    Abstract: 〈 b 〉 〈 i 〉 Background: 〈 /i 〉 〈 /b 〉 Upregulation of caveolin-1 (Cav-1) expression is correlated with histopathological grade and poor prognosis in several human cancers. However, in gastric cancer, its clinical utility as a useful prognostic molecular marker remains unclear. 〈 b 〉 〈 i 〉 Methods: 〈 /i 〉 〈 /b 〉 The prognostic importance of Cav-1 expression was retrospectively analyzed by immunohistochemistry in 148 patients with gastric cancer who had undergone radical gastrectomy. 〈 b 〉 〈 i 〉 Results: 〈 /i 〉 〈 /b 〉 Cav-1 expression was positive in 23 (15.5%) patients and negative in 125 (84.5%) patients. Tumor location, tumor grade, lymph node involvement, pT stage, pTNM stage, and the presence of recurrence were found to be significantly associated with Cav-1 expression. The median disease-free survival (DFS) of patients with negative Cav-1 expression was significantly better than that of patients with positive Cav-1 expression (not reached vs. 10.2 months, p 〈 0.001). Moreover, patients with positive Cav-1 expression had a worse median overall survival (OS) compared to patients with negative Cav-1 expression (14.2 vs. 40.3 months, p = 0.004). In the multivariate analysis, Cav-1 expression (positive vs. negative) was an independent prognostic factor for DFS (p 〈 0.001, hazard ratio (HR) 2.58) and OS (p = 0.031, HR 1.87), as was lymph node metastasis. 〈 b 〉 〈 i 〉 Conclusion: 〈 /i 〉 〈 /b 〉 Our results suggest that positive Cav-1 expression is associated with progression and poor prognosis in gastric cancer patients after radical gastrectomy. Targeting Cav-1 would be a potential option for future gastric cancer treatment.

    Abstract: 〈 i 〉 Objective: 〈 /i 〉 We investigated the presence of 〈 i 〉 Helicobacter pylori 〈 /i 〉 in laryngeal cancer specimens to reveal whether or not this is a risk factor in the development of squamous cell carcinoma of the larynx. 〈 i 〉 Methods: 〈 /i 〉 Sixty-nine total laryngectomy specimens with the pathologic diagnosis of squamous cell carcinoma and 30 laryngeal tissue samples that had been taken for the investigation of nonneoplastic (polyp, nodule) diseases were studied. Specimens of both tumor and control groups were stained with hematoxylin-eosin and modified Giemsa stains, and then they were examined under a light microscope. 〈 i 〉 Results: 〈 /i 〉 In both groups, 〈 i 〉 H. pylori 〈 /i 〉 could not be found in any of the cases. 〈 i 〉 Conclusion: 〈 /i 〉 The histologic examination of our series did not reveal any clue related to the possible etiologic association between 〈 i 〉 H. pylori 〈 /i 〉 and squamous cell carcinoma of the larynx.

    Abstract: 〈 b 〉 〈 i 〉 Introduction: 〈 /i 〉 〈 /b 〉 Data to guide the evaluation of living-related donor candidates for kidney transplant recipients with Alport syndrome (AS) spectrum are limited. We aimed to examine a cohort of living-related donors to recipients with AS and compare their outcomes with a control group to improve understanding of the clinical course and outcomes of living donation in this context. 〈 b 〉 〈 i 〉 Methods: 〈 /i 〉 〈 /b 〉 Living donors (LDs) of AS recipients and propensity score-matched control LDs without any family history of AS (control group) were followed for major cardiac events, death, post-donation estimated glomerular filtration rate (eGFR), and proteinuria. 〈 b 〉 〈 i 〉 Results: 〈 /i 〉 〈 /b 〉 There were 31 LDs (48.4% male), in whom relationship to AS recipient included mother (45.2%), father (32.3%), sibling (16.1%), grandparent (3.2%), and uncle (3.2%). Long-term outcomes over 10.0 (IQR, 3.0–15.0) years were evaluated in 25 and 25 LDs from study and control groups, respectively. During follow-up, 5 LDs (20.0%) in study group developed major cardiac event (acute coronary ischemia [ 〈 i 〉 n 〈 /i 〉 = 4] and severe congestive heart failure [ 〈 i 〉 n 〈 /i 〉 = 1]) after 5.5 (IQR, 4.5–10.3) years, whereas only 2 (8.0%) LDs in control group developed major cardiac events ( 〈 i 〉 p 〈 /i 〉 = 0.221). New-onset hypertension was higher in study group (56.0%) compared to the control group (16.0%) ( 〈 i 〉 p 〈 /i 〉 = 0.003). Three donors in study and 2 donors in control group who developed new-onset hypertension died during follow-up ( 〈 i 〉 p 〈 /i 〉 = 0.297). Major cardiac event rate was significantly higher in donors who developed hypertension after donation (0 vs. 28.0%, 〈 i 〉 p 〈 /i 〉 & #x3c; 0.001). There were no differences between study groups regarding last eGFR and proteinuria levels ( 〈 i 〉 p 〈 /i 〉 = 0.558 and 〈 i 〉 p 〈 /i 〉 = 0.120, respectively). 〈 b 〉 〈 i 〉 Discussion/Conclusion: 〈 /i 〉 〈 /b 〉 Although the risk of kidney disease can be minimized by careful donor evaluation, our findings suggest that hypertension risk after the donation is higher than expected in related donors of recipients with AS.

    Abstract: 〈 b 〉 〈 i 〉 Objective: 〈 /i 〉 〈 /b 〉 The purpose of this study was to evaluate the tenascin-C levels in severe rheumatic mitral stenosis before and after percutaneous mitral balloon valvuloplasty (PMBV). 〈 b 〉 〈 i 〉 Subjects and Methods: 〈 /i 〉 〈 /b 〉 Forty patients with severe mitral stenosis requiring PMBV and 20 age-matched healthy subjects were included in the study. The mitral valve areas, mitral gradients and systolic pulmonary artery pressure (sPAP) were measured by echocardiography. The sPAP values and mitral gradients were also measured by catheterization before and after PMBV. The blood tenascin-C levels were measured before PMBV and 1 month after the procedure. 〈 b 〉 〈 i 〉 Results: 〈 /i 〉 〈 /b 〉 The echocardiographic mean mitral gradients had a significant decrease after PMBV (11.7 ± 2.8 vs. 5.6 ± 1.7 mm Hg; p 〈 0.001) and also those of catheterization (13.9 ± 4.4 vs. 4.0 ± 2.4 mm Hg; p 〈 0.001). Mitral valve areas increased significantly after PMBV (from 1.1 ± 0.1 to 1.8 ± 0.2 cm 〈 sup 〉 2 〈 /sup 〉 , p 〈 0.001). Tenascin-C levels decreased significantly in patients after PMBV (from 15.0 ± 3.8 to 10.9 ± 3.1 ng/ml; p 〈 0.001). Tenascin-C levels were higher in patients with mitral stenosis before PMBV than in healthy subjects (15.0 ± 3.8 and 9.4 ± 2.9 ng/ml; p 〈 0.001, respectively). There were no significant differences between patients with mitral stenosis after PMBV and healthy subjects (10.9 ± 3.1 and 9.4 ± 2.9 ng/ml; p = 0.09, respectively). There was a significant positive correlation between tenascin-C levels and sPAP (r = 0.508, p 〈 0.001). In multivariant analysis, tenascin-C predicted mitral stenosis (p = 0.004, OR: 2.31). 〈 b 〉 〈 i 〉 Conclusions: 〈 /i 〉 〈 /b 〉 Tenascin-C was an independent predictor for rheumatic mitral stenosis.

    Abstract: 〈 b 〉 〈 i 〉 Introduction: 〈 /i 〉 〈 /b 〉 Multiple sclerosis (MS) is an autoimmune disease that can affect balance, gait, and improve fall risk. The aim of this study was to investigate peripheral vestibular system involvement in MS and associations with the disease severity. 〈 b 〉 〈 i 〉 Methods: 〈 /i 〉 〈 /b 〉 Thirty-five adult patients with MS and 14 age- and gender-matched healthy controls were evaluated using video head impulse test (v-HIT), cervical vestibular evoked myogenic potential (c-VEMP), ocular vestibular evoked myogenic potentials (o-VEMPs), and sensory organization test (SOT) of computerized dynamic posturography (CDP). The results of both groups were compared, and association with EDSS scores was evaluated. 〈 b 〉 〈 i 〉 Results: 〈 /i 〉 〈 /b 〉 There was no significant difference between the groups regarding v-HIT and c-VEMP results ( 〈 i 〉 p 〈 /i 〉 & #x3e; 0.05). There was no association of the v-HIT, c-VEMP, and o-VEMP results with EDSS scores ( 〈 i 〉 p 〈 /i 〉 & #x3e; 0.05). There was no significant difference between the o-VEMP results of the groups ( 〈 i 〉 p 〈 /i 〉 & #x3e; 0.05) except for N1-P1 amplitudes ( 〈 i 〉 p 〈 /i 〉 = 0.01). The amplitudes of N1-P1 were significantly lower in the patients compared to controls ( 〈 i 〉 p 〈 /i 〉 = 0.01). There was no significant difference between the SOT results of the groups ( 〈 i 〉 p 〈 /i 〉 & #x3e; 0.05). However, significant differences were found within and between groups when the patients were categorized according to their EDSS scores with a cutoff point of 3 ( 〈 i 〉 p 〈 /i 〉 & #x3c; 0.05). There were negative correlations between the EDSS scores and composite ( 〈 i 〉 r 〈 /i 〉 = −396, 〈 i 〉 p 〈 /i 〉 = 0.02) and somatosensory (SOM) scores ( 〈 i 〉 r 〈 /i 〉 = −487, 〈 i 〉 p 〈 /i 〉 = 0.04) of CDP in the MS group. 〈 b 〉 〈 i 〉 Conclusion: 〈 /i 〉 〈 /b 〉 Although central and peripheral balance-related systems are affected in MS, the impact of disease on the peripheral vestibular end organ is subtle. In particular, the v-HIT, which was mentioned previously as a detector of brainstem dysfunction could not be a reliable tool in the detection of brainstem pathologies in MS patients. The o-VEMP amplitudes may be affected in the early stages of the disease, possibly due to the crossed ventral tegmental tract, oculomotor nuclei, or interstitial nucleus of Cajal involvements. An EDSS score & #x3e;3 seems a cutoff level indicating abnormalities in balance integration.

    Abstract: 〈 i 〉 Background/Aims: 〈 /i 〉 To evaluate the correlation between visual acuity (VA) and photoreceptor integrity in eyes with resolved diabetic macular edema (DME) after pars plana vitrectomy (PPV). 〈 i 〉 Methods: 〈 /i 〉 Eleven eyes with resolved macular edema following PPV with internal limiting membrane removal for DME were included in this retrospective study. The integrity of the external limiting membrane (ELM) and inner and outer segments (IS/OS) of the photoreceptor junction was evaluated by spectral domain optical coherence tomography. The main outcome measures were percentage of disrupted ELM and IS/OS lines, and correlation between VA and photoreceptor integrity. 〈 i 〉 Results: 〈 /i 〉 The mean time after PPV was 78 ± 17 months. The mean lengths of the disrupted ELM and IS/OS lines were 223 ± 167 µm (63%) and 189 ± 175 µm (54%) in the foveola, and 900 ± 522 µm (60%) and 835 ± 582 µm (55%) in the fovea, respectively. Intact ELM and IS/OS lines were positively correlated with VA in both the fovea (p = 0.09 and p = 0.02, respectively) and foveola (p = 0.004 and p = 0.03, respectively). Linear regression analysis showed a statistically significant association of intact ELM and IS/OS lines with VA in the fovea. Disrupted ELM and IS/OS lines had a strong correlation with each other in both the fovea (r = –0.71, p = 0.013) and foveola (r = 0.81, p = 0.02). 〈 i 〉 Conclusions: 〈 /i 〉 The integrity of the ELM and IS/OS lines was positively correlated with VA in eyes with resolved DME after PPV.

    Abstract: 〈 i 〉 Background: 〈 /i 〉 Oxidative stress is one of the leading causes of cardiovascular morbidity and mortality in chronic kidney disease. Although it is clear that many metabolic abnormalities improve, the effects of kidney transplantation on oxidative state are obscure. 〈 i 〉 Methods: 〈 /i 〉 Twenty-three kidney transplant patients were included in the study. Eleven patients (mean age 27.9± 9.1 years) were treated with cyclosporine A (CsA) whereas 12 patients (mean age 22.4 ± 3.4 years) were treated with tacrolimus. Twenty-three healthy subjects served as controls. None of the patients or controls suffered from diabetes mellitus or an acute infection at the time of the study. Plasma malondialdehyde (MDA), plasma selenium (Se), erythrocyte glutathione peroxidase (GSH-Px), erythrocyte superoxide dismutase (SOD), erythrocyte Zn (EZn), and erythrocyte Cu (ECu) levels were studied before and in the 1st,3rd, 7th, 14th and 28th days after the transplantation. 〈 i 〉 Results: 〈 /i 〉 The GSH-Px, SOD, ECu, EZn and selenium levels were lower and MDA levels were higher in patients than controls before transplantation (p 〈 0.001 for all). MDA levels decreased and SOD, GSH-Px, ECu, EZn levels increased in parallel to the decrement of serum creatinine levels following the renal transplantation. No difference was found among the patients regarding the treatment regime. 〈 i 〉 Conclusion: 〈 /i 〉 The study data suggest that the improvement in oxidative state parameters begins at the first day of renal transplantation and continues at the 28th posttransplant day in living donor transplantations.

    Abstract: 〈 b 〉 〈 i 〉 Objective: 〈 /i 〉 〈 /b 〉 To report a case of broncholithiasis with different types of calculi in the tracheobronchial tree. 〈 b 〉 〈 i 〉 Clinical Presentation and Intervention: 〈 /i 〉 〈 /b 〉 A 50-year-old male who suffered from hemoptysis presented with recurrent broncholith expectoration due to past tuberculous middle lobe syndrome. Bronchoscopic examination revealed loose and embedded broncholiths located at two different bronchi. A surgical resection was suggested, but he refused. 〈 b 〉 〈 i 〉 Conclusion: 〈 /i 〉 〈 /b 〉 The diagnosis of broncholithiasis should be kept in mind in patients who had hemoptysis and calcified mediastinal lymph nodes on thorax computerized tomography, and diagnostic bronchoscopy should be done to prove the relationship of the tracheobronchial tree with a broncholith.

    Abstract: 〈 i 〉 Background: 〈 /i 〉 Lichen sclerosus (LS), is characterized by localized patches of atrophy and whitening of the skin. The cause of LS remains unknown, but genetic, hormonal, immunologic factors and autoimmune mechanisms have been incriminated. There are conflicting data regarding the association between LS and human leukocyte antigens (HLA). 〈 i 〉 Methods: 〈 /i 〉 We have analyzed the HLA alleles of a family, in which 4 of 5 children have lichen sclerosus. 〈 i 〉 Results: 〈 /i 〉 HLA-B*08 and HLA-B*18 alleles were detected in children with LS, but not in a healthy sister. None of the patients had autoimmune disease. 〈 i 〉 Conclusion: 〈 /i 〉 In our opinion, coexistence of these two alleles may play a role in the development of LS.
