    Abstract: A 1-year-old female Boer goat was presented for necropsy following spontaneous death and history of acute recumbency, nystagmus, and sialorrhea. A swollen area was grossly observed in the brainstem at the level of the pons. On cut surface, the right cerebellar peduncles were expanded by a focal, pale, poorly demarcated, slightly depressed, and soft area of malacia. Microscopically, this area contained diffuse edema and necrosis, with microabscesses, neuronal necrosis, neuronophagia, axonal spheroids, vasculitis, and perivascular accumulations of lymphocytes, plasma cells, macrophages, and neutrophils. The diagnosis was based on the morphologic findings, fluorescent antibody test results, and special staining.

    Extending access to care across the rural US south: Preliminary results from the Alabama eHealth programme

    Abstract: Using a mixed-methods formative evaluation, the purpose of this study was to provide a broad overview of the Alabama eHealth programme set-up and initial patient outcomes. The Alabama eHealth programme uses telemedicine to provide medical care to people living with HIV in rural Alabama. It was led by a community-based organisation, Medical Advocacy and Outreach (MAO), and supported by AIDS United and the Corporation for National Community Service’s Social Innovation Fund with matching support from non-federal donors. Methods We conducted and transcribed in-depth interviews with Alabama eHealth staff and then performed directed content analysis. We also tracked patients’ ( n = 240) appointment attendance, CD4 counts, and viral loads. Findings Staff described the steps taken to establish the programme, associated challenges (e.g., costly, inadequate broadband in rural areas), and technology enabling this programme (electronic medical records, telemedicine equipment). Of all enrolled patients, 76% were retained in care, 88% had antiretroviral therapy and 75% had a suppressed viral load. Among patients without missing data, 96% were retained in care, 97% used antiretroviral therapy and 93% had suppressed viral loads. There were no statistically significant demographic differences between those with and without missing data. Conclusions Patients enrolled in a telemedicine programme evaluation successfully moved through the HIV continuum of care.

He is currently Director of the program for executives at IBM in White Plains, New York.

    Abstract: Renal interstitial cell tumor (RICT) is a rare renal sarcoma of dogs that arises from renal interstitial cells. Herein we describe a RICT in an 8-y-old female Labrador Retriever dog that died after a 2-d history of lethargy and disorientation. Grossly, soft white nodules of 1–10 mm diameter were present in the renal cortex and corticomedullary junction of both kidneys, left cardiac ventricular wall, and right cerebral hemisphere. A pale-white to yellow, firm, irregular mass effaced 80% of the right pulmonary parenchyma, involving mainly the cranial and middle lobes, and the adjacent tracheobronchial lymph nodes. Histologically, the renal, myocardial, and cerebral neoplasm consisted of interlacing bundles of stellate-to-spindle cells with eosinophilic vacuolated cytoplasm and round-to-oval nuclei with finely stippled chromatin. The mitotic count was 28 per 2.37 mm 2 . Alcian blue stain revealed an extracellular myxomatous matrix throughout the neoplasm. Neoplastic cells had cytoplasmic immunolabeling for vimentin and cyclooxygenase 2. The pulmonary and tracheobronchial neoplasm consisted of infiltrative nodules of cuboidal epithelial cells that had a moderate amount of eosinophilic cytoplasm and round nuclei with coarsely stippled chromatin. There were 5 mitoses per 2.37 mm 2 . Neoplastic cells had cytoplasmic and nuclear immunolabeling for cytokeratin AE1/AE3 and thyroid transcription factor 1, respectively. Morphologic and immunohistochemical findings were consistent with a RICT with cardiac and cerebral metastases, and a pulmonary carcinoma with tracheobronchial lymph node metastasis.

    Abstract: Twenty-three patients with acute anterior cruciate ligament injuries, normal radiographs, and occult osteochondral lesions revealed by magnetic resonance imaging were reviewed 6 years after initial injury and anterior cruciate ligament hamstring autograft reconstruction. Each patient completed the Mohtadi Quality of Life outcome measure for anterior cruciate ligament deficiency, underwent clinical examination, and had a repeat magnetic resonance imaging scan. The index and follow-up magnetic resonance imaging scans were compared with respect to cartilage thinning and marrow signal. A significant number of patients had evidence of cartilage thinning adjacent to the site of the initial osteochondral lesion. Marrow signal changes persisted in 15 (65%) of the patients. Clinical comparison of patients with normal cartilage with those who had cartilage thinning revealed similar results on both KT-1000 arthrometry and on the Mohtadi outcome measure. This suggests that the initial injury resulted in irreversible changes in the knee. Injuries causing marrow signal changes may result in an alteration in the load-bearing properties of subchondral bone, which in turn allow for changes in the overlying cartilage. Further follow-up will determine the clinical significance of changes detected by magnetic resonance imaging.

    Abstract: PEGylation (the covalent binding of one or more polyethylene glycol molecules to another molecule) is a technology frequently used to improve the half-life and other pharmaceutical or pharmacological properties of proteins, peptides, and aptamers. To date, 11 PEGylated biopharmaceuticals have been approved and there is indication that many more are in nonclinical or clinical development. Adverse effects seen with those in toxicology studies are mostly related to the active part of the drug molecule and not to polyethylene glycol (PEG). In 5 of the 11 approved and 10 of the 17 PEGylated biopharmaceuticals in a 2013 industry survey presented here, cellular vacuolation is histologically observed in toxicology studies in certain organs and tissues. No other effects attributed to PEG alone have been reported. Importantly, vacuolation, which occurs mainly in phagocytes, has not been linked with changes in organ function in these toxicology studies. This article was authored through collaborative efforts of industry toxicologists/nonclinical scientists to address the nonclinical safety of large PEG molecules ( 〉 10 kilo Dalton) in PEGylated biopharmaceuticals. The impact of the PEG molecule on overall nonclinical safety assessments of PEGylated biopharmaceuticals is discussed, and toxicological information from a 2013 industry survey on PEGylated biopharmaceuticals under development is summarized. Results will contribute to the database of toxicological information publicly available for PEG and PEGylated biopharmaceuticals.
