    Abstract: This investigation focuses on the prevalence of motor, language, perceptual and memory impairments in long-term survivors of subarachnoid haemorrhage. The prevalence of different impairments was as follows: language 10%, motor 25%, perception 45% (a prevalence that may be age-dependent), and memory 73%. A total of 82% of the subjects had at least one impairment, and impairments of perception and long-term memory were particularly common with a ruptured anterior communicant artery. A factor analysis showed that perceptual function/impairment could be separated into two entities: lower- versus higher-order perception. Another factor analysis separated higher-order perception from memory, casting doubt upon assessment methods that aggregate these two entities.

    Abstract: Levels of satisfaction with life as a whole (happiness) and with eight different domains were investigated using mailed questionnaires in four age cohorts (25-, 35-, 45- and 55-year-olds) of men and women. With a few exceptions (vocational and financial satisfaction) levels of global and domain-specific satisfaction were not age-dependent and few gender differences were found. The generally high levels of satisfaction correspond well to those found in the USA and in Germany. Satisfaction with expressive (emotion-related) domains was greater in women than in men, and the provider items - satisfaction with vocational and financial situation -were influenced by age. The eight domains formed three meaningful factors: the first characterized satisfaction derived from expressive goals; the second from spare-time goals; and the third factor was instrumental (performance-related), characterizing satisfaction derived from provider goals. The three factors predicted gross level of happiness (happy/not happy) for 82% of subjects with complete answers, all three factors being significant predictors.

    Abstract: The clinical results in 29 patients who had undergone ACL reconstruction with quadriceps-patellar tendon graft over-the-top were studied at an average of 28 months postoperatively. Postoperative immobilization was at either 30° (N = 17) or 70° (N = 12) of knee flexion for 6 weeks. All patients then followed a com mon preparatory training program for 8 weeks. In the 14th postoperative week the patients were put on either progressive resistance training (N = 12) or isokinetic training (N = 17) for 6 weeks, after which all had identical training. Independent of primary knee immobilization angle or training regimens, no differences could be demon strated with respect to stability, range of motion, func tion, or isokinetic mechanical output. For all patients, Lysholm knee function score improved from 60.5 to 93.6 (P 〈 0.001) and activity level from 3.9 to 6.3 (P 〈 0.001). Isokinetic peak torque, contractional work, and mean power of the quadriceps at 30, 90, and 180 deg/ sec were lower in the injured compared to the nonin jured leg (78% to 90%; P 〈 0.01) both preoperatively and at followup, and there were no significant differ ences between the preoperative and follow-up values. Clinical stability improved (P 〈 0.001) in 19 of the 29 patients (66%) and in 11 of those 17 patients with early isokinetic training (65%). Performance tests were nor mal in 23 of 29 patients (79%), with minor abnormalities in the rest. We conclude that immobilization at a higher tension of the quadriceps or early isokinetic training did not results in any advantages in the long run, and that patients with early isokinetic training did not show any increased incidence of remaining instability.

    Abstract: On four occasions during a period of 17 months, iso kinetic maximum knee extensor output (peak torque and contractional work) and input (integrated electro myographic activity) during single and repetitive con tractions were measured in 10 male elite ice hockey players. The tests were performed in the middle and at the end of the competitive season, and after two off ice training periods involving different types of training. No significant changes in peak torque at single max imum contractions occurred for the whole group of players during the investigation. Nor did the slope of decline in contractional work or the plateau level of the ratio of contractional work:integrated electromyo graphic activity during repetitive contractions change significantly. The most obvious finding was great inter individual differences in muscular performance. Intrain dividually, changes in both peak torque as well as in slope of decline in contractional work and in plateau level of the ratio of contractional work:integrated elec tromyographic activity occurred during the different training periods. We conclude that local muscle adap tations, measured by isokinetic procedures, occur in the quadriceps as a result of ordinary training and games in elite ice hockey players. Because of the great interindividual differences in muscular performance and the different responses to similar training, we suggest individual programs for leg muscle training in ice hockey players.

    Abstract: The influence of the histone deacetylase (HDAC)-inhibitor, valproic acid (VPA), on bladder cancer cell adhesion in vitro was investigated in this paper. TCCSUP and RT-112 bladder cancer cells were treated with VPA (0.5 or 1 mM) twice or thrice weekly for 14 days. Controls remained untreated. Tumour cell interaction with immobilized collagen was evaluated by a flow-based adhesion assay using a shear force of 2 or 4 dyne/cm 2 . The effects of VPA on the integrin adhesion receptors α3, α5, β1, β3 and β4 were assessed by flow cytometry to determine integrin surface expression and by western blotting to determine the cytoplasmic integrin level. VPA of 0.5 mM and 1 mM significantly prevented binding of both RT-112 and TCCSUP cells to collagen, compared with the untreated controls. Adhesion was reduced to a higher extent when RT-112 (subjected to 2 dyne/cm 2 ) or TCCSUP (subjected to 2 or 4 dyne/cm 2 ) tumour cells were treated with VPA three times a week, compared to the two times a week protocol. VPA caused a significant up-regulation of the integrin α3, α5, β1, β3 and β4 subtypes on the TCCSUP cell surface membrane. In RT-112 cells, only integrin α5 was elevated on the cell surface following VPA exposure. Western blotting revealed an up-regulation of α3, α5, β3 and β4 integrins and down-regulation of the integrin β1 protein by VPA in TCCSUP. VPA also up-regulated α5 and down-regulated β1 integrin in RT-112 cells, but also reduced α3 and β3 in TCCSUP. VPA exerted adhesion-blocking properties on bladder cancer cells under physiologic flow conditions. The effects were accompanied by distinct modifications of the integrin expression profile, which differ depending on the cell lines used. Application of VPA might be an innovative option to prevent bladder cancer dissemination.

    Abstract: Using responses from mailed checklists, the actual and expected values of 10 occupational roles were identified for 201 randomly selected 25-, 35-, 45-, and 55-year-old Northern Swedish subjects, who also reported their levels of satisfaction with life as a whole and with eight domains of life. The 10 occupational roles were clustered into four factors labeled “meta-roles”: family, vocational, and two leisure meta-roles. The role preferences remained stable for men and women age 35 and 45; however, 25-year-olds expected to gain some roles and 55-year-olds expected to lose the worker role in 10 years. Hence, an age-related role career was identified. Several of the 10 occupational roles and the meta-roles (factors) exhibited logical adaptations to circumstances such as gender and parenthood, as well as role career. The meta-roles were closely related to level of satisfaction within six of the eight domains, indicating that fulfillment of different occupational roles is important for different aspects of life satisfaction.

    Abstract: Visual perceptual performance (12 items) and motor behavior were assessed in 109 patients within 2 weeks poststroke and were related to self-care activities of daily living (ADL) proficiency. A cluster analysis revealed that scores for motor behavior and self-care ADL were closely associated whereas visual-perception items could be separated into two groups One of these incorporated relatively meaningless (Low-Order Perception [L-OP]) and the other relatively meaningful (High-Order Perception [H-OP] ) stimuli. When the disability parameter, ADL, was excluded factor analyses identified three orthogonal factors—motor behavior, H-OP, and L-OP—which were similar to those of the cluster analysis and explained about 75% of the variance. Standardized factor scores were sizable predictors for self-care ADL-score for motor behavior and H-OP, but not for L-OP. The present findings of grouping of visual perception into two factors are contradictory to espoused categorizations and may shed new light on occupational therapy research, functional diagnosis, and the treatment of stroke patients.

    Abstract: An accepted landmark to assess feasibility of surgical aortic valve replacement (SAVR) via right anterolateral minithoracotomy (RALT) is the aortic-midpoint to right-sternal-edge distance. We aimed to evaluate single left lung positive-end-expiratory-pressure (SLL-PEEP) ventilation inducing an intraoperative rightward shift of the ascending aorta to improve exposure. Methods Nineteen patients with aortic stenosis undergoing SAVR via RALT were prospectively analyzed. SLL-PEEP ventilation (20,395 cmH 2 O) via a double-lumen endotracheal tube was applied immediately before transthoracic aortic cross-clamping, thereby inducing rightward shift of the ascending aorta to enhance exposure. We analyzed preoperative computed tomography (CT) reconstructions and intraoperative video recordings. Primary endpoint was extent of rightward shift induced by SLL-PEEP ventilation; secondary endpoints were procedure times and safety events. Results Mean age was 61 ± 14.8 years and 6 of 19 (31.6%) were female. Mean EuroSCORE II was 0.81% ± 0.04%, STS-PROM was 1.13% ± 0.74%, and mean aortic rightward shift induced by SLL-PEEP ventilation was 10.32 ± 4.14 mm (4 to 17 mm; P = 0.003). Median shift in the group considered suitable for the RALT approach by preoperative CT-scan evaluation was 14.2 mm (IQR 11) and in the less suitable group 11.5 mm (IQR 5). Mean procedure time was 167 ± 28.9 min, CPB time was 105.7 ± 18.4 min, and cross-clamp time was 64.5 ± 13 min. Fifteen patients (79%) received SAVR via RALT with implantation of a bioprosthesis, whereas a rapid-deployment-prosthesis was used in 4 patients (21%). Ten of 19 (53%) patients who were classified as less suitable preoperatively received SAVR via RALT after SLL-PEEP ventilation. No strokes were observed. Conclusions The SLL-PEEP ventilation maneuver during SAVR via RALT significantly enhances aortic exposure. There were no safety events associated with this maneuver and we were able to demonstrate significant rightward aortic shift in every single patient.

    Abstract: Objective: To compare the effect of walking training on a treadmill with body weight support (BWS) and walking training on the ground at an early stage of rehabilitation in patients with hemiparesis after stroke. Design: Randomized controlled experimental study. Setting: Multicentre design; three departments of rehabilitation medicine. Subjects: Seventy-three consecutive first stroke patients admitted to a rehabilitation clinic were randomized into a treatment group and a control group. Interventions: The treatment group received walking training on a treadmill with BWS for 30 minutes, 5 days a week. The control group received walking training according to the Motor Relearning Programme (MRP) on the ground for 30 minutes 5 days a week, not including treadmill training. During the time in the rehabilitation department (about two months), all patients in the study also received professional stroke rehabilitation besides the walking training in the two groups. Main outcome measures: Functional Independence Measure (FIM), walking velocity for 10 m, Functional Ambulation Classification (FAC), Fugl-Meyer Stroke Assessment and Berg's Balance Scale. The assessments were performed at admission, at discharge and at 10-month follow-up. Results: There were no statistically significant differences between the groups at discharge or at the 10-month follow-up with regard to FIM, walking velocity, FAC, Fugl-Meyer Stroke Assessment, and Berg's Balance Scale. Patients in both groups improved in these variables from admission to the 10-month follow-up. Conclusions: Treadmill training with BWS at an early stage of rehabilitation after stroke is a comparable choice to walking training on the ground.
