    Abstract: Polypeptide growth factors, including epidermal growth factor (EGF), play a central role in regulating hepatocyte growth both in vivo and in primary culture. To characterize EGF gene expression in the pathogenesis of regenerative cirrhotic fibrosis, we employed biotinylated antisense oligonucleotide probes to localize hepatic mRNA transcripts in situ. In control tissue and regenerative hepatic nodules, EGF receptor (EGFR) mRNA transcripts were expressed constitutively. In contrast, oligonucleotide probes targeting the human EGF coding region showed that EGF transcription was extremely low in control liver but was highly elevated and localized to regenerative hepatic nodules and bile duct epithelia of cirrhotic liver. To determine whether EGF mRNA accumulation accompanied a comparable increase in the EGF peptide, we performed immunohistochemistry using an antibody specific for the nonprocessed peptide aminoterminus. We observed that positive localized EGF staining paralleled its mRNA transcript. These results indicate that EGF upregulation is a characteristic of cirrhotic liver disease and suggest that persistent de novo ligand synthesis and its signaling contribute to an autocrine-mediated hepatocyte proliferation within the regenerative nodule.

    Abstract: As an alternative to established laboratory protocols, partial least-squares (PLS) regression models based on Fourier transform infrared (FT-IR) spectra were developed for the rapid and simultaneous determination of several middle distillate fuel properties. Requiring less than 2 mL of sample, the PLS models use a single wavelength range of baseline-uncorrected FT-IR spectra. Several preprocessing parameters were investigated, including mean-centering of the raw spectra and mean-centering of first- and second-difference spectra. In addition, modification to the calibration parameters included varying the number of cross-validation rotation samples from 5 to 50, applying a baseline correction to the spectra, and using restricted wavelength regions. In order to obtain an accurate estimate of the prediction error, we applied the method provided by Faber and Kowalski [Appl. Spectrosc. 51, 660 (1997)] that corrects for the measurement error in the reference values. On the basis of this work, the following middle distillate fuel properties may be confidently estimated by FT-IR: gravity, API°; density, g/mL; kinematic viscosity, cSt, 40 °C; boiling point at 50% recovery, °C; cetane index; carbon, wt %; hydrogen, wt %; carbon-to-hydrogen ratio, C/H; net heat of combustion, MJ/Kg (or BTU/lb); monocyclic aromatics, wt %; dicyclic aromatics, wt %; polycyclic aromatics, wt %; and total aromatics, wt %. Estimation of several other properties is also possible where reduced precision is acceptable.

    Abstract: The mother–daughter relationship is seen as complex, unique, and emotionally charged, with adolescence being the most difficult period for this dyad. Yet much of the literature has highlighted the struggle of adolescents, while underemphasizing that many of their mothers themselves are confronting their own developmental issues at this time. In this article, the developmental tasks and dyadic issues of adolescent daughters (aged 13–19) and their midlife mothers (aged 35–55) are examined from a psychodynamic perspective. The authors posit that both mother and daughter are experiencing a lifecycle crisis of separation and self-definition. Drawing from psychoanalytic and lifespan developmental literature as well as their own clinical experience, the authors propose that the current generation of mothers and daughters requires a new look at old models, parts of which may still be relevant and parts of which need to be revised or dropped. The issues of competition and rebellion as they relate to separation and self-definition are also addressed.

    Abstract: To report a case of bilateral nonarteritic anterior ischemic optic neuropathy (NAION) in a hepatitis C (HCV) infected patient and demonstrate the relationship between HCV and the development of NAION. Methods Case report. Results A 43-year-old woman with chronic HCV infection and long-term euthyroid autoimmune thyroiditis suddenly lost vision in her right eye, and 6 months later in her left eye, due to NAION. Slightly elevated levels of aminotransferases suggested liver infection activity. Anti-HCV antibody was detected; the genotype of the virus was 1b and the viral RNA level was 1.8 × 106 IU/mL. Liver biopsy proved chronic active hepatitis (Ishak score grading: 7, staging: 2). Except for the elevated levels of antithyroid antibodies and a weak antinuclear factor, the detailed laboratory examinations (thrombophilia, cryoglobulin, anticardiolipin antibodies, co-infections) revealed no other abnormalities; a causative relationship between the underlying chronic hepatitis C and bilateral NAION therefore seems probable. The patient was treated with pegylated interferon and ribavirin for 1 year and a sustained viral remission could be achieved. Her vision has neither improved nor deteriorated further. Conclusions This appears to be the first reported case of bilateral NAION presumably caused by HCV infection.

    Abstract: The aim of this experimental cadaver study was to verify that thumb traction radiography can be used to diagnose scapholunate interosseous ligament (SLIL) injury. Eight cadaver forearms were positioned vertically so that the thumb could be held in a Chinese finger trap and traction force applied using a 5 kg weight. Fluoroscopy was performed with the thumb unloaded and under traction, and then unloaded and under traction after division of the SLIL. The scapholunate joint gaps were measured electronically. The difference between the unloaded and loaded wrists with sectioned SLIL was not statistically significant. These results suggest that thumb traction radiography might not reliably detect acute, complete SLIL tears.
