    Abstract: Vitamin D deficiency is reported to be associated with increased incidence of allergic airway diseases and is correlated with the severity of asthma. This study was designed to determine if serum Vitamin D level is lower in chronic rhinosinusitis with nasal polyposis (CRSwNP) patients and if low serum Vitamin D level is correlated with the severity of CRSwNP. Methods New CRSwNP patients undergoing elective endoscopic sinus surgery were recruited. Patients with malignancies or asthma were excluded. Twenty chronic rhinosinusitis without nasal polyposis (CRSsNP) patients were used as control. Demographic characteristic information was collected. The severity of CRSwNP was assessed with the Lund-Mackay (LM) score and polyp grading system. Vitamin D status was assessed by measuring circulating 25-hydroxyvitamin D (25OHD) by using commercial chemiluminescence immunoassay. Data were stratified by factors known to affect serum 25OHD, including sex, race, and body mass index. Results Serum 25OHD levels (ng/mL ± SD) were significantly lower in patients with CRSwNP (21.4 ± 5.7) than in those with CRSsNP (28.8 ± 6.2; p 〈 0.001). The incidences of vitamin D deficiency ( 〈 20 ng/mL) in CRSwNP and CRSsNP patients were 45.5 and 6.3%; however, the incidences of vitamin D insufficiency (20–30 ng/mL) in these patients were 50.0 and 62.5%, respectively. A significantly negative relationship was found between serum 25OHD level and polyp grade (r = −0.63; p = 0.001), indicating lower serum 25OHD was associated with higher polyp grade. Serum 25OHD was inversely related to both LM score and total IgE level as well; however, statistical significance was not found. Conclusion A significantly lower vitamin D level was found in a group of Taiwanese CRSwNP patients, which revealed an association with greater nasal polyp size. Serum vitamin D levels could be added to the routine workup of patients suffering from CRS and these data could be used to potentially help determine the disease severity.

    Abstract: Obtaining representative samples from populations of emerging adults who do not attend college is challenging for researchers. This article introduces respondent-driven sampling (RDS), a method for obtaining representative samples of hard-to-reach but socially interconnected populations. RDS combines a prescribed method for chain referral with a mathematical model to adjust for potential sampling bias. A case study is presented of the use of RDS to obtain a sample of rural African American emerging adults and to document the feasibility, effectiveness, and efficiency of the method with this population. RDS-related findings indicate that rural African American emerging adults form interconnected social networks that are appropriate for using RDS. RDS procedures are an acceptable and relatively efficient strategy for rural African Americans that is likely to yield more representative samples than traditional snowball sampling and provides data on potential sampling biases for planning multivariate analyses.

    Abstract: Plain language summary A prognostic index in locally advanced and metastatic gastric cancer To date, no recognized systematic prognostic score has been established for advanced gastric cancer (AGC). Our research aims to construct an effective prognostic index to predict overall survival (OS) for AGC patients to aid clinical decision-making and individual risk stratification. In our research, seven prognostic factors were identified: primary tumor site in the non-proximal gastric area, signet-ring cell carcinoma (SRCC)/mucinous carcinoma, peritoneal metastasis, neutrophil count higher than the upper limit of normal value (ULN), lymphocyte count lower than the lower limit of normal value, lactate dehydrogenase level higher than the ULN, and alkaline phosphatase level higher than the ULN as significant for prognosis. A prognostic index named the Fudan advanced gastric cancer prognostic risk score (FARS) index was constructed, and patients in the high-risk group had significantly shorter OS than those in low-risk group (median OS, 15.5 months vs. 8.0 months, P 〈 0.001). Moreover, three triplet regimen efficacy parameters were identified: SRCC/mucinous adenocarcinoma, primary tumor location in the non-proximal gastric area, and peripheral neutrophil count higher than the ULN; a TRIS index was subsequently conducted. In patients with any two of the three parameters, the triplet regimen showed significantly longer OS than the doublet regimen (P = 0.018).

    Abstract: This study examined the relationships between parentification, family relationship, political interest, and civic beliefs using a large sample of Taiwanese adolescents. Data were collected from 1648 junior and senior high school students in Taichung City, Taiwan. Parentification was hypothesized to positively affect family relationship, which in turn influences adolescents’ political interest and civic beliefs. A series of structural equation models were constructed and tested. Implications of the findings for practice and policy-making are discussed. Findings Two dimensions of parentification, instrumental and emotional caregiving, were found to be positively associated with the parent–child relationship while perceived unfairness was negatively associated with the family relations. Family relationship was positively related to youth’s civic beliefs as well as their political interests. Potential gender, age (junior high/senior high), and family (living with/not with both parents) effects were examined through multi-group comparisons. The model was robust with minor differences between gender, age, and family type subgroups. Applications Social workers should recognize the developmental benefits of parentification and utilize such experiences when working with adolescents. With cultural sensitivity, a healthy inter-dependence between youth and parents can be appreciated as a source of strength, which facilitates young people’s civic responsibility and concern for public affairs beyond family.

    Abstract: Natural gas is a potential alternative to conventional liquid fuels for utilization in automotive internal combustion engines. Its utilization can significantly reduce the levels of nitrogen oxide and particulate matter emissions from dual-fuel diesel/natural-gas engines. However, this reduction in the particulate matter emissions may induce an increase in the quantity of particles of smaller size, which are harmful to human health. The engine parameters, namely the diesel injection timing and the pilot quantity directly, affect the homogeneity of the fuel–air mixture, the spatial distribution of initial ignition centres and the quantity of initial ignition centres. In this paper, the effects of these two engine parameters on the combustion and the particle emission characteristics of micro-diesel pilot-ignited natural-gas fuel are investigated. The combustion parameters were calculated from the cylinder pressure data. An electrical low-pressure impactor was employed to measure the particle number concentration and particle size distribution. Experimental results indicate that the ignition delay period is prolonged and the particle number concentration progressively increases with advance of the injection timing, while the particle mass concentration undergoes a steady decrease and then a rapid increase. On the other hand, increasing the pilot diesel quantity can shorten the ignition delay period and enhance the burning rate. Meanwhile, the particle mass concentration increases evidently but the particle number concentration decreases unexpectedly. It should be noted that the total number of fine particles is sensitive to advancing injection and decreasing pilot mass, especially above the 20° crank angle before top dead centre injection timing and below the basic pilot mass.

    Abstract: This study’s purpose was to learn why some youth who participated in the Strong African American Families (SAAF) program increased alcohol use after 2 years whereas other youth did not. Using a sample of 28 African American caregiver-youth dyads, the authors collected qualitative data to explore these issues. Findings underscore the importance of caregivers’ practicing vigilant monitoring to keep their adolescents from experimenting with alcohol. Recommendations for limiting access to alcohol and encouraging vigilant parenting are discussed.

    Abstract: Octahedral truss is a kind of geometry-rich structure, which can be used extensively in space missions. Regular octahedron is the basis of other various octahedral trusses. Firstly, five basic units of octahedral truss or derivatives are proposed, and the structural performances are investigated briefly. Mechanism features and formations are then studied for fundamental deployable modules in conjunction with the basic units. And knowledge of geometry is formulated in detail for different deployable modules, especially for variable geometry truss and tensegrity. The corresponding mast formations are also developed. Finally, kinematic analysis approach is proposed concisely for statically determinate deployable octahedral truss, and spares matrix storage and reduced matrix technique are employed to improve the computation efficiency.

    Abstract: This article uses a sample of 867 African American households to investigate differences in parenting practices and child outcomes by type of household. Results indicate that mothers provide similar levels of parenting regardless of family structure. Secondary caregivers, however, show a great deal of variation in quality of parenting. Fathers and grandmothers engage in the highest quality parenting, stepfathers the poorest, with other relatives falling in between. These differences in parenting do not explain family structure differences in child behavior problems. Results suggest that children do best when there are two caregivers in the household, although stepfathers are an exception to this rule. Child behavior problems are found to be no greater in either mother-grandmother or mother-relative families than in households with two biological parents. In terms of risk for child behavior problems, these family forms seem to be functionally equivalent.

    Abstract: In this brief report, the hypothesis that self-regulatory problems would mediate the association between contextual stressors and unprotected intercourse among rural African American young adult men was investigated. Family support and religiosity were hypothesized to ameliorate the influence of contextual stressors on self-regulatory problems. Hypotheses were tested on 79 sexually active men from a sample recruited with Respondent Driven Sampling; episodes of unprotected intercourse constituted the criterion variable. Analyses supported the mediating role of self-regulatory problems in linking young adult men’s contextual stressors with a heightened likelihood of unprotected intercourse. Religious involvement and family support interacted with contextual stressors to predict diminished associations with self-regulatory problems.

    Abstract: The immune makers including CD4+CD25+ T cells, natural killer cells, and T cells subgroup were retrospectively analyzed to find the relationship between apatinib and the immune system in the patients treated with apatinib. Method: Forty-two patients with advanced malignant tumors orally took apatinib as treatment and 16 patients with the same situation did not take apatinib as a control group. These patients were all included in the study, and they orally received apatinib 500 mg daily as monotherapy or combination. The treatment was continued until disease progression or intolerable toxicity. CD4+CD25+ T cells, natural killer cells, and T cells subgroup were detected before and 1 month after therapy for all the patients. The relationship between the changing number of immune cells and progression-free survival was analyzed in this study. Result: For the apatinib group, the rate of CD4+CD25+ T cells significantly increased ( P = .048). The median progression-free survival was 3.25 months for the 42 patients. The median progression-free survival in the patients with the rate of CD4+CD25+ T cells increased and decreased was 5.8 months and 2.9 months, respectively ( P = .012). Multivariate analysis found the increased rate of CD4+CD25+ T cells was an independent prognostic factor for a longer progression-free survival. The rate of natural killer cells and T cells subgroup did not change much after apatinib therapy, and they were not independent prognostic factors for progression-free survival. Conclusion: The rate of CD4+CD25+ T cells is very important in patients with apatinib treatment. The changing number of CD4+CD25+ T cells may be a good indicator for apatinib prognosis. Natural killer cells and T cells subgroup did not change much after apatinib, and they were not independent prognostic factors for progression-free survival.
