    Abstract: To identify biotypes in patients with newly diagnosed Parkinson disease (PD) and to test whether these biotypes could explain interindividual differences in longitudinal progression. Methods In this longitudinal analysis, we use a data-driven approach clustering PD patients from the Parkinson's Progression Markers Initiative (n = 314, age 61.0 ± 9.5, years 34.1% female, 5 years of follow-up). Voxel-level neuroanatomic features were estimated with deformation-based morphometry (DBM) of T1-weighted MRI. Voxels with deformation values that were significantly correlated ( p 〈 0.01) with clinical scores (Movement Disorder Society–sponsored revision of the Unified Parkinson’s Disease Rating Scale Parts I–III and total score, tremor score, and postural instability and gait difficulty score) at baseline were selected. Then, these neuroanatomic features were subjected to hierarchical cluster analysis. Changes in the longitudinal progression and neuroanatomic pattern were compared between different biotypes. Results Two neuroanatomic biotypes were identified: biotype 1 (n = 114) with subcortical brain volumes smaller than heathy controls and biotype 2 (n = 200) with subcortical brain volumes larger than heathy controls. Biotype 1 had more severe motor impairment, autonomic dysfunction, and much worse REM sleep behavior disorder than biotype 2 at baseline. Although disease durations at the initial visit and follow-up were similar between biotypes, patients with PD with smaller subcortical brain volume had poorer prognosis, with more rapid decline in several clinical domains and in dopamine functional neuroimaging over an average of 5 years. Conclusion Robust neuroanatomic biotypes exist in PD with distinct clinical and neuroanatomic patterns. These biotypes can be detected at diagnosis and predict the course of longitudinal progression, which should benefit trial design and evaluation.

    Abstract: The aim of the study was to discuss the application of biological optimization and its difference from physical optimization in hypofractionated radiotherapy for breast cancer after conservative surgery. Methods This retrospective study enrolled 15 randomly chosen patients with left-sided breast cancer who received radiotherapy. The volumetric arc therapy (VMAT) technique was used to redesign treatment plans with physical functions (PF) group, biological-physical functions combined (BF + PF and PF + BF) groups, and biological functions (BF) group. The dosimetric differences based on the above four optimization methods were assessed by calculating and analyzing the corresponding dose-volume parameters. Results The target parameters of the four groups differed significantly ( P 〈 0.05) except for the conformity index (CI). The tumor control probability (TCP) values in the BF and BF + PF groups were higher than those in the PF and PF + BF groups. Moreover, the dose-volume parameters of the ipsilateral lung in the BF group were less than those of three other groups, while the monitor unit (MU) in the BF group was approximately 16% lower than those of the PF and PF + BF groups. Conclusion Biological functions were useful to increase the equivalent uniform dose (EUD) and TCP values of the target, decrease the dose-volume parameters of the organs-at-risk (OARs), and improve treatment efficiency.

    Abstract: Metallothioneins (MTs) were reported to be associated with many kinds of tumors’ prognosis, although MTs expression varied greatly among tumors. To assess the prognostic value of Metallothioneins (MTs) in different kinds of tumors, comprehensive literature search was conducted to perform a meta-analysis. Methods: Eligible studies were identified by PubMed, MEDLINE, Web of Science (WOS), the Cochrane Library of Systematic Reviews, EMBASE, China National Knowledge Infrastructure (CNKI), WANFANG database and SinoMed database up to December 2017, which was designed to assess the prognostic value of MTs in different kinds of tumors. The main endpoint events were overall survival (OS) and disease-free survival (DFS). Hazard ratios (HRs) and its variance were retrieved from the original studies directly or calculated using Engauge Digitizer version 4.1. Random or fixed effects model meta-analysis was employed depending on the heterogeneity. Publication bias was evaluated by funnel plots, Begg and Egger tests. Results: A total of 22 studies were enrolled in this meta-analysis, including 2843 tumor tissues (1517 were MTs negative/low, and 1326 were MTs high). Results showed that there was significant association between MTs expression and tumors’ OS (HR = 1.60; 95%CI 1.34∼1.92, P   〈  .00001). Subgroup analysis showed that high level of MTs expression was associated with prolonged OS in liver cancer (HR = 0.65, 95%CI 0.48∼0.89, P  = .007), but it was on the contrary in the tumor of ovary (HR = 1.47, 95%CI 1.01∼2.14, P  = .04), bladder (HR = 1.71, 95%CI 1.21∼2.42, P  = .002), intestine (HR = 3.13, 95%CI 1.97∼4.97, P   〈  .00001), kidney (HR = 3.31, 95%CI 1.61∼6.79, P  = .001). However, there was no significant association between MTs expression and OS in breast (HR = 1.02, 95%CI 0.69∼1.51, P  = .93). Conclusions: MTs could be taken as a potential prognostic biomarker for tumors, and uniqueness of MTs prognostic value in liver cancer deserved further study.

    Abstract: Wounds caused by firearms are intractable problems in treating war traumas and clinical management. Conventional open surgery inflicts large injury and leads to slow recovery. At the same time, most patients suffer from compound injuries with the critical condition and poor operation tolerance. Thus, it is crucial to probe into the minimally invasive surgical removal of residual kidney bullets. Patient concerns: We report a case where a bullet remained in the right renal parenchyma on the patient, with penetrating injury in his liver. Diagnosis: Obviously the patient has suffered gunshot wound with a bullet stuck in his kidney, while his liver function was impacted. Interventions: Six months after the injury, we performed the minimally-invasive procedures on the patient with percutaneous nephroscope technology and laser technology under the guidance of ultrasound localization. The bullet and ammunition granulation and scar surrounding tissue were fully removed. Intraoperative bleeding was little, while the incision was small. The patient could leave the bed and walk on the 1st postoperative day. The drainage tube was removed on the 3rd postoperative day, after which the patient was discharged on the 4th postoperative day. Outcomes: The patient recovered well after surgery and was followed up for 5 years. The latest examination of his liver and kidney function was as follows: alanine aminotransferase 61IU/L, aspartate aminotransferase 33 IU/L, albumin/globulin 46.6/26.0, total bilirubin 19.1μmol/L, direct bilirubin 4.9μmol/L, indirect bilirubin 14.2μmol/L, alkaline phosphatase 111 IU/L, creatinine 57μmol/L, urea 5.16mmol/L, cystatin 0.73mg/L. The plain computed tomography scan showed a few calcifications in the liver and a patchy low-density shadow in the right kidney. It was proved that the liver and kidney function of the patient recovered well, and his living qualify has come back to the track, with no postoperative complications. Lessons: Innovative integration of percutaneous nephroscopy technology and laser was used to remove kidney foreign bodies and developed the optimal surgical plan, small trauma, fast recovery, and the treatment of kidney foreign bodies was newly explored.

    Abstract: Accumulating evidence has suggested that the XRCC1 Arg399Gln and Arg194Trp polymorphisms might be related to cutaneous melanoma susceptibility. However, epidemiologic findings have been inconsistent. We have assessed reported studies by meta-analysis to perform a more precise estimation of the association between the XRCC1 two polymorphisms (Arg399Gln, Arg194Trp) and risk of cutaneous melanoma. A total of seven eligible articles were selected for this meta-analysis, including 3454 cases and 3811 controls for the XRCC1 Arg399Gln polymorphism and 1256 cases and 1575 controls for the XRCC1 Arg194Trp polymorphism. Overall, no significant associations were found in all genetic models when the studies were pooled into the meta-analysis for the Arg399Gln and Arg194Trp polymorphisms. When stratified by source of control, significant associations were found for the Arg399Gln polymorphism in the population-based subgroup under AA versus GG [odds ratio (OR)=1.43, 95% confidence interval (CI)=1.08–1.88]; the dominant model AA/GA versus GG (OR=1.25, 95% CI=1.04–1.51); and the recessive model AA versus GA/GG (OR=1.31, 95% CI=1.01–1.68). No significant associations were found for the Arg194Trp polymorphism in the subgroup analysis. This meta-analysis suggested that the XRCC1 Arg399Gln polymorphism was a risk factor for cutaneous melanoma in population-based subgroup.

    Abstract: To introduce an end-to-end automatic segmentation method for organs at risk (OARs) in chest computed tomography (CT) images based on dense connection deep learning and to provide an accurate auto-segmentation model to reduce the workload on radiation oncologists. Methods CT images of 36 lung cancer cases were included in this study. Of these, 27 cases were randomly selected as the training set, six cases as the validation set, and nine cases as the testing set. The left and right lungs, cord, and heart were auto-segmented, and the training time was set to approximately 5 h. The testing set was evaluated using geometric metrics including the Dice similarity coefficient (DSC), 95% Hausdorff distance (HD95), and average surface distance (ASD). Thereafter, two sets of treatment plans were optimized based on manually contoured OARs and automatically contoured OARs, respectively. Dosimetric parameters including Dmax and Vx of the OARs were obtained and compared. Results The proposed model was superior to U-Net in terms of the DSC, HD95, and ASD, although there was no significant difference in the segmentation results yielded by both networks ( P 〉 0.05). Compared to manual segmentation, auto-segmentation significantly reduced the segmentation time by nearly 40.7% ( P 〈 0.05). Moreover, the differences in dose-volume parameters between the two sets of plans were not statistically significant ( P 〉 0.05). Conclusion The bilateral lung, cord, and heart could be accurately delineated using the DenseNet-based deep learning method. Thus, feature map reuse can be a novel approach to medical image auto-segmentation.

    Abstract: The aim of the study was to compare flattening filter-free (FFF) beams and conventional flattening filter (FF) beams in volumetric modulated arc therapy (VMAT) for cervical cancer after surgery, through a retrospective planning study. Methods VMAT plans of FFF beams and normal FF beams were designed for a cohort of 15 patients. The prescribed dose was 45 Gy to 1.8 Gy per fraction, and at least 95% of the planning target volume received this dose. Doses were computed with a commercially available treatment planning system using a Monte Carlo (MC) algorithm. Plans were compared according to dose-volume histogram analysis in terms of planning target volume homogeneity and conformity indices (HI and CI), as well as organs at risk (OAR) dose and volume parameters. Results FFF-VMAT was similar to FF-VMAT in terms of CI, but inferior to FF-VMAT considering HI. No statistically differences were observed between FFF-VMAT and FF-VMAT in following organ at risks including pelvic bone marrow, small bowel, bladder, rectum, and normal tissue (NT). . Conclusion For patients with cervical cancer after hysterectomy, the FFF beam achieved target and OAR dose distribution similar to that of the FF beam. Reduction of beam-on time in cervical cancer is beneficial.
