    Abstract: In this paper we present an improved zero-power controlled magnetic suspension system that has the capability to adjust stiffness. The conventional zero-power control system produces constant negative stiffness, and the stiffness solely depends on the capacity of the permanent magnets or the gap–force coefficient of the magnets. This becomes an obstacle to the fields of application of zero-power control where the adjustment of stiffness is necessary, such as developing a vibration isolation system. In this research, the basic zero-power control system is modified such that it can adjust stiffness by adding a minor proportional feedback of displacement to the zero-power controller. Finally, a vibration isolation system is developed by combining a positive stiffness spring in series with a negative stiffness suspension realized by the modified zero-power controller. The proposed control system and the developed vibration isolation system are investigated both analytically and experimentally.

    Abstract: This study was performed to investigate serum procalcitonin (PCT) and albumin (Alb) as prognostic biomarkers in elderly patients at risk of bacterial infection. Methods Serum PCT was measured in 270 hospitalized patients (mean age, 77.4 years) with suspected bacterial infection. The PCT-negative ( 〈 0.5 ng/mL) and PCT-positive (≥0.5 ng/mL) groups comprised 155 and 115 patients, respectively. Logistic regression analysis was performed with various clinical laboratory test values as independent variables and PCT positivity/negativity as the dependent variable. Results C-reactive protein (CRP) was the only independent variable significantly associated with PCT positivity/negativity. In the survival analysis, the 30-day in-hospital death rate was significantly higher in the PCT-positive than -negative group. Within the Alb-positive group ( 〉 2.5 g/dL), no significant difference in survival was observed between the PCT-positive and -negative groups. However, within the Alb-negative group (≤2.5 g/dL), the survival rate was significantly lower in the PCT-positive than -negative group. PCT was strongly associated with CRP and Alb, and having both PCT positivity and Alb negativity was a prognostic factor for elderly people at risk of bacterial infection. Conclusions Combined measurement of PCT with Alb is expected to be a valuable tool to assess prognosis in elderly people at risk of bacterial infection.

    Abstract: Endothelium-derived nitric oxide (NO) plays an important role in the prevention of platelet aggregation and adhesion to the vascular wall. Endothelial nitric oxide synthase (eNOS) and L-arginine/NO pathway are both present in human platelets. Platelet-derived NO inhibits excessive activation and aggregation of platelets. However, the expression level of the eNOS gene in human platelets has yet to be elucidated. The current study investigates the individual expression level of platelet eNOS mRNA using the real-time reverse transcriptase-polymerase chain reaction (RT-PCR) detection method. eNOS mRNA expression was examined in platelets isolated from 50 subjects: 11 male smokers, 15 male nonsmokers, and 24 female non-smokers. After extraction of platelet total RNA, eNOS (target) and GAPDH (internal control) mRNA expression levels were quantitated using real-time RT-PCR. The expression levels of eNOS mRNA (relative copy numbers) were significantly lower in male smokers (59±17) than in male nonsmokers (195±71, P 〈 .03), and higher in female nonsmokers (285±60) than in the male nonsmokers (195±71, P 〈 .03). By multiple linear regression analysis, cigarette smoking ( P = .008) and diabetes mellitus ( P = .047) were found to be significantly negative predictors, and antioxidant (vitamin E) treatment ( P = .01) was a significantly positive predictor of platelet eNOS mRNA expression. Age, other medications, and other risk factors for coronary artery disease were not significant. Using this method, eNOS mRNA abundance in human platelets was detected and quantitated in real-time. The intraplatelet eNOS mRNA expression levels were significantly decreased in cigarette smokers. Low platelet NO synthesis in smokers may result in the augmentation of platelet aggregation and thrombus formation, developing into acute coronary syndromes.

    Abstract: The environment surrounding Helicobacter pylori eradication treatment is dramatically changing. Recently, vonoprazan, a first-in-class potassium-competitive acid blocker (P-CAB), was introduced onto the market in 2015. The aging of Japan’s demographic structure is becoming pronounced. In this study, we examined the trend of the eradication rate of H. pylori in the metropolitan area and examined factors concerning successful eradication. Methods: We collected data from 20 hospitals in the Tokyo metropolitan area on patients who received first-line eradication therapy with a proton-pump inhibitor (PPI)/P-CAB, amoxicillin, and clarithromycin for 1 week and second-line eradication therapy with a PPI/P-CAB, amoxicillin, and metronidazole for 1 week from 2013 to 2018. The annual eradication rate and associated factors for successful eradication were analyzed. Results: We collected data of 4097 and 3572 patients in the first- and second-line eradication therapies, respectively. The eradication rate decreased from 2013 to 2014 and increased again from 2015 to 2018 with the first-line therapy [the eradication rates in 2013, 2014, 2015, 2016, 2017 and 2018 were 71.8%, 63.7%, 78.5%, 84.6%, 89.7 and 90.1%, respectively, in the per protocol (PP)]. The second-line eradication rates were 90.0%, 82.6%, 88.8%, 87.5%, 91.8% and 90.1% in 2013, 2014, 2015, 2016, 2017 and 2018, respectively, in PP. Vonoprazan was an independent factor for successful eradication in not only first-line, but also second-line eradication. Age over 75 years was an independent factor for eradication failure in both first- and second-line eradication therapies. Conclusion: The eradication rate improved from 2015 to 2018 with the first-line therapy because of the introduction of vonoprazan in the market. The eradication rates with first- and second-line regimens in elderly patients were lower than those in younger patients.

    Abstract: A control method for achieving zero compliance to direct disturbance is proposed for a vibration isolation system. The structure of the proposed zero-compliance system is as follows. A middle mass is suspended by a spring from the base. The isolation table is suspended and moved by a linear actuator that is fixed to the middle mass. When direct disturbance acts on the isolation table, the middle mass begins to move in the same direction as the disturbance. The actuator operates to move the isolation table in the opposite direction to cancel the displacement of the middle mass to the base. To verify the effectiveness of this principle, a single-axis apparatus was fabricated. In this apparatus, the isolation table and the middle mass are guided to move horizontally to study characteristics without the influence of gravity. A voice coil motor (VCM) is installed between the isolation table and the middle mass, and integral—proportional—derivative (I-PD) control is implemented to achieve displacement cancellation control. Another VCM is installed between the middle mass and the base, and PD control is implemented for realizing the prescribed stiffness and damping. The efficacy of the designed control system is confirmed experimentally.

    Abstract: Complex degenerative tears of the medial meniscus in the knee are usually treated using meniscectomy. However, this procedure increases the risk of osteoarthritis, while other treatments aimed at meniscal repair remain challenging due to the high possibility of failure. The use of synovial mesenchymal stem cells (MSCs) is an attractive additional approach for meniscal repair, as these cells have high proliferative and chondrogenic potential. In this case report, we surgically repaired a complex degenerative tear of the medial meniscus and then transplanted autologous synovial MSCs. We evaluated clinical outcomes at 2 years and assessed adverse events. We enrolled patients with clinical symptoms that included a feeling of instability in addition to pain caused by their complex degenerative tears of the medial meniscus. Two weeks after surgical repair of the torn meniscus, autologous synovial MSCs were transplanted onto the menisci of five patients. The total Lysholm knee score, the Knee Injury and Osteoarthritis Outcome Scale scores for “pain,” “daily living,” “sports activities,” and the Numerical Rating Scale were significantly increased after 2 years. Three adverse events, an increase in c-reactive protein, joint effusion, and localized warmth of the knee were recorded, although these could have been due to the meniscal repair surgery. This first-in-human study confirmed that the combination of surgical repair and synovial MSC transplantation improved the clinical symptoms in patients with a complex degenerative tear of the medial meniscus. No adverse events occurred that necessitated treatment discontinuation. These findings will serve as pilot data for a future prospective study.

    Abstract: Cardiac amyloidosis usually presents with heart failure, but rarely leads to coronary insuffi ciency. The authors report a case of a 69-year-old Japanese woman with cardiac amyloidosis presenting as microvascular angina. She had exertional angina with positive exercise test and normal coronary angiograms. However, heart failure developed, and she died 3 years after symptom onset. On autopsy, coronary arteries were patent. In contrast to that of the epicar dial coronary arteries, histologic examination of the heart revealed severe obstructive alter ations of the intramural coronary arteries with amyloid. Furthermore, amyloid was present mainly in the endocardium and the intramural coronary arteries, but there was little present in the myocardium. This case was a rare AL amyloidosis. There have been only 4 reported cases of cardiac amyloidosis that presented with exertional angina, a positive exercise test, and normal coronary angiograms and histologic examination.

    Abstract: To assess the effect of 12 months of treatment with tofogliflozin on electrolytes and dehydration in Japanese patients with type 2 diabetes mellitus (T2DM) Methods This retrospective study involved mainly elderly patients with T2DM who had received tofogliflozin for 12 months. Data on glycated haemoglobin (HbA1c), serum electrolytes (sodium, potassium, chloride), haematocrit, estimated glomerular filtration rate (eGFR) and blood urea nitrogen (BUN)/creatinine ratio were retrieved and analysed. Results Data from 69 patients (77% of whom were ≥65 years) showed that there was a significant reduction in HbA1c over the 12-month treatment period with tofogliflozin. However, the drug had no significant effect on levels of haematocrit, electrolytes, eGFR or BUN/creatinine ratio. Conclusion This retrospective analysis of data from mainly elderly Japanese patients with T2DM showed that 12-month administration of tofogliflozin exhibited glucose-lowering capabilities with accompanying low risk of electrolyte abnormalities and dehydration.

    Abstract: Mass measurement using relay feedback of velocity and restoring force compensation is investigated for determining the mass of an object under weightless conditions. In the measurement system, the velocity of the object is fed back through a relay with hysteresis and the force acting on the object is switched from a positive value to a negative value when the velocity reaches a positive threshold and vice versa. As a result, a limit cycle is induced in the measurement system and the mass is estimated based on the period of the limit cycle. In addition, restoring force compensation with a spring is introduced to avoid the drift of the trajectory. This compensation makes the static equilibrium state unique. However, the trajectory still drifts slightly. It causes some error in measurement when a simple formula of estimating mass is applied. To eliminate such an error, a new formula is derived to estimate the mass independently of the position of the trajectory that is determined by the switching positions in the relay actions. When the switching positions deflect from the origin at which the spring is in the natural length, the trajectory is not at the center and becomes asymmetric. It is analytically shown that the period of the limit cycle is minimum when the switching positions are at the origin. It indicates that mass is overestimated with the simple estimation formula when the trajectory is not at the center. The validity of the modified formula and the analytical results are confirmed experimentally.
