    Abstract: Portal hypertension is the main consequence of liver cirrhosis, leading to severe complications such as variceal hemorrhage, ascites or hepatic encephalopathy. As an attempt to decompress the portal venous system, portal flow is derived into the systemic venous system through spontaneous portosystemic shunts (SPSSs), bypassing the liver. In this review, we aim to provide an overview of the published reports in relation to the prevalence and physiopathology behind the appearance of SPSS in liver cirrhosis, as well as the complications derived from its formation and its management. The role of SPSS embolization is specifically discussed, as SPSSs have been assessed as a therapeutic target, mainly for patients with recurrent/persistent hepatic encephalopathy and preserved liver function. Furthermore, different aspects of the role of SPSS in liver transplantation, as well as in candidates for transjugular intrahepatic portosystemic shunt are reviewed. In these settings, SPSS occlusion has been proposed to minimize possible deleterious effects, but results are so far inconclusive.

    Abstract: Active (new/enlarging) T2 lesion counts are routinely used in the clinical management of multiple sclerosis. Thus, automated tools able to accurately identify active T2 lesions would be of high interest to neuroradiologists for assisting in their clinical activity. Objective: To compare the accuracy in detecting active T2 lesions and of radiologically active patients based on different visual and automated methods. Methods: One hundred multiple sclerosis patients underwent two magnetic resonance imaging examinations within 12 months. Four approaches were assessed for detecting active T2 lesions: (1) conventional neuroradiological reports; (2) prospective visual analyses performed by an expert; (3) automated unsupervised tool; and (4) supervised convolutional neural network. As a gold standard, a reference outcome was created by the consensus of two observers. Results: The automated methods detected a higher number of active T2 lesions, and a higher number of active patients, but a higher number of false-positive active patients than visual methods. The convolutional neural network model was more sensitive in detecting active T2 lesions and active patients than the other automated method. Conclusion: Automated convolutional neural network models show potential as an aid to neuroradiological assessment in clinical practice, although visual supervision of the outcomes is still required.

    Abstract: Sentinel lymph node biopsy has become an ordinary method for breast cancer staging. Neoadjuvant chemotherapy has been considered one of the contraindications for sentinel lymph node biopsy due to potential secondary fibrosis and lymphatic distortion. Timing and influence on sentinel lymph node biopsy result by primary systemic therapy are current and controversial topics. Methods and study design The experience in the medical literature is reviewed. A search was performed in the following databases: Medline (through Pubmed), EMBASE, Tripdatabase and Cochrane Library, between January 1998 and December 2008. Results and conclusions After analyzing the conclusions from 42 series and waiting for the end of related prospective trials, it could be concluded that sentinel lymph node biopsy is a useful diagnostic tool that should be integrated in the algorithm for the management of breast cancer patients when primary systemic therapy is needed.

    Abstract: This article presents an analysis of the characteristics that are associated with union membership duration using data from the membership registers of the largest Spanish trade union: Comisiones Obreras (CCOO, Workers’ Commissions). Making use of survival analysis techniques, the results indicate that the shortest membership durations and the highest risks of leaving are associated with workers with poor employment conditions, mainly youth and foreigners, as well as those in firms, economic sectors, and territories where the union has a rather weak presence. As workers in these situations represent the majority of both current joining and leaving rates, the article concludes that retention policies should focus on the early stages of union membership.

    Abstract: This article analyses the characteristics of members leaving trade unions in Spain – specifically the Catalonia branch of Workers' Commissions (CCOO-Catalonia) – alongside their reasons for leaving, using a variety of data sources. Our findings indicate higher union attrition among members in more precarious employment situations (i.e. temporary employment, low seniority). In general, union leavers confirm that their job situation is an important reason for leaving the union. We therefore conclude that efforts made by unions to support members in vulnerable labour market positions are also important in order to reduce rates of union attrition. Cet article analyse les caractéristiques des membres qui quittent les syndicats en Espagne – plus particulièrement la branche catalane des Commissions ouvrières (CCOO-Catalonia) – ainsi que les motifs de leur départ, en utilisant des données provenant d’un large éventail de sources. Il montre que la désyndicalisation concerne davantage les membres qui connaissent une situation d’emploi plus précaire (par ex. emploi temporaire, ancienneté limitée). En général, ceux qui quittent le syndicat confirment que leur situation professionnelle est un motif important d'abandon du syndicat. Nous concluons dès lors que les efforts consentis par les syndicats pour soutenir les membres qui se trouvent dans des positions vulnérables sur le marché du travail sont également importants pour réduire le taux de désyndicalisation. Dieser Beitrag untersucht die Merkmale von Gewerkschaftsmitgliedern, in diesem Fall Mitgliedern des katalanischen Gebietsverbandes der CC.OO, die aus der Gewerkschaft ausgetreten sind, sowie die Gründe, die sie dazu veranlasst haben. Die Analyse von Daten aus unterschiedlichen Quellen zeigt, dass vor allem Gewerkschaftsmitglieder in prekären Beschäftigungsverhältnissen (das heißt mit zeitlich befristen Arbeitsverträgen oder kurzer Betriebszugehörigkeit) aus der Gewerkschaft austreten. Im Allgemeinen geben die Betroffenen an, dass ihre berufliche Situation ein wichtiger Grund für den Austritt aus der Gewerkschaft war. Um die Zahl der Abgänge zur verringern, ist es daher wichtig, dass die Gewerkschaften auch Anstrengungen unternehmen, um Mitglieder mit einer unsicheren Stellung auf dem Arbeitsmarkt zu unterstützen.

    Abstract: Background: The development of trigger digit after carpal tunnel syndrome release surgery has been widely reported. Lluch described reconstruction of the flexor retinaculum in elongated position to prevent such complication. Methods: We conducted a retrospective review to determine whether patients who undergo reconstruction in elongated position of the flexor retinaculum have a lower incidence of trigger digit postoperatively. In total, 1050 patients were included, 865 of whom had undergone traditional carpal tunnel release and 185 flexor retinaculum reconstruction. Results: No differences were found in the incidence of trigger digit after surgery (8.7% of the patients who underwent traditional release vs 11.9% in the reconstruction group). Neither difference was found when comparing mean time with the development of trigger digit. Conclusions: In the absence of randomized long-term studies comparing traditional release and reconstruction in elongated position after carpal tunnel release, given our results, we see no reason to favor reconstruction over standard carpal tunnel release as a means to prevent postoperative triggering of digits.

    Abstract: Stavudine (d4T)-containing regimens are associated with a potential for lipoatrophy and dyslipidaemia. We assessed the safety and efficacy of reducing the dose of stavudine compared with switching to tenofovir or maintaining the standard dose of d4T. Methods Clinically stable HIV-infected patients receiving antiretroviral therapy containing stavudine 40 mg twice daily with a plasma HIV RNA 〈 200 copies/ml for at least 6 months were randomized to maintain stavudine 40 mg twice daily (d4T40 arm), to reduce to 30 mg twice daily (d4T30 arm), or to switch from d4T to tenofovir (TDF arm). Results Fifty-eight (93% male) patients were included: 22 in the d4T40 arm, 19 in the d4T30 arm and 17 in TDF arm. At baseline, median time on d4T was 6 years (interquartile range [IQR] 2.6–7.1), median age 43 years (IQR 36–51) and median CD4 + T-cell count was 587/mm 3 (IQR 329–892). At week 24, median limb fat changes (g) were as follows: d4T40=-182 (95% CI: -469—5); d4T30=527 (95% CI: -343–694); and TDF=402 (95% CI: 130–835; d4T40 versus TDF, P=0.0003). Significant differences between median values of laboratory parameters were detected: triglycerides (mg/dl): d4T40=19; d4T30=-23 and TDF=-79 (d4T40 versus TDF, P=0.03); and total cholesterol (mg/dl): d4t40=22, d4T30=-4, and TDF=-28 (d4T40 versus TDF, P=0.04). No significant difference was observed in mitochondrial function assessed in peripheral blood mononuclear cells. Conclusions Although both strategies were associated with a trend toward a decrease in plasma lipids and an increase in body fat, the only significant changes were observed among those who switched to tenofovir.
