    Abstract: Benzo[ a]pyrene (BaP) is a prototype for studying carcinogenesis of polycyclic aromatic hydrocarbons (PAHs). We have long been interested in studying the phototoxicity of PAHs. In this study, we determined that metabolism of BaP by human skin HaCaT keratinocytes resulted in six identified phase I metabolites, for example, BaP trans-7,8-dihydrodiol (BaP t-7,8-diol), BaP t-4,5-diol, BaP t-9,10-diol, 3-hydroxybenzo[a] pyrene (3-OH-BaP), BaP (7,10/8,9)tetrol, and BaP (7/8,9,10)tetrol. The photocytotoxicity of BaP, 3-OH-BaP, BaP t-7,8-diol, BaP trans-7,8-diol- anti-9,10-epoxide (BPDE), and BaP (7,10/8,9)tetrol in the HaCaT keratinocytes was examined. When irradiated with 1.0 J/cm 2 UVA light, these compounds when tested at doses of 0.1, 0.2, and 0.5 μM, all induced photocytotoxicity in a dose-dependent manner. When photoirradiation was conducted in the presence of a lipid (methyl linoleate), BaP metabolites, BPDE, and three related PAHs, pyrene, 7,8,9,10-tetrahydro-BaP trans-7,8-diol, and 7,8,9,10-tetrahydro-BaP trans-9,10-diol, all induced lipid peroxidation. The formation of lipid peroxides by BaP t-7,8-diol was inhibited by NaN 3 and enhanced by deuterated methanol, which suggests that singlet oxygen may be involved in the generation of lipid peroxides. The formation of lipid hydroperoxides was partially inhibited by superoxide dismutase (SOD). Electron spin resonance spin trapping experiments indicated that both singlet oxygen and superoxide radical anion were generated from UVA photoirradiation of BPDE in a light dose responding manner.

    Abstract: High tibial osteotomy is a valuable option for patients with varus gonarthrosis. To avoid difficulties with closing-wedge osteotomies, medial opening-wedge high tibial osteotomies have been advocated. Hypothesis Opening-wedge high tibial osteotomy is a good option in highly active patients with varus gonarthrosis who would like to delay or prevent progression to total knee arthroplasty without activity restrictions. Study Design Case series; Level of evidence, 4. Methods Twenty consecutive patients with varus gonarthrosis were treated with a medial opening-wedge high tibial osteotomy using the Puddu plate and allograft bone graft for a prospective study (14 men and 6 women; average age, 49.4 years [range, 36-67 years]). Gait analysis was performed preoperatively and at 6 months postoperatively. Preoperative radiographs, subjective ratings, and knee scores (Lysholm and Hospital for Special Surgery [HSS] scores) were obtained. At 2 years postoperatively and at the latest follow-up visit (average, 8.3 years), the subjective ratings and knee scores were repeated. Results Gait analysis revealed an abnormal weightbearing pattern preoperatively with the vertical ground-reaction force. The postoperative vertical ground-reaction force revealed a normal double peak pattern. The preoperative adduction moment was 29% greater than the 6-month postoperative adduction moment. The preoperative varus averaged 3.6° and was corrected to an average of 7.5° of valgus postoperatively. All patients subjectively rated their preoperative knee as poor. At 2 years postoperatively, most patients (14) rated their knee as good, with 5 excellent and only 1 fair rating. The average preoperative Lysholm and HSS knee scores were 54.2 and 75.9, respectively, compared with the 2-year postoperative averages of 89.1 and 92.7, respectively. At 8 years postoperatively, there was 70% survivorship with 42% of patients rating their knees as good or excellent. Five patients (25%) had undergone total knee arthroplasty. Lysholm and HSS knee scores were 83.0 and 86.8, respectively, for the surviving knees at 8 years postoperatively. Conclusion Medial opening-wedge high tibial osteotomy produces good results in the midterm. After the osteotomy, a more normal appearing weightbearing pattern with double peaks was seen. The adduction moment significantly decreased, resulting in less contact pressure through the medial degenerative compartment of the knee. The authors recommend medial opening-wedge high tibial osteotomy for young patients with varus alignment and medial compartment arthritis to allow this patient population to remain highly active and delay progression to total knee arthroplasty without activity restrictions.

    Abstract: To determine the accuracy and reliability of diagnosing aggressive posterior retinopathy of prematurity (AP-ROP). Methods: A total of 1220 eye examinations from 230 infants were prospectively obtained at 8 major ROP centers. An ophthalmologist at each center provided a clinical diagnosis using indirect ophthalmoscopy. Wide-angle retinal images were then obtained, which were independently read by 2 ROP experts using a web-based system for an image-based diagnosis. Sensitivity and specificity of image-based AP-ROP diagnosis by the ROP experts were calculated using the clinical diagnosis as the reference standard. Agreement of AP-ROP diagnosis through image-based diagnosis and clinical diagnosis was calculated using the unweighted κ statistic. Results: One hundred four (9%) of the 1220 examinations had a clinical diagnosis of AP-ROP. Sensitivity and specificity for the presence of AP-ROP were 35% and 96% for expert 1 and 17% and 99% for expert 2. Using the κ statistic, expert image-based versus clinical diagnostic agreement for the diagnosis of AP-ROP was 0.34 (fair) for expert 1 and 0.24 (fair) for expert 2. Agreement for the diagnosis of AP-ROP between the image-based diagnoses of expert 1 and expert 2 was 0.49 (moderate). Conclusion: There are inconsistencies between the clinical diagnosis of AP-ROP (as determined by indirect ophthalmoscopy) and the image-based diagnosis of AP-ROP. This may have important implications for ROP management and the current international ROP classification system.

    Abstract: The spatial variability of Holocene relative sea level (RSL) in the South China Sea is unknown, with data restricted to Thailand, the Malay Peninsula, and a few other isolated sites. In this study, we present new continuous RSL records for Borneo using surveyed and U–Th dated coral microatolls from four sites in western Sarawak. The record spans 450 years of RSL from 7450 to 7000 yr BP. Our data suggest that RSL was higher than present and rapid RSL rise had ceased by 7450 yr BP. We compare these RSL reconstructions with a regional model of glacial-isostatic adjustment (GIA). The RSL reconstructions from three sites off the coast of Sarawak show a spatial gradient opposite to that predicted by the GIA model. This disagreement can best be explained by tectonic deformation since 7000 yr BP, which was previously unrecognized. We propose vertical land motion of 0.7–1.45 m due to slip on the Serabang fault, which runs between our four sites. This slip may have occurred in response to the loading of the Sunda Shelf by rising sea level.
