    Abstract: We present a family with a mitochondrial DNA 3243A 〉 G mutation resulting in mitochondrial myopathy, encephalopathy, lactic acidosis, and stroke-like episodes (MELAS), of which some members have hearing loss in which a novel mutation in the P2RX2 gene was identified. Methods: One hundred ninety-four (194) Japanese subjects from unrelated families were enrolled in the study. Targeted genomic enrichment and massively parallel sequencing of all known nonsyndromic hearing loss genes were performed to identify the genetic causes of hearing loss. Results: A novel mutation in the P2RX2 gene that corresponded to c.601G 〉 A (p.Asp201Tyr) was identified. Two patients carried the mutation and had severe sensorineural hearing loss, while other members with MELAS (who did not carry the P2RX2 mutation) had normal hearing. Conclusion: This is the first case report of a diagnosis of hearing loss caused by P2RX2 mutation in patients with MELAS. A potential explanation is that a decrease in adenosine triphosphate (ATP) production due to MELAS with a mitochondrial 3243A 〉 G mutation might suppress activation of P2X2 receptors. We also suggest that hearing loss caused by the P2RX2 mutation might be influenced by the decrease in ATP production due to MELAS.

    Abstract: The authors reviewed the surgical treatment and outcome in patients with juxtarenal aortic occlusion. From 1975 to 1993, 44 patients (37 men) with a mean age of 62.6 years underwent surgery for this condition. The vascular lesions included renal-level segmental aortic occlusion in 2 patients, infrarenal complete aortic occlusion in 30, and high aortic occlusion with a patent inferior mesenteric artery in 12. Four patients had subacute occlusion and 40 had chronic occlusion. In the chronic group, the symptom was intermittent claudication in 31 cases, rest pain in 4, and gangrene or ulceration in 5. Impotence was present in most of the men. Surgical treatment included transaortic thromboendarterectomy in 2 patients, axillobifemoral bypass in 12, and high aortic thromboendarterectomy plus aortobifemoral bypass in 29. Suprarenal aortic clamping was done in 22 patients, with a mean renal occlusion time of 9.3 minutes. Simultaneous renal artery reconstruction was performed in 3 patients. There were 4 operative deaths (mortality rate 9%), with 2 in the subacute group. Late death from unrelated causes occurred in 9 patients within five years. Lower limb ischemia was cured in all patients and impotence cured in 4 cases. Graft patency was satisfactory after both extra-anatomic and anatomic bypass. Transanal Doppler ultrasound monitoring during the surgery was easy to set and gave the authors useful information about accurate visceral circulation in reference to aortic reconstruction.

    Abstract: The authors studied the effect of prolonged physical exercise on the mechanical properties of rat aorta in relation to the amounts and qualities of arterial connective tissue fibrous proteins. Twelve male rats were divided into two groups: 6 sedentary rats (S) and 6 training rats (T), which were forced to swim from nine weeks to twenty-five weeks of age. The ultimate tensile stress and the ultimate tensile extension ratio of ring specimens at the descending thoracic aorta were larger in T than in S (192.3 ± 47.9 g/mm 2 , mean ± SD, vs 147.8 ± 26.0, p 〈 0.05; 3.52 ± 0.13 vs 3.18 ± 0.27, p 〈 0.05; respectively). The elasticity parameter, calculated by fitting stress-strain curves to exponential function in the stress level of 0-20 g/mm 2 , was lower in T than in S (1.79±0.15 vs 2.13±0.24, p 〈 0.01). The contents of elastin (alkali-insoluble elastin preparation) and collagen were higher in T than in S (0.44 ± 0.11 g/g dry aorta vs 0.30 ± 0.06, p 〈 0.05; 0.15 ± 0.04 g/g dry aorta vs 0.11 ± 0.04, NS, respectively). Although the aortic calcium content did not significantly change in the training rats (T 1.17 ± 0.23 mg/g dry aorta, S 0.95 ± 0.34), the content of calcium in elastin was lower in T than in S (1.75 ± 0.51 mg/g dry elastin vs 2.63 ± 1.00, p 〈 0.05). Elastin isolated from the training rats showed significant decreases in some of the polar amino acids: arginine (3.18±1.00 residues/1000 residues vs 4.12 ± 0.40, p 〈 0.05), threonine (6.79 ± 1.91 vs 8.69 ± 1.51, p 〈 0.05), and serine (9.66 ± 1.56 vs 11.97± 1.41, p 〈 0.05). The authors conclude that prolonged physical exercise from an early age provides training rat with a strong and extensible aorta, which is related to the larger amount of elastin with probably fewer degenerative changes.

    Abstract: Hematoma in the retropharyngeal space (RPS) is a life-threatening condition that leads to rapid airway obstruction. However, the indication for airway management remains unclear. Additionally, the requirement for surgical hematoma evacuation remains undetermined. Therefore, we attempt to suggest some criteria for the management of hematoma in such cases. Methods We report three cases of hematoma in the RPS wherein one patient was treated without surgery and the other two underwent tracheotomy followed by hematoma evacuation. Results We found that airway management should be based on whether the glottis could be visible on laryngoscopy and dyspnea severity. The degree of hematoma, swelling, subcutaneous bleeding in the anterior neck, and emotional stability should also be considered. Proper management during the acute phase may allow for conservative treatments. Hematomas extending below the tracheal bifurcation may help ease upper airway obstruction due to pressure distribution, allowing for conservative treatment. When hematomas are surgically evacuated, tracheotomy should be performed simultaneously. Our report suggests that mediastinal hematoma evacuation could be avoided. Conclusion We should determine a therapeutic strategy for hematoma in RPS based on glottis visualization, patient’s condition, and extent of hematoma growth under careful observation.

    Abstract: We measured cochlear blood flow (CBF) in 55 patients who received cochlear implants, using a laser-Doppler probe placed over the site of drilling in the cochlear bony wall. The subjects included 29 patients with congenital deafness of unknown cause, 8 with idiopathic progressive sensorineural hearing loss, 4 with postmeningitic deafness, 3 with Waardenburg's syndrome, 3 with congenital cytomegalovirus infection, and 8 whose deafness had other causes. There was a wide range of CBF values in patients with congenital deafness of unknown cause. In the patients with idiopathic progressive sensorineural hearing loss, the CBF was significantly lower in patients more than 40 years old. Intracochlear calcification following meningitis appears to be associated with a reduced CBF.

    Abstract: Cotton fabrics and cellulose films treated with fluorocarbon resin (Asahi Guard AG-480 ) with and without crosslinking agents (isocyanate blocked copolymer and aziridine) were washed and subsequently heat treated. Water repellency of the fabrics decreased with washing and recovered with heat treatment. The decreased water re pellency with washing was controlled by using the resin with a crosslinking agent. To investigate surface tension and surface chemical composition of the fabrics after washing and subsequent heat treatment, the contact angle to water and critical surface tension of the films were measured along with an ESCA analysis of the fabrics. Adding cross linking agents to the fluorocarbon resin treatment controlled the decrease in F 1s intensity and the increase in O 1s intensity with subsequent washing.

    Abstract: The etiology of Takayasu's arteritis is unknown; however, abnormality of the immune system plays an important role. To clarify a possible association between inflammatory diseases and Takayasu's arteritis, the authors retrospectively reviewed the medical records of 36 patients (2 men and 34 women), admitted to their department with this diagnosis. They found that 11 of the 36 patients had at least 1 other chronic or subacute inflammatory disease: Chronic tonsillitis, tuberculous lymphadenitis of the neck, chronic hepatitis B (infectious diseases); pyoderma gangrenosum, erythema nodosum (skin lesions); subacute thyroiditis, chronic thyroiditis (autoimmune diseases); Crohn disease, and ulcerative colitis (inflammatory diseases with unknown etiology). The present observation revealed a high prevalence of other inflam matory diseases in patients with Takayasu's arteritis and raises the possibility that Takayasu's arteritis may be associated with abnormality of the immune system activated by other inflam matory diseases, such as infection, autoimmune diseases, or inflammatory diseases of unknown origin.

    Abstract: Many studies have shown the existence of cycles of approximately 90 to 100 minutes (corresponding to Kleitman's basic rest-activity cycle) and several hours (‘slow ultradian rhythm’ cycles). EEG power spectra, mood, performance, and self-evaluation of performance were measured every 15 minutes for 9 hours for 10 male university students. Principal component analysis was applied to extracted ultradian fluctuations in EEG activity, task performance, and the subjective variables. The analysis indicated that two common temporal fluctuations were present, one in the behavioral and subjective variables, and the other in EEG activity. Spectral analysis indicated that the former fluctuated at a rate of 12 cycles per day (corresponding to basic rest-activity cycle), and the latter was comprised of both a slower (6 cycles per day) and a faster (10 to 18 cycles per day) cycle, thereby supporting the multioscillator hypothesis of ultradian rhythm.
