    Abstract: To present the tympanometric findings in 552 patients (115 over 10 years of age) with unrepaired cleft palate (256 had audiologic findings) and to show the natural history and outcome of these cases. Setting: The cleft lip and palate clinic for the Division of Cleft Lip and Palate Surgery at the West China College of Stomatology, Sichuan University, Chengdu, People's Republic of China. Design: Pure-tone audiometric and tympanometric evaluations were performed on 552 patients with an unrepaired cleft palate. Results were analyzed by looking at the patient's age and cleft palate type. Results: This study demonstrated an age-related decrease in the frequency of hearing impairment and abnormal tympanometry. The frequency of hearing impairment and abnormal tympanometry in patients with submucous cleft palate was significantly lower than in patients from the other four major cleft palate categories (p  =  .001, p  =  .006, respectively). Conclusions: The middle ear function and hearing levels of unrepaired cleft palate patients improved with age, but at least 30% of the patients’ ears demonstrated a hearing loss and abnormal tympanometry in each age group, including those over 19 years of age. In the crucial language-learning stage, the frequency of hearing impairment and abnormal tympanometry was as high as 60%. Considering these results, palate repair and surgical intervention, such as tube insertion, for otological problems should be considered at an early age.

    Abstract: A morphing aircraft can maintain optimal flight performance by adaptively changing shape. However, large deformation and fast motion of aircraft modules lead to complicated dynamics during morphing process. This paper proposes a mechanistic equivalent model with parameters identified by optimization method. Based on the dynamics of the aircraft modules with large deformation and fast motion and considering the coupling characteristics of rigid bodies and gimbal joints, an equivalent dynamic model of morphing aircraft is built in this study. Considering the huge amount of highly coupled parameters in the equivalent model, particle swarm optimization algorithm is used to identify the equivalent parameters based on the sample data of flexible model. By comparing the simulation results of proposed model to those of rigid model and flexible model, it can be seen that the accuracy of the proposed equivalent model is comparable to that of the flexible model, but the computational load is only 10% of that of flexible model. Further, based on this high-fidelity model with low computational load, an optimized morphing process is obtained, and the attitude variation during morphing is reduced by 4.23%.

    Abstract: Single-dose intravenous ketamine has rapid but time-limited antidepressant effects. We aimed to examine the sustained effects of six consecutive ketamine infusions in Chinese patients with major depressive disorder. Methods: Seventy-seven patients with major depressive disorder were eligible to receive augmentation with six ketamine infusions (0.5 mg/kg over 40 min) administered over the course of 12 days (Monday–Wednesday–Friday). The coprimary outcome measures were the rates of response and remission as measured on the 10-item Montgomery-Asberg Depression Rating Scale. Psychotomimetic and dissociative symptoms were measured with the Brief Psychiatric Rating Scale-positive symptoms and the Clinician Administered Dissociative States Scale, respectively. Results: After the first ketamine infusion, only 10 (13.0%) and 6 (7.8%) patients responded and remitted, respectively; after six ketamine infusions, 52 (67.5%) patients responded and 37 (48.1%) remitted. There was a significant mean decrease in Montgomery-Asberg Depression Rating Scale score at four hours after the first ketamine infusion (7.0±7.5, p 〈 0.001), and this decrease was maintained for the duration of the infusion period. The response to ketamine treatment was positively associated with no history of psychiatric hospitalization (odds ratio=3.56, p=0.009). Suicidal ideation rapidly decreased across the entire study sample, even among the nonresponder group. No significant differences were found regarding Brief Psychiatric Rating Scale and Clinician Administered Dissociative States Scale scores from the first infusion at baseline to four hours post-infusion. Conclusion: Six ketamine infusions increased rates of response and remission when compared to a single-dose ketamine infusion in patients with major depressive disorder. Future controlled studies are warranted to confirm and expand these findings.

    Abstract: Ketamine produces significant rapid-onset and robust antidepressant effects in patients with major depressive disorder. However, this drug also has transient cardiovascular stimulatory effects, and there are limited data about potential predictors of these cardiovascular effects. Methods: A total of 135 patients with unipolar and bipolar depression received a total of 741 ketamine infusions (0.5 mg/kg over 40 min). Blood pressure and pulse were monitored every 10 min during the infusions and 30 min after the infusions. Depressive, psychotomimetic and dissociative symptom severity was assessed at baseline and 4 hours after each infusion. Results: The maximum blood pressure and pulse values were observed at 30–40 min during infusions. The largest mean systolic/diastolic blood pressure increases were 7.4/6.0 mmHg, and the largest mean pulse increase was 1.9 beats per min. No significant change in blood pressure and pulse was found in the second to sixth infusions compared with the first infusion. Patients who were older (age⩾50 years), hypertensive and receiving infusions while exhibiting dissociative symptoms showed greater maximal changes in systolic and diastolic blood pressure than patients who were younger (age 〈 50 years), normotensive and without dissociative symptoms (all p 〈 0.05). Hypertensive patients had less elevation of pulse than normotensive patients ( p 〈 0.05). Ketamine dosage was positively correlated with changes in systolic and diastolic blood pressure (all p 〈 0.05). Conclusions: Blood pressure and pulse elevations following subanaesthetic ketamine infusions are transient and do not cause serious cardiovascular events. Older age, hypertension, large ketamine dosage and dissociative symptoms may predict increased ketamine-induced cardiovascular effects.

    Abstract: Adenomyosis is a common and refractory disease in gynecology. Preserving the uterus during treatment for adenomyosis remains a problem. High-intensity focused ultrasound (HIFU) is widely used in treatment of solid tumors. This study aimed to analyze patients with adenomyosis who were treated by HIFU and to preliminarily examine the characteristics of patients who are more suitable for HIFU to treat adenomyosis with reliable efficacy. Methods Over 2 years, 67 women who were diagnosed with adenomyosis and treated with HIFU at our gynecology department were included in this study. We investigated outcomes of their symptoms (dysmenorrhea and hypermenorrhea) and the volume of their uterine lesions. We also compared the patients’ clinical profiles. Results The women had a mean follow-up duration of 11.6 ± 0.46 months. In the numerical rating scale, used to assess the degree of dysmenorrhea, the score was significantly lower (mean difference: −1.94, 95% confidence interval: −2.704 to −1.176) 3 months after HIFU treatment compared with before treatment, then it remained stable for 3 to 12 months. Hypermenorrhea was reduced to a certain degree, with a mean difference of −0.54 (−1.01–0.02). Conclusions HIFU is a new noninvasive treatment method for adenomyosis that may help relieve dysmenorrhea.

    Abstract: Currently used morphologic criteria have limitations in assessing tumor response to chemotherapy because of the relatively slow tumor shrinkage as measured by conventional morphologic imaging. Functional imaging techniques show promising results in early assessment of tumor response to treatment. Purpose To quantitatively detect changes in tumor perfusion during chemotherapy with contrast-enhanced ultrasound. Material and Methods Twenty-three MCF-7 breast cancer bearing nude mice treated by either adriamycin ( n = 11) or sterile saline ( n = 12) were imaged before and after treatment with an ultrasound scanner after bolus injection of SonoVue. Regions of interest within the tumor were analyzed offline to determine perfusion parameters including peak enhancement (PE), area under the curve of wash-in (WiAUC), rise time (RT), wash-in rate (WiR), wash-in perfusion index (WiPI), and quality of fit (QOF). Hematoxylin and eosin was used to assess tumor cell density and immunohistochemical analysis was performed for evaluation of microvascular density (MVD). Results Treatment with adriamycin significantly reduced tumor growth in comparison to the control group ( P 〈 0.001). There was no significant difference in perfusion parameters before treatment. Treatment with adriamycin resulted in a significant decrease in PE, WiAUC, WiR, and WiPI in comparison with control group ( P 〈 0.01). The tumor cell density estimated by pathology slice was significantly lower in treated tumors than in control tumors after treatment ( P 〈 0.001). Immunohistochemistry showed significant decreases of MVD in treated tumors as compared with control tumors ( P 〈 0.001) after treatment. Conclusion Quantitative contrast-enhanced ultrasound can detect the change of tumor perfusion after chemotherapy, which may enable early assess tumor response to chemotherapy.

    Abstract: Evidence from many countries shows that as societies modernise and women’s status rises, son preference declines. Yet in China the sex ratio at birth has been the highest in the world for over two decades despite rapid modernisation, urbanisation and huge improvements in women’s status. This study explored this apparent contradiction through interviews with 212 men and women in urban and rural areas of Zhejiang, Guizhou and Yunnan provinces. Results showed that women’s status is perceived as high across a range of factors, including educational attainment and opportunity, labour participation and roles at home and the workplace. The majority of interviewees expressed gender indifference and had clear views about why the sex ratio is persistently high in China. High sex ratios persist probably because, while the majority is essentially gender indifferent, it takes only a small minority undergoing selective abortion to skew the sex ratio.

    Abstract: This paper addresses the stability analysis and adaptive state feedback control for a class of nonlinear discrete-time systems with multiple interval time-varying delays and non-symmetric dead-zone input. The multiple interval time-varying delays uncertainties are bounded by the nonlinear function with unknown coefficients. The non-symmetric dead-zone input without the knowledge of the dead-zone parameters is considered for the nonlinear system. The smooth adaptive state feedback controller is designed. By introducing the new Lyapunov-Krasovskii functional, it can be seen that the solutions of the closed-loop error system converge to an adjustable bounded region. In general, the proposed adaptive state feedback control strategy does not require the knowledge of maximum and minimum values for the characteristic slopes, and the knowledge of all system states. The control design conditions are relaxed because the system output is available for the controller design. Finally, three simulation examples are performed to show the effectiveness of the proposed methods.

    Abstract: To evaluate the neglect of left-behind children (LBC) in China. Method: Participants: Children separated from one or both parents for at least 6 months. Intervention: Trauma of separation. Comparison: Non-left-behind children (NLBC). Outcomes: Neglect rates and severity. Only case–control studies were included. Results: Thirteen studies were included; there were 18,688 LBC in a large sample ( N = 42,003) of children aged 0–18 years in China. The overall neglect rate was significantly higher in LBC compared to NLBC (odds ratio [ OR] = 1.58, 95% confidence interval [CI] = [1.50, 1.67], p 〈 .01) based on the Chinese Rural Child Neglected Evaluation Model (CRCNEM) and the Parents–Child Conflict Tactics Scales ( OR = 1.44, 95% CI [1.35, 1.54], p 〈 .01). The overall neglect severity in LBC was also significantly higher than NLBC ( SMD = 0.31, 95% CI [0.28, 0.33], p 〈 .01). The same trends were observed in sex subgroups. With regard to subtypes, LBC were significantly more likely to have emotional neglect ( OR = 2.29, 95% CI [1.88, 2.78], p 〈 .01), medical neglect ( OR = 1.79, 95% CI [1.62, 1.98], p 〈 .01), physical neglect ( OR = 1.75, 95% CI [1.60, 1.91], p 〈 .01), security neglect ( OR = 1.52, 95% CI [1.32, 1.75], p 〈 .01), educational neglect ( OR = 1.50, 95% CI [1.31, 1.72], p 〈 .01), and social neglect ( OR = 1.33, 95% CI [1.18, 1.51], p 〈 .01). Furthermore, LBC had significantly higher severity in medical neglect ( SMD = 0.31, 95% CI [0.27, 0.35], p 〈 .01), emotional neglect ( SMD = 0.28, 95% CI [0.24, 0.32], p 〈 .01), physical neglect ( SMD = 0.24, 95% CI [0.18, 0.29], p 〈 .01), security neglect ( SMD = 0.26, 95% CI [0.23, 0.29], p 〈 .01), educational neglect ( SMD = 0.25, 95% CI [0.20, 0.31], p 〈 .01), and social neglect ( SMD = 0.25, 95% CI [0.10, 0.40], p 〈 .01). Conclusion: The neglect rates and severity in LBC in China were both significantly higher than those in NLBC. There was a strong association between neglect and LBC. Public policy changes are urgently needed to improve the dire situation and the well-being of the LBC.
