lower brachial plexus- or forearm-injured patients were enrolled. After at least 12 months following surgery, the middle-finger-to-palm distance was less than 2.5 cm in six patients. In the other four patients with less satisfactory results, secondary tenolysis surgery was performed and the middle-finger-to-palm distances were reduced to 2.0–4.0 cm. The average grasp strength was 20 ± 4 kg. Elbow flexion was not adversely affected. In conclusion, brachialis muscle transfer is an effective method for reconstructing digital flexion, not only in lower brachial plexus injury, but also in forearm injury patients.     Abstract: Fine particulate matter (PM2.5) is a risk factor for stroke, and patients with pre-existing diseases appear to be particularly susceptible. We conducted a case-crossover study to examine the association between short-term exposure to fine particulate matter (PM2.5) and hospital admission for stroke in individuals with atrial fibrillation (AF), hypertension, diabetes, or hyperlipidemia. Methods: Patients diagnosed with acute ischemic stroke (AIS) were recruited from 2015 to 2017 in Chinese Stroke Center Alliances. We estimated daily PM2.5 average exposures with a spatial resolution of 0.1° using a data assimilation approach combining satellite measurements, air model simulations, and monitoring values. Conditional logistic regression was used to assess PM2.5-related stroke risk in patients with pre-existing medical co-morbidities. Results: A total of 155,616 patients diagnosed with AIS were admitted. Patients with a history of AF ( n = 15,430), hypertension ( n = 138,220), diabetes ( n = 43,737), or hyperlipidemia ( n = 16,855) were assessed separately. A 10 µg/m 3 increase in daily PM2.5 was associated with a significant increase in AIS for individuals with AF at lag 4 (odds ratio (OR), 1.008; 95% confidence interval (CI), 1.002–1.014), and with hypertension (OR, 1.008; 95% CI, 1.006–1.010), diabetes (OR, 1.006; 95% CI, 1.003–1.010), and hyperlipidemia (OR, 1.007; 95% CI, 1.001–1.012) at lags 0–7. Elderly (⩾ 65 years old) and female patients with AF had significantly higher associations at lag 5 (OR, 1.009; 95% CI, 1.002–1.015) and lag 5 (OR, 1.010; 95% CI, 1.002–1.018), respectively. Conclusion: Short-term exposure to PM2.5 is significantly associated with hospital admission for stroke in individuals with pre-existing medical histories, especially in older or female patients with AF. Preventive measures to reduce PM2.5 concentrations are particularly important in individuals with other medical co-morbidities.

    Abstract: To evaluate the relationship between pain and quality of life (QoL) in patients newly admitted to Wuhan Hospice Center, China. A total of 1,634 patients were analyzed in this retrospective study. A Numerical Rating Scale and Chinese-QoL instrument were used to assess pain score and QoL, respectively. Most patients experienced moderate to severe pain, which significantly impaired QoL. The pain was significantly correlated with appetite, mood, sleep, fatigue, pain intensity, daily activity, side effect, general appearance, and support from family. But there was no correlation with support from society, understanding of cancer, or attitude toward treatment. In our study, the relationship between pain and QoL was found to be reciprocal. The staff can offer a multidisciplinary care perspective for improving hospice care for this special group of population.

    Abstract: Genome-wide sequencing investigations have identified numerous long noncoding RNAs (lncRNAs) among mammals, many of which exhibit aberrant expression in cancers, including esophageal squamous cell carcinoma (ESCC). Herein, this study elucidates the role and mechanism by which LINC01419 regulates the DNA methylation of glutathione S-transferase pi 1 (GSTP1) in relation to ESCC progression and the sensitivity of ESCC cells to 5-fluorouracil (5-FU). Methods: LINC01419 and GSTP1 levels were quantified among 38 paired ESCC and adjacent tissue samples collected from patients with ESCC. To ascertain the contributory role of LINC01419 in the progression of ESCC and identify the interaction between LINC01419 and GSTP1 promoter methylation, LINC01419 was overexpressed or silenced, and the DNA methyltransferase inhibitor 5-Aza-CdR was treated. Results: Data from the GEO database (GSE21362) and the Cancer Genome Atlas displayed elevated levels of LINC01419 and downregulated levels of GSTP1 in the ESCC tissues and cells. The silencing of LINC01419 led to decreased proliferation, increased apoptosis, and enhanced sensitivity to 5-FU in ESCC cells. Notably, LINC01419 could bind to the promoter region of the GSTP1 gene, resulting in elevated GSTP1 methylation and reduced GSTP1 levels via the recruitment of DNA methyltransferase among ESCC cells, whereby ESCC progression was stimulated accompanied by reduced ESCC cell sensitivity to 5-FU. GSTP1 demethylation by 5-Aza-CdR was observed to reverse the effects of LINC01419 overexpression in ESCC cells and the response to 5-FU. Conclusion: Highly expressed LINC01419 in ESCC promotes GSTP1 methylation, which ultimately acts to promote the event of ESCC and diminish the sensitivity of ESCC cells to 5-FU, highlighting a novel potential strategy to improve 5-FU-based chemotherapy in ESCC.

    Abstract: Tracking control, which is applied to the target mobile robots in the process of rushing toward the trainees, is one of the critical technologies in the advancement of anti-terrorist training. Considering the disadvantages of various types of traditional tracking methods, this paper proposes a novel laser ray tracking mechanism and a backstepping controller for the target mobile robot that is used in shooting ranges. The mechanism and principle of the laser ray tracking is illustrated in detail. Based on the unique structure, the light intensity distributions are measured to further locate the laser spots on the cut-ray boards. Then, the relationship between the positions of the laser spots on the cut-ray boards and the pose of the target mobile robot is demonstrated. According to the features of the tracking situation, a backstepping controller is designed to achieve the laser ray tracking. After that, the inverse kinematics of the wheeled skid-steering mobile robot is analyzed to map the linear and angular velocities of the robot to the velocities of its left wheels and right wheels. The conventional proportional–integral–derivative controller is applied in the experiments to compare with the proposed backstepping controller. The experimental results show that the proposed controller is more robust, and converges faster for the laser ray tracking.

    Abstract: The objective of this systematic review is to evaluate the safety ad feasibility of the totally implantable vascular access devices (TIVADs) flushed more than 4 weeks. We searched the following electronic databases from the date their build-up to February 2020: PubMed, EMBASE, The Cochrane Central Register of Controlled Trials (CENTRAL), and CINAHL. The final selection resulted in 14 trials fulfilling the inclusion criteria and being included in our review. A pooled frequency of port-related late complications with longer flushing intervals ( 〉 4 weeks) was 8.0%, and the pooled frequency of occlusions, infections, and mechanical complications was 5.0%, 2.0%, and 3.0%, respectively. Then, we compared the frequency of port-related complications between standard and longer flushing intervals. There were no differences between the group’s changes in the frequency of total late complications, occlusions, infections, and mechanical complications. This systematic review and meta-analysis demonstrates that longer flushing intervals for ports are safe. However, more prospective, power appropriated randomized trials are needed to explore the specific flushing time for ports.

    Abstract: For robots with flexible joints, the joint torque dynamics makes it difficult to control. An effective solution is to carry out a joint torque controller with fast enough dynamic response. This article is dedicated to design such a torque controller based on sliding mode technique. Three joint torque control approaches are proposed: (1) The proportional-derivative (PD)-type controller has some degree of robustness by properly selecting the control gains. (2) The direct sliding mode control approach which fully utilizes the physical properties of electric motors. (3) The sliding mode estimator approach was proposed to compensate the parameter uncertainties and the external disturbances of the joint torque system. These three joint torque controllers are tested and verified by the simulation studies with different reference torque trajectories and under different joint stiffness.

    Abstract: This paper seeks to study high-temperature effect on mechanical performance of screwed single-lap carbon fiber-reinforced polyimide–TC4 titanium alloy joints repaired with metal inserts. Quasi-static tension tests were conducted at room temperature (RT) and 250℃ to determine the joint strength and stiffness of repaired joints with metal inserts. Based on the experimental results, high-temperature effect on joint strength and stiffness and insert repair efficiency were analyzed and discussed. A new analytical model was established to evaluate the effect of high temperature on joint stiffness. It is concluded that (1) joint strength and stiffness for all configurations are lower at 250℃ than that at RT, showing the expected detrimental effect of high temperature on joint strength and stiffness. The reductions in joint strength and stiffness depend on the joint configuration; (2) the repair efficiencies of embedded conical nut for joint strengths of protruding and countersunk head screw joints decrease, but those for joint stiffness increase at 250℃ as against at RT. Unlike the repair efficiencies of embedded conical nut, the repair efficiency of bushing for joint strength is slightly greater, but that for joint stiffness is less at 250℃ than at RT; and (3) the developed analytical model is capable of predicting the displacement of screwed single-lap carbon fiber-reinforced polyimide–TC4 joints at RT and high temperature, and there is good agreement between the experimental data and the predicted curves.

    Abstract: This paper outlines progressive damage characteristics of screwed single-lap CFRPI-metal joints subjected to tensile loading at RT (room temperature) and 350°C. Quasi-static tensile tests were performed on screwed single-lap CCF300/AC721-30CrMnSiA joint at RT and 350°C, and the load versus displacement curve, strength and stiffness of joint were gauged and discussed. With due consideration of thermal-mechanical interaction and complex failure mechanism, a modified progressive damage model (PDM) based on the mixed failure criterion was devised to simulate progressive damage characteristics of screwed single-lap CCF300/AC721-30CrMnSiA joint, and simulations correlate well with experiments. By using the PDM, the effects of geometry dimensions on mechanical characteristics of screwed single-lap CCF300/AC721-30CrMnSiA joint were analyzed and discussed.
