    Notes: Abstract A novel class of multigrid algorithms for the variable-coupling isotropic Gaussian models is presented. In addition to the elimination of the critical slowing down (which otherwise might become much worse than usual in the case of strongly varying coupling values), the “volume factor” is also eliminated. That is, the need to produce many independent fine-grid configurations for averaging out their statistical deviations is removed, by applying multigrid cycles that sample mostly on coarse grids. Thermodynamic limits can be calculated to relative accuracy ε r in just $$O(\varepsilon _r^{ - 2} )$$ computer operations, where ε r is the error relative to the standard deviation of the observable. In this paper, such an optimal algorithm is obtained for the calculation of the susceptibility in the d-dimensional variable-coupling isotropic Gaussian model (with numerical experiments for d = 1, 2). Some basic general rules for the operation of multigrid algorithms, applicable to much wider classes of models, are derived.

    Notes: Abstract Beyond eliminating the critical slowing down, multigrid algorithms can also eliminate the need to produce many independent fine-grid configurations for averaging out their statistical deviations, by averaging over the many samples produced in coarse grids during the multigrid cycle. Thermodynamic limits can be calculated to accuracy ɛ in justO(ε-2) computer operations. Examples described in detail and with results of numerical tests are the calculation of the susceptibility, the σ-susceptibility, and the average energy in Gaussian models, and also the determination of the susceptibility and the critical temperature in a two-dimensional Ising spin model. Extension to more advanced models is outlined.

    Notes: Abstract Aims/hypothesis. A computer model was developed to determine the health outcomes and economic consequences of different combinations of diabetes interventions in newly diagnosed patients with Type I (insulin-dependent) diabetes in Switzerland.¶Methods. We modelled seven complications of diabetes: hypoglycaemia, ketoacidosis, acute myocardial infarction, stroke, lower extremity amputation, nephropathy, and retinopathy. Transition probabilities and costs were taken from published literature. The Swiss health insurance payer perspective was taken. Various combinations of diabetes management strategies, including intensive or conventional insulin therapy and screening and treatment strategies for renal and eye disease were defined. Life expectancy, cumulative incidences of complications, and mean expected total lifetime costs per patient were calculated under six different management strategies. Incremental cost-effectiveness ratios were calculated in terms of costs per life-year gained compared with conventional insulin therapy alone.¶Results. The addition of screening for microalbuminuria and retinopathy followed by appropriate treatment, if detected, were cost saving, with reduction in cumulative incidence of end stage renal disease and blindness respectively, and, in the case of microalbuminuria screening and treatment, an improvement in life expectancy. Intensive therapy improved life expectancy but increased total lifetime costs.¶Conclusion/interpretation. Optimal management of Type I diabetic patients, including secondary and tertiary prevention, leads to reduced complications and improved life expectancy, with the increased costs of prevention offset to varying degrees by cost savings due to complications avoided.     Notes: Abstract Glucocorticoids are the most potent antiinflammatory drugs. Large doses of dexamethasone and other glucocorticoids are widely used in the clinic but the mechanism of the beneficial effects of megadoses still remains to be explained. We tested the effects of a dexamethasone (DEX) megadose therapy in a rat model of anaphylactic shock. By combining DEX with the potent glucocorticoid receptor antagonist RU 486 we looked for evidence of a non-specific action of the glucocorticoid at high doses. A dose of 10 mg/kg DEX given 2h before challenge protected the heart against the symptoms of cardiac anaphylaxis: heart rate was improved by 22%, ventricular contractility by 13% and coronary flow by 5%. Administration of 20 mg/kg RU 486, 30 min before dexamethasone, reduced the beneficial effect of DEX by about 30%, although this failed to reach statistical significance. We found no dose-response relationship for the high dexamethasone doses of 0.5–10 mg/kg. In experimental allergic encephalomyelitis (EAE) there was a surprising toxic action of DEX after daily doses of 0.25 mg/kg for 5 days, and also following single administration of 2.5 or 10 mg/kg. A single dose of 1.25 mg/kg, given on the day of immunization alone or with additional doses at weekly intervals for 3 weeks, caused a strong inhibition of the EAE. In summary we conclude that the present data of cardiac anaphylaxis hardly point to extra glucocorticoid megadose effects. The present dexamethasone effects in cardiac anaphylaxis and in EAE have to be cleared up by further experiments.
