    Abstract: In recent years, smoking water pipes or hookah has increased among adolescents in most countries. Although there is evidence in support of the negative effects of this type of smoking on human health, such as the increased risk of lung disease, little is known about the potential effects of hookah smoking on the male reproductive system, especially on the molecular aspects of sperm. Patients and methods: This cross-sectional study examined sperm DNA fragmentation index, protamine 1 and 2 (PRM1 and PRM2) genes expression, and oxidant status in normozoospermic hookah smokers in comparison with non-smoker controls. Results: Our results showed significantly higher rates of DNA fragmentation, protamine deficiency, and abnormal chromatin condensation in the spermatozoa of hookah smokers ( P  〈  .0001). Also, protamine gene expression showed a remarkable decrease in hookah smokers (1.55 ± 2.54 and 0.33 ± 0.54) compared to the controls (3.49 ± 5.41 and 1.22 ± 1.96), although the reduction was not statistically significant ( P = .155 and P = .066, respectively). Moreover, a significantly higher level of semen MDA was observed in the case group compared to the controls (0.39 ± 1.04 vs 0.15 ± 0.21; P = .013). Conclusion: According to our study, although hookah smoking does not have a significant effect on sperm parameters, it may have deleterious effects on DNA integrity, oxidative status, and nuclear protein levels of spermatozoa.

    Abstract: Invasive candidiasis (IC) is associated with high mortality in intensive care unit (ICU) patients. Timely diagnosis of this potentially fatal condition remains a challenge; on the other hand, the criteria for initiating empirical antifungal therapy in critically ill patients are not well defined in different patient population and ICU settings. Alongside the international guidelines, reaching regional and local consensus on diagnosis and management of IC in ICU setting is essential. This report summarizes our present status of IC management in ICU, considered by a group of Iranian experts in the fields of intensive care and infectious diseases. A round table of 17 experts was held to review the available data and discuss the optimal treatment strategies for IC in critical care setting. Comparative published data on the management of IC were analytically reviewed and the commonly asked questions about the management of IC in ICU were isolated. These questions were interactively discussed by the panel and audience responses were taken to consolidate point-to-point agreement with the panel arriving at consensus in many instances. The responses indicated that patients’ risk stratification, clinical discretion, fungal diagnostic techniques and the empirical therapy for IC are likely to save more patients. Treatment options were recommended to be based on the disease severity, prior azole exposure, and the presence of suspected azole-resistant Candida species. This report was reviewed, edited and discussed by all participants to include further evidence-based insights. The panel expects such endorsed recommendations to be soon formulated for implementation across the country.

    Abstract: The COVID-19 pandemic has challenged the capacity of health systems in various countries. This study was designed to evaluate the response rates of hospitals in the prevention and control of COVID-19 in Hamadan province, Iran. In this cross-sectional study, all 20 hospitals in Hamadan province were monitored in February 2020. The instrument used in this study was the WHO checklist, entitled “Infection prevention and control health-care facility response for COVID-19.” In each hospital, the hospital infection control officials completed the checklists under the supervision of the hospital managers and were then provided to the project manager. Data analyzed by SPSS 22 software. There were 3482 hospital beds (the average beds of each hospital was 174) in the studied hospitals. Of 15 055 patients admitted to all hospitals, 2196 (14.6%) individuals were COVID-19 patients. The total average score obtained from the checklist was 62.25. Among 7 domains studied, the lowest scores belonged to 2 domains of “infrastructure and equipment” and “patient screening and triage.” The scores of domains “IPC programs” and “visitors” were significantly higher in hospitals with ICU beds than other hospitals ( P-value = .03 in both domains). A comparison between university-teaching hospitals with other hospitals and those in Hamadan city with other cities revealed no significant differences in any of the domains. The mean response rate (62.25) of hospitals in terms of COVID-19 in Hamadan province indicates their relative readiness to prevent and control the COVID19 pandemic. The shortage of infrastructure and equipment and screening and triage problems of patients were the main challenges of hospitals in managing the COVID-19 pandemic in Hamadan province, Iran.

    Abstract: This article presents an observer-based control scheme for nonlinear bilateral teleoperation systems under variable asymmetric time delays. Estimated velocities are used in control laws, which may make the system unstable. A novel stability analysis is addressed to guarantee the performance of the controller–observer scheme. The proposed scheme is composed of feedback controllers, consisting of two proportional–derivative plus damping controllers in which d-terms are used to passify the delayed proportional and derivative control actions. Two nonlinear high-gain observers are designed in order to estimate the velocity signals of the master and the slave. The stability of the overall closed-loop system is proved under the assumption of passivity of the terminal operator and environment. The performance of the proposed approach is verified using experiments performed on a 2-degree-of-freedom teleoperation system. Experimental results verify that both controller and observer design objectives are satisfied.

    Abstract: Implementing social-emotional learning skills into Collaborative Strategic Reading (CSR), the current study intended to extend the efficacy of CSR for teaching reading strategies when applying it to students in rural areas from a working-class community. To this purpose, forty-four students who made the comparison and the experimental groups were taught reading strategies through CSR and ECSR (Extended Collaborative Strategic Reading), respectively. A reading comprehension test with different question types was implemented to the students as pretest and posttest, and an interview was given at the end of the study to investigate the perception of the students toward reading strategy instruction through CSR and ECSR. Analysis of data indicated that only the ECSR group improved significantly in overall reading comprehension, but the componential analysis of the reading test showed that despite the fact that the CSR group showed no significant improvement in the reading tests in four formats (true–false, multiple-choice, matching, and cloze), the ECSR group improved significantly in reading tests with multiple-choice and cloze test formats. Moreover, although the students in both groups showed a positive view toward the interventions, the students in the ECSR group improved in social-emotional and communication skills. It seems that CSR can be improved to be effective by implementing the emotional component to it.

    Abstract: This study uses a comprehensive control approach to deal with the trajectory tracking problem of a two-flexible-link manipulator subjected to model uncertainties. Because the control inputs of two-flexible-link manipulators are less than their state variables, the proposed controller should be able to tackle the stated challenge. Practically speaking, there is only a single control signal for each joint, which can be used to suppress link deflections and control joint trajectories. To achieve this objective, a novel optimal robust control scheme, with an updated gain under the adaptive law, has been developed in this work for the first time. In this regard, a nonsingular terminal sliding mode control approach is used as the robust controller and a control Lyapunov function is used as the optimal control law, to benefit from the advantages of both methods. To systematically deal with system uncertainties, an adaptive law is used to update the gain of nonsingular terminal sliding mode control. The advantage of this approach over the existing methods is that it not only can robustly and stably control an uncertain nonlinear system against external disturbances but also can optimally solve a quadratic cost function (e.g. minimization of control effort). The Lyapunov stability theory has been applied to verify the stability of the proposed approach. Moreover, to show the superiority of this method, the computer simulation results of the proposed method have been compared with those of an adaptive sliding mode control scheme. This comparison shows that the presented approach is capable of optimizing the control inputs while achieving the stability of the examined two-flexible-link manipulator in the presence of model uncertainties and external disturbances.

    Abstract: The aim of this study was to identify the effect of web-based communication between a nurse and a family member of a patient with COVID-19 on his/her perceived stress. In this multicenter parallel randomized controlled trial, 67 family members of COVID-19 patients admitted to the Intensive Care Unit (ICU) were investigated. In the intervention group, web-based communication was performed for four consecutive days for 10 to 15 minutes. The Perceived Stress Scale (PSS-14) were completed in both groups before and after the intervention. Mean and standard deviation of perceived stress scores in the two groups were not significantly different ( p = 0.26) before the intervention; however, after the intervention, the mean PSS-14 in the intervention group was significantly lower than that of the control group ( p  〈  0.001). Due to the need to follow the physical and social distancing to protect against Coronavirus disease, the use of web-based communication recommended in future studies.

    Abstract: Lack of lateral resistance is one of the emerging problems in continuously welded rails. Lateral resistance is a type of resistance in which the railway track mobilizes against the applied lateral forces. Thus, curved tracks, especially sharp curves (curves with small radiuses), are the most susceptible segments for the lack of lateral resistance. The reason is, on curved tracks, in addition to the lateral forces applied by the train, longitudinal forces are decomposed into two parts, with one part being tangential (tangent to the curve) and the other part being radial. This radial force causes some defects to the track such as track buckling, transversal shifting of the track, pulling out of fastening shoulders, etc. Therefore, according to the general track instructions, welding of rails is not possible on curved tracks with a radius less than 400 m. With the help of laboratory tests, the authors of this paper previously showed that using two stiffeners under the steel sleeper increased the lateral resistance of the track adequately (by 140%) compared to the track with normal steel sleepers. In this paper, the effect of using two plate stiffeners under the steel sleepers was examined by field investigation on a real track. Two test methods, including the Single Tie (sleeper) Push Test (STPT) and the Multiple Tie (sleeper) Push Test (MTPT), were used to investigate the lateral resistance of the track. The results obtained by the STPT and MTPT methods showed an increase in lateral resistance by 139.6% and 135.5%, respectively. The obtained results are in accordance with the results of the laboratory tests. Moreover, the results showed that using two stiffeners under steel sleepers increased the lateral resistance of the track adequately, thereby enabling rails to be welded on curved tracks.

    Abstract: To investigate the prevalence of congenital hypothyroidism (CH) among neonates in East Azerbaijan, Iran. Methods A cross-sectional study conducted from 2009 to 2010. Most heel blood samples were taken between three and seven days of birth. Thyroid stimulating hormone (TSH) was tested using the enzyme-linked immunosorbent assay method. Neonates with abnormal screening results (TSH 〉 20 mIU/L, T4 〈 6.5 mg/dL) were re-examined. Data were analysed using SPSS software. Results A total of 62,459 infants were screened. Of 1562 re-called subjects (re-call rate 2.5%), 94 cases with CH were detected, yielding a CH prevalence of 1:666 (female:male ratio 1:1.4). Conclusion The prevalence of CH in East Azerbaijan province is higher in comparison with worldwide levels. These results emphasize the importance of the neonatal screening programme.
