    Abstract: The purpose of this study was to identify how older Korean people seek information and their desire to participate in decision making about their health care. A total of 165 elderly people living in Seoul, South Korea, participated in the study. Data were collected during individual interviews using the Autonomy Preference Index. The mean information-seeking score was high. The mean score for their desire to participate with a physician in decision making was lower, but this was higher when family members were involved. The study indicates that many older people want to receive information about their health care. Families (or guardians), as well as older people themselves, should be included in the decision-making process. Nurses can encourage older people to express their wishes, while treating each individual with respect.

    Abstract: Chondromyxoid fibroma is a rare benign primary bone tumor of cartilage. Despite a characteristic radiographic appearance, chondromyxoid fibroma with atypical radiographic findings may mimic more common tumors. Purpose To describe the MR findings of chondromyxoid fibroma. Material and Methods MR images of 19 histopathologically confirmed chondromyxoid fibromas were retrospectively analyzed for signal intensity, periosteal reaction, adjacent abnormal bone marrow and soft tissue signal, and patterns of contrast enhancement. Results All cases of chondromyxoid fibroma showed hypointense to intermediate signal intensity and internal hyperintense foci were observed in seven (37%) cases on T1-weighted images. On T2-weighted images, all lesions were hyperintense: peripheral intermediate signal band with central hyperintense signal in 11 (58%) of 19 lesions, whereas diffusely hyperintense with heterogeneous pattern in eight (42%). Periosteal reaction was observed in 11 (58%) of 19 cases. Adjacent abnormal bone marrow or soft tissue signal was observed in 12 (63%) or 14 (74%) of 19 cases, respectively. On contrast-enhanced T1-weighted images, peripheral nodular enhancement was observed in 69% (11/16) and diffuse contrast enhancement was observed in 31% (5/16) with homogeneous ( n = 3) or heterogeneous ( n = 2) patterns. Among the cases with peripheral nodular enhancement, the peripheral nodular enhancing portion generally corresponded to the peripheral intermediate signal band on T2-weighted images, although the peripheral enhancement was not as wide as a band of intermediate signal intensity. On the other hand, the central non-enhancing portion generally corresponded to the central hyperintense signal intensity on T2-weighted images. Conclusion The helpful features of chondromyxoid fibroma are the peripheral intermediate signal band and central hyperintense signal on T2-weighted images, generally corresponding to the peripheral nodular enhancement and central non-enhancing portion on contrast-enhanced T1-weighted images, respectively.

    Abstract: Three-dimensional (3D) fast spin-echo sequence with variable flip-angle refocusing pulse allows retrospective alignments of magnetic resonance imaging (MRI) in any desired plane. Purpose To compare isotropic 3D T2-weighted (T2W) turbo spin-echo sequence (TSE-SPACE) with standard two-dimensional (2D) T2W TSE imaging for evaluating lumbar spine pathology at 3.0 T MRI. Material and Methods Forty-two patients who had spine surgery for disk herniation and had 3.0 T spine MRI were included in this study. In addition to standard 2D T2W TSE imaging, sagittal 3D T2W TSE-SPACE was obtained to produce multiplanar (MPR) images. Each set of MR images from 3D T2W TSE and 2D TSE-SPACE were independently scored for the degree of lumbar neural foraminal stenosis, central spinal stenosis, and nerve compression by two reviewers. These scores were compared with operative findings and the sensitivities were evaluated by McNemar test. Inter-observer agreements and the correlation with symptoms laterality were assessed with kappa statistics. Results The 3D T2W TSE and 2D TSE-SPACE had similar sensitivity in detecting foraminal stenosis (78.9% versus 78.9% in 32 foramen levels), spinal stenosis (100% versus 100% in 42 spinal levels), and nerve compression (92.9% versus 81.8% in 59 spinal nerves). The inter-observer agreements (κ = 0.849 vs. 0.451 for foraminal stenosis, κ = 0.809 vs. 0.503 for spinal stenosis, and κ = 0.681 vs. 0.429 for nerve compression) and symptoms correlation (κ = 0.449 vs. κ = 0.242) were better in 3D TSE-SPACE compared to 2D TSE. 3D TSE-SPACE with oblique coronal MPR images demonstrated better inter-observer agreements compared to 3D TSE-SPACE without oblique coronal MPR images ( κ = 0.930 vs. κ = 0.681). Conclusion Isotropic 3D T2W TSE-SPACE at 3.0 T was comparable to 2D T2W TSE for detecting foraminal stenosis, central spinal stenosis, and nerve compression with better inter-observer agreements and symptom correlation.

    Abstract: This study examines the effects of presence when respondents played a golf game using virtual golf simulators. The respondents were able to use real golf clubs and accurate swing-and-ball-tracking technology for a realistic experience. The perceived enjoyment, perceived value, and behavioral intention were measured. Data were collected using a convenience sample of 275 virtual golf simulator players. Multiple regression analyses revealed that social presence had a pivotal role in explaining perceived enjoyment, perceived value, and behavioral intention. However, telepresence was not a significant predictor for those variables. Results of simple regression showed that perceived enjoyment was significantly associated with perceived value and behavioral intention. The results also showed that perceived value significantly predicted behavioral intention. The findings imply that the success of virtual golf simulators may be more related to an enhanced sense of socialization with other players, rather than the virtual reality technology itself.

    Abstract: Although many studies have implicated interleukin (IL)-4 promoter polymorphisms as potential determinants of disease susceptibility, there are no reports on the association between IL-4 promoter polymorphisms and nasal polyps. The aim of this study was to investigate the relationship between an IL-4 promoter polymorphism and nasal polyps. Methods The C-590T promoter polymorphism of the IL-4 gene was genotyped by polymerase chain reaction-restriction fragment length polymorphism analysis in 106 Korean chronic rhinosinusitis patients with or without nasal polyps and 70 healthy Korean subjects. Results The frequency of the T allele at position -590 of the IL-4 gene in a Korean population was 0.85, which was significantly higher than those of other ethnic groups, and the T/T allele at position -590 of IL-4 was associated with protection against nasal polyps if compared with the C/C allele (relative risk, 0.529; 95% confidence interval, 0.307–0.912; p = 0.028). Conclusion T-590, which is dominant at position -590 of the IL-4 promoter, appears to be associated with a protective mechanism against nasal polyps in Korean populations.

    Abstract: This study examined depression in low-income elementary school children and identified gender differences in factors that influence depression from an ecological perspective. Participants were 262 first- to sixth-grade children recruited from six Korean community centers. Personal factors were anxiety and self-concept. Environmental factors consisted of caregiver (perceived caregiving style), school (perceived school life and quality of peer relations), and neighborhood. The outcome variable was depression. The predictors of boys’ depression were anxiety, perceived school life, and neighborhood. Predictors of girls’ depression were anxiety, self-concept, caregiving style, and neighborhood. School nurses are encouraged to screen elementary school children for depressive symptoms and consider gender differences and environmental factors when intervening in depression. School-focused interventions may be more effective for boys; family-focused interventions may be more beneficial for girls.

    Abstract: Background: Fat-suppressed (FS) proton density (PD)-weighted magnetic resonance imaging (MRI) and FS three-dimensional (3D) gradient-echo imaging such as spoiled gradient-recalled (SPGR) sequence have been established as accurate methods for detecting articular cartilage defects. Purpose: To retrospectively compare the diagnostic efficacy between FS PD-weighted and FS 3D gradient-echo MRI for differentiating between grade 3 and grade 4 cartilage defects of the knee with arthroscopy as the standard of reference. Material and Methods: Twenty-one patients who had grade 3 or 4 cartilage defects in medial femoral condyle at arthroscopy and knee MRI were included in this study: grade 3, 〉 50% cartilage defects; grade 4, full thickness cartilage defects exposed to the bone. Sagittal FS PD-weighted MR images and FS 3D gradient-echo images with 1.5 T MR images were independently graded for the cartilage abnormalities of medial femoral condyle by two musculoskeletal radiologists. Statistical analysis was performed by Fisher's exact test. Inter-observer agreement in grading of cartilage was assessed using κ coefficients. Results: Arthroscopy revealed grade 3 defects in 17 patients and grade 4 defects in 4 patients in medial femoral condyles. For FS 3D gradient-echo images grade 3 defects were graded as grade 3 ( n=15) and grade 4 ( n=2), and all grade 4 defects ( n=4) were correctly graded. However, for FS PD-weighted MR images all grade 3 defects were misinterpreted as grade 1 ( n=1) and grade 4 ( n=16), whereas all grade 4 defects ( n=4) were correctly graded. FS 3D gradient-echo MRI could differentiate grade 3 from grade 4 defects ( P=0.003), whereas FS PD-weighted imaging could not ( P=1.0). Inter-observer agreement was substantial (κ=0.70) for grading of cartilage using FS PD-weighted imaging, whereas it was moderate (κ=0.46) using FS 3D gradient-echo imaging. Conclusion: FS 3D gradient-echo MRI is more helpful for differentiating between grade 3 and grade 4 cartilage defects than is FS PD-weighted imaging.

    Abstract: In this paper, we present the result of investigations pertaining to the pipe stiffness (PS) of glass fiber reinforced polymer plastic (GFRP) flexible pipes buried underground. PS formula for the parallel plate loading condition is derived based on the elasticity theory. Vertical and horizontal displacements are also derived. In the study, the mechanical properties and structural behavior of GFRP flexible pipes are investigated experimentally and the results are used in the analysis and design of these flexible pipes. Pipe stiffness of GFRP flexible pipe is investigated by the parallel plate loading test, the finite element analysis, and theoretical analysis. The results of pipe stiffness of the GFRP flexible pipe obtained by the experiment, theory, and the finite element analysis are compared and close agreements are observed.

    Abstract: To investigate biomechanical effects of cleft type (unilateral/bilateral cleft lip and palate), facemask anchorage method (tooth-borne and miniplate anchorage), and alveolar bone graft on maxillary protraction. Design Three-dimensional finite element analysis with application of orthopedic force (30° downward and forward to the occlusal plane, 500 g per side). Model Computed tomography data from a 13.5-year-old girl with maxillary hypoplasia. Intervention Eight three-dimensional finite element models were fabricated according to cleft type, facemask anchorage method, and alveolar bone graft. Main Outcome Measure(s) Initial stress distribution and displacement after force application were analyzed. Results Unilateral cleft lip and palate showed an asymmetric pattern in stress distribution and displacement before alveolar bone graft and demonstrated a symmetric pattern after alveolar bone graft. However, bilateral cleft lip and palate showed symmetric patterns in stress distribution and displacement before and after alveolar bone graft. In both cleft types, the graft extended the stress distribution area laterally beyond the infraorbital foramen. For both unilateral and bilateral cleft lip and palate, a facemask with a tooth-borne anchorage showed a dentoalveolar effect with prominent stress distribution and displacement on the upper canine point. In contrast, a facemask with miniplate anchorage exhibited an orthopedic effect with more favorable stress distribution and displacement on the middle maxilla point. In addition, the facemask with a miniplate anchorage showed a larger stress distribution area and suturai stress values than did the facemask with a tooth-borne anchorage. The pterygopalatine and zygomatico-maxillary sutures showed the largest suturai stress values with a facemask with a miniplate anchorage and after alveolar bone grafting, respectively. Conclusion In this three-dimensional finite element analysis, it would be more advantageous to perform maxillary protraction using a facemask with a miniplate anchorage than a facemask with a tooth-borne anchorage and after alveolar bone graft rather than before alveolar bone graft, regardless of cleft type.

    Abstract: Ride quality became a very important factor in the performance of railway vehicles according to the expansion of high-speed railways and speedup of velocity of railway vehicles. In this study, the results of applying the MR (magnetorheological) lateral damper on the secondary suspension to reduce the vibration of the car body, directly relating to the ride quality of railway vehicles, were mentioned. In order to verify the control performance of MR dampers, a 1/5 scaled railway vehicle model was constructed, and numerical simulation and experimental tests were conducted. The MR damper for the experimental tests was produced and was attached between the car body and bogie of a full scaled vehicle, and a vibration controlling test was performed to improve ride quality on a roller rig. The skyhook control algorithm was used as the controlling technique, and regarding the test results, the RMS (root mean square) value was found by compensating the frequency of the lateral vibration based on the UIC 513 R Standard about the ride quality of railway vehicles. As a result of the test, it could be confirmed that vibration was reduced by approximately 24% when attaching the MR damper between the bogie and the car body compared to when applying a passive damper.
