Abstract:
- Đặt vấn đề: Tổn thương thận cấp là biến chứng phổ biến trên bệnh nhân nhiễm khuẩn huyết. Biến chứng trên thận làm kéo dài thời gian và chi phí điều trị, gia tăng đáng kể tỷ lệ tử vong cũng như đưa đến suy thận giai đoạn cuối. Dự đoán sớm những bệnh nhân có nguy cơ tiến triển tổn thương thận cấp giúp nhà lâm sàng có thái độ xử trí tích cực, phòng ngừa tổn thương thận xảy ra. Hiện nay creatinine vẫn là tiêu chuẩn vàng trong chẩn đoán tổn thương thận cấp. Tuy nhiên creatinine thường tăng ở giai đoạn tương đối muộn và bị ảnh hưởng bởi khối lượng cơ cũng như tình trạng dinh dưỡng của bệnh nhân, điều này có thể dẫn đến chậm trễ trong chẩn đoán và điều trị. Một số dấu ấn sinh học mới giúp phát hiện sớm tổn thương thận nhưng chi phí cao và chưa sẵn có trong thực hành lâm sàng. Trong khi đó, nhiều nghiên cứu cho thấy tỷ số neutrophil trên lymphocyte có giá trị tương đối cao trong tiên đoán biến chứng này là một xét nghiệm thường quy, chi phí thấp, không xâm lấn. Vì thế chúng tôi thực hiện đề tài nghiên cứu này nhằm đánh giá vai trò của tỷ số neutrophil trên lymphocyte trong tiên đoán sớm tổn thương thận cấp trên bệnh nhân nhiễm khuẩn huyết và sốc nhiễm khuẩn.
- Mục tiêu: Xác định các yếu tố nguy cơ của tổn thương thận cấp (TTTC) trên bệnh nhân nhiễm khuẩn huyết (NKH) và sốc nhiễm khuẩn (SNK). So sánh một số đặc điểm lâm sàng, cận lâm sàng và tỷ lệ tử vong ở bệnh nhân NKH và SNK có TTTC và không TTTC. Xác định điểm cắt tối ưu, độ nhạy, độ đặc hiệu, giá trị tiên đoán dương, giá trị tiên đoán âm, AUC của tỷ số neutrophil trên lymphocyte (NLR) trong tiên đoán tổn thương thận cấp ở bệnh nhân NKH và SNK. - Phương pháp nghiên cứu: Nghiên cứu đoàn hệ trên bệnh nhân NKH và SNK tại khoa Hồi sức tích cực - Chống độc Bệnh viện Nhân dân 115 từ tháng 12/2021 đến tháng 6/2022.
- Kết quả: Tỷ lệ TTTC là 59,18%, trong đó giai đoạn 2 chiếm đa số (46,55%). Dân số nghiên cứu có tuổi trung bình là 66 ± 12,02, điểm APACHE II và SOFA trung vị lần lượt là 21 và 7 điểm, HATB 68,35 mmHg và tỷ lệ cần sử dụng thuốc vận mạch là 62,1%. Tỷ lệ tử vong trong nhóm có TTTC cao hơn có ý nghĩa thống kê so với nhóm không TTTC (51,72% so với 22,50 %). Hô hấp là cơ quan nhiễm trùng thường gặp nhất (47,96%). Tỷ lệ cấy máu và cấy bệnh phẩm dương tính lần lượt là 18,37% và 81,6%. Trong đó, tác nhân gây bệnh thường gặp nhất là Klebsiella Pneumoniae (22,67%). Các yếu tố nguy cơ độc lập của TTTC sau khi phân tích hồi quy đơn và đa biến bao gồm: TC (OR = 0,99 (0,98 - 0,99)), tỷ số NLR (OR = 1,17 (1,03 - 1,34)) và điểm APACHE II (OR = 1,29 (1,07 - 1,54)). Điểm cắt NLR ≥ 12,28 có độ nhạy và độ đặc hiệu lần lượt là 90,24% và 75% và AUC = 0,83 trong tiên đoán tổn thương thận. - Kết luận: Có thể sử dụng tỷ số NLR với ngưỡng điểm cắt ≥ 12,28 từ mẫu công thức máu tại thời điểm bệnh nhân được chẩn đoán NKH hoặc SNK để xác định những bệnh nhân có nguy cơ cao diễn tiến TTTC.
Abstract - Background: Acute kidney injury is a common complication among septic patients, which is associated with prolonged treatment duration, significant increase in mortality, treatment costs and development of chronic complications such as endstage renal failure. Early prediction patients at risk of developing AKI can facilitate proactive intervention by healthcare providers and prevents the occurrence of renal damage. Although creatinine is a conventional predictor for AKI, its dependence on the patient’s muscle mass, nutritional status and late increase can potentially lead to delayed diagnosis and treatment. Several urinary biomarkers aid diagnosis of AKI earlier than a rise in serum creatinine; however, their high costs make the measurements are not possible nowadays. There is now increasing experimental and clinical studies for an undeniable role of inflammation in the pathophysiology of AKI. Neutrophil - lymphocyte ratio is an indicator of systemic inflammation, calculated easily from complete blood count which is a routine and cost - effective test with minimal invasiveness. Therefore, this study aims to evaluate the role of the Neutrophil - lymphocyte ratio in early prediction of AKI in patients with sepsis and septic shock.
- Objectives: Determination the risk factors of acute kidney injury (AKI) in patient with sepsis and septic shock. Comparison the clinical, laboratory, and mortality outcomes between patients with and without AKI. Determination the optimal cut - off value, sensitivity, specificity, positive predictive value, negative predictive value and AUCof the Neutrophil - to - Lymphocyte Ratio (NLR) in predicting AKI in patients with sepsis and septic shock. - Method: Prospective cohort study conducted on patients with sepsis and septic shock admitted to the Intensive Care Unit and Toxicology Department of Hospital 115 from December 2021 to June 2022.
- Results: The rate of AKI accounted for 59,18%, of which stage 2 had the highest proportion (46.55%). The average age was 66 ± 12,02, the median APACHE II and SOFA scores were 21 and 7 points respectively, the average blood pressure was 68,35 mmHg and the rate of vasopressor use was 62,1%. The mortality rate in the AKI group was statistically significant higher compared to the non - AKI group at (51,72% vs 22,5%). Respiratory tract was the most common source of infection (47,96%). The rates of blood and specimen culture positivity were 18,37% and 81,6%, respectively. Among them, the most common pathogen was Klebsiella pneumoniae (22,67%). Independent risk factors of septic AKI after multivariate logistic regression analysis included: platelet (OR = 0,99, 95% CI: 0,98 - 0,99), NLR (OR = 1,17, 95% CI:1,03 - 1,34), and APACHE II score (OR = 1,29, 95% CI: 1,07 - 1,54). NLR cut - off value 12,28 had a sensitivity 90,24%, specificity 75% and an AUC of 0,83 in predicting AKI. - Conclusion: The NLR from the blood sample at the time of diagnosing sepsis or septic shock with a cut - off point ≥ 12,28 can be used to identify patients at high risk of progressing to AKI.
