    Abstract: Highway traffic incidents induce a significant loss of life, economy, and productivity through injuries and fatalities, extended travel time and delay, and excessive energy consumption and air pollution. Traffic emergency management during incident conditions is the core element of active traffic management, and it is of practical significance to accurately understand the duration time distribution for typical traffic incident types and the factors that influence incident duration. This study proposes a dual-learning Bayesian network (BN) model to estimate traffic incident duration and to examine the influence of heterogeneous factors on the length of duration based on expert knowledge of traffic incident management and highway incident data collected in Zhejiang Province, China. Fifteen variables related to three aspects of traffic incidents, including incident information, incident consequences, and rescue resources, were included in the analysis. The trained BN model achieves favorable performance in several areas, including classification accuracy, the receiver operating characteristic (ROC) curve, and the area under curve (AUC) value. A classification matrix, and significant variables and their heterogeneous influences are identified accordingly. The research findings from this study provide beneficial reference to the understanding of decision-making in traffic incident response and process, active traffic incident management, and intelligent transportation systems.

    Abstract: Since entering the 21 st century, there has been an increasing interest in minimally invasive surgery for spinal diseases, which has led to the continued development of minimally invasive spine surgery (MISS), with major breakthroughs in technology and technical skills. However, in recent years, there is little relevant research using bibliometrics to analyze the field of MISS research. The purpose of this study is to sort out the publication situation and topic trends of articles in the field of MISS research from the perspective of bibliometrics. Methods The articles and reviews related to MISS from 2000 to 2022 were retrieved and downloaded from the Web of Science Core Collection (WOSCC). Visualization and knowledge mapping were performed using three bibliometric tools, including online bibliometric platform, CiteSpace and VOSviewer software. Curve fitting and correlation analysis were performed using Microsoft Excel software. The global research publication output, contributions of countries, institutions, authors, and journals, average citations per item (ACI), Hirsch index ( H-index), research hot keywords, etc., in this field were analyzed. Results A total of 2384 papers were retrieved, including 2135 original papers and 249 review papers. In the past 22 years, the number of annual publications of MISS research has shown a steady growth trend. China contributed the most papers, and the United States ranked second, but the United States had the highest total citations, and H-index value. The most prolific institutions were Soochow University, Capital Medical University and Wooridul Spine Hospital. In this field, Professors Lee SH, Ahn Y and Yang HL have made significant achievements. However, there is relatively little international collaboration between institutions or researchers. World Neurosurgery is the most published journal on MISS research. According to the keyword co-occurrence analysis, recent keywords mainly focus on researches on minimally invasive modalities, techniques and prognosis, while on the keyword analysis of the ongoing bursts, percutaneous transforaminal endoscopic discectomy, lumbar diskectomy, spinal stenosis, recompression, diskectomy, endoscopic spine surgery, laminectomy, transforaminal lumbar interbody fusion, etc., will likely continue to be a research hotspot in the near future. Conclusion Looking at the temporal trend in the number of publications per year, the number of publications for the MISS study will increase in the near future. China has the highest number of publications, but the US has the highest quality publications. International cooperation needs to be further strengthened. Our findings can provide useful information for the academic community and identify possible research fronts and hotspots in the coming years.

    Abstract: We investigated the preventive effect of alprostadil on contrast-induced nephropathy (CIN) in patients with renal insufficiency undergoing percutaneous coronary intervention (PCI). A total of 300 patients with creatinine clearance (crCl) ≤60 mL/min undergoing PCI were randomly assigned to alprostadil or a control group. The primary end point was the incidence of CIN defined as an increase in serum creatinine (Scr) levels by ≥0.5 mg/dL or≥ 25% after administration of the contrast media within 72 hours. The secondary end points were (1) changes in Scr and crCl within 72 hours and (2) the incidence of major adverse events during hospitalization. The incidence of CIN was 2.7% (4/150) in the alprostadil group, and 8.7% (13/150) in the control group (χ 2 = 5.05, P = .043).There was no difference regarding the incidence of major adverse events during hospitalization between the alprostadil group and control groups (2.7% vs 4.0%, P = .750). Multivariate logistic regression analysis showed that alprostadil was an independent protective factor for CIN (odds ratio = 0.136, 95% confidence interval: 0.020-0.944, P = .044). Prophylactic administration of alprostadil may prevent CIN in patients with renal insufficiency undergoing PCI.

    Abstract: Extended Kalman filter is well-known as a popular solution to the simultaneous localization and mapping problem for mobile robot platforms or vehicles. In this article, the development of a neuro-fuzzy-based adaptive extended Kalman filter technique is presented. The objective is to estimate the proper values of the R matrix at each step. We design an adaptive neuro-fuzzy extended Kalman filter to minimize the difference between the actual and theoretical covariance matrices of the innovation consequence. The parameters of the adaptive neuro-fuzzy extended Kalman filter is then trained offline using a particle swarm optimization technique. With this approach, the advantages of high-dimensional search space can be exploited and more effective training can be achieved. In the experiments, the mobile robot navigation with a number of landmarks under two benchmark situations is evaluated. The results have demonstrated that the proposed adaptive neuro-fuzzy extended Kalman filter technique provides the improvement in both performance efficiency and computational cost.

    Abstract: Breast-conserving surgery is facing the challenge of objective tumor margin identification intraoperatively. Near-infrared fluorescence imaging would be an ideal approach to visualize tumor margins during surgeries. In this preliminary study, the feasibility of methylene blue–based near-infrared fluorescence imaging technique for breast cancer detection was assessed in resected human breast specimens after breast cancer surgeries. Thirty patients with breast cancer scheduled for surgical treatment were enrolled, including 10 patients with preoperative chemotherapy and 20 patients without. Each of them received an injection of 1 mg/kg methylene blue intravenously 3 hours before the surgery. Then, a home-developed methylene blue–specific near-infrared fluorescence imaging system was employed to image the resected breast tissues and identify the tumor by the fluorescence contrast. Specimens were taken for pathological examinations as the reference. There were no severe adverse events attributable to methylene blue. Of 20 patients, who did not receive preoperative chemotherapy, 16 exhibited fluorescent contrast on their resected tissues (signal-to-background ratio: 1.94 ± 0.71). In contrast, tumors were identified in 3 of 10 specimens from patients who underwent preoperative chemotherapy (signal-to-background ratio: 1.63 ± 0.38). A total of 35 tissues were sampled from 30 specimens. Besides 30 tumor samples, 5 more suspicious samples with fluorescence signal were confirmed to be benign hemorrhagic tissues. Therefore, a sensitivity of 0.63 and a positive predictive value of 0.79 were achieved by the methylene blue fluorescence imaging strategy. Here, we demonstrate the feasibility of using methylene blue fluorescence imaging to identify breast cancer. Preoperative chemotherapy had an impact on imaging effect, which may reduce the detection rate.     Abstract: The on-track/off-track concept for shoulder instability primarily describes the medial-lateral rotational relationship between an engaging Hill-Sachs lesion and a Bankart defect. Though clinically more protective, on-track lesions retain some risk for failure following primary arthroscopic Bankart repair. While some of this risk can be explained by the “near-track” concept, the role of the superior-inferior position of the Hill-Sachs lesion has never been studied in the context of failure of primary Bankart repair. This study aims to identify the relationship between the superior-inferior position of a Hill-Sachs lesion and risk for failure following primary arthroscopic bankart repair. Our hypothesis is that inferiorly-based Hill-Sachs lesions may engage with the arm in neutral and thus be higher risk for failure following primary Bankart repair. Methods: We performed a retrospective analysis of 201 individuals with on track lesions who underwent primary arthroscopic Bankart repair (without remplissage) between 2007 and 2019 who have minimum 2 year follow-up. Patients with failure were defined as those who sustained a dislocation or subluxation after the index procedure. A pre-operative sagittal MRI cut showing the maximum Hill-Sachs diameter was used for position analysis. Sagittal position of the Hill-Sachs was defined the angle formed by the Hill-Sachs bisecting line through the humeral head center, against the mid-humeral axis on a sagittal MRI cut (Figure 1); for example, an angle of 0 is twelve o’clock on the humeral head, while an angle of 90 is equatorial. We defined a priori four Hill-Sachs quadrants for semi-quantitative analysis, based on physiologic arm positions: Superior (angle 〈 40), Mid-Superior (40-60), Mid (61-90), and Inferior ( 〉 90). Hill-Sachs quadrants were then correlated against failure following primary arthroscopic Bankart repair. Results: Failure rates following arthroscopic bankart repair as it relates to superior-inferior position of the Hill-Sachs lesion is as follows (Table 1): No Hill-Sachs (10 of 73, 13.7%), Superior (0 of 7, 0%), mid-superior (6 of 36, 16.7%), Mid (19 of 71, 26.8%), and Inferior (1 of 6, 16.7%). We grouped Hill-Sachs lesions into low grade (No Hill-Sachs, Superior, and Mid-Superior quadrants) and high grade (Mid and Inferior quadrants). Low grade represented a 13.8% risk of failure, while High grade represented a 26% risk for failure (p=0.034). Conclusions: The superior-inferior sagittal position of a Hill-Sachs lesion may contribute to risk for failure of primary arthroscopic Bankart repair for on-track lesions. Inferiorly-based Hill-Sachs lesions may risk engagement at lower degrees of arm abduction, and in our study represent nearly double the risk of failure of arthroscopic Bankart repair as compared to superior Hill-Sachs positions.     Abstract: This event-related potential (ERP) study examined the effect of contextual arousal on emotional word integration during discourse comprehension. We used two-sentence discourses as experimental materials. The first sentence served as an emotional context and described a high-arousal positive event, a low-arousal positive event, a high-arousal negative event, or a low-arousal negative event. The second sentence contained one negative word as the critical word, which was identical among different conditions. Thus, four conditions were included in the present study: high-arousal inconsistent, low-arousal inconsistent, high-arousal consistent, and low-arousal consistent. The ERP results showed that inconsistent emotional words elicited larger P200 and LPC than consistent words in the high-arousal context. However, only a P200 effect was observed for inconsistent emotional words in the low-arousal context. Our results indicate that a high-arousal context could lead to more elaborated emotional evaluation in the later stage of emotional word integration and suggest an important role of contextual arousal on the processing of emotional words during discourse processing.

    Abstract: Autosomal dominant polycystic kidney disease (ADPKD) is characterized by progressive renal cyst formation and expansion. Several clinical trials show that somatostatin analogs halt cyst growth and progression of ADPKD by inhibiting adenosine 3′,5′-cyclic monophosphate (cAMP) signaling. However, two studies suggest that the effect of the somatostatin analog octreotide on kidney growth during the first year of treatment is reduced in the subsequent follow-ups and the kidney enlargement resumes. We hypothesize that this biphasic change in kidney growth during octreotide treatment may be due to changes in somatostatin receptor 2 (SSTR2) expression. Here we analyzed the expression of renal SSTR2 in various polycystic kidney disease (PKD) mouse models in which PKD1 gene expression was disrupted on postnatal day 10 or 18 by tamoxifen. Using immunohistochemical analysis, we showed that the distribution of SSTR2 in murine kidneys is mainly in distal tubules and collecting ducts. In addition, in both PKD models, we observed a significant decrease in SSTR2 expression in epithelia of dilated tubules and cystic epithelia in mice with end stage of PKD compared to wild-type mice. These findings were further confirmed by quantitative PCR (qPCR) on mRNA levels of SSTR2. In conclusion, our data show that SSTR2 expression levels are reduced during kidney cyst growth, which may suggest reduced efficacy in long-term treatment with somatostatin analogs.

    Abstract: The aim of the present study was to determine the neuroimaging predictors of poor participation after acute ischemic stroke. A total of 443 patients who had acute ischemic stroke were assessed. At 1-year recovery, the Reintegration to Normal Living Index was used to assess participation restriction. We also assessed the Activities of Daily Living Scale and modified Rankin Scale (mRS) score. Brain MRI measurement included acute infarcts and pre-existing abnormalities such as enlarged perivascular spaces, white matter lesions, ventricular-brain ratio, and medial temporal lobe atrophy (MTLA). The study included 324 men (73.1%) and 119 women (26.9%). In the univariate analysis, patients with poor participation after 1 year were older, more likely to be men, had higher National Institutes of Health Stroke Scale (NIHSS) score on admission, with more histories of hypertension and atrial fibrillation, larger infarct volume, more severely enlarged perivascular spaces and MTLA, and more severe periventricular hyperintensities and deep white matter hyperintensities. Patients with participation restriction also had poor activities of daily living (ADL) and mRS score. Multiple logistic regression showed that, in model 1, age, male gender, NIHSS score on admission, and ADL on follow-up were significant predictors of poor participation, accounting for 60.2% of the variance. In model 2, which included both clinical and MRI variables, male gender, NIHSS score on admission, ADL on follow-up, and MTLA were significant predictors of poor participation, accounting for 61.2% of the variance. Participation restriction was common after acute ischemic stroke despite good mRS score. Male gender, stroke severity, severity of ADL on follow-up, and MTLA may be predictors of poor participation.
