    Abstract: To evaluate the efficacy and safety of a plasma-derived factor VIII concentrate containing von Willebrand Factor (pdVWF/FVIII) in standard clinical practice in von Willebrand Disease (VWD) patients. Methods A retrospective, multicentric, observational study of VWD patients treated with Fanhdi ® , a pdVWF/FVIII concentrate, from January 2011 to December 2017 was conducted at 14 centers in Spain. Efficacy and safety were evaluated for acute bleeding episodes, for prevention of bleeding in surgeries, and for secondary long-term prophylaxis. Results Seventy-two eligible patients, type 1, 2, 3 VWD (25%/38.9%/36.1%) were treated for spontaneous and traumatic bleeding (140 episodes, n = 41 patients), to prevent surgical bleeding (69 episodes, n = 43 patients); and for secondary long-term prophylaxis (18 programs, n = 13 patients). Replacement therapy with pdVWF/FVIII showed an excellent to good clinical efficacy in 96.7% of the bleeding episodes, 100% during surgical procedures and 100% during prophylaxis. No adverse events (AEs), nor serious AEs related to the product were observed. Conclusions Fanhdi ® was effective, safe and well tolerated in the management of bleeding episodes, the prevention of bleeding during surgeries, and for secondary long-term prophylaxis in VWD patients.

    Abstract: Changes in bone mineral content of calcium (Ca), phosphorous (P), magnesium and potassium for male and female Wistar rats during their development from 3 weeks old to adulthood (27 weeks old) were measured. Bone mineral content was related to areal bone mineral density (BMD) which was measured in vivo at the femoral neck using a calibrated X-ray transmission system to obtain basal curves as a function of the age of the specimen. Diagnostic curves were built to determine low BMD (osteopaenia) and osteoporosis in female rats fed a Ca-depleted diet (50%) based on the obtained data and the criteria established by the World Health Organization. Bone mineral content is directly related to sex and age, but P did not change throughout the experimental period. P content did not exhibit significant changes with growing, while Ca was greatest in male rats, producing significant differences in the Ca:P ratio. Male rats reach the Ca:P ratio peak before female rats. However, areal BMD does not follow the same trend. On the other hand, osteoporosis produced a 45% decrease in this parameter for young and mature adults. These results make Z-score values available to diagnose bone-mass losses and hence the possibility of improving the conditions of non-contact measurement of BMD in vivo. This technique can be used for future experiments with Wistar rats.

    Abstract: Based on the observation that the parasite Onchocerca volvulus selectively absorbs vitamin A from the host, and the known toxicity of vitamin A in higher concentration, it was hypothesized that dying microfilariae (mf) release their stores of vitamin A (retinoids) into the host circulation in toxic concentrations, inducing the signs and symptoms of onchocerciasis. We conducted a pilot study to test the hypothesis in Songea communities in Southern Tanzania, where mass drug administration with ivermectin had not been implemented by the time of the survey. The specific aim was to evaluate the correlation between the diagnosis of onchocerciasis and increased levels of retinoic acid at infection sites. The analysis was performed by determining copy numbers of a genome of O volvulus present in skin snip samples of persons with onchocerciacis, and correlating these numbers with expression levels of retinoic acid receptor-α (RAR-α), which is inducible by retinoic acid. Total DNA and RNA were extracted from each of 25 mf-positive and 25 mf-negative skin samples and evaluated using quantitative polymerase chain reaction with appropriate negative controls. Analysis of the samples, adjusted with glyceraldehyde 3-phosphate dehydrogenase gene levels, revealed that most samples with detectable RAR-α transcripts had higher levels of RAR-α expression than the assay control. However, the quality and number of samples were insufficient for statistical analysis. Fold data on the expression levels of both O volvulus DNA and RAR RNA suggested a possible trend toward higher relative RAR-α expression in samples with higher levels of O volvulus DNA ( r 2  = 0.25, P = .079). Evidence of a contribution of vitamin A to the pathology of onchocerciasis thus remains elusive. Future studies on the role of retinoids in onchocerciasis will require larger groups of participants as well as careful monitoring of the cold chain and tissue storage procedures in view of the sensitivity of vitamin A to heat and light.

    Abstract: To compare the effectiveness of two different exercise-based programs through telerehabilitation in patients with coronavirus disease 2019. Design Randomized, controlled, parallel, double-blinded, three-arm clinical trial. Setting Patients’ homes through telerehabilitation devices. Subjects Subjects with coronavirus disease 2019 in the acute phase. Interventions Subjects were divided into three groups: breathing exercises group, strength exercises group or no treatment/control group. Main measures We analysed visual analogue scale for fatigue, 6-minute walking test, 30-seconds sit-to-stand test, multidimensional dyspnoea-12 questionnaire and Borg scale at baseline and 14 days later. Results From 93 subjects recruited, 88 were enrolled, and 77 patients (mean [SD] age 39.40 [11.71] ) completed the 14-days intervention and were included in the analysis: 26 in strength exercises group, 29 in breathing exercises group and 22 in control group. The intergroup analysis shows significant differences between the study groups and control group in all variables ( p  〈  0.05); Borg scale, multidimensional dyspnoea-12 questionnaire (pre–post intervention score: strength exercises group: 7.85 [6.82] – 4.54[4.82] , breathing exercises group: 11.04 [6.49] – 5.32 [3.63] , control group: 10.27 [6.49] – 10.59[6.58] ), visual analogue scale for fatigue, 6-minute walking test and 30-seconds sit-to-stand test (pre–post intervention score: strength exercises group: 12.19 [4.42] – 13.58 [5.37] , breathing exercises group: 11.18 [3.42] – 12.79 [4.00] , control group: 10.45 [2.15] – 9.86[1.88] ). The greatest effect sizes were found in the variables Borg Scale ( R 2  = 0.548) and multidimensional dyspnoea-12 questionnaire ( R 2  = 0.475). Conclusions Strength exercises group and breathing exercises group obtained significant improvements in fatigue, dyspnoea, perceived effort, and physical state, compared to control group, although the greatest benefits were found for dyspnoea and aerobic capacity in breathing exercises group.

    Abstract: This article analyzes the influence of Spanish citizens’ computer knowledge and level of education on their tolerance of, or aversion to, risk in the online purchase of goods and services. To this end, annual survey data from Spain’s National Statistics Institute were used to know the incidence in e-commerce of the following variables: “computer knowledge,” “electronic commerce,” and “socioeconomic characteristics.” A method based on the design of a computer knowledge indicator was used, followed by a bivariate logistic regression to determine computer knowledge and level of education. The results show that the higher the level of computer knowledge, the greater the impulse to buy online and the higher the tolerance of risk. There was also a positive relation between level of education and propensity to buy; the higher the level of education, the greater the propensity.

    Abstract: This article studies the performance of a wireless sensor network with cognitive radio capabilities to gather information about structural health monitoring of buildings in case of seismic activity. Since the use of the local area network is intensive in office and home environments, we propose the use of empty cellular channels (primary system). As such, the structural health monitoring does not degrade the local communications. Thus, the wireless sensor network for structural health monitoring acts as secondary network. Two discrete-time analytical approaches are proposed and developed to evaluate the system performance in terms of both the average packet delay and average energy consumption. The first one is an approximation suitable for the case when the time slot duration is small relative to the mean call inter-arrival time. The second model is accurate for any time slot duration and inter-arrival times.

    Abstract: One of the greatest challenges that health professionals face is providing humanized care, especially when technological advancements contribute to the depersonalization of care delivery. In this sense, nursing care not only requires the nurse to be scientific, academic, and clinical but also a humanitarian and moral agent, as a partner in human transactions. Method: Quantitative, descriptive, and transversal study. In a nonprobabilistic sampling for convenience, in 150 surgical patients, with more than 3 days of hospital stay, the instrument “Perception of Behaviors of Humanized Nursing Care” was used (third version). The study adhered to the legal and ethical research guidelines in Mexico. Results: According to the general objective of the study, the findings determined were that 67% of the participants perceived humanized nursing care as favorable Conclusion: More than half of the patients always perceived behaviors of humanized care, provided by nurses, during their hospitalization in surgical services.

    Abstract: There are few reports in oral squamous cell carcinoma (OSCC) that indicate the expression of macrophage migration inhibitory factor (MIF) in tissues, serum, or saliva of patients with OSCC. The aim of this study was to evaluate the mRNA expression and protein of MIF in tissues and serum, respectively, in OSCC patients and its association with the TNM stage. A cross-sectional study was performed. Serum and tissues of 25 patients with OSCC and 25 healthy control subjects (HCS) were included to evaluate the MIF mRNA expression and protein serum levels by real-time PCR and ELISA, respectively. Serum MIF levels were significantly higher in OSCC compared with control subjects. Furthermore, in the OSCC group, MIF was significantly increased in accordance with tumor disease stage (TNM III–IV), as well as in poorly differentiated tumors. The mRNA showed significantly higher levels in HCS, as well as in more differentiated tumors. The results of this study suggest that MIF could be an indicator of severity and progression of OSCC. Further studies are required to explore the role of MIF as a serological biomarker for OSCC.

    Abstract: En México, la población indígena supera los siete millones de habitantes, en Puebla el grupo más representativo es el Náhuatl. Sin embargo, las condiciones de vida, salud, educación y transporte son precarias para esta población. En los adolescentes, las responsabilidades como el matrimonio, la familia y los compromisos ante la comunidad, favorecen conductas de riesgo sexual que dificultan su desarrollo económico, social y reproductivo. El objetivo fue proponer un modelo explicativo del uso del condón en adolescentes nahuas. Método. Bajo el marco de la teoría social cognitiva, el concepto de valores culturales de Leininger y el proceso de la sustracción teórica, se desarrolló este artículo. Se muestran las relaciones del modelo con las proposiciones y los factores que influyen en el uso del condón para este grupo específico. Finalmente, el modelo explica las variables de interés, los niveles de abstracción y las relaciones entre sí en el contexto náhuatl. El siguiente paso será implementar los indicadores empíricos para conocer el grado de influencia de los factores personales y ambientales hacia el uso del condón en adolescentes nahuas. Resultados que aportarán información para el desarrollo del conocimiento en enfermería y la reducción de riesgo sexual de esta población.

    Abstract: Deployable mechanisms in CubeSat satellites have many problems with the system that provides the anchor position. The main defect of the traditional deployment mechanisms for solar panels in CubeSats is the lack of position system to block the back-driving of the panel when it reaches the final phase of the deployment. This generates spurious oscillations in the panel, affecting the photovoltaic process as well as generating fatigue in the mechanical elements of the mechanism (hinge or pin). In this work, the design, analysis and manufacture of a deployment mechanism for CubeSat solar panels is shown. A finite element method analysis was carried out in a hinge with an integrated blocking system as well as a double torsion spring, which can be used on CubeSats. The outcome shows the layout of the described anchor hinge and the used double-torsion spring, which provides a positive direction torque transfer. Likewise, the performed numerical analyses on the designed system, reduce the weight and optimise the geometry of the mechanism, showing its feasibility as well as the potential applications and further research in the area.
