    Kurzfassung: In intensive case management (ICM), users receive a wide variety of services of varying content, which makes it difficult to understand the global features of ICM programs. Aims: The aim of this study was to examine the features of ICM programs using network analysis. Methods: A total of 233 ICM users in two Japanese medical institutions were recruited to participate. All received services were recorded for 2 months. In the network analysis, nodes represented types of ICM services and edges between two nodes depicted when over 5% of participants received both types of services. Results: We found high centrality values for ‘H5. Hospital-based counseling’, ‘O13. Outreach support for mental health medications’, ‘H13. Hospital-based support for mental health medication’, ‘T5. Counseling via telecommunication’, ‘H3. Hospital-based coordination of services in the medical institution’ and ‘T2. Coordination of services with other institutions via telecommunication’. These results indicated that these services were associated with various other types of services. Social functioning was related to ‘O13. Outreach support for mental health medication’, whereas need for ICM was related to ‘H13. Hospital-based support for mental health medications’, ‘T5. Counseling via telecommunication’ and ‘T2. Coordination of services with other institutions via telecommunication’ Conclusion: Based on these findings, we speculated that there are at least five types of core services in ICM: regular face-to-face contact, outreach services, hospital-based services, easy contacts and coordination. These findings clarified the features of ICM programs, which may help improve the understanding of case managers’ practice.

    Kurzfassung: Few studies have examined the more nuanced experiential facets of self-conscious emotion from a cross-cultural perspective. The present study’s aim was to investigate shame and embarrassment experiences in relation to shame logics (or appraisals), shame antecedents and intensity across cultures in Australia and Japan, drawing on Fessler’s Dual Logics Model of Shame ( Fessler, 2004 ), and applying a new instrument (The Self-Conscious Emotion Questionnaire). There were 157 participants from two cultures, Japan (75) and Australia (82) who completed both paper-based and web-based questionnaires. Previous findings showing a higher experienced shame intensity found in Japan were corroborated across all shame and embarrassment logics. While the logic of ‘norm non-conformity’ was the strongest logic in both cultures, the logic of ‘status lowness’ was prominent in Japan but not Australia, and the novel logic of ‘broken positive assumptions about the self’ was prominent in both cultures. Shame in Japan appeared to be stronger with an introspective ‘eyes of self’ but explicitly described trigger, whereas in Australia, it was more publicly ‘eyes of other’ and implicitly induced counter to some expectations. Findings support the Self-Conscious Emotion Questionnaire as an instrument for exploring nuanced aspects of self-conscious emotion in cross-cultural research and lend support to a novel third logic of ‘broken positive assumptions about the self’ in both Australian and Japanese samples.

    Kurzfassung: Intensive case management is an essential component of community mental health care system for people with severe mental illness. We examined the association of service types with previous and future functioning and hospital admissions using a longitudinal data, to clarify the case managers’ activity. Methods The study included baseline assessment, recording and analysis of services, and follow-up assessment. At the baseline and follow-up assessments, we examined functioning (ie, Personal and Social Performance Scale: PSP) and hospital admission of users for past periods. For recording and analysis of services, we recorded services provided by case managers over 2 months, and five core services were used for the analyses: regular face-to-face contact, outreach services, hospital-based (outpatient) services, and easy contacts and coordination. Results Outreach services were significantly associated with hospital admissions at baseline and follow-up assessments. Hospital-based services were significantly associated with lower PSP scores and hospital admission at follow-up assessment. Easy contacts and coordination were associated with lower PSP scores at follow-up assessment. Conclusion In Japan, most services are provided in the outpatient department of hospital, and outreach services are relatively time consuming for case managers; thus, the indications for providing outreach services need to be carefully considered. Therefore, we suggest that case managers provide flexible services to users according to the circumstances in Japan.
