    Abstract: As the prevalence of chronic diseases is continuously increasing, the socioeconomic cost of those conditions in Korea is also rising. In order to effectively manage chronic diseases, the “Community-Based Primary Care Project” was implemented from 2014 to 2016 and focused on primary medical care and physician-led chronic disease management. The purpose of this study is evaluating the effects of the project through the DID (difference in difference) model. The project’s database and the National Health Insurance claims database were both used to compare the project and control groups (n = 6092 vs 24 368). Results of the analysis show that medication adherence was increased more in the project group compared with the control group. Hospitalization days, outpatient days, and number of primary medical clinic visits increased more in the participant group than the control group. As the project showed an improvement in treatment persistence, it will be necessary to monitor for a longer period of time.

    Abstract: In an environmentally polluted environment, woven curtain fabrics that collect dust and pollen play an important health-protective role in interior decoration. However, devices or evaluation factors for measuring the dust adsorption ability of interior products have not been developed. To resolve this deficiency, we developed an apparatus that directly measures the dust adsorption and devised two test parameters (dust interrupting capability and fine dust adsorption capacity) for quantifying the dust adsorption. We also investigated the physical properties affecting the dust adsorption ability of fabrics with different denier weaves of air-textured yarns and draw-textured yarns, which adsorb dust in their fluffs and crimps, respectively. The air permeability and weight of the fabrics were strongly correlated with fine dust interrupting capability. The air permeability, dust adsorption, and interruption capability of the air-textured yarns largely depended on the loops formed by the overfeed rate in the weft direction. The strong sound-absorbing performance of the samples (especially at higher frequencies) was also obtained by loops, fluffs, and crimps of air-textured yarns and draw-textured yarns of fabrics. The best sound-absorbing performance was exhibited by S20, made from 760 D with many loops on the yarn surface.

    Abstract: Three monoterpene glucosides were isolated from the flowers of Brugmansia arborea L. using repeated silica gel and octadecyl SiO 2 column chromatography. Based on spectroscopic data including 1d-NMR ( 1 H, 13 C, and distortionless enhancement by polarization transfer (DEPT)), 2D-NMR (gradient correlation spectroscopy (gCOSY), gradient heteronuclear single quantum coherence (gHSQC), and gradient heteronuclear multiple bond coherence (gHMBC)), Infrared Spectroscophy, and Mass Spectroscophy, the glucosides were identified as citronellol O- β-D-glucopyranoside (1), jasminoside N (2), and jasminoside P (3). The EtOAc ( Brugmansia arborea Flowers ethyl acetate fraction [BAFE] ) and n-BuOH ( Brugmansia arborea Flowers n-butanol fraction [BAFB]) fractions showed high inhibition of tyrosinase activity (BAFE: IC 50 = 68.0 and BAFB: IC 50 = 59.3 μg/mL), and all isolated monoterpenes inhibited tyrosinase activity (1: IC 50 = 156.5, 2: IC 50 = 198.2, and 3: IC 50 = 191.0 μM).

    Abstract: Immune checkpoints are involved in mechanisms by which tumors escape from the host immune system. Our aim was to evaluate AML patients to determine expression levels of checkpoint molecules according to diagnosis and treatments, and to identify optimal candidates for checkpoint blockade. Bone marrow (BM) samples were obtained from 279 AML patients at different disease status and from 23 controls. Programmed death1 (PD-1) expression levels on CD8+T cells at AML diagnosis were increased compared to controls. PD-L1 and PD-L2 expression levels on leukemic cells at diagnosis were significantly higher in secondary AML than in de novo AML. PD-1 levels on CD8+ and CD4+T cells after allo-SCT were significantly higher than those at diagnosis and after CTx. PD-1 expression on CD8+T cells increased in the acute GVHD group than in the non-GVHD group. The overall survival of patients with high PD-1 expression on CD8+T cells was significantly shorter than that of patients with low PD-1 expression. In conclusion, patients who underwent allo-SCT exhibited high PD-1 expression, suggesting that allo-SCT increases PD-1 expression on T cells, and the patients with high PD-1 expression on CD8+T cells after allo-SCT showed the poor prognosis. For these patients, PD-1 blockade could be an immunotherapeutic strategy.

    Abstract: The bispectral index (BIS) has been used to monitor sedation during spinal anesthesia. We evaluated the correlation between BIS and the Observer’s Assessment of Alertness/Sedation Scale (OAA/S) in patients sedated with dexmedetomidine, propofol, or midazolam. Methods This prospective, randomized study included 46 patients scheduled for knee arthroplasty under spinal anesthesia with sedation. The patients were randomized to receive sedation with dexmedetomidine (n = 15), propofol (n = 15), or midazolam (n = 16). Correlation between BIS and OAA/S was assessed during sedation in the three groups. Results A linear correlation was observed between BIS and OAA/S, and there was no significant difference in BIS score between the groups during mild to moderate sedation status (OAA/S 3–5). During deep sedation (OAA/S 1–2), the BIS score in the midazolam group was significantly higher than that in the propofol and dexmedetomidine groups (74.4 ± 11.9 vs 67.7 ±  9.5 vs 62.6 ± 12.2). Conclusions BIS values differed at the same level of sedation between different sedative agents. Objective sedation scores should therefore be used in combination with BIS values for the assessment of sedation levels during spinal anesthesia.

    Abstract: This study investigated the effects of acetylsalicylic acid (ASA) and clopidogrel, standardly used in the secondary prevention of vascular occlusions, on cerebral arteriogenesis in vivo and in vitro. Cerebral hypoperfusion was induced by three-vessel occlusion (3-VO) in rats, which subsequently received vehicle, ASA (6.34 mg/kg), or clopidogrel (10 mg/kg). Granulocyte colony-stimulating factor (G-CSF), which enhanced monocyte migration in an additional cell culture model, augmented cerebrovascular arteriogenesis in subgroups (40 μg/kg). Cerebrovascular reactivity and vessel diameters were assessed at 7 and 21 days. Cerebrovascular reserve capacity was completely abolished after 3-VO and remained severely compromised after 7 (−14 ± 14%) and 21 (−5 ± 11%) days in the ASA groups in comparison with controls (4 ± 5% and 10 ± 10%) and clopidogrel (4 ± 13% and 10 ± 8%). It was still significantly decreased when ASA was combined with G-CSF (1 ± 4%) compared with G-CSF alone (20 ±8%). Posterior cerebral artery diameters confirmed these data. Monocyte migration into the vessel wall, improved by G-CSF, was significantly reduced by ASA. Acetylsalicylic acid, but not clopidogrel, inhibits therapeutically augmented cerebral arteriogenesis.

    Abstract: Objective. This study aimed to compare postoperative pain and surgical outcomes after transumbilical single-port access (SPA) and conventional multiport laparoscopic surgery for adnexal lesions. Methods. A retrospective case–control study was conducted matched by age, body mass index, and frequency of previous abdominal surgery. A total of 110 SPA laparoscopy patients (cases) were matched with a cohort of 107 patients who underwent conventional laparoscopy (controls) for benign adnexal lesions. SPA system consisted of a wound retractor, surgical glove, two 5-mm trocars, and one 11-mm trocar. Postoperative pain scores were measured immediately after surgery and at 6, 24, and 48 hours postsurgery using the numerical rating scale. Results. Postoperative pain scores did not differ between the 2 groups ( P = .552). However, higher number of painkiller administrations was observed in the SPA laparoscopy group (median 3 vs 1, P 〈 .001). The type of surgery and intraoperative blood loss were the significant factors influencing the number of painkiller administrations after controlling for other parameters by linear regression ( P 〈 .0001). The SPA laparoscopy group had less intraoperative blood loss (45.3 vs 87.5 mL, P 〈 .001) and shorter hospital stay (2.1 ± 0.8 vs 2.7 ± 1.0 days, P 〈 .001) compared with the conventional laparoscopy group. Operative time and perioperative complications did not differ between groups. Conclusions. There was no difference in pain intensity between the SPA and conventional laparoscopic group in this study. Future trials are warranted to better define the benefits of SPA surgery in terms of postoperative pain.

    Abstract: There remains no consensus regarding which repair technique provides the most optimal results for unstable distal clavicle fractures. Purpose: To compare radiologic and clinical outcomes between locking plate fixation and anatomic coracoclavicular (CC) ligament reconstruction for patients with unstable distal clavicle fractures. Study Design: Cohort study; Level of evidence, 3. Methods: The study included 41 patients with Neer type 2B distal clavicle fracture. In group A (n = 15), patients were treated using CC ligament reconstruction with autologous palmaris longus tendon, artificial tape, and Steinmann pin fixation; in group B (n = 26), patients were treated using anatomic locking plate fixation. All patients had a minimum 2 years of follow-up. CC distance and arthrosis of the acromioclavicular joint were assessed radiographically. Clinical outcomes—including range of motion, visual analog scale for pain, American Shoulder and Elbow Surgeons rating scale, and Constant score—were compared between the groups using the paired Student t test and Fisher exact test. Results: Bone union was attained in all patients. In both groups, the CC distance decreased significantly from presurgery to final follow-up: group A, from 16.25 ± 4.75 to 7.66 ± 2.61 mm ( P 〈 .001); group B, from 17.3 ± 4.07 to 9.33 ± 2.01 mm ( P 〈 .001). The final CC distance was significantly greater in group B (7.66 vs 9.33 mm in groups A and B, respectively; P = .028). Osteoarthritis of the acromioclavicular joint occurred in 13 of 41 patients (3 in group A and 10 in group B). At final follow-up, there was no statistical significance between the groups in range of motion, and clinical outcome scores were satisfactory in both groups, with no statistical difference between them. Conclusion: Anatomic locking plate and anatomic reconstruction of the CC ligament showed good clinical results, so both techniques can be considered reliable for restoring stability. However, CC ligament reconstruction had better CC distance on radiologic assessment and did not require removal surgery, so it may be the preferable surgical option to treat unstable distal clavicle fractures (Neer type 2B).

    Abstract: To report an unusual presentation of disseminated, intraocular, extranodal natural killer/T-cell lymphoma, nasal type (NK/T-cell lymphoma), originating from nasal NK/T-cell lymphoma. Methods Case report. Results A 63-year-old woman who had been treated with systemic chemotherapy and radiotherapy for NK/T-cell lymphoma in the nasal cavity presented with vitreous haze of the right eye. Despite anti-inflammatory therapy, the right eye showed poor clinical response and received diagnostic vitrectomy. The vitreous opacity resembled a thin sheet, with no sign of subretinal infiltrate or vascular sheathing. The vitreous specimen contained many large, pleomorphic lymphoma cells. The malignant cells were positive for CD3, CD8, and granzyme B. Systemic workups showed no involvement of other organs. A diagnosis of T-cell lymphoma in the vitreous was made; the tumor likely originated from nasal NK/T-cell lymphoma. The patient was treated with intrathecal chemotherapy and intravitreal methotrexate injection. The eye was clinically clear of malignant cells after the injections. Conclusions Vitreous infiltration without uveoretinal involvement can be an unusual manifestation of intraocular NK/T-cell lymphoma. Clinician awareness of possible ocular involvement may assist in the diagnosis of disseminated NK/T-cell lymphoma.
