    Kurzfassung: A brief review of the factors influencing plasma tricyclic concentrations and the relationship between clinical response and plasma concentration is presented. It is concluded that routine monitoring of tricyclics is unwarranted. In some specific instances monitoring tricyclic antidepressant plasma concentrations may be indicated: in cases of therapeutic failure a low level may indicate poor compliance or rapid metabolism and the need to increase the dose; for the management of side effects, especially when cardiac abnormalities exist prior to therapy; in cases of polypharmacy when drug interactions can occur; the management of overdoses and in patients with physical illnesses especially renal and hepatic disease. The elderly also constitute a group of patients in whom drug monitoring may be advisable. This group of patients tends to develop higher plasma levels than younger patients on equivalent doses. Some guidelines for the interpretation of laboratory results are presented and discussed.

    Kurzfassung: Fifty-three patients suffering from depressive illness were classified as ‘responders’ or ‘non-responders’ on the basis of an “amelioration score” of the Hamilton Rating Scale for depression after six weeks treatment with nortriptyline hydrochloride at 150 mgs. daily. A multivariate analysis of variance was performed on the four psychological rating scale scores used, to determine whether rating scales could predict treatment response to tricyclic antidepressants. There was a significant difference between the two groups after adjustment for age. Subjects were then classified as ‘non-responders’ and ‘responders’ on the basis of their discriminant scores. The prediction of response to tricyclic antidepressants was found to be at 75% probability.

    Kurzfassung: Pairs of depressed patients were matched for age, sex, and severity of depression, and treated for four weeks with nortriptyline so that plasma levels of below 49 ng/ml and above 140 ng/ml were, obtained. Comparisons of pairs of patients as regards to improvement of depression, using a sequential skew restricted design, suggested that it was unlikely that a significant difference between the two treatment regimes would be detected.

    Kurzfassung: A double-isotope derivative dilution procedure for measuring plasma levels of Nortriptyline is described. The assay is specific, sensitive, precise and reproducible. Studies on the variation in steady-state plasma levels are described on a fixed oral dosage of nortriptyline. Between individual variations in steady-state plasma levels are large. Between day within individual variation is 15–20%. Dosage regime, time of blood sampling after hist oral dose and the method of blood handling are unlikely to affect the steady-state plasma levels. Within an individual, the oral dose, duration of treatment and concomitant drug treatment are all important determinants of the steady-state plasma level.
