Notes: Abstract Rechargeable cells with lithium metal anodes have a very large theoretical energy density and are a promising cell system. However, rechargeable lithium metal cells are not yet currently commercially available. One of the biggest problems with the cells is the poor safety aspect resulting from the high chemical reactivity of lithium. We have been studying a cell system consisting of an amorphous (a-)V2O5P2O5 (95:5 in molar ratio) cathode, a lithium (Li) metal anode and an organic electrolyte in fabricating an AA-size prototype. In this paper, we report recent progress on our rechargeable lithium metal cell focusing on its safety.

Notes: Summary 1-Hexylcarbamoyl-5-fluorouracil (HCFU) and its parent compount 5-fluorouracil (5-FU) were tested PO for antitumor activity against mouse colon adenocarcinoma 26 (colon 26), colon adenocarcinoma 38 (colon 38), and Lewis lung carcinoma. The drugs were given orally at 2–4 days intervals for a total of ten doses. 5-FU was moderately active against colons 26 and 38 but not against Lewis lung carcinoma. In this treatment regiment the most impressive antitumor activity was obtained with HCFU against colons 26 and 38, especially colon 38 tumor. At 300 mg HCFU/kg, one out of seven mice inoculated with colon 26 and five out of ten mice inoculated with colon 38 became tumor-free. HCFU, however, was marginally effective in the prolongation of survival time of mice bearing Lewis lung carcinoma. 5-FU is approximately twice as active as HCFU against cultured cell lines from colon 26, colon 38, and Lewis lung carcinoma. Lewis lung carcinoma cells were most sensitive against HCFU, which is in contrast to the results obtained in the in vivo experiment. The IC50 value of HCFU against Lewis lung carcinoma cells was approximately half that against colon 26 and colon 38 cells. This higher sensitivity of Lewis lung cells against HCFU could be explained by the higher cellular uptake of the drug.

Notes: Summary The effectiveness of acute stage vascular reconstruction in cases of progressing stroke is reported. The clinical material consists of 28 cases of progressing stroke in the anterior circulation upon which vascular reconstruction was performed. Following admission, brain protective substances (500 ml mannitol, 500 mg vitamin E, 500 mg phenytoin) and dextran, were administered to the patients, and induced hypertension was also performed. Changes in symptoms were then observed and vascular reconstruction was carried out in cases where symptoms progressed. The vascular lesion was on the internal carotid artery in 8 cases and on the middle cerebral artery in 20 cases. Complete disappearance of neurological symptoms was obtained in 12 cases, whereas only mild neurological symptoms remained but a return to normal social life was possible in 11 other cases. Symptoms remained in three cases and there were two fatalities. In a long-term follow-up study, there were no cases of aggravation of symptoms due to ischaemic stroke. Moreover, the reconstruction was judged as effective on the basis of cerebral blood flow and metabolism. We concluded that acute vascular reconstruction for progressing stroke under the administration of brain protective substances is effective in preventing the progression of neurological symptoms.

Notes: Summary Antigens on the rat pancreatic islet cell surface were redistributed into patch and cap formation when the cells were incubated in the presence of the phosphodiesterase inhibitor, 3-isobutyl-1-methylxanthine, in tissue culture medium 199 for 24 h, before addition of rat pancreatic islet cell surface antibody. In contrast, if the cells were cultured in tissue culture medium 199 supplemented with glucose (5.5 or 16.7mmol/l) and 10% heat-inactivated fetal calf serum without 3-isobutyl-1-methylxanthine, cap formation was not detectable. These results suggest that mobile antigen on the surface of pancreatic B cells can be induced to aggregate into patch and cap formations during conditions of increased cellular metabolism.

Notes: Summary A study has been made of the cerebral blood flow (CBF) in moyamoya disease from the perspective of hemispheric cerebral blood flow and regional CBF (rCBF). The material includes 21 children between the ages of 5 and 15 years with moyamoya disease, and 19 adult moyamoya cases-all of which had virtually no neurological symptoms at the time of the study. CBF was measured using the133Xe intravenous injection method. Comparsion was made with the measurements from 16 normal children and 14 normal adults. Study was also made of the relationship between the angiographic stage of the disease and the CBF. With the exception of the more elderly patients, CBF was found to be significantly lower in the moyamoya cases than in normal subjects of the same age group. In all age groups, the distribution of rCBF showed a dominant posterior distribution, dissimilar to the dominant anterior distribution found in the normals. Among the juvenile moyamoya cases, there was a tendency toward decreasing hemispheric blood flow together with advancing disease-as determined angiographically. Moreover, with advancing stages of the disease, there was a continuing transition from the normal pattern of frontal dominance to one of occipital dominance. This dominance of posterior rCBF is thought to be a characteristic feature of moyamoya disease.

Notes: Summary This report describes our retrospective evaluation of CT features of the acute phase in 34 cases of ruptured cerebral aneurysms of the posterior cranial fossa. The results are as follows. 1. Examination of cisternal haematoma distribution revealed that SAH had extended to the supra- and infratentorial cisterns in 28 cases (82%). There were only 6 cases (18%) in which SAH was confined to the posterior cranial fossa only but even when there was subarachnoid haematoma in both the supra- and infratentorial cisterns, thick haematomas were seen at the periphery of the brain stem. In cases of vertebral artery-posterior inferior cerebellar artery aneurysms (VA-PICA AN), haematomas in the ambient cistern were thicker on the aneurysm side. In addition, in cases of basilar arterybifurcation (BA-Bifurcation AN) and basilar artery-superior cerebelli artery aneurysms (BA-SCA AN), there were many thick, highdensity haematomas in the interpeduncular cistern. 2. The rate of intracerebral haemorrhage was extremely low (1 patient). 3. The rate of intraventricular haemorrhage was high, and these haemorrhages demonstrated a reflux pattern. 4. The rate of hydrocephalus was high (76.5%) in comparison with that noted in association with SAH due to the rupture of anterior circulation aneurysms.

Notes: Summary For vascular reconstruction in cases of atherosclerotic stenosis at the origin of the vertebral artery, we use vertebral to subclavian artery transposition. We discuss the advantages and effectiveness of such treatment based on a study of 32 cases. We have experienced neither surgical mortality nor morbidity and the outcome at the time of discharge has been favourable. Follow-up revealed no deaths, however, three cases exhibited symptoms of cerebral ischaemia. One had a supratentorial completed stroke, and the other two hat TIA or RIND, but without any notable lesion in the angiograms. There were no cases of cerebral infarction of the posterior fossa. We believe that this method should be the first choice for treatment of cases without lesions of the subclavian artery for the following reasons: serious operative complications have not been encountered, surgical invasion is minimal, temporary occlusion of the common carotid artery is unnecessary, the operation can be done by occluding only the vertebral artery, and unlike various bypass operations, anastomosis is required at only one location and is consequently technically uncomplicated. Following anastomosis the cerebral blood pathway is physiological.

Notes: Summary Subarachnoid haemorrhage (SAH) often leads to subarachnoid fibrosis and resultant normal pressure hydrocephalus; however, how subarachnoid fibrosis occurs is unknown. We examined the changes within arachnoid granulations (AGs) and the subarachnoid space (SAS) chronologically at the parasagittal region obtained from patients with SAH at autopsy and made comparison with controls by immunostaining for cytokeratin, specific marker for leptomeningeal cells and by the elastica Masson-Goldner methods. Within a week some AGs were torn, and many inflammatory cells filled the AGs and SAS. Cytokeratin positive cells were scarce. During the next two weeks cytokeratin positive cells increased. After three weeks, AGs and SAS were filled by dense deposits of extracellular matrices surrounded by multiple layers of leptomeningeal cells.

Notes: Summary A series of nine patients over 70 years of age with saccular aneurysms, operated on by direct surgery, is reviewed in order to discuss aneurysm surgery in older patients. The median age of the patients was 74 years, and the median interval from haemorrhage to surgery was 18 days. Operative mortality was 11%, and morbidity was 33%. Because of arteriosclerosis round the aneurysm neck and parent artery, special considerations for surgical treatment on the older patients are required.

Notes: Summary We have studied the correlations between computed tomograms (CT), clinical severity at the time of onset, and subsequent development of cerebral infarction due to vasospasm in 32 cases with subarachnoid haemorrhage secondary to rupture of cerebral aneurysms. It was found that the clinical severity could be judged by CT as shown by the amount of blood in subarachnoid space. Furthermore, it was possible to predict the subsequent occurrence of cerebral infarction by sequential findings of CT. A Hounsfield number of the high density area over 60 showed good correlation with the development of cerebral infarction.
