    Abstract: Due to the low temperatures, the heating efficiency of air source heat pump systems during the winter is very low. To address this problem, a low-temperature solar hot water system was added to a basic air source heat pump system. Several parameters were tested and analyzed. The heat collection efficiency of the solar collector was analyzed under low-temperature conditions. The factors that affect the performance of the heat pumps, such as the fluid temperature, pressure, and energy savings, were analyzed for cases where the solar energy auxiliary heat pump and the air source heat pump are used independently. The optimal heating temperature and the changes in the fluid temperature were determined. The influence of the compression ratio and the coefficient of performance (COP) were investigated theoretically. The results revealed the parameters that are important to the performance of the system. Several measures for improving the COP of the heat pump units are provided for other applications and future research.

    Abstract: Our present study intended to examine the associations of RPEL1 and miR-1307 gene polymorphisms (rs4917385 and rs7911488) with susceptibility, glucocorticoids (GCs) efficacy, anxiety, depression, and health-related quality of life (HRQoL) in Chinese systemic lupus erythematosus (SLE) patients. Methods Initially, 1000 participants (500 SLE cases and 500 controls) were recruited for the case–control study. Then, 429 cases who received GCs were followed through 12 weeks to explore GCs efficacy, depression, anxiety, and HRQoL. We selected the iMLDR technique for genotyping: RPEL1: rs4917385 (G/T) and miR-1307: rs7911488 (A/G). Results The minor G allele of rs7911488 reduced the risk of SLE ( p = .024). Four haplotypes consisting of rs4917385 and rs7911488 were associated with SLE susceptibility ( p 〈 .025). Both rs4917385 and rs7911488 were associated with anxiety symptoms and physical function (PF) in SLE patients ( p 〈 .025). The rs4917385 was associated with depression and its improvement. No statistical significance was found between RPEL1 and miR-1307 gene polymorphisms with GCs efficacy. Meanwhile, additive interaction analysis showed a significant association between RPEL1 and miR-1307 gene polymorphisms with tea consumption in anxiety. Conclusion RPEL1 and miR-1307 gene polymorphisms (rs4917385 and rs7911488) might be related to SLE susceptibility in Chinese population. Additionally, the two polymorphisms were possibly associated with depression, anxiety, and HRQoL in Chinese SLE population.

    Abstract: Hepatocellular Carcinoma (HCC) has the highest mortality rate worldwide with the intractability of its extremely complicated pathogenesis and unclear mechanism. The limited survival highlights the need for the further detection of prognosis for HCC. MicroRNAs (miRNAs) and messenger RNAs (mRNAs) have been identified as regulatory factors and target genes in human cancers, while some studies also found post-transcriptional modification plays a crucial role in the occurrence and development of HCC. The present study aimed to elucidate the prognostic significance of miRNA and mRNA models in HCC. Methods: Data were obtained from The Cancer Genome Atlas (TCGA), International Cancer Genome Consortium (ICGC), and Gene Expression Omnibus (GEO) databases. The miRNA and mRNA expressions were tested by the Wilcoxon and used funrich software to predict mRNA that might be related to miRNA. Then we determined the intersection with overlapped mRNA and miRNA Venn diagram, and screened out hub gene by using Degree algorithm in Cytoscape software. The COX models, with TCGA data as the training set and ICGC data as the test set, were constructed. All patients were divided into high-risk and low-risk groups. Data on overall survival of different groups were collected and analyzed by Kaplan-Meier method, and independent risk factors affecting prognosis were assessed by Cox analysis. Results: The miRNA and mRNA polygenic risk model showed a good true positive rate. Kaplan-Meier curve and Cox analysis suggested that the high-risk group was associated with poor prognosis, and the risk score could be used as an independent risk factor for HCC. Conclusion: Tumor risk models constructed in this study could effectively predict the prognosis of patients, which is expected to provide a reference for the prognostic stratification and treatment strategy development of HCC.

    Abstract: Electroacupuncture (EA) can produce analgesia by increasing the β-endorphin level and activation of peripheral μ-opioid receptors in inflamed tissues. Endogenous cannabinoids and peripheral cannabinoid CB2 receptors (CB2Rs) are also involved in the antinociceptive effect of EA on inflammatory pain. However, little is known about how peripheral CB2Rs interact with the endogenous opioid system at the inflammatory site and how this interaction contributes to the antinociceptive effect of EA on inflammatory pain. In this study, we determined the role of peripheral CB2Rs in the effects of EA on the expression of β-endorphin in inflamed skin tissues and inflammatory pain. Results: Inflammatory pain was induced by injection of complete Freund's adjuvant into the left hindpaw of rats. Thermal hyperalgesia was tested with a radiant heat stimulus, and mechanical allodynia was quantified using von Frey filaments. The mRNA level of POMC and protein level of β-endorphin were quantified by real-time PCR and Western blotting, respectively. The β-endorphin-containing keratinocytes and immune cells in the inflamed skin tissues were detected by double-immunofluorescence labeling. The CB2R agonist AM1241 or EA significantly reduced thermal hyperalgesia and mechanical allodynia, whereas the selective μ-opioid receptor antagonist β-funaltrexamine significantly attenuated the antinociceptive effect produced by them. AM1241 or EA significantly increased the mRNA level of POMC and the protein level of β-endorphin in inflamed skin tissues, and these effects were significantly attenuated by pretreatment with the CB2R antagonist AM630. AM1241 or EA also significantly increased the percentage of β-endorphin-immunoreactive keratinocytes, macrophages, and T-lymphocytes in inflamed skin tissues, and these effects were blocked by AM630. Conclusions: EA and CB2R stimulation reduce inflammatory pain through activation of μ-opioid receptors. EA increases endogenous opioid expression in keratinocytes and infiltrating immune cells at the inflammatory site through CB2R activation.

    Abstract: In the present study, we have investigated the effects of acupuncture on (1) serum protein expression that might have a beneficial effect on stroke patients and (2) the strength of limb muscles in stroke patients. A total of 35 acute ischemic stroke (IS) patients were divided into two groups, one receiving drug treatment alone and the other receiving electroacupuncture (EA) and drug treatment. EA treatment was performed on eight acupuncture points once a day for 10 consecutive days. Serum proteins were detected using a proteomics method based on two-dimensional gel electrophoresis, and the specificity of proteins was confirmed by Western blotting. Changes of limb muscle strength were measured using a modified Medical Research Council grading scale. After EA, SerpinG1 protein expression in serum was down-regulated while the expressions of gelsolin, complement component I, C3, C4B and beta-2-glycoprotein I proteins were up-regulated in patients. The changes of serum protein expression were further confirmed by Western blotting in a majority of the cases. The muscle strength of limbs was increased after EA in 18 patients. EA appears to be effective in regulating differential expression of multiple serum proteins involved in stroke, and also in enhancement of muscle strength recovery in acute IS patients despite an individual variation.

    Abstract: Although fieldwork experience plays an important role in an occupational therapy (OT) student's choice of future area of practice, studies on the influences of fieldwork on the preference of practice areas remain limited. The goal of the study was to further understand how OT students choose their future areas of practice. Methods: A questionnaire, “The preferred future practice areas of occupational therapy students”, was created by systematic review of the relevant literature and collection of opinions from occupational therapists, students, and faculty. In total, 152 OT students undergoing their Level II fieldwork participated in the survey. Reliability tests, factor analysis tests, and discrimination analysis were carried out. Results The questionnaire demonstrated good to moderate reliability and validity. The factorial analysis indicated that the most influential factors for the preference of future practice areas of senior occupational therapy students are as follows: fieldwork experiences with clinical supervisors, fieldwork experiences with peers, social factors, college courses, and pressure from future work. The discrimination analysis revealed that social factors and college courses are the best factors for discriminating the variable “Choosing physical dysfunction, paediatric, or psychiatric OT as the most preferred future practice area.” Conclusion The choice of future area of practice is proved to be influenced by the experience in fieldwork for OT students. The psychometric properties of the questionnaire used in this study are supported, and it is recommended for use in future studies.

    Abstract: The pathogenesis of sudden cardiac death may differ between younger and older adults in schizophrenia, but evidence remains scant. This study investigated the age effect on the incidence and risk of the physical and psychiatric comorbidity for sudden cardiac death. Methods: Using 2000 to 2016 data from the Taiwan National Health Insurance Research Database and Department of Health Death Certification System, we identified a national cohort of 170,322 patients with schizophrenia, 1,836 of whom had a sudden cardiac death. Standardized mortality ratios (SMRs) were estimated. Hazard ratios and population attributable fractions of distinctive comorbidities for sudden cardiac death were assessed. Results: The SMRs of sudden cardiac death were all 〉 1.00 across each age group for both sexes, with the highest SMR in male patients aged 〈 35 years (30.88, 95% CI: 26.18–36.18). The fractions of sudden cardiac death attributable to hypertension and congestive heart failure noticeably increased with age. By contrast, the fraction attributable to drug-induced mental disorder decreased with age. Additionally, chronic hepatic disease and sleep disorder increased the risk of sudden cardiac death in patients aged 〈 35 years. Dementia and organic mental disorder elevated the risk in patients aged between 35–54 years. Ischemic heart disease raised the risk in patients aged ≥55 years. Conclusions: The risk is increased across the lifespan in schizophrenia, particularly for younger male patients. Furthermore, physical and psychiatric comorbidities have age-dependent risks. The findings suggest that prevention strategies targeted toward sudden cardiac death in patients with schizophrenia must consider the age effect.

    Abstract: The purpose of this research was to establish a conceptual model of volunteer interpreters’ satisfaction in Taiwan. The objectives were: (1) to understand Taiwanese volunteer interpreters’ activity involvement, place attachment, and personal satisfaction and (2) to propose a hypothetical structural model representing the association between activity involvement, place attachment, and interpreters’ satisfaction. A total of 378 surveys were collected from volunteer interpreters at six Taiwan national parks and the Taipei Zoo. Based on the findings, research targets’ activity involvement was high and place attachment was moderate. Their satisfaction, however, was high. Furthermore, the results showed that the volunteer interpreters’ activity involvement and place attachment positively and significantly influenced satisfaction; their activity involvement also positively influenced place attachment. Based on the results, researchers proposed suggestions for agency managers who use volunteers, among them promoting interpreters’ activity involvement and place attachment in order to enhance volunteer satisfaction.

    Abstract: The objectives of this phase II study were to determine the clinical activity of the MET tyrosine kinase inhibitor capmatinib (INC280) in patients with MET-dysregulated advanced hepatocellular carcinoma (HCC) and to assess the safety, pharmacokinetics, and correlation of biomarkers with the response. Methods: This phase II, open-label, single-arm study evaluated twice daily (BID) oral capmatinib in a dose-determining stage, utilizing a Bayesian Logistic Regression Model (BLRM) subject to Escalation with Overdose Control criteria, safety, pharmacokinetics, and pharmacodynamic information to determine a recommended dose for expansion (RDE) evaluating efficacy in patients with MET-dysregulated HCC. Results: A total of 38 patients received treatment. In the dose-determining stage, patients received capmatinib 300 mg BID capsules ( n = 8), and in the expansion, patients received 600 mg BID capsules ( n = 28) or 400 mg BID tablets ( n = 2) based on the BLRM and other relevant clinical data. No predefined qualifying adverse events (AEs) were observed during the first 28 days of treatment, and the RDE was 600 mg BID capsules (equivalent pharmacokinetics to 400 mg BID tablets). The most common any causality AEs were nausea (42%), vomiting (37%), and diarrhea (34%). In the expansion stage, in a subgroup of 10 patients with MET-high HCC, the overall response rate was 30%, including 1 durable complete response ( 〉 600 days) and 2 partial responses [1 durable ( 〉 600 days)]. Conclusions: Single agent capmatinib at the RDE is tolerable with a manageable safety profile. Antitumor activity was seen in a subset of patients with MET-dysregulated (MET-high) HCC.     Abstract: The measurement performance of the roller tester for the vehicle brake force is evaluated by both proposed dynamic calibration and traditional static calibration to find an effective way for ensuring the vehicle safety. Three static parameters, brake force error of a single bench, difference of the left and right benches, and measurement repeatability, are verified to be eligible in the experiments. However, the experimental results of the dynamic calibration prove that the same brake tester fails on the repeatability with a 10.4% error. The dynamic calibration method improves the measurement performance of the brake tester in practical applications.
