    Abstract: To analyze vitreoretinal findings, immunoglobulin clonality tests, and interleukin (IL) levels for diagnosing vitreoretinal lymphoma (VRL). Methods: Forty-three patients who underwent diagnostic vitrectomy for suspected VRL were retrospectively reviewed. Of those patients finally diagnosed with VRL and nonlymphoma, ophthalmic evaluation and cytology results, IL-6 and IL-10 levels, and immunoglobulin heavy chain and immunoglobulin kappa light chain clonality assays were compared. Results: Sub-retinal pigment epithelium infiltration and veil-pattern vitreous opacity were specific vitreoretinal findings in patients with VRL. The area under the receiver operating characteristic curve of the IL-10-to-IL-6 ratio and of IL-10 levels was 0.972 and 0.931, respectively. A combined immunoglobulin heavy chain and immunoglobulin kappa light chain assay showed increased sensitivity, whereas the determined specificity of immunoglobulin kappa light chain, at 94.12%, was much higher than the 78.95% of immunoglobulin heavy chain. Patients with VRL with atypically elevated IL-6 levels showed extensive and severe sub-retinal pigment epithelium infiltration. Conclusion: Newly screened immunoglobulin kappa light chain clonality assays may be useful to distinguish VRL from uveitis with high specificity. When sub-retinal pigment epithelium or retinal infiltration is severe and extensive, the IL-10-to-IL-6 ratio may not be typical and should be carefully interpreted.

    Abstract: To investigate the prevalence of pachydrusen, soft drusen, and subretinal drusenoid deposits in eyes with different neovascular age-related macular degeneration (nAMD) subtypes, determine the relationship between each drusen type and the choroidal thickness, and analyze the distinct features of each nAMD subtype according to the drusen type. Methods: Medical records involving 454 eyes from 454 patients with nAMD were retrospectively reviewed. The prevalence of each drusen type and the choroidal thickness and choroidal characteristics were evaluated according to the nAMD subtype. Results: Pachydrusen were prevalent in the typical nAMD (40.4%) and polypoidal choroidal vasculopathy (47.8%) groups and were not detected in the retinal angiomatous proliferation group. No significant drusen were detected in 24.3% of typical nAMD, 43.3% of polypoidal choroidal vasculopathy, and 0% of retinal angiomatous proliferation groups. Regardless of the nAMD subtype, pachydrusen, soft drusen, and subretinal drusenoid deposits were associated with a thick, moderately thick, and thin choroid, respectively. For eyes with typical nAMD, the prevalence of choroidal vascular hyperpermeability and extrafoveal neovascularization was significantly higher in the pachydrusen group than in the other groups. By contrast, the prevalence of Type 2 neovascularization was significantly lower in the pachydrusen group than in the subretinal drusenoid deposit group ( P 〈 0.001 for all). Conclusion: The prevalence of various drusen differed according to the nAMD subtypes, and each drusen type was strongly associated with the choroidal thickness. Typical nAMD showed distinct features according to the accompanying drusen type.

    Abstract: Background: Although non-alcoholic fatty liver disease (NAFLD) is the hepatic manifestation of metabolic syndrome, its influence on hypertension development is poorly understood. Hypothesis: We investigated whether fatty liver disease, as assessed by the fatty liver index (FLI), could predict the development of hypertension independently of systemic insulin resistance and inflammatory status. Methods: Prospective cohort study of 1,521 adults (484 men and 1037 women) aged 40 to 70 years without baseline hypertension examined in 2005 to 2008 as a baseline and 2008 to 2011 for follow-up. An equation was used to calculate FLI and classify patients as follows: FLI 〈 30, no NAFLD; FLI ≥60, NAFLD; and 31 〈 FLI 〈 59, intermediate FLI. Results: During an average of 2.6 years of follow-up, 153 subjects (10.06 %) developed hypertension. FLI was positively associated with baseline blood pressure, homeostasis model assessment of insulin resistance (HOMA-IR), leptin, urinary albumin/creatinine excretion, and high sensitivity C-reactive protein (hsCRP). After adjustment for confounding factors, including markers of insulin resistance and systemic inflammation, the odds ratio (95% confidence interval) for the incident hypertension increased in a graded manner with FLI ( 〈 30 vs. 30-59 vs. ≥60= 1 vs. 1.77 [1.16-2.71] vs. 2.06 [1.09-3.89] , respectively). Conclusions: NAFLD assessed by FLI was an independent risk factor for hypertension. Our findings suggest that FLI, a simple surrogate indicator of fatty liver disease, might be useful for identifying subjects at high risk for incident hypertension in clinical practice.

    Abstract: Although regional metastasis to the lymph nodes is common in advanced oral cancer, extensive local invasion into surrounding structures such as the mandible, skin and soft tissue of the neck, and masticator space is relatively rare. Sometimes surgical treatment cannot be performed and only palliative chemotherapy and radiation therapy are offered to preserve the quality of life of patients with advanced oral cancer. Nevertheless, the surgical resection of tumors remains the most effective treatment. This study presents a case of aggressive mouth floor cancer in which extensive composite defects on the mouth floor, oral mucosa, mandible, skin and soft tissue of the neck caused by tumor resection were reconstructed. Patient concerns: A 66-year-old man and a 65-year-old man with no significant personal or family history visited our clinic due to a large and multiple masses on the floor of the mouth and both sides of the neck. Diagnosis: Histopathological evaluation of the biopsy specimen revealed squamous cell carcinoma. Interventions: A fibula osteocutaneous free flap and customized titanium plate were used for the intraoral lining. Mandibular reconstruction was performed using a 3D-printed bone model, and an anterolateral thigh free flap was used to resurface the anterior of the neck. Outcomes: Reconstruction using this method was successful, and excellent functional and aesthetic outcomes were achieved without cancer recurrence. Lessons: This study show that the reconstruction of extensive composite defects of the oral mucosa, mandible, and neck soft tissue following surgical resection of mouth floor cancer can be performed in a single-stage operation. Through a single-stage reconstruction, both excellent functional aspects without cancer recurrence and satisfactory aesthetic outcomes can be obtained.

    Abstract: To evaluate the characteristics of bacillary layer detachment (BALAD) in acute Vogt–Koyanagi–Harada (VKH) disease and determine its prognostic value. Methods: Seventy patients with acute VKH disease with a minimum follow-up of 6 months were studied. The primary outcomes were clinical characteristics associated with BALAD, including features on multimodal imaging at baseline and follow-up. The secondary outcomes included best-corrected visual acuity and VKH with recurrence features. Results: Of 70 eyes (36 patients), 41 (58.6%) showed BALAD. The mean baseline best-corrected visual acuity and mean best-corrected visual acuity after resolution of serous retinal detachment were significantly lower in the BALAD group than in the no-BALAD group (0.90 ± 0.49 vs. 0.35 ± 0.35 log minimum angle of resolution, P 〈 0.001 and 0.39 ± 0.27 vs. 0.20 ± 0.20 log minimum angle of resolution, P = 0.020). The loss of ellipsoid zone integrity at baseline, proportion of serous retinal detachment, duration of serous retinal detachment, loss of ellipsoid zone integrity at 1 month, and subfoveal choroidal thickness at baseline were significantly higher in the BALAD group ( P = 0.017, P = 0.006, P = 0.023, P = 0.002, and P = 0.046, respectively). The mean best-corrected visual acuity and subfoveal choroidal thickness did not differ between the two groups at 6 months ( P = 0.380 and P = 0.180, respectively). Bacillary layer detachment at baseline was found to be a significant prognostic factor for VKH with recurrence features ( P = 0.007). Conclusion: Vogt–Koyanagi–Harada with BALAD featured more severe clinical characteristics than VKH without BALAD during the acute phase. Patients with baseline BALAD require more vigilant monitoring as they are more likely to show recurrence features within the first 6 months.

    Abstract: To investigate the prevalence of a newly defined drusen type, pachydrusen, soft drusen, and subretinal drusenoid deposits in eyes with polypoidal choroidal vasculopathy and fellow eyes and the relationship between each drusen type and the choroidal thickness, vascular morphology, and hyperpermeability. Methods: The 169 eyes of 90 patients with polypoidal choroidal vasculopathy were retrospectively reviewed. The prevalence of each drusen type was evaluated using color fundus photography and optical coherence tomography. The choroidal thickness and presence of pachyvessels on optical coherence tomography and choroidal vascular hyperpermeability on indocyanine green angiography were compared among the drusen groups. Results: Pachydrusen, soft drusen, and subretinal drusenoid deposits were found in 49.3%, 12.3%, and 6.9% in polypoidal choroidal vasculopathy eyes. The mean subfoveal choroidal thickness of the pachydrusen, soft drusen, and subretinal drusenoid deposit groups was 403.1, 184.4, and 176.4 µ m. The pachydrusen group showed significantly thicker choroid than the others. The choroidal hyperpermeability was noticed at 41.7%, 0%, and 0% and the pachyvessel was observed at 80.6%, 44.4%, and 40% in pachydrusen, soft drusen, and subretinal drusenoid deposit groups, respectively. Conclusion: In patients with polypoidal choroidal vasculopathy, pachydrusen was prevalent and associated with thicker choroid. Polypoidal choroidal vasculopathy with pachydrusen was highly associated with choroidal vascular hyperpremeability and pachyvessel morphology than other types of drusen.
