Abstract:
Đặt vấn đề: Mất đoạn AZF (azoospermic factor) là nguyên nhân di truyền thường gặp nhất gây hiếm muộn nam. Tầm soát AZF được khuyến cáo khi giảm số lượng tinh trùng. Nghiên cứu xác định tần suất và đặc điểm mất đoạn AZF ở nam giới hiếm muộn tại Bệnh viện Hùng Vương.
Phương pháp nghiên cứu: 526 BN hiếm muộn nam có kết quả mất đoạn AZF, thăm khám lâm sàng, tinh dịch đồ, nội tiết tố sinh dục nam, và nhiễm sắc thể (NST) đồ được hồi cứu hồ sơ từ 5/2018 đến 5/2021. Có 4 nhóm BN: vô tinh, thiểu tinh rất nặng, nặng và vừa hay nhẹ. Kết quả: Tỷ lệ mất đoạn AZF là 25,9% (136 BN), gồm AZFc (92,6%) và AZFbc (7,4%). AZFc bán phần và toàn phần (b2/b4) lần lượt chiếm 85,7% và 14,3%. AZFc bán phần gồm gr/gr (89,8%) và b2/b3 (10,2%). Tỷ lệ mất đoạn AZF tăng theo mức độ giảm số lượng tinh trùng: cao nhất ở vô tinh không bế tắc (33,5%) và thiểu tinh rất nặng (32,9%), và thấp hơn ở thiểu tinh nặng (23,9%) (χ2 test, p 〈 0.0001). Mất đoạn AZF cũng ghi nhận ở 11,1% thiểu tinh vừa hay nhẹ.
38/396 BN (9,6%) được làm NST đồ có bất thường NST, gồm hội chứng Klinefelter (24), thể khảm 45X/46XY [1] , đảo đoạn/chuyển đoạn [9], và bất thường cấu trúc NST khác [4] . Tỷ lệ mất đoạn AZF ở NST đồ bình thường và bất thường là 25,9% và 26,3%. Tỷ lệ mất đoạn AZF ở vô tinh không bế tắc 46XY, hội chứng Klinefelter, chuyển đoạn/đảo đoạn NST và bất sản ODT bẩm sinh lần lượt là 34,4%, 25%, 22,2% và 11,4%. Một thể khảm 45X/46XY cũng kèm mất đoạn AZF.
Kết luận: Mất đoạn AZF khá phổ biến ở nam giới hiếm muộn do giảm số lượng tinh trùng nặng ( 〈 5 x 106/mL) và có thể xảy ra đồng thời với bất thường NST. Vì mất đoạn AZF cũng gặp ở 1 trên 10 BN thiểu tinh vừa hay nhẹ, cân nhắc chỉ định tầm soát AZF theo khuyến cáo và thực tế ở Việt Nam cũng như các yếu tố khác của cặp vợ chồng hiếm muộn.
Abstract Introduction: AZF (azoospermic factor) microdeletions are the most common genetic cause of male infertility. AZF microdeletion screening is recommended in case of reduced sperm counts. This study was aimed at determining the frequencies and characteristics of AZF microdeletions at Hung Vuong hospital.
Patients and Methods: 526 infertile men whose medical records contained information about physical examination, spermatogram, male sex hormones, AZF microdeletion and karyotype were recruted between May 2018 and May 2021. Patients were categorized in 4 groups: azoospermia and very severe / severe / moderate or mild oligozoospermia.
Results: The rate of AZF microdeletions was of 25.9% (136 patients), consisting of AZFc (92.6%) and AZFbc (7.4%). Partial and complete AZFc microdeletions were of 85.7% and 14.3%, respectively. Partial AZFc microdeletions included gr/gr (89.8%) and b2/b3 (10.2%). The rate of AZF microdeletions increased with the severity of sperm count reduction: higher in nonobstructive azoospermia (33.5%) and very severe oligozoospermia (32.9%), and lower in severe oligozoospermia (23.9%) (χ2 test, p 〈 0.0001). AZF microdeletions were found in 11.1% of moderate or mild oligozoospermia.
38/396 patients (9.6%) who were indicated for karyotype had chromosomal disorders, including Klinefelter’s syndrome (24), 45X/46XY mosaicism [1] , inversion/translocation [9], and other chromosomal structural anomalies [4] . The rate of AZF microdeletions among patient with and without chromosomal anomalies were similar (25.9% and 26.3%, respectively). The rates of AZF microdeletions were 34.4%, 25%, 22.2% and 11.4%, respectively, in nonobstructive azoospermia with 46XY karyotype, Klinefelter’s syndrome, inversion/translocation, and congenital absence of vas deferens. One 45X/46XY mosaicism patient had AZF microdeletion.
Conclusions: AZF microdeletions are quite common in infertile men caused by severely reduced sperm counts ( 〈 5 x 106/mL) and could occur simultaneously with chromosomal anomalies. Since AZF microdeletions could be found in approximately one over ten oligozoospermic patients, AZF microdeletion screening should be considered in respect of global and regional recommendations, clinical practice in Vietnam and infertile couples’ related factors.
