    Abstract: The frequency of isolated Lisfranc ligament (ILL) injuries has been increasing recently with the increase in low-energy trauma resulting from sports injuries. For ILL injuries, the optimal method of fixation still remains controversial. The traditional fixation method is achieved by trans-articular screws, but recently, dorsal bridge plates and suture button (SB) fixation have become alternatives. Some biomechanical studies have showed that SB fixation can provide adequate strength compared to trans-articular screws. Therefore, a meta-analysis is imperative to provide evidence on whether flexible fixation is comparable to screw fixation for treatment of ILL injuries. Methods: We will conduct a comprehensive literature search in PubMed, Cochrane Library, EMBASE and Web of Science databases and for comparative studies. We will apply the risk-of-bias tool of the Cochrane Collaboration for Randomized Controlled Trials to assess the methodological quality. Risk-of-Bias Assessment Tool for Non-randomized Studies was used to evaluate the quality of comparative studies. Statistical analysis will be conducted using RevMan 5.4 software (Cochrane Collaboration, London, England). Results: This systematic review will evaluate the functional outcomes and radiographic results of flexible fixation for treatment of ILL injuries. Conclusion: The conclusion of this study will provide evidence for judging whether flexible fixation is superior to screw fixation for treatment of ILL injuries.

    Abstract: Many potential causative factors are related to the initiation and progression of osteonecrosis of the femoral head. The aim of this research was to investigate the etiology and clinical features of osteonecrosis of the femoral head in Chinese patients. Methods From January 1990 to July 2011, 643 cases of osteonecrosis of the femoral head were investigated retrospectively to analyze the potential causative factors, age, gender, latency period, time from the onset of pain to diagnosis, and Association Research Circulation Osseous stage. Results Of 643 cases, 315 cases were bilateral and 328 cases were unilateral, with an average age of (47.55±15.27) years. In the steroid-induced group, the average age at symptom onset was (41.80±15.47) years, and the median duration from taking steroid to the onset of pain was 36 months. The underlying diseases in the steroid-induced osteonecrosis of the femoral head group consisted of autoimmune and other diseases, of which systemic lupus erythematosus was the most common. In the alcohol-induced group, the average age at onset of symptoms was (48.06±11.90) years and the median time of habitual alcohol use was 240 months. In the traumatic group, the average age was (51.43±14.23) years and the median time from trauma to the onset of pain was 20 months. In the idiopathic group, the average age was (50.33±15.88) years. Of the total of 958 hips, 647 were at stage III or IV. The stage at diagnosis was earlier in the steroid-induced group than in the alcohol-induced, traumatic, or idiopathic groups. Conclusions Steroid use is the most common cause for osteonecrosis of the femoral head in this study. The age at diagnosis, time from the onset of pain to diagnosis, and stage were significantly earlier in the steroid-induced group.

    Abstract: Venous thromboembolism (VTE) is a common and serious complication after colorectal cancer (CRC) surgery. Few large-sample studies have reported VTE incidence and management status after CRC surgery in China. This study aimed to investigate the incidence and prevention of VTE in Chinese patients after CRC surgery, identify risk factors for developing VTE, and construct a new scoring system for clinical decision-making and care planning. Methods: Participants were recruited from 46 centers in 17 provinces in China. Patients were followed up for 1 month postoperatively. The study period was from May 2021 to May 2022. The Caprini score risk stratification and VTE prevention and incidence were recorded. The predictors of the occurrence of VTE after surgery were identified by multivariate logistic regression analysis, and a prediction model (CRC-VTE score) was developed. Results: A total of 1836 patients were analyzed. The postoperative Caprini scores ranged from 1 to 16 points, with a median of 6 points. Of these, 10.1% were classified as low risk (0–2 points), 7.4% as moderate risk (3–4 points), and 82.5% as high risk (≥5 points). Among these patients, 1210 (65.9%) received pharmacological prophylaxis, and 1061 (57.8%) received mechanical prophylaxis. The incidence of short-term VTE events after CRC surgery was 11.2% (95% CI 9.8–12.7), including deep venous thrombosis (DVT) (11.0%, 95% CI 9.6–12.5) and pulmonary embolism (PE) (0.2%, 95% CI 0–0.5). Multifactorial analysis showed that age (≥70 years), history of varicose veins in the lower extremities, cardiac insufficiency, female sex, preoperative bowel obstruction, preoperative bloody/tarry stool, and anesthesia time at least 180 min were independent risk factors for postoperative VTE. The CRC-VTE model was developed from these seven factors and had good VTE predictive performance ( C -statistic 0.72, 95% CI 0.68–0.76). Conclusions: This study provided a national perspective on the incidence and prevention of VTE after CRC surgery in China. The study offers guidance for VTE prevention in patients after CRC surgery. A practical CRC-VTE risk predictive model was proposed.

    Abstract: Elevated Ang II (angiotensin II) level leads to a range of conditions, including hypertensive kidney disease. Recent evidences indicate that FGFR1 (fibroblast growth factor receptor 1) signaling may be involved in kidney injuries. In this study, we determined whether Ang II alters FGFR1 signaling to mediate renal dysfunction. Methods: Human archival kidney samples from patients with or without hypertension were examined. Multiple genetic and pharmacological approaches were used to investigate FGFR1-mediated signaling in tubular epithelial NRK-52E cells in response to Ang II stimulation. C57BL/6 mice were infused with Ang II for 28 days to develop hypertensive kidney disease. Mice were treated with either adeno-associated virus expressing FGFR1 shRNA or FGFR1 inhibitor AZD4547. Results: Kidney specimens from subjects with hypertension and mice challenged with Ang II have increased FGFR1 activity in renal epithelial cells. Renal epithelial cells in culture initiate extracellular matrix programming in response to Ang II, through the activation of FGFR1, which is independent of both AT1R (angiotensin II receptor type 1) and AT2R (angiotensin II receptor type 2). The RNA sequencing analysis indicated that disrupting FGFR1 suppresses Ang II–induced fibrogenic responses in epithelial cells. Mechanistically, Ang II–activated FGFR1 leads to STAT3 (signal transducer and activator of transcription 3) activation, which is responsible for fibrogenic factor expression in kidneys. In the mouse model of hypertensive kidney disease, genetic knockdown of FGFR1 or pharmacological inhibition of its activity protected kidneys from dysfunction and fibrosis upon Ang II challenge. Conclusions: Our studies uncover a novel mechanism causing renal fibrosis in hypertension and indicate FGFR1 as a potential target to preserve renal function and integrity.

    Abstract: Introduction: Vascular smooth muscle cell (VSMC) proliferation, migration and dedifferentiation are critical for vascular diseases, such as atherosclerosis and restenosis after angioplasty. Recently, it is demonstrated that after vascular injury Notch 1and Notch 3 knockout (KO) mice have decreased intima formation whereas Notch 2 KO mice have increased one. Therefore, it is important to investigate the specific regulatory pathways that activate the different Notch receptors. Methods and Results: There was a time and dose dependent activation of Notch1, as determined by western blot of N1-ICD but not Notch 2 and 3 in VSMC. As Akt activates Notch signaling in endothelial cells and phospholipase C gamma-1 (PLCγ1) binds to and activates Akt in tumor cells, we therefore propose that PLCγ1 mediated Akt -Notch1 pathway is critical for intima formation by increasing VSMC proliferation, migration and dedifferentiation. To test our hypothesis, we measured PLCγ1 and Akt interaction in VSMC. We observed an association between PLCγ1 and Akt, which was increased upon AngII or PDGF stimulation. Depletion of PLCγ1 by siRNA significantly inhibited Akt and Notch1 activation. As the major target gene of Notch1, Hey2 protein expression was increased 1.8 fold and 2.3 fold respectively after AngII and PDGF stimulation. PLCγ1 siRNA treatment inhibited it by 68.3% and 64.7%. Depletion of PLCγ1 significantly inhibited PDGF induced VSMC proliferation, migration and dedifferentiation but increased apoptosis. In vivo, PLCγ1 and control siRNA were delivered periadventitially in pluronic gel after carotid ligation. Morphometric analysis 21days after ligation demonstrated that PLCγ1 depletion attenuated intima area (2.2 ± 0.7 vs 4.3 ± 1.0, *104 μm2) and intima/media ratio (0.21 ± 0.09 vs 0.43 ± 0.12) compared to the control group. Conclusions: In conclusion, PLCγ1 mediated Akt-Notch1 signaling is crucial for intima formation. The PLCγ1 and Akt interaction will be a promising target for therapeutic stratergies that inhibit vascular remodeling.

    Abstract: We report the first case of coronavirus disease 2019 (COVID-19) comorbid with leukemia in a patient hospitalized in Beijing, China. The patient showed a prolonged manifestation of symptoms and a protracted diagnosis period of COVID-19. It is necessary to extend isolation time, increase the number of nucleic acid detections and conduct early symptomatic treatment for children with both COVID-19 and additional health problems.

    Abstract: Medication adherence plays an important role in improving symptoms and ameliorating the progression of systemic lupus erythematosus (SLE). The purpose of this study was to examine the medication adherence rate and explore the related factors for nonadherence in Chinese SLE patients. Methods A cross-sectional study from The Second Affiliated Hospital of Nantong University between January 2017 and February 2018 was conducted. Patients with SLE completed the Compliance Questionnaire on Rheumatology to evaluate adherence, the Systemic Lupus Erythematosus Disease Activity Index (SLEDAI) for disease activity, and the Hospital Anxiety and Depression Scale to assess anxiety and depression. χ 2 Analyses, independent samples t tests, and logistic regression models were used to analyze data. Results This study included 144 SLE patients, and we found that 43.1% (n = 62) of the patients were nonadherence. Education, SLEDAI, anxiety, and depression were correlated with adherence in SLE patients. Moreover, logistic regression models identified education, SLEDAI, and depression as important associated factors of nonadherence. Conclusions This is the first time to evaluate demographic, clinical, and psychological related factors for medication adherence in Chinese SLE patients using SLEDAI and Hospital Anxiety and Depression Scale. Our study showed that 56.9% of SLE patients were not adherent, and education, SLEDAI, and depression were significantly related to medication adherence in SLE. These findings suggested that it was necessary to pay more attention to medication adherence of SLE patients, especially those with lower level of education, higher score of SLEDAI, and depression by effective intervention to improve SLE patients' adherence, so as to improve their quality of life.
