    Abstract: This study aimed to investigate the current contract rate and residents’ willingness to contract with general practitioner (GP) services in Guangzhou, China, during the policy trial phase, and also to explore the association of behavior contract and contract willingness with variables based on Andersen’s Behavioral Model of Health Services Use (ABM). In total, 160 residents from community health centers (CHCs) and 202 residents from hospitals were recruited in this study. The outcome variables were behavior contract and contract willingness. Based on the framework of ABM, independent variables were categorized as predisposing factors, enabling factors, need factors, and CHC service utilization experiences. Univariate and multivariate logistic regression analysis models were applied to explore the associated factors. Out of 362 participants, 14.4% had contracted with GP services. For those who had not contracted with GP services, only 16.4% (51 out of 310) claimed they were willing to do so. The contract rate for community-based participants was significantly higher than that for hospital-based participants. Major reasons for not choosing to contract were perceiving no benefit from the service and concerns about the quality of CHCs. Community health center experiences and satisfaction were significantly associated with contracting among hospital-based participants. A need factor (diagnosed with hypertension or diabetes) and CHC service utilization experiences (have gotten services from the same doctor in CHCs) were significantly associated with contract willingness among CHC-based participants. Intervention to improve awareness of GP services may help to promote this service. Different intervention strategies should be used for varying resident populations.

    Abstract: Circulating tumor cells and serum tumor markers have been found significant in predicting outcome for several malignancies. However, their role in gastric cancer is not fully clarified. We conducted a retrospective study to explore the prognostic value of circulating tumor cells and their applicability in assessing the treatment efficacy in gastric cancers. Methods: From September 2015 to December 2018, 116 patients with newly pathologically diagnosed gastric adenocarcinoma were enrolled. At both baseline and two courses after chemotherapy, the data of circulating tumor cells and serum tumor markers, such as CEA, CA72-4, CA19-9, CA50, and CA242, were collected. The relationships between the change trend of circulating tumor cells and the treatment efficacy were analyzed after chemotherapy, with a paired t-test. Univariate and multivariable analysis were used to find prognostic factors for overall survival (OS). Results: We found there is a significant difference between the circulating tumor cells-positive and circulating tumor cells-negative before and after therapy (mOS 12.6 vs. 31.6 months, P 〈 0.001; mOS 12.4 vs. 24.2 months, P=0.002), respectively. Also, differentiation, pre-therapeutic circulating tumor cells and therapeutic response were independent predictors of overall survival. Following two courses of chemotherapy, the number of circulating tumor cells increased obviously in the progressive disease group ( P=0.002), while they decreased in the non-progressive disease group ( P=0.02). Thus, the change in the circulating tumor cells count had a close association with the therapeutic response ( P=0.004). Conclusion: Generally, circulating tumor cells provide a novel tool to evaluate the outcomes of gastric cancer patients. Since the change of circulating tumor cells was highly related to treatment response, it may be an auxiliary to assess the effect of chemotherapy, leading an earlier adjustment of following regimens.

    Abstract: Yarn twist in textile technology is an important characteristic since it considerably affects the properties of knitted or woven fabrics. Many researchers have investigated the effect of staple-spun yarn twist on the properties of the yarns and fabrics. However, the effects of twist level of Kevlar® 29 filament yarn on the properties of yarn and its resin-impregnated self-lubricating fabric liner are not fully known yet. In this study, we have investigated the effects of Kevlar® 29 twist level on the tensile and tribological properties of the fabric liner (Kevlar® 29/polytetrafluoroethylene fabric-resin composite). Two unexpected findings about the effect of yarn twist have been observed, namely (1) asynchronous twist effect on the yarn’s and the liner’s tensile strength and (2) dissimilar yarn twist effect on the liner’s performance. These findings are mainly attributed to the synergic contributions of the yarn twist and strength and the interaction of the resin with the yarn orientation in the woven fabric structure of the liner.

    Abstract: Frictional failure is inevitable for friction pairs. A common sense about the frictional failure of steel-polymer composite friction pair is that the polymer composites are the primary failure part rather than the steel counterpart. However, an opposite case was newly found in the case of fabric self-lubricating liner (Kevlar/PTFE hybrid composite) spherical plain-bearing under elevated load: the steel counterbody suffered a fatigue failure before the fabric composite fails, which resulted in the final failure of the bearing. This paper investigated such fatigue failure of surface chrome plated 45# steel by systematically studying the wear characteristics of 45# steel and fabric composite, the stress distribution of the 45# steel and fatigue features within cross section of the 45# steel. Results showed that wear characteristics of both fabric composite and 45# steel exhibited regional characters. Failure mechanism of the 45# steel was found to be fatigue failure (mainly fatigue pitting), which was revealed by mathematical analysis of the stress distribution of the 45# steel and the fatigue cracks found within subsurface of the cross section of the 45# steel.

    Abstract: In this study, we aimed to clarify the effects of long noncoding ribonucleic acid prostrate androgen-regulated transcript-1 on bladder cancer cell proliferation and apoptosis. Methods: Microarrays were implemented to investigate the long noncoding ribonucleic acid expression profiles in bladder cancer tissue (N = 9) and in noncancer bladder tissue (N = 5). Relative prostrate androgen-regulated transcript-1 expression levels in tissue samples or cell lines were detected by real-time quantitative reverse transcription-polymerase chain reaction. Prostrate androgen-regulated transcript-1 expression was enhanced by the transfection of pcDNA3.1-prostrate androgen-regulated transcript-1 and downregulated by the infection with pcMV-sh prostrate androgen-regulated transcript-1. Additionally, cell proliferation and apoptosis were measured by the cell counting kit-8 assay and flow cytometry, respectively. Cell invasion was determined by a Transwell assay. Results: Prostrate androgen-regulated transcript-1 expression was upregulated in bladder cancer tissues compared to adjacent nontumor tissues. Furthermore, prostrate androgen-regulated transcript-1 levels were successfully upregulated by pcDNA3.1-prostrate androgen-regulated transcript-1 and depleted by pCMV-sh prostrate androgen-regulated transcript-1 in bladder cancer cell lines (5637, T24). Enhanced prostrate androgen-regulated transcript-1 expression promoted cell proliferation and invasion and inhibited cell apoptosis. However, knockdown of prostrate androgen-regulated transcript-1 expression inhibited cell proliferation and invasion and induced cell apoptosis. Conclusion: In summary, these data suggest that the knockdown of prostrate androgen-regulated transcript-1 represents a tumor suppressor player in bladder cancer and contributes to the inhibition of tumor proliferation, the promotion of cell apoptosis, and the suppression of cell invasion. Prostrate androgen-regulated transcript-1 may function as a new prognostic biomarker and as a feasible therapeutic target for patients with bladder cancer.

    Abstract: The performance of a power-split hybrid system and power coupling quality of its power components directly depend on the optimal design of its configuration and characteristic parameters. For numerous potential configurations, determining the best system configuration and corresponding planetary gear (PG) sets characteristic parameters to achieve the desired vehicle performance target is the key issue during a power-split system design. In this article, all potential configurations, different numbers of shifting actuators, connection positions of power components, and combinations of vehicle working modes are considered for a two-planetary-gear (2-PG) power-split hybrid system. A new algorithm, named Reborn after the Doom based on multi-objective particle swarm optimization (RAD-MOPSO) algorithm, is proposed to optimize the 2-PG system configuration and parameters. First, based on the analysis, evaluation, and screening of the system configuration, the design of the 2-PG hybrid system is considered as a multi-objective optimization design problem. The RAD-MOPSO algorithm is then proposed and applied, and the effectiveness and competitiveness of the proposed algorithm are validated by relevant benchmark functions. Finally, RAD-MOPSO is applied to the parameter optimization of the potential 2-PG system configuration. The calculation results demonstrate that this algorithm has faster convergence speed and better diversity performance compared with MOPSO and can also achieve efficient configuration screening and parameter optimization of a power-split hybrid system, and the Pareto-optimal solution set is obtained.
