    Abstract: 〈 b 〉 〈 i 〉 Objectives: 〈 /i 〉 〈 /b 〉 Bifurcation angle has emerged as a predictor of outcome after percutaneous coronary intervention (PCI) for bifurcation lesions. We investigated the impact of bifurcation angle on clinical outcomes in patients undergoing bifurcation lesion PCI. 〈 b 〉 〈 i 〉 Methods: 〈 /i 〉 〈 /b 〉 Consecutive patients who received PCI for bifurcation lesions were enrolled from 16 centers in Korea between January 2004 and June 2006. Patients were divided into low-angle and high-angle groups using the median bifurcation angle (50°). We compared major adverse cardiac events, including cardiac death, myocardial infarction and target lesion revascularization as well as periprocedural outcomes between the 2 groups. 〈 b 〉 〈 i 〉 Results: 〈 /i 〉 〈 /b 〉 We evaluated 1,432 patients with bifurcation lesions with a median follow-up duration of 21 months. The rates of interventional side branch procedures such as guide-wiring of side branches, side-branch ballooning, final kissing ballooning and side-branch stenting were higher in the low-angle group. However, the incidences of major adverse cardiac events and target lesion revascularization were not significantly different between the 2 groups (6.6 vs. 6.9%, p = 0.856 and 4.6 vs. 5.7%, p = 0.375, respectively). 〈 b 〉 〈 i 〉 Conclusions: 〈 /i 〉 〈 /b 〉 Bifurcation angle may not influence long-term clinical outcome in patients with non-left main bifurcation lesion undergoing PCI despite its association with more interventional side-branch procedures.

    Abstract: 〈 b 〉 〈 i 〉 Background/Aims: 〈 /i 〉 〈 /b 〉 Some minimal changes (MCs) are believed to have a certain relationship with gastroesophageal reflux (GERD). Nonetheless, the individual meaning of MC is still unclear. Our aim was to compare the overall and individual prevalence of MC between patients with non-erosive reflux disease (NERD) and healthy controls (HC). 〈 b 〉 〈 i 〉 Methods: 〈 /i 〉 〈 /b 〉 Twelve endoscopic findings in the esophagogastric junction were prospectively compared between NERD (n = 64) and control (n = 104). 〈 b 〉 〈 i 〉 Results: 〈 /i 〉 〈 /b 〉 Overall frequency of MC (≥1 out of 12 criteria) was higher in the NERD group (71.9%) than in the HC group (45.2%). In individual analysis, white mucosal turbidity, irregular Z-line, horizontal erosions, and mucosal protrusion of cardia were significantly more common in the NERD group compared to controls. Among them, only white mucosal turbidity was independently associated with the NERD group (OR 3.97, 95% CI 1.72–9.13). Individuals with male gender, reflux symptoms, higher height, current smoking, ethanol intake and hiatal hernia were more likely to have white mucosal turbidity compared to the group without white turbidity. 〈 b 〉 〈 i 〉 Conclusions: 〈 /i 〉 〈 /b 〉 MC could be a useful marker to support clinical diagnosis of GERD. White mucosal turbidity in particular might be a GERD-specific sign related to acid-induced mucosal damage.

    Abstract: 〈 i 〉 Objective: 〈 /i 〉 The serine protease urokinase-type plasminogen (uPA) and its receptor (uPAR) appear to have a major function in tumor invasion and metastasis. Interleukin-8 (IL-8) acts as an angiogenic factor in solid cancer. The purpose of this study was to determine whether the expression of IL-8 and the uPAR gene correlates with clinicopathological parameters in human gastric carcinoma. 〈 i 〉 Methods: 〈 /i 〉 We examined the expression of uPAR mRNA and IL-8 mRNA using Northern blot analysis and RT-PCR in 35 gastric carcinomas and the surrounding normal mucosa. Macroscopic and histopathological tumor findings and survival rates were obtained from the patient records. 〈 i 〉 Results: 〈 /i 〉 uPAR and IL-8 mRNA expression levels were higher in most neoplasms compared to the corresponding normal mucosal tissue. A correlation between uPAR and IL-8 expression in tumors was observed (r = 0.447, p 〈 0.01). uPAR mRNA expression in the tumors correlated well with lymph node metastasis (p 〈 0.02), and its stage (p 〈 0.01). The IL-8 MRNA expression in the tumors correlated with diffuse-type gastric cancer (p 〈 0.05). The survival rates of patients with tumors displaying high uPAR expression levels were significantly lower (p 〈 0.04) than those of patients without uPAR expression, but patients with IL-8 expression only showed a tendency to a lower survival rate. 〈 i 〉 Conclusion: 〈 /i 〉 These results suggest that uPAR and IL-8 may be important prognostic factors in human gastric carcinomas.

    Abstract: 〈 i 〉 Background: 〈 /i 〉 ( 〈 i 〉 Z 〈 /i 〉 )-Stellettic acid C, an acetylenic acid from the marine sponge 〈 i 〉 Stelletta 〈 /i 〉 sp., has been shown to have cytotoxic activity in some cancer cells; however, its mechanisms on malignant cell growth are not known. In this study, the potential of ( 〈 i 〉 Z 〈 /i 〉 )-stellettic acid C to induce apoptosis in human leukemic U937 cells and its effects on telomerase activity were investigated. 〈 i 〉 Methods: 〈 /i 〉 Cytotoxicity was evaluated by MTT assays. Apoptosis was detected using DAPI staining and annexin V fluorescein. The mRNAs of Bcl-2, inhibitor of apoptosis proteins (IAPs) family and Fas/FasL system were determined by RT-PCR. Caspases and telomerase activities were measured using colorimetric assay and telomeric repeat amplification protocol enzyme-linked immunosorbent assay (TRAP-ELISA), respectively. 〈 i 〉 Results: 〈 /i 〉 Exposure of U937 cells to ( 〈 i 〉 Z 〈 /i 〉 )-stellettic acid C resulted in growth inhibition and induction of apoptosis in a dose-dependent manner, which was associated with the modulation of Bcl-2 family expression, activation of caspases and downregulation of IAPs family members. ( 〈 i 〉 Z 〈 /i 〉 )-Stellettic acid C treatment markedly inhibited the activity of telomerase in a dose-dependent fashion. Additionally, the expression of human telomerase reverse transcriptase, a main determinant of the telomerase enzymatic activity, was progressively downregulated by ( 〈 i 〉 Z 〈 /i 〉 )-stellettic acid C treatment. 〈 i 〉 Conclusions: 〈 /i 〉 These results suggest that ( 〈 i 〉 Z 〈 /i 〉 )-stellettic acid C could have a possible cancer therapeutic potential.

    Abstract: 〈 b 〉 〈 i 〉 Background/Aims: 〈 /i 〉 〈 /b 〉 Although the hepatobiliary manifestations of 〈 i 〉 Mycoplasma pneumoniae 〈 /i 〉 infection have been described in several previous studies, controversies still remain. The aim of this study was to evaluate the clinical characteristics of patients with 〈 i 〉 M. pneumoniae- 〈 /i 〉 related hepatitis and compare to those with 〈 i 〉 M. pneumoniae 〈 /i 〉 infection but not hepatitis. 〈 b 〉 〈 i 〉 Methods: 〈 /i 〉 〈 /b 〉 We reviewed the medical chart of patients diagnosed with 〈 i 〉 M. pneumoniae 〈 /i 〉 infection serologically. 〈 b 〉 〈 i 〉 Results: 〈 /i 〉 〈 /b 〉 Among the 117 patients with 〈 i 〉 M. pneumoniae 〈 /i 〉 infection enrolled in the study, 25 showed acute hepatitis without any evidence of another cause. Patients with hepatitis presented with prodromal symptoms more frequently and also had a higher body temperature and C-reactive protein (CRP) levels than patients without hepatitis. Frequencies of respiratory tract involvement were not different between two groups. Clinical signs recovered within three weeks in both groups following the institution of antibiotic therapy. Multivariate analysis revealed that male sex, age 〈 40 years, and high CRP levels were significantly linked to 〈 i 〉 M. pneumoniae- 〈 /i 〉 related hepatitis. 〈 b 〉 〈 i 〉 Conclusions: 〈 /i 〉 〈 /b 〉 We found that acute hepatitis associated with 〈 i 〉 M. pneumoniae 〈 /i 〉 occurred in about 21% of all 〈 i 〉 M. pneumoniae 〈 /i 〉 infections, and gender, age, and CRP levels were factors related to the occurrence of 〈 i 〉 M. pneumoniae 〈 /i 〉 -related hepatitis.

    Abstract: 〈 b 〉 〈 i 〉 Background: 〈 /i 〉 〈 /b 〉 The neutrophil-to-lymphocyte ratio (NLR) is an independent prognostic marker in solid and hematological cancers. While the derived NLR (dNLR) was shown to be non-inferior to the NLR in large cohorts of patients with different cancer types, it has not been validated as a prognostic marker for multiple myeloma (MM) to date. 〈 b 〉 〈 i 〉 Methods: 〈 /i 〉 〈 /b 〉 Between May 22, 2011 and May 29, 2014, 176 patients with MM from 38 centers who were ineligible for autologous stem cell transplantation were analyzed. The dNLR was calculated using complete blood count differential data. The optimal dNLR cut-off value according to receiver operating characteristic analysis of overall survival (OS) was 1.51. All patients were treated with melphalan and prednisone combined with bortezomib. 〈 b 〉 〈 i 〉 Results: 〈 /i 〉 〈 /b 〉 The complete response rate was lower in the high dNLR group compared to the low dNLR group (7 vs. 26.1%, respectively; 〈 i 〉 p 〈 /i 〉  = 0.0148); the corresponding 2-year OS rates were 72.2 and 84.7%, respectively ( 〈 i 〉 p 〈 /i 〉  = 0.0354). A high dNLR was an independent poor prognostic factor for OS (hazard ratio 2.217, 95% CI 1.015–4.842; 〈 i 〉 p 〈 /i 〉  = 0.0458). 〈 b 〉 〈 i 〉 Conclusion: 〈 /i 〉 〈 /b 〉 The dNLR is a readily available and cheaply obtained parameter in clinical studies, and shows considerable potential as a new prognostic marker for transplantation-ineligible patients with MM.

    Abstract: Choledochal cysts are congenital anomalies of the biliary tract manifested by cystic dilatation of the extrahepatic and intrahepatic bile ducts. Choledochal cyst is not rare in far-East Asian countries. Type II choledochal cysts account for 2% of all such cysts. They are true diverticula of the extrahepatic bile duct and communicate with the bile duct through a narrow stalk. This condition is associated with significant complications, such as ductal strictures, stone formation, cholangitis, rupture and secondary biliary cirrhosis. We describe a case of a huge impacted stone in a diverticular choledochal cyst which masqueraded as an unusual cystic duct stone causing Mirizzi's syndrome.

    Abstract: 〈 i 〉 Objective: 〈 /i 〉 Vascular endothelial growth factor (VEGF) is known to be a potent proangiogenic factor. This study evaluates the potential association of three VEGF gene polymorphisms (–460T 〉 C, +405G 〉 C, and 936C 〉 T) with the susceptibility to and clinicopathologic characteristics of gastric cancer. 〈 i 〉 Methods: 〈 /i 〉 The VEGF genotypes were determined using paraffin-embedded tissue from 413 patients who underwent a surgical resection and peripheral blood lymphocytes from 413 healthy controls by PCR-RFLP assay. 〈 i 〉 Results: 〈 /i 〉 There was no difference in the allele frequency of –460T 〉 C polymorphism. However, for the +405G 〉 C polymorphism, the +405C allele was associated with a significantly decreased susceptibility to gastric cancer [odds ratio (OR) 0.686; 95% confidence interval (CI) 0.564–0.834]. Although there was no significant difference in the distribution of the 936C 〉 T polymorphism between the two groups, the 936T allele was associated with a decreased susceptibility to gastric cancer (OR 0.757; 95% CI 0.591–0.970). In the haplotype analyses, the haplotype TCT (OR 0.405; 95% CI 0.263–0.624) was most closely associated with a decreased susceptibility to gastric cancer. However, no significant association was observed between the frequency of the genotypes or alleles and the clinicopathologic characteristics of gastric cancer. 〈 i 〉 Conclusion: 〈 /i 〉 These observations imply that the VEGF gene polymorphisms may be associated with the susceptibility to gastric cancer. However, further studies of other VEGF sequence variants and their biological functions are needed to understand the role of the VEGF polymorphisms in determining the susceptibility to gastric cancer.

    Abstract: 〈 b 〉 〈 i 〉 Background: 〈 /i 〉 〈 /b 〉 Dysfunctional interplay between the heart and kidneys may lead to the development of anemia. The aim of this study was to evaluate the impact of cardiorenal anemia syndrome (CRAS) on short- and long-term outcomes among patients hospitalized with heart failure (HF). 〈 b 〉 〈 i 〉 Methods: 〈 /i 〉 〈 /b 〉 We enrolled 303 patients hospitalized with HF. We divided the patients into two groups: a CRAS group (n = 64) and a non-CRAS group (n = 239). We defined CRAS as HF accompanied by (1) an estimated glomerular filtration rate 〈 60 ml/min/1.73 m 〈 sup 〉 2 〈 /sup 〉 calculated by the Modification of Diet in Renal Disease at admission and (2) a hemoglobin level 〈 12 g/dl for females and 〈 13 g/dl for males at admission. The primary outcome was a composite of cardiac death, non-fatal myocardial infarction and rehospitalization for HF. 〈 b 〉 〈 i 〉 Results: 〈 /i 〉 〈 /b 〉 During a median follow-up period of 25.6 months (range 0.1-35.3 months), the patients with CRAS had a significantly increased risk for the primary outcome (27.5 vs. 10.7%, p 〈 0.001) compared with the patients in the non-CRAS group. Using Cox proportional hazard analyses, the hazard ratio (HR) for the presence of CRAS was found to be 1.874 (95% confidence interval [CI] 1.011-3.475, p = 0.046); HRs were also computed for the presence of diabetes mellitus (HR = 2.241, 95% CI 1.221-4.112, p = 0.009), New York Heart Association class III or IV HF (HR = 2.948, 95% CI 1.206-7.205, p = 0.018) and the use of intravenous loop diuretics (HR = 2.286, 95% CI 0.926-5.641, p = 0.073). 〈 b 〉 〈 i 〉 Conclusions: 〈 /i 〉 〈 /b 〉 Renal dysfunction and anemia are a fatal combination and are associated with poor prognosis in patients with HF.
