    Abstract: 〈 b 〉 〈 i 〉 Background: 〈 /i 〉 〈 /b 〉 Psychological problems are common in cancer patients. For the purpose of planning psycho-oncological interventions and services tailored to the specific needs of different cancer patient populations, it is necessary to know to what extent psychological problems meet the criteria of mental disorders. The purpose of this study was to estimate the 12-month and lifetime prevalence rates of mental disorders in cancer patients. 〈 b 〉 〈 i 〉 Methods: 〈 /i 〉 〈 /b 〉 A representative sample of patients with different tumour entities and tumour stages (n = 2,141) in outpatient, inpatient and rehabilitation settings underwent the standardized computer-assisted Composite International Diagnostic Interview for mental disorders adapted for cancer patients (CIDI-O). 〈 b 〉 〈 i 〉 Results: 〈 /i 〉 〈 /b 〉 The overall 12-month prevalence for any mental disorder was 39.4% (95% CI: 37.3-41.5), that for anxiety disorders was 15.8% (95% CI: 14.4-17.4), 12.5% (95% CI: 11.3-14.0) for mood disorders, 9.5% (95% CI: 8.3-10.9) for somatoform disorders, 7.3% (95% CI: 6.2-8.5) for nicotine dependence, 3.7% (95% CI: 3.0-4.6) for disorders due to general medical condition, and 1.1% (95% CI: 0.7-1.6) for alcohol abuse or dependence. Lifetime prevalence for any mental disorder was 56.3% (95% CI 54.1-58.6), that for anxiety disorders was 24.1% (95% CI: 22.3-25.9), 20.5% (95% CI: 18.9-22.3) for mood disorders, 19.9% (95% CI: 18.3-21.7) for somatoform disorders, 18.2% (95% CI: 16.6-20.0) for nicotine dependence, 6.4% (95% CI: 5.4-7.6) for alcohol abuse or dependence, 4.6% (95% CI: 3.8-5.6) for disorders due to general medical condition, and 0.2% (95% CI: 0.1-0.6) for eating disorders. 〈 b 〉 〈 i 〉 Conclusions: 〈 /i 〉 〈 /b 〉 Mental disorders are highly prevalent in cancer patients, indicating the need for provision of continuous psycho-oncological support from inpatient to outpatient care, leading to an appropriate allocation of direct personnel and other resources.

    Abstract: 〈 b 〉 〈 i 〉 Background: 〈 /i 〉 〈 /b 〉 The German Cancer Aid set up a priority research programme with the intention to generate high-quality information based on evidence and to make this information easily accessible for health-care professionals and advisors, researchers, patients, and the general public. 〈 b 〉 〈 i 〉 Summary: 〈 /i 〉 〈 /b 〉 The 〈 i 〉 Kompetenznetz Komplementärmedizin in der Onkologie 〈 /i 〉 (KOKON) received 2 funding periods within this programme. During the first funding period, KOKON assessed patients’ and health-care professionals’ informational needs, developed a consulting manual for physicians, developed an education programme for self-help groups, set up a knowledge database, and developed a pilot information website for patients. Funding period 2 continues with work that allows cancer patients and health-care professionals to make informed decisions about complementary and alternative medicine (CAM). For this aim, KOKON evaluates training programmes for physicians (oncology physicians, paediatric oncologists, and general practitioners) and for self-help groups. All training programmes integrate results from an analysis of the ethical, psychological, and medical challenges of CAM in the medical encounter, and the knowledge database is being extended with issues related to CAM for supportive and palliative care. 〈 b 〉 〈 i 〉 Key Message: 〈 /i 〉 〈 /b 〉 A Germany-wide collaborative research project to identify needs, provide information, foster communication, and support decision-making about CAM in oncology is being set up.

    Abstract: During development, targets of outgrowing axons produce neurotrophins. These signaling molecules exert short-distance effects on neuronal growth cones as well as long-distance effects on neuronal cell bodies and nuclei. The retrograde signaling from axon tips to cell bodies is essential for the regulation of neuronal survival during the period of naturally occurring programmed cell death in the nervous system. During this time window, neurons that fail to establish appropriate connections with their target tissues are eliminated by apoptosis. Neurotrophins (nerve growth factor, brain-derived neurotrophic factor, neurotrophin-3 and neurotrophin-4/5) may influence neuronal survival not only via signaling through Trk receptors but also through interactions with p75NTR. Retrograde transport of neurotrophins and their receptors within organelles coined ‘signaling endosomes’ is one potential mechanism contributing to retrograde signaling. However, alternative pathways not involving retrograde transport of neurotrophins are emerging. The current models of retrograde neurotrophin signaling from synapses to cell bodies are discussed, with special focus on potential roles of the enigmatic p75NTR receptor in neurotrophin trafficking.

    Abstract: 〈 b 〉 〈 i 〉 Introduction: 〈 /i 〉 〈 /b 〉 Prolonged mechanical ventilation (PMV) and weaning failure are factors associated with prolonged hospital length of stay and increased morbidity and mortality. In addition to the burden these places on patients and their families, it also imposes high costs on the public health system. The aim of this systematic review was to identify risk factors for PMV and weaning failure. 〈 b 〉 〈 i 〉 Methods: 〈 /i 〉 〈 /b 〉 The study was conducted according to PRISMA guidelines. After a comprehensive search of the COCHRANE Library, CINHAL, Web of Science, MEDLINE, and the LILACS Database a PubMed request was made on June 8, 2020. Studies that examined risk factors for PMV, defined as mechanical ventilation ≥96 h, weaning failure, and prolonged weaning in German and English were considered eligible; reviews, meta-analyses, and studies in very specific patient populations whose results are not necessarily applicable to the majority of ICU patients as well as pediatric studies were excluded from the analysis. This systematic review was registered in the PROSPERO register under the number CRD42021271038. 〈 b 〉 〈 i 〉 Results: 〈 /i 〉 〈 /b 〉 Of 532 articles identified, 23 studies with a total of 23,418 patients met the inclusion criteria. Fourteen studies investigated risk factors of PMV including prolonged weaning, 9 studies analyzed risk factors of weaning failure. The concrete definitions of these outcomes varied considerably between studies. For PMV, a variety of risk factors were identified, including comorbidities, site of intubation, various laboratory or blood gas parameters, ventilator settings, functional parameters, and critical care scoring systems. The risk of weaning failure was mainly related to age, previous home mechanical ventilation (HMV), cause of ventilation, and preexisting underlying diseases. Elevated PaCO 〈 sub 〉 2 〈 /sub 〉 values during spontaneous breathing trials were indicative of prolonged weaning and weaning failure. 〈 b 〉 〈 i 〉 Conclusion: 〈 /i 〉 〈 /b 〉 A direct comparison of risk factors was not possible because of the heterogeneity of the studies. The large number of different definitions and relevant parameters reflects the heterogeneity of patients undergoing PMV and those discharged to HMV after unsuccessful weaning. Multidimensional scores are more likely to reflect the full spectrum of patients ventilated in different ICUs than single risk factors.

    Abstract: 〈 b 〉 〈 i 〉 Einleitung: 〈 /i 〉 〈 /b 〉 Die Misteltherapie ist im deutschsprachigen Raum ein häufig angewandtes komplementärmedizinisches Verfahren (KM) in der Onkologie. Diese Studie hatte das Ziel, die Beweggründe für oder gegen eine Inanspruchnahme der Misteltherapie zu untersuchen und Themenfeldern zuzuordnen. 〈 b 〉 〈 i 〉 Patienten und Methoden: 〈 /i 〉 〈 /b 〉 Es wurden qualitative leitfadengestützte Interviews mit Krebspatienten geführt. Der Interviewleitfaden fragte nach der Inanspruchnahme von KM, der Motivation zur Inanspruchnahme, Informationsverhalten und -bedürfnissen zu KM sowie nach der Krebserkrankung. Um die Beweggründe für die Inanspruchnahme oder Nichtinanspruchnahme der Misteltherapie zu verstehen, wurden die Interviews inhaltsanalytisch ausgewertet. 〈 b 〉 〈 i 〉 Ergebnisse: 〈 /i 〉 〈 /b 〉 Insgesamt wurden Interviews mit 88 Krebspatienten geführt, davon nutzen 18 (20,5%) die Misteltherapie. Die Beweggründe für oder gegen eine Inanspruchnahme der Misteltherapie ließen sich den 2 Themenfeldern «Wahrgenommene Indikation» und «Abwägungen bei der Entscheidungsfindung» zuordnen. 〈 b 〉 〈 i 〉 Diskussion und Schlussfolgerungen: 〈 /i 〉 〈 /b 〉 Mit der Misteltherapie wird sowohl ein Einfluss auf das Tumorwachstum als auch eine supportive Wirkung assoziiert. Anwender sehen die Misteltherapie als sicheres Verfahren; Nichtnutzer befürchten eher Neben- oder Wechselwirkungen. Die Empfehlung von Fachpersonal spielt eine wichtige Rolle bei der Inanspruchnahme. Zum Teil waren die Nichtnutzer interessiert an der Anwendung der Misteltherapie, befanden sich jedoch noch im Klärungsprozess.

    Abstract: Myopathies comprise a heterogeneous group of disorders characterized by variable phenotypes. The increasing use of next-generation sequencing allows identification of the causative genes in a much higher percentage of patients with hereditary muscle disorders and also illustrates a considerable degree of overlap with other clinical entities, including connective tissue disorders. Here, we present a 14-year-old German patient who was initially suspected to suffer from myopathy based on his clinical, radiological, and muscle biopsy findings. Exome sequencing revealed a novel homozygous nonsense mutation in the 〈 i 〉 SLC39A13 〈 /i 〉 gene, causative for spondylocheiro dysplastic Ehlers Danlos syndrome (SCD-EDS), suggesting a connective tissue disorder. Including our patient, only 9 affected individuals from 4 families have been described for SCD-EDS so far. The previously reported patients did not show obvious evidence of myopathy, suggesting a broader clinical presentation than originally suspected. We summarize herein the current knowledge on clinical features as well as pathophysiological pathways for this rare connective tissue disease and discuss the high degree of clinical overlap between myopathic and connective tissue disorders.

    Abstract: 〈 b 〉 〈 i 〉 Hintergrund: 〈 /i 〉 〈 /b 〉 Die Tanztherapie ist ein Verfahren der künstlerischen Therapie, welches den unmittelbaren Körperbezug im individuellen Tanz zur Erfahrung und zum Ausdruck emotionaler und kognitiver Themen einsetzt. Sie stellt für Krebspatienten eine wichtige Hilfe zur Krankheitsverarbeitung dar. Bislang liegen nur wenige wissenschaftliche Studien zur Tanztherapie bei Krebspatienten vor. 〈 b 〉 〈 i 〉 Methodik: 〈 /i 〉 〈 /b 〉 In der vorliegenden explorativen Studie wurden Wirkmechanismen der Tanztherapie bei Krebspatientinnen (n = 115) im Setting der onkologischen Rehabilitation auf der Basis eines Ein-Gruppen-prä-post-Designs einschließlich einer 3-monatigen Katamnese untersucht. Hierbei wurden Lebensqualität, Angst und Depression sowie Fragen zum Selbstkonzept mit standardisierten Messinstrumenten erhoben (EORTC QLQ-C30, HADS, FSKN). Außerdem konnten die Studienteilnehmerinnen den Therapieerfolg durch subjektive Einschätzung des Nutzens und des Erfolgs der Tanztherapie bewerten. Die folgende Darstellung fokussiert die Ergebnisse der Prä-post-Messung. 〈 b 〉 〈 i 〉 Ergebnisse: 〈 /i 〉 〈 /b 〉 Als Wirkfaktoren der Tanztherapie konnten emotionale Ausdrucksfähigkeit, Stärkung des Selbstwerts/Selbstbewusstseins, Persönlichkeitsentwicklung, Vitalität, allgemeine Ausgeglichenheit sowie Herstellung eines Bezugs zum eigenen Körper identifiziert werden. In Bezug auf die Lebensqualität und psychische Befindlichkeit konnten mittlere bis große Effektstärken festgestellt werden. 〈 b 〉 〈 i 〉 Schlussfolgerungen: 〈 /i 〉 〈 /b 〉 Wenngleich die dargestellten Effektstärken aufgrund des Studiendesigns nicht als Wirksamkeitsnachweis interpretiert werden können, gibt die Untersuchung wichtige Aufschlüsse über Wirkprinzipien und Therapieprozesse des fokussierten kurzzeit-tanztherapeutischen Vorgehens im Rahmen der stationären Rehabilitation.
