    Kurzfassung: This study aimed to elucidate the structure of the Rivermead Postconcussion Symptoms Questionnaire (RPQ) and evaluate its longitudinal and group variance. Factor structures were developed and compared in 1,011 patients with mild traumatic brain injury (mTBI; i.e., Glasgow Coma Scale score 13-15) from the Transforming Research and Clinical Knowledge in TBI study, using RPQ data collected at 2 weeks, and 3, 6, and 12 months postinjury. A bifactor model specifying a general factor and emotional, cognitive, and visual symptom factors best represented the latent structure of the RPQ. The model evinced strict measurement invariance over time and across sex, age, race, psychiatric history, and mTBI severity groups, indicating that differences in symptom endorsement were completely accounted for by these latent dimensions. While highly unidimensional, the RPQ has multidimensional features observable through a bifactor model, which may help differentiate symptom expression patterns in the future.

    Kurzfassung: Recent advances in medical care have provided more effective therapies for the treatment of various malignancies and increased the number of successful bone marrow and organ transplantations. However, these advances often place the patient in a severely immunocompromised state for several days or weeks resulting in one or more life-threatening infections. This article discusses some general principles, current pharmacotherapeutic strategies, and novel treatment strategies for the management of immunocompromised patients.

    Kurzfassung: The Personality Assessment Inventory (PAI) is a new generation multiscale inventory that is widely used in clinical settings. The current study examined its convergent validity with emergency referrals in a metropolitan correctional facility. A sample of 80 referrals completed two phases of an assessment study. As a measure of convergent validity, results on select PAI scales were compared to the Schedule of Affective Disorders and Schizophrenia (SADS), the Structured Interview of Reported Symptoms (SIRS), and the Suicide Probability Scale (SPS). Overall, results indicated moderate to good convergent validity for the following: (a) screening for feigned profiles, (b) establishing clinical correlates of common disorders, and (c) evaluating the potential for suicidal ideation.

    Kurzfassung: This study evaluates the factor structure and concurrent validity of a measure of self-care, the Denyes Self Care Practice Instrument (DSCPI-90©), when used with adults. Three hundred eight participants complete the 18-item DSCPI-90© and a measure of general health status. Data are examined using descriptive statistics, exploratory factor analysis, and correlation. An efficient 12-item, two-factor model is identified. Reducing the number of items from 18 to 12 does not have an untoward effect on either scale reliability or the amount of variance explained. Correlations with a measure of health status are modest, lending validity to the construct of self-care as being distinct from, yet related to, health status. It is concluded that the 12-item version of the DSCPI© is reliable and valid when used with adults.

    Kurzfassung: Methylene Blue (MB) is a brain-penetrating drug with putative neuroprotective, antioxidant and metabolic enhancing effects. In vitro studies suggest that MB enhances mitochondrial complexes activity. However, no study has directly assessed the metabolic effects of MB in the human brain. We used in vivo neuroimaging to measure the effect of MB on cerebral blood flow (CBF) and brain metabolism in humans and in rats. Two doses of MB (0.5 and 1 mg/kg in humans; 2 and 4 mg/kg in rats; iv) induced reductions in global cerebral blood flow (CBF) in humans (F (1.74, 12.17) 5.82, p = 0.02) and rats (F (1,5) 26.04, p = 0.0038). Human cerebral metabolic rate of oxygen (CMRO 2 ) was also significantly reduced (F (1.26, 8.84) 8.01, p = 0.016), as was the rat cerebral metabolic rate of glucose (CMRglu) (t = 2.6 (16) p = 0.018). This was contrary to our hypothesis that MB will increase CBF and energy metrics. Nevertheless, our results were reproducible across species and dose dependent. One possible explanation is that the concentrations used, although clinically relevant, reflect MB’s hormetic effects, i.e., higher concentrations produce inhibitory rather than augmentation effects on metabolism. Additionally, here we used healthy volunteers and healthy rats with normal cerebral metabolism where MB’s ability to enhance cerebral metabolism might be limited.

    Kurzfassung: The U.S. Environmental Protection Agency's MOBILE emission rate model is used to evaluate carbon monoxide, hydrocarbon, nitrogen oxide, and particulate matter impacts of major transportation projects. Using fleet characteristics, environmental conditions, and on-road operating conditions, MOBILE estimates fleet average emission rates. Coupling projected traffic volumes and physical roadway characteristics yields pollutant mass flux rates with which dispersion models predict downwind pollutant concentrations. Distance-flux relationships may also prove useful in pollutant exposure modeling to help quantify relative environmental risks of living or working near major roadways. Applying the MOBILE model requires the creation and execution of scenario-specific input files, which must be properly structured. Researchers developed the MOBILE-matrix model to facilitate more readily the use of MOBILE emission rates in transportation modeling. The matrix creates large emission rate lookup tables that can be applied to traffic in any facility or subregion. It performs all necessary model runs well before a future modeling need. The goal of this specific application is to integrate MOBILE-matrix capabilities into the multimodal transportation planning tool (MTPT) employed by the Georgia Department of Transportation so that it can be used to estimate emissions from rural road facilities throughout the state. Thousands of MOBILE6.2 runs were executed to generate database output files, which were post-processed to create MOBILE-matrix lookup tables for 159 Georgia counties. For each transportation link in the MTPT, emission rates are extracted from the lookup matrix by facility type, average speed, temperature, and so forth. The predicted emissions can then be used for a variety of transportation planning purposes.

    Kurzfassung: Considerable efforts have been made over the past decade to address vitamin and mineral deficiencies. An increasing number of countries in the Association of Southeast Asian Nations (ASEAN) are adopting mandatory food fortification as one of the primary strategies to overcome these deficiencies. Experience shows that fortified foods can reach large parts of the population, including the poor, if the fortification is done on a mandatory rather than a voluntary basis and if the food vehicle is widely consumed. Objective To review the importance of regulatory monitoring as an essential component of food fortification efforts in selected ASEAN countries, with special focus on the available information on regulatory monitoring systems for iodized salt and fortified wheat flour. Methods The role of regulatory monitoring in strengthening food fortification programs was discussed during a joint regional meeting of the World Health Organization, UNICEF, the Flour Fortification Initiative, the Global Alliance for Improved Nutrition, the Micronutrient Initiative, and the World Bank on regulatory monitoring of salt and wheat flour fortification programs in Asia, which took place in Manila, Philippines, on 27–29 September 2011. This paper reviews the regulatory monitoring systems of selected ASEAN countries that participated in this meeting. Results Problems and challenges in regulatory monitoring systems for iodized salt and fortified wheat flour in selected ASEAN countries are identified, and a description of the role of regulatory monitoring in strengthening food fortification initiatives, particularly of salt and flour, and highlights of areas for improvement are presented. Conclusions Regulatory monitoring consists of monitoring activities conducted at the production level, at customs warehouses, and at retail stores by concerned regulatory authorities, and at the production level by producers themselves, as part of quality control and assurance efforts. Unless there are appropriate enforcement and quality assurance mechanisms in place to stimulate compliance by food producers, i.e., regulatory monitoring, having national legislation will not necessarily lead to increased coverage of fortified products and associated outcomes.

    Kurzfassung: The functional perspective is a normative approach to describing and predicting group performance that focuses on the functions of inputs and/or processes. The aim of theory and research from this perspective is to understand why some groups are successful and others are not. This article investigates theory and, to a lesser extent, research of small groups based on the functional perspective. The authors present the underlying theoretical assumptions and review theories that fit into the functional perspective from several representative areas of research. They conclude by outlining notable strengths and weaknesses associated with viewing groups from this perspective and propose some directions for future theory development.

    Kurzfassung: Fine cell granularity in modern climate models can produce terabytes of data in each snapshot, causing significant I/O overhead. To address this issue, a method of reducing the I/O latency of high-resolution climate models by identifying and selectively outputting regions of interest is presented. Working with a global cloud-resolving model and running with up to 10,240 processors on a Cray XE6, this method provides significant I/O bandwidth reduction depending on the frequency of writes and the size of the region of interest. The implementation challenges of determining global parameters in a strictly core-localized model and properly formatting output files that only contain subsections of the global grid are addressed, as well as the overall bandwidth impact and benefits of the method. The gains in I/O throughput provided by this method allow dual output rates for high-resolution climate models: a low-frequency global snapshot as well as a high-frequency regional snapshot when events of particular interest occur.
