    Abstract: Nestin, which was initially identified as a marker of neural stem cells, has been reported in regenerating pancreas as well as in early embryonic stem (ES) cell derivatives. However, little is known about its specific roles in stem cells as a functional regulator. We investigated the source of the action of nestin in ES and adult pancreatic ductal stem (PDS) cells in regard to the neogenesis of insulin-secreting β-cells. In ES cells, suppression of nestin by gene silencing led to an increased expression of the pluripotency-associated genes, including Oct 4, Nanog, and SSEA-1, before embryoid body (EB) formation, whereas it reduced endodermal and pancreatic transcription factors in EBs. Inhibition of nestin expression in adult PDS cells caused a low expression of pancreatic transcription factors and islet hormones, leading to poor β-cell development and insulin secretion. These data may indicate not only that nestin is a simple stem cell marker, but also that it constitutes a functional factor at the time of stem cell differentiation. We suggest that nestin plays pivotal roles as an intermediate regulator governing both stemness and differentiation of stem cells in the process of their differentiation into insulin-secreting cells.

    Abstract: We investigated the physical traits, school performance, and behavior of Korean children with sleep-disordered breathing (SDB). Methods: We recruited 679 students from an elementary school in Seoul, Korea. We used a survey to collect information on the absence or presence of SDB at both the child's preschool age and his or her current age and on the degree of behavioral disturbance. Physical traits and examination scores were also analyzed. We divided the children into 4 groups: Non-SDB group, past SDB group, recent SDB group, and continuous SDB group. Comparisons between these four groups were conducted. Results: Sixty-one students were excluded because of incomplete information. The current body mass index was significantly higher in the past (19.7 ± 3.6), recent (21.2 ± 3.6), and continuous SDB groups (20.7 ± 3.9) than in the non-SDB group (18.8 ± 3.2), but only for male students (p 〈 0.001). The examination scores were not different among the four groups, but the behavioral disturbance scores were much higher in the past, recent, and continuous SDB groups than in the non-SDB group for both genders. Conclusions: Among these Korean elementary school students in the upper grade levels, the presence of current or past SDB appeared to influence the current body mass index in boys and the presence of behavioral disturbances in both genders. However, SDB was not associated with school performance.

    Abstract: It is known that children with sleep-disordered breathing (SDB) often have accompanying growth retardation and learning and behavior disabilities. However, these results are based mainly on studies of children from European and North American countries. The objective of this study was to investigate the school performance and behavior of Korean children with SDB. Methods: We enrolled 302 third-grade elementary students from an elementary school in Seoul. A survey was conducted, using information from the children's parents for the diagnosis of SDB. The children's height, weight, midterm examination scores, and behavioral disturbances were analyzed. Results: Overall, 299 parents returned the survey. Of the 299 students, 29 (9.7%) were considered to have SDB. They showed no difference from the control in terms of body mass index. The mean examination score was higher in the control group than in the SDB group. However, there was a statistical difference only in social science. The prevalences of students who were inattentive (58.6% versus 23.7%) and lacking in self-control (44.8% versus 14.1%) were significantly higher among students with SDB. Conclusions: SDB is not closely associated with poor school performance among Korean elementary students. However, behavioral disturbance is more frequent among the students with SDB than in those without.

    Abstract: For enhancing the security of ubiquitous communication, we have to consider three keywords: mobility, wireless, and low computing capability. In this paper, we study one of suitable security protocols for the ubiquitous communication environment. We discuss RSA-based password-authenticated key exchange (RSA-PAKE) protocols for imbalanced wireless networks where a party uses a low-power device to communicate with another party equipped with a powerful computing device. For imbalanced wireless network applications, it is important to reduce the cost of communication for a low-power device even though the cost for powerful devices is increasing. The most power-consuming operation in RSA-PAKE protocols is the reliability test of unauthorized RSA public keys. Hence, it is important to design an efficient reliability test method to construct an efficient RSA-PAKE protocol. In this paper, we propose a new reliability test technique and design a provably secure RSA-PAKE protocol using the technique. Our protocol is suitable for securing the communications conducted over imbalanced wireless networks since the operations computed by one communicating party are efficient enough to be implemented on most low-power devices such as mobile phones and PDAs. The cost of a low-power device is reduced by 84.25% compared with CEKEP, the most efficient RSA-PAKE protocol. We prove the security of our protocol under a firmly formalized security model.

    Abstract: Glycated hemoglobin A1c (HbA1c) is widely used for monitoring and diagnosing human diabetes mellitus, but is rarely used in veterinary clinics. The goal of our study was to validate the commercial HbA1c testing system SD A1cCare analyzer (Bionote, Gyeoggi-do, South Korea) for use in dogs. Dogs were recruited with owner’s consent. Diabetic status was determined based on clinical signs, fasting hyperglycemia, and glycosuria. Intra-assay precision and linearity were evaluated with EDTA, heparin, or citrate as anticoagulants, and had excellent precision with mean coefficients of variation (CVs) of 2.47%, 2.26%, and 1.92%, respectively. Diluted anticoagulated blood samples showed excellent linear relationships with R 2 of 0.991, 0.996, and 0.994, respectively. Inter-assay precision revealed that the mean CV of the normal control was 2.18% and that of the high control was 2.01% (30 repeats). Observed total error of a normal control was 7.81%, and 6.12% for the high control. HbA1c level measured before and after removal of plasma and replacement by saline showed minimal interference by lipid contents ( p = 0.929). The HbA1c concentrations of diabetic dogs were significantly higher than those of non-diabetic dogs ( p 〈 0.001). HbA1c value 〉 6.2% indicated canine diabetes through a classification and regression tree model. In most cases, fructosamine and HbA1c were highly correlated ( r = 0.674, p 〈 0.001). The HbA1c testing system could be a valuable testing system to evaluate canine diabetes mellitus, providing an alternative in-house option for use by veterinary clinicians.

    Abstract: Among the risk factors for cerebrovascular/cardiovascular disease or thromboembolic events caused by the administration of second-generation antipsychotics, clinicians have mainly focused on metabolic side effects, with little interest in the effects on platelet activity. Because excessive platelet activity can increase the risk for cerebrovascular/cardiovascular disease, the aim of this study was to investigate the effect of second-generation antipsychotics on platelet activity in patients with schizophrenia. Methods The medical records of patients with schizophrenia who were treated with second-generation antipsychotics were retrospectively reviewed. The degree of platelet activation was assessed by measuring the mean platelet component. Results Wilcoxon signed-rank test revealed that mean platelet component levels were significantly decreased by the administration of second-generation antipsychotics (V = 20; p  〈  0.05), suggesting that the administration of second-generation antipsychotics may increase platelet activation. Conclusion Because platelet activation is an additional risk factor for the occurrence of cerebrovascular/cardiovascular disease, results of this study suggest that clinicians should carefully monitor the degree of platelet activation after the administration of second-generation antipsychotics.

    Abstract: We investigated the effect of transforming growth factor-β1 (TGF-β1) on the differentiation of nasal polyp–derived fibroblasts (NPDFs) and on their production of collagen, and investigated the effect of peroxisome proliferator-activated receptor γ (PPARγ) agonists on the effects of TGF-β1. Methods: Primary fibroblast lines were established from 5 human nasal polyp tissues. We activated the NPDFs with TGF-β1 and exposed them to the PPARγ agonists ciglitazone and 15-deoxy-A-prostaglandin J2. The expression of α-smooth muscle actin (α-SMA) and collagen was measured by reverse transcriptase–polymerase chain reactions, immunocytochemical staining, and collagen assays. Results: The treatment with TGF-β1 increased the expression of α-SMA and collagen, and expression was markedly attenuated by the PPARγ agonists. Conclusions: PPARγ agonists inhibit the differentiation of TGF-β1–activated NPDFs and their production of collagen.

    Abstract: Peripherally inserted central catheter (PICC) has become a common procedure. Although ultrasound (US)-guidance has improved success rates, a small percentage of malposition is inevitable. The purpose of our study is to evaluate malposition rates of US-guided bedside PICC catheter insertion, and the clinical factors associated with malposition. Methods: This is a retrospective cohort study evaluating 5981 patients who had undergone ultrasound-guided bedside PICC placement from January 2017 to December 2021 at a single tertiary center. Final tip location was confirmed on chest radiograph. Results: Patients were categorized into optimal, suboptimal, and malposition groups according to final tip location. 4866 cases (81.7%) showed optimal tip position, 790 (13.3%) were suboptimal, and 299 (5.0%) were malpositioned. Logistic regression analysis identified six variables associated with tip malposition; height (odds ratio (OR) 1.044; 95% confidence interval (CI), 1.028–1.061; p 〈 0.001), body mass index (BMI) (OR 1.051; 95% CI, 1.017–1.087; p = 0.003), prior failure at accessing peripheral intravenous (IV) access (OR 1.718; 95% CI, 1.215–2.428; p = 0.002), side of the arm (OR 3.467; 95% CI, 2.457–4.891; p 〈 0.001), length of the catheter (OR 0.763; 95% CI, 0.734–0.794; p 〈 0.001), and number of previous central catheter insertions (OR 1.069; 95% CI, 1.004–1.140; p = 0.038). Malpositioned catheters were corrected by either bedside repositioning, bedside reinsertion, fluoroscopic reinsertion, switching to jugular catheters or catheter removal. No patient related factors were significantly associated with malposition or success of reposition. Conclusion: US-guidance can help reduce catheter malposition during bedside PICC insertion. Patients with risk factors such as multiple previous central vein insertions, failed peripheral line insertions, left arm insertion, or high BMI should undergo thorough sonographic evaluation of the arm vessels to prevent malposition.

    Abstract: Complete tumor removal and damaged nail bed repair are critical factors that determine the success of subungual tumor excision. We examined a modified proximally based nail bed flap approach for excision of subungual bone tumors of the distal phalanx and present cases where this approach was used, along with postoperative functional and cosmetic outcomes. Methods: Twenty-four benign subungual bone tumors, identified from 23 patients (9 males and 14 females), were included in this study. All patients underwent tumor excision by the modified proximally based nail bed flap approach. Pain was assessed using a numeric rating scale, and cosmesis was self-assessed using a visual analog scale preoperatively and 1 year postoperatively. We measured sensation of the involved digit tip, relative to opposite-side sensation, using static and moving two-point discrimination and Semmes–Weinstein monofilament tests 1 year postoperatively. Lastly, we assessed postoperative nail deformities and tumor recurrence as potential surgical complications. Results: Mean pain severity and cosmesis were significantly improved 1 year postoperatively. There were no significant differences in the digit tip sensation between the involved and the opposite-side digits 1 year postoperatively. We observed one case of tumor recurrence and four cases of postoperative nail deformities. Conclusions: The modified proximally based nail bed flap approach showed satisfactory functional and cosmetic outcomes. This approach enables a low rate of recurrence after removal of benign subungual bone tumors in the short term and cosmetically superior nail bed repair. Level of evidence: Therapeutic, IV
