    Abstract: Background It is believed that the excessive cardiovascular (CV) burden of patients on peritoneal dialysis (PD) is closely associated with chronic inflammation. Neutrophil-lymphocyte ratio (NLR) is an inflammatory marker that was shown to correlate with CV outcomes. However, little is known about the significance of serial monitoring of serum NLR. We aimed to determine the prognostic value of serial NLR on all-cause mortality and CV mortality in PD patients. Methods Serial measurement of NLR was obtained from 225 incident PD patients in a single center, with each measurement one year apart. Patients were divided into two groups (‘high’ versus ‘low’) by the median value of NLR. The primary and secondary outcome measure was all-cause and CV mortality respectively. Results After a median of follow up for 43.9 months, patients with lower baseline NLR demonstrated a higher survival rate (p=0.01). Patients with persistently high NLR values on serial measurement had the lowest survival rate (p=0.03). Multivariate Cox regression showed that this group of patients had significantly higher all-cause mortality (HR 1.74, 95%CI 1.09 to 2.79, p=0.02). However, the NLR failed to demonstrate a statistically significant relationship with CV mortality. Conclusions While baseline NLR was an independent predictor of all-cause mortality in PD patients, persistent elevation in NLR appeared to further amplify the risk. Regular monitoring of serial serum NLR may enable early identification of patients who are at risk of adverse outcome.

    Abstract: Coelomic fluid and fetal parts were obtained in 35 patients undergoing surgical termination of pregnancy. Fetal gender was determined by Y-sequence-specific polymerase chain reactions (PCR). Fourteen male pregnancies out of 35 cases were identified by examination of fetal parts. The overall concordance rate of gender determination between coelomic fluid and fetal parts was 91.4%. There were 2 cases of male pregnancies not being identified and 1 case of false positive result by coelocentesis. The gestation of pregnancy and the volume of coelomic fluid obtained appeared not to be related to the discrepancy in the paired results. We conclude that sex determination using coelocentesis and PCR remains unreliable to be used as a definitive prenatal diagnostic test for sex-linked disorders.

    Abstract: 〈 b 〉 〈 i 〉 Background: 〈 /i 〉 〈 /b 〉 Coughing due to respiratory tract infections may lead to internal carotid artery (ICA) dissection. 〈 b 〉 〈 i 〉 Aim: 〈 /i 〉 〈 /b 〉 We are presenting a patient with an unusual cough-induced ICA dissection. 〈 b 〉 〈 i 〉 Case Report: 〈 /i 〉 〈 /b 〉 A 42-year-old health care worker presented with bilateral hand numbness which resolved spontaneously. This initial episode was followed 9 days later with intermittent episodes of right hand and leg weakness with speech difficulty. Two days later, he had another episode of speech difficulty. One week prior to the first presentation, he had upper respiratory tract infection with ongoing strong bouts of coughing. Magnetic resonance imaging (MRI), diffusion-weighted imaging (DWI), and apparent diffusion coefficient (ADC) of the brain showed early ischaemic changes at the left frontal and left parietal regions. MR angiography (MRA) showed high signal intensity at the left proximal ICA and poor flow beyond the left carotid bulb. Cerebral angiography revealed left ICA dissection. 〈 b 〉 〈 i 〉 Conclusion: 〈 /i 〉 〈 /b 〉 Proper identification of cough-induced extracranial ICA dissection is important because this is treatable.

    Abstract: 〈 b 〉 〈 i 〉 Background: 〈 /i 〉 〈 /b 〉 With global aging, robots are considered a promising solution for handling the shortage of aged care and companionships. However, these technologies would serve little purpose if their intended users do not accept them. While the socioemotional selectivity theory predicts that older adults would accept robots that offer emotionally meaningful relationships, selective optimization with compensation model predicts that older adults would accept robots that compensate for their functional losses. 〈 b 〉 〈 i 〉 Objective: 〈 /i 〉 〈 /b 〉 The present study aims to understand older adults’ expectations for robots and to compare older adults’ acceptance ratings for 2 existing robots: one of them is a more human-like and more service-oriented robot and the other one is a more animal-like and more companion-oriented robot. 〈 b 〉 〈 i 〉 Methods: 〈 /i 〉 〈 /b 〉 A mixed methods study was conducted with 33 healthy, community-dwelling Taiwanese older adults (age range: 59–82 years). Participants first completed a semi-structured interview regarding their ideal robot. After receiving information about the 2 existing robots, they then completed the Unified Theory of Acceptance and Use of Technology questionnaires to report their pre-implementation acceptance of the 2 robots. 〈 b 〉 〈 i 〉 Results: 〈 /i 〉 〈 /b 〉 Interviews were transcribed for conventional content analysis with satisfactory inter-rater reliability. From the interview data, a collection of older adults’ ideal robot characteristics emerged with highlights of humanlike qualities. From the questionnaire data, respondents showed a higher level of acceptance toward the more service-oriented robot than the more companion-oriented robot in terms of attitude, perceived adaptiveness, and perceived usefulness. From the mixed methods analyses, the finding that older adults had a higher level of positive attitude towards the more service-oriented robot than the more companion-oriented robot was predicted by higher expectation or preference for robots with more service-related functions. 〈 b 〉 〈 i 〉 Conclusion: 〈 /i 〉 〈 /b 〉 This study identified older adults’ preference toward more functional and humanlike robots. Our findings provide practical suggestions for future robot designs that target the older population.

    Abstract: 〈 i 〉 Background: 〈 /i 〉 Inflammatory events may contribute to the pathogenesis of dementia and interleukin-1 (IL-1) may exert both neurotoxic and neuroprotective effects. We investigated whether IL-1α –889 C/T and IL-1β –511 C/T promoter polymorphisms are associated with the risk of Alzheimer’s disease (AD) and vascular dementia (VaD). 〈 i 〉 Methods: 〈 /i 〉 AD patients (n = 219) and VaD patients (n = 82), who fulfilled the criteria of the NINCDS-ADRDA and NINDS-AIREN, and ethnic-matched and nondemented controls (n = 209) were analyzed by means of genotype association method. 〈 i 〉 Results: 〈 /i 〉 No significant difference in the genotype distribution of the analyzed single nucleotide polymorphisms was found between AD or VaD cases and controls. However, the frequency of the IL-1α –889 CT genotype was notably lower in VaD patients aged over 70 years than the age-matched controls (9.1 vs. 22.9%, p = 0.036) andtheIL-1α –889 CT genotype demonstrated a trend toward decrease in risk of developing VaD (odds ratio: 0.34; 95% confidence interval: 0.12–0.83, p = 0.026). Multivariate analysis revealed that the IL-1β –511T-carrying genotype slightly strengthens the negative association of the IL-1α –889 CT genotype with VaD (odds ratio: 0.26; 95% confidence interval: 0.08–0.79, p = 0.024). 〈 i 〉 Conclusion: 〈 /i 〉 Our data suggest a protective role of the IL-1α –889 CT genotype in VaD susceptibility among Taiwanese aged over 70 years.

    Abstract: 〈 b 〉 〈 i 〉 Background/Aims: 〈 /i 〉 〈 /b 〉 The Remission in Schizophrenia Working Group has defined remission as ‘a low-mild symptom intensity level, maintained for a minimum of 6 months, where such symptoms do not affect an individual's behaviour' [Andreasen et al.: Am J Psychiatry 2005;162:441-449]. Since brain morphology relates to symptomatology, treatment and illness progression, MRI may assist in predicting remission. 〈 b 〉 〈 i 〉 Methods: 〈 /i 〉 〈 /b 〉 Thirty-nine patients newly diagnosed with DSM-IV schizophrenia underwent MRI brain scan prior to antipsychotic exposure. The Global Assessment of Functioning (GAF) score was entered into a voxel-based analysis to evaluate its relationship with cerebral grey matter volume from the baseline MRI. We entered age, total intracranial volume and intake GAF score as co-variates. Males and females were analysed separately because gender is a potent determinant of outcome. 〈 b 〉 〈 i 〉 Results: 〈 /i 〉 〈 /b 〉 Males had lower GAF scores than females, both at intake and at 1 year. Males comprised only 40% (12 out of 39) of the early remission group. For females only, early remission was strongly and positively correlated with bilateral lentiform and striatal volumes. For males, there was no such relationship. 〈 b 〉 〈 i 〉 Conclusion: 〈 /i 〉 〈 /b 〉 Larger striato-thalamic volume correlated with early remission in females only. These baseline MRI findings were unlikely to be confounded by antipsychotic treatment and chronicity. These brain morphological markers show gender dimorphism and may assist in the prediction of early remission in newly diagnosed schizophrenia.
