    Abstract: We report on a 15-year-old female with a 3-month history of a pruritic, erythematous cutaneous eruption on the right cheek and perioral area. The lesion had a linear distribution following the lines of Blaschko. Histopathological findings and direct immunofluorescence were compatible with chronic cutaneous lupus erythematosus (LE). Treatment with topical steroids and systemic antimalarial agents over 2 months showed hardly any improvement contrary to similar cases reported in the literature in the past. Histological findings move this case close to LE. However, the unusual clinical presentation as well as the resistance to antimalarial drugs do not fully allow to confirm this suspicion. Therefore, we recommend to call this new entity LE-like facial Blaschkitis of the adult.

    Abstract: 〈 b 〉 〈 i 〉 Background: 〈 /i 〉 〈 /b 〉 Surgical excision is the gold standard for cutaneous squamous cell carcinoma (cSCC), however its application is limited in specific cases. Superficial radiotherapy (RTx) is an alternative treatment option, but long-term follow-up data are limited. 〈 b 〉 〈 i 〉 Objective: 〈 /i 〉 〈 /b 〉 To determine the outcome of superficial RTx of cSCC in correlation to histological differentiation grade and tumor localization. 〈 b 〉 〈 i 〉 Methods: 〈 /i 〉 〈 /b 〉 The outcome of 180 large cSCCs after superficial RTx between 1960 and 2004 was retrospectively reviewed. 〈 b 〉 〈 i 〉 Results: 〈 /i 〉 〈 /b 〉 Mean tumor size was 3.5 cm 〈 sup 〉 2 〈 /sup 〉 (SD 7.5) and mean follow-up period was 4.9 years (SD 4.7). Relapse-free survival was 95.8 and 80.4% after 1 and 10 years. Two-year relapse-free survival was 94.8% for good, 88.9% for moderate and 85.7% for poor differentiated tumors. Five-year relapse-free survival was highest in cSCCs located around the eyes (100%) and cheeks (90.9%). 〈 b 〉 〈 i 〉 Conclusion: 〈 /i 〉 〈 /b 〉 Superficial RTx is an effective alternative for cSCC if surgery is difficult due to localization or concomitant disease.

    Abstract: A 50-year-old patient, suffering from familial polyposis (Gardner’s syndrome), initially presented with several nodules on his left arm. Histological examination revealed primary cutaneous T-cell-rich B-cell lymphoma (TCRBCL). Staging procedures failed to detect any systemic involvement. Three years after total excision of the tumours, the patient presented with a non-specific dermatitis, enlarged axillary lymph nodes and splenomegaly. Histological and immunohistochemical examination of lymph node and spleen biopsy specimens resulted in the diagnosis of Hodgkin’s disease (HD) of the nodular sclerosis type. Sequence analysis of single cells micromanipulated from skin and from lymph node lesions indicated that both lymphoma infiltrates were derived from the same precursor germinal centre B-cell clone. This is a case showing a clonal relationship between TCRBCL and HD, providing support to the B-cell origin of Hodgkin and Reed-Sternberg cells.

    Abstract: Immunosuppression profoundly influences the prevalence of skin disorders in transplant recipients. Skin tumors occur with high incidence and constitute a major part of transplantation-related morbidity and mortality. We report on 2 immunosuppressed patients presenting with rapidly growing epithelial and mesenchymal neoplasms after renal transplantation. The diagnostic approach, differential diagnosis and treatment options are discussed emphasizing the characteristics of cutaneous lesions in immunosuppressed transplant recipients.

    Abstract: 〈 i 〉 Background: 〈 /i 〉 The excimer laser is a new therapeutic option in the treatment of psoriasis vulgaris. 〈 i 〉 Objective: 〈 /i 〉 The purpose of this study was to determine the response of psoriasis lesions to the 308-nm excimer laser compared to 311-nm UVB phototherapy. 〈 i 〉 Methods: 〈 /i 〉 In this prospective right/left comparative, open, single-blinded trial, selected psoriasis plaques of 16 patients were treated with the excimer laser whereas the rest of the body was treated with UVB narrow-band phototherapy. A modified PASI score was used to evaluate the results. 〈 i 〉 Results: 〈 /i 〉 After 12 treatments, 15 patients were evaluated. In 2 patients no difference between the two body sides was observed. In 9 patients the laser-treated lesions showed better results, whereas in 4 patients the side treated with 311-nm UVB showed more clearing. The mean reduction in PASI score was 5.6 and 4.9, respectively (difference not significant). 〈 i 〉 Conclusion: 〈 /i 〉 The use of the 308-nm xenon chloride excimer laser is an additional effective therapeutic option for the treatment of psoriasis vulgaris.

    Abstract: 〈 i 〉 Background: 〈 /i 〉 Baseline staging is important in all melanoma types, including melanoma with unknown primary site (MUP). Staging includes different examination strategies, each with different accuracy. 〈 i 〉 Objective: 〈 /i 〉 To determine the value of serum S100 protein levels and positron emission tomography (PET) in the baseline staging of MUP. 〈 i 〉 Methods: 〈 /i 〉 Twenty patients with MUP were evaluable for the analysis between 1996 and 2007 with both S100 assessment and PET performed for baseline staging. 〈 i 〉 Results: 〈 /i 〉 Serum S100 was elevated in 7 patients (35%). The PET scan detected the metastases in 6 of 7 patients with elevated serum S100 protein showing a strong correlation (p = 0.005). Patients with metastases had significantly higher serum S100 levels (p = 0.01) than the ones without. Serum S100 protein was shown to be discriminative between patients with and without metastases (receiver-operating characteristic, p = 0.012) with 75% sensitivity and 92% specificity. 〈 i 〉 Conclusion: 〈 /i 〉 Serum S100 protein appears to be a sensitive as well as specific marker to detect metastases. We therefore might recommend serum S100 assessment to be included in the baseline staging of MUP.

    Abstract: Recent work suggests that in some patients with the hypereosinophilic syndrome, a clone of abnormal T cells produces large amounts of interleukin-5. In this study, we examined 60 patients with idiopathic eosinophilia. Sixteen patients had circulating T cells with an aberrant immunophenotype that, in most cases, were associated with different forms of skin inflammation. The abnormal T cells produced large amounts of interleukin-5, which may have increased eosinophil differentiation in the bone marrow of these patients.

    Abstract: 〈 i 〉 Borrelia burgdorferi 〈 /i 〉 infection has been implicated in cutaneous B-cell lymphoma. We report a case of multilesional primary cutaneous large B-cell lymphoma without extracutaneous spread in a patient with elevated 〈 i 〉 B. burgdorferi 〈 /i 〉 titers. After antibiotic therapy, clinical remission and a subsequent drop in 〈 i 〉 B. burgdorferi 〈 /i 〉 antibody titers were obtained.

    Abstract: 〈 i 〉 Background: 〈 /i 〉 Polymorphous light eruption (PLE) is an idiopathic eruption induced by ultraviolet (UV) radiation (UVR). 〈 i 〉 Objective: 〈 /i 〉 Evaluation of the clinical aspects, diagnostic criteria of PLE in a major Swiss referral center. 〈 i 〉 Methods: 〈 /i 〉 25 patients with PLE were tested with a standardized protocol for the assessment of photodermatoses. 〈 i 〉 Results: 〈 /i 〉 25 patients (22 women vs. 3 men) were identified. Papular and papular-vesicular eruptions were the most common clinical presentations. 6 of 25 patients had a reduced minimal erythema dose (MED) für UVA and 8 of 25 patients had a reduced MED for UVB. Photoprovocation was positive in 11 of 20 patients for UVA and 7 of 20 patients for UVB. Photohardening with narrow-band UVB was successful in 8 of 10 patients. Combined UVA/UVB therapy had a satisfactory effect in 10 of 15 patients. Narrow-band UVB therapy was still successful after ineffective UVA/UVB therapy. 〈 i 〉 Conclusion: 〈 /i 〉 The MED was of no value for the diagnosis of PLE. The typical lesions were reproduced by UVA and UVB photoprovocation. We recommend photohardening with narrow-band UVB (311 nm).

    Abstract: 〈 b 〉 〈 i 〉 Background: 〈 /i 〉 〈 /b 〉 Due to the importance of function and cosmetics, periocular skin malignancies represent a therapeutic challenge. 〈 b 〉 〈 i 〉 Objective: 〈 /i 〉 〈 /b 〉 To evaluate the safety and efficacy of radiotherapy (RT) treating periocular skin tumors. 〈 b 〉 〈 i 〉 Methods: 〈 /i 〉 〈 /b 〉 Data of patients with periocular tumors treated with grenz or soft X-rays at the University Hospital Zurich, Switzerland, between 2009 and 2014 were reviewed. Basal cell carcinoma (BCC) and squamous cell carcinoma (SCC) with associated in situ lesions, cutaneous melanoma, lentigo maligna (LM), cutaneous lymphoma and Kaposi’s sarcoma were included in the analysis. 〈 b 〉 〈 i 〉 Results: 〈 /i 〉 〈 /b 〉 We found 159 periocular lesions in 145 patients. Overall recurrence was highest for actinic keratosis and Bowen’s disease (27%), melanoma (17%) and LM (15%), whereas SCC and BCC showed recurrence in 11 and 10%, respectively. 45% of all recurrences occurred within 12 months after treatment, with a median time to recurrence of 13 months (range 3–73). 〈 b 〉 〈 i 〉 Conclusion: 〈 /i 〉 〈 /b 〉 RT, which provides a good therapeutic response with minimal adverse events, is a therapy option for periocular cutaneous malignancies.
