    Abstract: Introduction: After more than 20 years of sustained work, the Latin American Group for the Study of Lupus (GLADEL) has made a significant number of contributions to the field of lupus, not only in the differential role that race/ethnicity plays in its course and outcome but also in several other studies including the beneficial effects of using antimalarials in lupus patients and the development of consensus guidelines for the treatment of lupus in our region. Methods: A new generation of “Lupus Investigators” in more than 40 centers throughout Latin America has been constituted in order to continue the legacy of the investigators of the original cohort and to launch a novel study of serum and urinary biomarkers in patients with systemic lupus erythematosus. Results: So far, we have recruited 807 patients and 631 controls from 42 Latin-American centers including 339 patients with SLE without renal involvement, 202 patients with SLE with prevalent but inactive renal disease, 176 patients with prevalent and active renal disease and 90 patients with incident lupus nephritis. Conclusions: The different methodological aspects of the GLADEL 2.0 cohort are discussed in this manuscript, including the challenges and difficulties of conducting such an ambitious project.

    Abstract: This study compared decision-making (DM) of experienced and novice volleyball coaches while measuring blood flow brain activation with functional near-infrared spectroscopy. We sampled 34 coaches (mean [ M] age of 32.5, standard deviation [ SD]  = 9.4 years) divided into two experience groups: (a) novice ( M = 2.8, SD = 1.9 years) and (b) experienced (M = 19, SD = 7.2 years). We evaluated coaches’ DM through their responses to video-based scenarios of attacks performed in the extremities of the net within the Declarative Tactical Knowledge Test in Volleyball. We found no significant DM differences between the two groups of coaches ( p = .063), though novice (vs. experienced) coaches showed greater blood flow of the prefrontal cortex when visualizing the game situations. While experienced coaches may have better prefrontal neural efficiency during DM in these situations, further research is needed to evaluate other cerebral areas; since blood flow is an indirect measure of neural efficiency, and activity in remaining cortical components was unknown in this study.

    Abstract: This study examined 142 volleyball games of the Men’s Super League 2014/2015 seasons in Brazil from which we analyzed 24–26 games of each participating team, identifying 5,267 Zone 4 attacks for further analysis. Within these Zone 4 attacks, we analyzed the association between the effect of the attack carried out and the separate effects of serve reception, tempo and type of attack. We found that the reception, tempo of attack, second tempo of attack, and power of diagonal attack were predictors of the attack effect in Zone 4. Moreover, placed attacks showed a tendency to not yield a score. In conclusion, winning points in high-level men's volleyball requires excellent receptions, a fast attack tempo and powerfully executed of attacks.

    Abstract: Sports; Ankle Introduction/Purpose: The number of professional and amateur athletes has increased, consequently leading to an increase in the incidence of sports injuries. Understanding the epidemiological profile of foot and ankle's injuries in athletes is relevant for the prevention and treatment its occurrence. As far as we know, there is a lack of studies that report the epidemiological profile of athletes with foot and ankle disorders considering different sports. The aim of this study is to describe the epidemiology of foot and ankle injuries in athletes treated at an outpatient clinic specializing in injuries resulting from sports practice. The primary hypothesis is that ankle instability is the most frequent injury in athletes in the foot and ankle specialty outpatient clinic. Methods: Observational, retrospective, descriptive and comparative level IV of evidence study. The sample consisted of patients treated at an outpatient clinic specializing in foot and ankle injuries in sport, between January 1, 2015 and December 31, 2020. Three certified orthopedic physicians reviewed the charts and documented patient demographics, sport, diagnosis and lesion laterality. Age was analyzed according to age groups. The inclusion criteria were patients practicing amateurs or sports professionals of all age groups. We only count the first visits and the main modality and diagnosis of the patient. Exclusion criteria were insufficient data documented in chart, injuries that did not occur secondary to sports practice, and patients with conditions unrelated to the ankle and foot. Statistical analyzes were performed using software: SPSS V20, Minitab 16 and Excel Office 2010. We performed a multivariate logistic regression analysis to verify the association of gender, age and laterality with the main injuries documented. Results: A total of 763 patients (387 men and 376 women) met the inclusion criteria. The mean age was 35 years (SD= +-14.1; min=8.9; max=77). The most prevalent age group was young adults with 51.7% and the least prevalent was children with 1.4%. The most prevalent diseases were chronic lateral ankle instability (n=134, 17.6%), acute lateral ankle sprain (n=120, 15.7%) and plantar fasciitis (n=69, 9%). The multivariable analysis by logistic regression identified as a risk factor for lateral ankle instability the age group of young adults and females. In acute lateral ankle sprain, age was identified as a risk factor with statistical significance. In overuse injuries, only the female gender was evidenced as a risk factor. In non-insertional Achilles tendinopathy, bilaterality was associated as a risk factor with disease while in insertional Achilles tendinopathy, only the male gender was identified as a risk factor, as well as in Achilles tendon rupture. Conclusion: The literature specifically describing foot and ankle injuries during sport is limited, despite being highly prevalent injuries. Our work correlated several factors such as age, gender and laterality with specific sports through multivariate analysis, adding epidemiological data to the literature that help in the prevention and treatment of foot and ankle injuries in sports.

    Abstract: Transplantation of autologous bone marrow mononuclear cells (BMMCs) has been proven safe in animal and human studies. However, there are very few studies in stroke patients. In this study, intra-arterial autologous BMMCs were infused in patients with moderate to severe acute middle cerebral artery infarcts. The subjects of this study included 20 patients with early or late spontaneous recanalization but with persistent deficits, in whom treatment could be initiated between 3 and 7 days after stroke onset. Mononuclear cells were isolated from bone marrow aspirates and infused at the proximal middle cerebral artery of the affected hemisphere. Safety analysis (primary endpoint) during the 6-month follow-up assessed death, any serious clinical events, neurological worsening with ≥ 4-point increase in National Institutes of Health Stroke Scale (NIHSS) scores, seizures, epileptogenic activity on electroencephalogram, and neuroimaging complications including new ischemic, hemorrhagic, or neoplastic lesions. Satisfactory clinical improvement (secondary endpoint) at 90 days was defined according to the pretreatment NIHSS scores as follows: modified Rankin Scale score of 0 in patients with NIHSS 〈 8, modified Rankin Scale scores of 0–1 in patients with NIHSS 8–14, or modified Rankin Scale scores 0–2 in patients with NIHSS 〉 14. Good clinical outcome was defined as mRS ≤2 at 90 days. Serial clinical, laboratory, electroencephalogram, and imaging evaluations showed no procedure-related adverse events. Satisfactory clinical improvement occurred in 6/20 (30%) patients at 90 days. Eight patients (40%) showed a good clinical outcome. Infusion of intra-arterial autologous BMMCs appears to be safe in patients with moderate to severe acute middle cerebral artery strokes. No cases of intrahospital mortality were seen in this pilot trial. Larger prospective randomized trials are warranted to assess the efficacy of this treatment approach.

    Abstract: Exposure to particulate matter (PM) air pollution is associated with increased asthma morbidity. Residual oil flash ash (ROFA) is rich in water-soluble transition metals, which are involved in the pathological effects of PM. The objective of this study was to investigate the effects of intranasal administration of ROFA on pulmonary inflammation, pulmonary responsiveness, and excess mucus production in a mouse model of chronic pulmonary allergic inflammation. BALB/c mice received intraperitoneal injections of ovalbumin (OVA) solution (days 1 and 14). OVA challenges were performed on days 22, 24, 26, and 28. After the challenge, mice were intranasally instilled with ROFA. After forty-eight hours, pulmonary responsiveness was performed. Mice were sacrificed, and lungs were removed for morphometric analysis. OVA-exposed mice presented eosinophilia in the bronchovascular space ( p 〈 .001), increased pulmonary responsiveness ( p 〈 .001), and epithelial remodeling ( p = .003). ROFA instillation increased pulmonary responsiveness ( p = .004) and decreased the area of ciliated cells in the airway epithelium ( p = .006). The combined ROFA instillation and OVA exposure induced a further increase in values of pulmonary responsiveness ( p = .043) and a decrease in the number of ciliated cells in the airway epithelium ( p = .017). PM exposure results in pulmonary effects that are more intense in mice with chronic allergic pulmonary inflammation.
