    Abstract: We discuss an international immersion research experience conducted by an interdisciplinary team of business and environmental engineering students and faculty in Tanzania. Using this experience as “data,” we bring out methodological and substantive issues in studying the intersection of poverty and the environment. The approach we describe is bottom-up, beginning with micro-level details of life circumstances. Such micro-level details are often the most challenging to learn and the most neglected in terms of informing practice. Our work speaks to a number of issues such as organizing a short research immersion for effectiveness and efficiency, covering different units of analysis in terms of individuals, households, communities, village-level leadership, and the outside ecosystem of organizations using a bottom-up approach, and combining natural science analytical testing with social science to focus on the substantive domains of environments in subsistence contexts.

    Abstract: Purpose: To evaluate the effectiveness of a pharmacist management regarding the percentages of patients meeting American Diabetes Association (ADA) treatment goals for diabetes and specific Veterans Health Administration (VA) performance measures individually and in combination after 6 months of intervention. Methods: We retrospectively evaluated the electronic medical record of all new patients seen between October 1, 2007 and March 31, 2009, with an A1c 〉 7%. Primary objectives included the percentages of patients meeting ADA treatment goals individually and in combination after 6 months in addition to the percentage meeting the 3 VA performance goals individually and in combination. Results: One-hundred and ninety-seven patients met inclusion criteria. There were 6% of patients who met all ADA goals and 79% who met all VA performance measure goals at study end. Individual goal analyses revealed 43% of patients at a goal A1c of 〈 7%, 55% of patients were at low-density lipoprotein (LDL) goal, 45% of patients at systolic blood pressure (SBP) goal, and 51% of patients at diastolic blood pressure (DBP) goal. Evaluation for VA performance measures showed that 91% patients at goal for A1c, 55% at LDL goal, 70% at systolic BP goal, and 87% met diastolic BP goal. Conclusions: Six months of pharmacist intervention resulted in improvement in patients achieving ADA and VA performance measure goals individually and in combination.

    Abstract: Synthetic and semi-synthetic polymeric materials were originally developed for their durability and resistance to all forms of degradation, including biodegradation. Nanotechnology has the potential to revolutionize many sectors, including pharmaceuticals, information technology, medical devices, materials science, chemicals, and energy. Nanofibres provide a connection between the nanoscale world and the macroscale world, since their diameters are in the range of 1 to 100 nanometres and several metres in length. Therefore, the current emphasis of research is to exploit such properties and focus on determining appropriate conditions for electrospinning various polymers and biopolymers for eventual applications including: multifunctional membranes; biomedical structural elements (scaffolds used in tissue engineering, wound dressing, drug delivery, artificial organs, vascular grafts); protective shields in specialty fabrics; and filter media for submicron particles in the separation industry. This paper reviews the research on recent biomedical applications of electrospun nanofibres.

    Abstract: This paper presents wave crest statistics for storm waves in deep water obtained from data collected over the last 7 years using wave altimeters mounted on the North Alwyn fixed oil and gas platform in the northern North Sea. The evolution of wave characteristics during the rise, peak and, decay of storms has been examined and it has been found that, as storms develop, there tends to be a focusing of the directional energy as the peak of the storm is approached followed by a directional defocusing as it decays. The effect of this upon the normalized crest elevations has been examined and sample distributions of crest heights taken from the start, peak and end of storms show some differences that are discussed. Three crest elevation probability distribution models based on second-order wave theory have been examined using these data and found to produce quite good predictions of observed crest heights except in the most extreme values.

    Abstract: The paper describes the analyses of wave data recorded at various locations and water depths around northern Europe to determine the temporal characteristics of individual wave periods and other wave period statistics. These analyses show that, just as there are group characteristics for wave heights, there are similar, but less pronounced, characteristics for wave periods. This is observed in three separate sets of data from different locations in water depths of 18, 50, and 130 m. It is also found in time series simulated using random linear wave theory from a Jonswap spectrum. A simple, new statistic, R, is introduced that measures the rate of change in the wave period from one wave to the next. This is relevant to wave energy devices that may try to tune themselves to obtain optimum power output from each individual wave. The characteristics of this statistic and its variation with significant wave height, mean energy period, and spectral bandwidth have been examined for the three datasets and are discussed. It is found that the R statistic can be fitted quite well by a Gaussian distribution for all the datasets examined. In a real sea there will be many small waves with comparatively very little energy, and the effect of filtering these out upon the R statistic has been examined. It is seen that removing the small waves has very little effect upon the energy available for extraction but significantly reduces the rate at which the wave energy device must retune to obtain optimum power conversion. This is illustrated by considering a hypothetical wave energy device, with a representative power transfer function, that can retune at prescribed rates. It is shown that being able to tune to individual waves can greatly increase power output.

    Abstract: Methamphetamine and its related compounds are among the most widely abused recreational drugs worldwide. While a myriad of clinical complications of methamphetamine use have been described, there is a paucity of literature regarding the effects of maternal abuse during pregnancy on neonatal hearts. In this report, we describe a neonate who underwent Norwood-type palliation and subsequently developed catecholamine-resistant cardiogenic shock, likely related to methamphetamine exposure, which recovered after a period of venoarterial extracorporeal membrane oxygenation support.

    Abstract: Budd-Chiari syndrome (BCS) describes a variety of clinical conditions that result in outflow obstruction of hepatic venous return to the right heart, ultimately leading to liver failure and death if left untreated. The authors present successful management to 24 months of a patient with BCS by radiologic interventional techniques that included percutaneous transluminal angioplasty and placement of metallic stents.

    Abstract: Symptom control may become challenging for terminally ill patients as they near the end of life. Patients often seek hospital admission to address symptoms, such as pain, nausea, vomiting, and restlessness. Alternatively, palliative medicine focuses on the control and mitigation of symptoms, while allowing patients to maintain their quality of life, whether in an outpatient or inpatient setting. Hospice care provides, in addition to inpatient care at a hospice facility or in a hospital, the option for patients to receive symptom management at home. This option for symptom control in the outpatient setting is essential to preventing repeated and expensive hospital readmissions. This article discusses the impact of hospice care on hospital readmission rates.
