    Abstract: Incisional hernias occur in up to 11 per cent of patients undergoing abdominal surgery. Up to 50 per cent of these patients with hernias will require repeat operative procedures. Management of these hernias have focused primarily on tensile strength of the mesh material, have not addressed currently used materials, and have not compared the strength of these repairs with each other. Forty-nine adult Sprague–Dawley rats had an incisional hernia created by removing a portion of their abdominal wall that was then repaired primarily, using either a composite mesh, Dual mesh (Gore-Tex), or polypropylene mesh. Six weeks after the repair, the rats were euthanized. Hydrostatic distension of the abdominal cavity was performed to compare bursting strength of each repair. Wound tensile strength was assessed and compared. Tissue samples were also taken to compare repair types for incorporation of prosthetic materials. The gross weight of the animals subjected to hydrostatic distention was equivalent between groups, as was the volume required prior to failure of the repair. There was a trend toward improved tensile strength of the Prolene mesh repair, which had a lower average inflammatory and fibrosis score on histology. Overall, the type of mesh used for repair does not seem to impact significantly the strength of the repair when assessed 6 weeks postoperatively. Choice of prosthetic material to repair the hernia should be made based on economics and handling characteristics alone. Prolene mesh has satisfactory strength with the least amount of inflammation and fibrosis.

    Abstract: To comprehensively assess associations of site-specific CD4 + -T-cell hypomethylation of the CD40-Ligand gene ( CD40L) with disease activity of women with systemic lupus erythematosus (SLE). Methods CpG-sites within the DNA of the promotor and two enhancer regions (n = 22) of CD40L were identified and numbered consecutively. The rate of methylated DNA in isolated CD4 + -T-cells of women with SLE were quantified for each methylation site by MALDI-TOF. Disease activity was assessed by SLE Disease Activity Index (SLEDAI). Associations of site-specific methylation rates with the SLEDAI scores were assessed by linear regression modelling. P values were adjusted according to Bonferroni-Holm as indicated. Results 60 female SLE patients participated in the study (age 45.7 ± 11.1 years, disease duration 17.0 ± 8.3 years). Significant associations to the SLEDAI were noted for CpG22 hypomethylation of the promotor (β = −40.1, p = 0.017, adjusted p = 0.027), trends were noted for CpG17 hypomethylation of the promotor (β = −30.5, p = 0.032, adjusted p = 0.6), and for CpG11 hypermethylation of the second enhancer (β = 15.0, p = 0.046, adjusted p = 0.8). Conclusion Site-specific hypomethylation of the CD40L promotor in CD4 + -T-cells show associations with disease activity in female SLE patients.

    Abstract: To assess whether a healthy lifestyle is associated to beneficial effects on various systemic lupus erythematosus (SLE) health domains. Methods In a cross-sectional study, Mediterranean Diet Adherence Score (MEDAS), physical activity energy expenditure (PAEE), and smoking status were assessed by questionnaires, along with clinical parameters and various health domains including Systemic Lupus Disease Activity Score (SLEDAI), Depression Scale (CES-D), Fatigue Severity (FSS), functional status (FFbH), physical and mental quality of life (PCS, MCS). Lifestyle choices were assessed with respect to health domains by linear regression modeling. Additionally, SLE patients with a healthy lifestyle (MEDAS ≥ 4, ≥ 1 h sport per week, no smoking) were compared to those without by Wilcoxon’s signed-rank test. Results 49 of 145 SLE patients (44.3 ± 31.7 years, 87.6% female) followed a healthy lifestyle and showed a higher physical quality of life (β = 4.5 (95%-CI 1.5–7.9) p = 0.01), lower depression (β = −5.0 (−8.2 to −0.2) p = 0.02) and lower fatigue (β = −0.8 (−1.5 to −0.2) p = 0.01) independently of SLE disease activity. Furthermore, dsDNA-antibodies were lower (146 ± 540 vs 266 ± 146 U/mL, p = 0.049). In a more detailed analysis, physical activity had the highest impact on the various health domains when compared to smoking or diet adherence, which was consistent even after adjusting for multiple potential confounders. Each 1,000 kcal of weekly PAEE was associated to a 1.8 (0.9–2.6) point increase in the PCS ( p = 0.0001), a 0.2 (0.03–0.4) point decrease in the CES-D ( p = 0.01) and a 2.8 (1.2–4.4) point increase in the FFbH ( p = 0.0006). Conclusion A healthy lifestyle, especially physical activity is associated with beneficial effects including quality of life, depression and fatigue in SLE.

    Abstract: The Peruvian coastal upwelling is one of the most productive systems in the global ocean, with important impacts on the carbon cycle. Primary productivity there displays strong variations at the interannual to decadal timescales. However, down-core investigations rarely reach sufficient temporal resolution to assess the response of productivity to climatic variations at these timescales beyond the instrumental and historical periods. We here analyzed diatom assemblages, sea-surface temperatures (SSTs), and nitrogen and organic carbon contents on a laminated sediment core from the Peruvian continental shelf to trace variations in regional productivity over the last 3000 years. Our record provides evidence for different climatic and oceanic conditions with more humid and less productive conditions older than 2500 cal. yr BP and drier and more productive conditions younger than 2500 cal. yr BP. The last 2500 years also present much stronger centennial-scale variability with the occurrence of six intervals with higher total diatom abundances and stronger percentages in upwelling-related diatom species, representative of intensified productivity, congruent to lower percentages in benthic diatoms, indicative of reduced rainfall. These six periods were synchronous to intervals of enhanced Walker circulation, suggesting a strong imprint of the Pacific zonal circulation on productivity variations off Peru. Our record also demonstrates that SSTs did not vary in phase with productivity, arguing against the idea of regional SSTs controlled by the upwelling intensity, but were rather in agreement to SST records off southern Chile, suggesting that Peruvian SST variations were largely controlled by oceanic currents at southern high latitudes.

    Abstract: The aim of this study was to identify factors associated with impaired work productivity and impaired daily activities in patients with systemic lupus erythematosus (SLE). Methods The LuLa study is a longitudinal patient-reported study. Beyond sociodemographic data, work productivity, daily activities and fatigue, several other clinical outcome parameters (e.g. mental health–related quality of life and physical functioning, disease activity, damage and pain) were surveyed with validated questionnaires. The effects of confounders on work productivity (WPAI 2) and daily activity domains (WPAI 4) were studied by multivariate regression analysis. Results A total of 585 patients completed the questionnaire of whom 259 were employed and analysed. The median impairment in work productivity (WPAI 2) was 20% (Q1–3 0–40), and the median impairment in daily activities (WPAI 4) was 30% (Q1–3 10–50%). Multivariate regression analysis revealed that fatigue, pain, disease activity and health-related quality of life affected WPAI 2 and 4. Furthermore, we observed distinct synergistic effects of fatigue, disease activity and pain on both work productivity and daily activities: a higher impact of fatigue was associated with the reported extent of pain or disease activity. Conclusion In employed patients with SLE, impaired work productivity and impaired daily activities were frequently reported. Fatigue, pain, disease activity and health-related quality of life demonstrated a detrimental impact, with a synergistic effect of fatigue, disease activity and pain. Hence, both optimized pain management and targeted immunomodulatory therapy are important for preserving active participation in life among patients with fatigue.

    Abstract: Planktic and benthic foraminifera assemblages from a set of sediment cores, collected on the Adriatic shelf and the Southern Adriatic deep basin, provide compelling evidence of submillennial-scale environmental changes during the last 6000 years. Repeated peaks in Globigerinoides sacculifer represent warm-dry intervals, including the `Mediaeval Warm Period', the `Roman Age', the late `Bronze Age' and the `Copper Age'. The Last Occurrence (LO) of G. sacculifer (550 years BP) approximates the base of the `Little Ice Age' (LIA). Significant turnovers in the structure of the water column reflect changes in the rate of formation and depth of flow of the North Adriatic Dense Water (NAdDW) and the Levantine Intermediate Water (LIW). About 7500 years BP the benthic oxygen isotope records mark the timing when the NAdDW formation intensified on the slope and shifted to its modern route. About 5500 years BP, when sea level reached its modern high stand, oxygen isotope records of intermediate planktic dwellers indicate a northward intrusion of the LIW on the slope. The oscillating isotope trends during the last five millennia document a discontinuous invasion of LIW into the Central Adriatic, possibly reflecting short-term climate changes with weakened LIW production during colder and wetter intervals.

    Abstract: There is a scarcity of data supporting the association between atherosclerosis and dolichoectasia in unbiased samples. Aims To test the hypothesis that the association between dolichoectasia and extracranial carotid atherosclerosis depends on the degree of collateral circulation. Methods The Northern Manhattan Study magnetic resonance imaging substudy consists of 1290 participants who remained stroke-free at the time of magnetic resonance imaging. Arterial diameters were collected in all participants with available magnetic resonance angiography. Dolichoectasia was defined as a head-size adjusted diameter 〉 2 standard deviation for each artery. Carotid Doppler was used to evaluate for carotid atherosclerosis (carotid plaque, maximum plaque thickness and carotid intima media thickness). Results We included 994 participants with available Doppler and magnetic resonance angiography data (mean age 63 years, 60% female). Any dolichoectasia was reported in 16% of participants, 54% had at least one carotid plaque and the mean carotid intima media thickness was 0·92 ± 0·09 mm. After adjusting for demographic and clinical characteristics, there was no association between markers of carotid atherosclerosis and dolichoectasia. However, stratifying by collaterals, it was observed that dolichoectasia was more likely in the anterior and posterior circulations when collaterals were available among participants with carotid atherosclerosis. These associations were confirmed by noting an increment in arterial diameters in the corresponding arteries ipsilateral and contralateral to each carotid as well as in the posterior circulation. Conclusions We did not find an association of extracranial carotid atherosclerosis with dolichoectasia. However, we found that dolichoectasia is more frequent when intracranial collaterals are available suggesting a compensatory process that needs further investigation.

    Abstract: The Labrador Current is part of the anticlockwise subpolar gyre and plays a major role in the formation of North Atlantic Deep Water. It is influenced by the West Greenland and Baffin currents supplying warmer Atlantic and cold polar waters, respectively. During the early Holocene, at the final stage of the last deglaciation, meltwater and iceberg discharge caused highly variable conditions in the Labrador Current. In order to assess its sensitivity to such freshening, this study provides a well-resolved Holocene paleoclimatic record from the Labrador Shelf. Based on benthic foraminiferal faunal and alkenone biomarker analyses, we differentiated four distinct climatic periods in the western Labrador Sea. From 8.9 to 8.6 ka BP, the Labrador Shelf was dominated by polar water outflow from Baffin Bay and covered by perennial sea ice. Between 8.6 and 7.4 ka BP, a strong subsurface inflow of warmer Atlantic water masses is ascribed to an intensification and redirection of the West Greenland Current. At 7.4 ka BP, the decreased influence of Atlantic water masses on the Labrador Shelf marks the establishment of winter convection leading to the formation of Labrador Sea Water in the central basin. Concurrently, an intensified polar water outflow through the Canadian Gateways strengthened the inner Labrador Current, and higher primary productivity suggests longer spring blooms because of a shorter sea-ice season during the Holocene Thermal Maximum. In the late Holocene after 3 ka BP, periodic fluctuations of primary productivity may tentatively be correlated with stronger and weaker northwesterly winds.
