    Abstract: Background: Shivering is one of the commonly encountered adverse affect after spinal anaesthesia. Shivering can be very discomforting to the patient and hampers operative maneuvering. Pharmacological therapies have been studied for control and management of shivering. One such drug is ondansetron, a 5-HT3 antagonist. Aim of current study is to evaluate the efficacy of prophylactic administration of ondansetron 8 mg vs. 4 mg for prevention of shivering.Methods: A prospective, randomized, and double blind study was conducted on 100 patients, from either gender, aged 20-60 years, of the American society of anesthesiologists grade I or II, scheduled for various surgeries under spinal anesthesia. The patients were randomly divided into two groups of 50 each to receive either ondansetron 8 mg (group E) or ondansetron 4 mg, (group F) as slow intravenous infusion prior to spinal anesthesia. The primary end point were intraoperative shivering and secondary outcomes included hypotension, adverse reaction, cardiac dysrhythmia’s.Results: A total of 10 patients in group E (20%) and 20 (40%) patients in group F experienced shivering (p=0.029). Incidence of nausea was similar in both groups, total of 8 (16%) patients in group E and 5 (10%) in group B had hypotension (p=0.27). 1 (2%) patient in group E experienced bradycardia.Conclusions: Prophylactic administration of ondansetron 8 mg has better efficacy in prevention of spinal anaesthesia induced shivering with minimal side effects as compared to 4 mg dosage.

    Abstract: Intra-testicular epidermoid cyst of testis is a benign tumour and accounts for 1-2% of all testicular neoplasms. It is a benign lesion, and should be distinguished from dermoid cyst or teratoma, which have a considerable malignant potential. We describe a rare case of testicular epidermoid cyst in an elderly male presenting with gradually increasing testicular mass since 8 months. Local examination revealed a non-tender, irregular, hard mass in the right testis. All routine laboratory investigations along with tumour markers were within normal limits. Ultrasound showed features of a testicular neoplasm following which orchidectomy was performed. Grossly, cut surface of testis showed a cyst measuring 4x2x1cm and microscopic examination revealed features of an epidermoid cyst. Extesive sampling excluded the possibility of a teratomatous focus. This case is described owing to the rarity of this neoplasm and the unusual clinico-radiological presentation. The pre-operative diagnosis of this tumour can be made if characteristic sonographic and MRI findings are obtained in the absence of which these masquerade neoplastic lesions. Histopathological examination thus is vital for a decisive diagnosis.

    Abstract: 〈 p 〉 〈 strong 〉 Background 〈 /strong 〉 : Proximal femoral nail is emerging as the treatment of choice for unstable extra capsular proximal femoral fractures. This study comprises of functional evaluation of the treated patients by using SF 36 score. 〈 /p 〉 〈 p 〉 〈 strong 〉 Methods 〈 /strong 〉 : This was a prospective hospital-based study done on 54 patients with unstable proximal femoral fractures classified according to the AO/ASIF classification system and were treated by using PFN in the department of Orthopaedics DRPGMC, Tanda. Functional outcome was assessed by using SF-36 score on, 14th postoperative day, at three months and six months. 〈 /p 〉 〈 p 〉 〈 strong 〉 Results 〈 /strong 〉 : Our study has mean scores of short form-36 subscales as physical function 70.6, role limitation due to physical health 68.1, role limitation due to emotional health 90, energy/fatigue 66, emotional wellbeing 78, social functioning 67.7, pain 75 and general health 73.3. 〈 /p 〉 〈 p 〉 〈 strong 〉 Conclusions 〈 /strong 〉 : The results of this study confirm that intramedullary nailing with the use of a PFN in unstable proximal femoral fractures is a safe method. A significant change was seen in quality of life postoperatively as evidenced by various scores used in present study. Weaknesses of this study include a short follow-up period and the inclusion of a small study group. 〈 /p 〉

    Abstract: Background: Science of anaesthesia has revolutionized from traditional only application of relieving patient from pain induced during surgery to wide spectrum approaches like palliative care, critical care, perioperative care along with pain management. Despite of such critical roles in health care sector there is still a lack of awareness about anaesthesiology and anaesthesiologists. Current investigation was aimed towards assessing the awareness of paramedical staff regarding the role of anesthesiology.Methods: Current investigation was a prospective cross-sectional questionnaire-based study conducted on 100 paramedical staff posted in different departments of military hospital Jaipur.  All the study participants were interviewed face to face orally by an investigator using the set of questions listed in the questionnaire. Participants responses were documented and statistically represented as percentage.Results: Current study findings reveal that even though majority of the study participants (91) had experience more than 5 years in the hospital and most of them (˃90%) of participants were having educational level of higher secondary or graduation still the role of anaesthesiologist outside operation theatre was poorly known to paramedical staff. It was also observed in current investigation that role of anesthetist in pain clinic was poorly known to most of the participating paramedical staff.Conclusions: Providing knowledge about expertise of anesthesiologist among paramedical staffs by incorporating anaesthesia in the curriculum, in house training programs, use of pamphlets, internet and other easily assessable techniques is highly desirable for spreading awareness of anaesthesia and anesthetist among paramedical staff, so that patient can avail anaesthesia related services effectively.

    Abstract: Background: Delayed presentation of patients of cancer is associated with poorer survival. Early presentation, timely diagnosis and treatment are critical factors in improving long-term survival for patients with cancer. Thus, reducing delays in diagnosis of cancer should be a priority. Against this backdrop this study was conducted to identify factors leading to delayed presentation of patients in advanced stages of cancer at our institute.  Methods: Newly diagnosed patients of cancer coming to radiation oncology, surgery and medical oncology outpatient departments were included in this study. Data was collected in a pre-validated questionnaire format.Results: 360 patients were included. Majority of patients 212 (58.9%) were male. 66.7% patients were from rural areas. Most of the patient’s educational level was upto primary schooling 178 (49.4%) and caregiver educational level was upto primary schooling 153 (42.5%). Reason for alternative treatment correlated significantly with patient awareness about the disease (p 〈 0.03). Most common reason of the time gap given by patients was symptom appraisal 258 (71.7%).Conclusions: Educational level of patients, symptom appraisal, fear of side effects of treatment and awareness about disease were main reasons for delay in presentation of cancer patients. So, education regarding warning signs of cancer and symptom appraisal are important in reducing delays in presentation.

    Abstract: Background: Globally, neonatal deaths constitute 44% of all deaths in less than 5 years age group .The concept of SNCU is based on the learning from the “Purulia model”. This research study was undertaken, to assess the profile of sick newborns admitted in the SNCU.Methods The investigators analyzed this data and establish the morbidity profile of newborns admitted in SNCU Hamirpur in 2019.Results: Total of 422 newborns were admitted 197 (46.7%) were females and 225 (53.3%) were males. Out of these 381 (90.3%) were inborn and 41(9.7%) were out born. In inborn 293 (76.9%) weighed more than 2.5 kg, 83 (21.8%) were low birth weight 〈 2.5 kg to 1.5 kg and 2 (0.52%) were very low birth weight i.e 〈 1.5 kg to 1 kg and one newborn was extreme low birth weight i.e 〈 1 kg. In the out born group, 23 (56.1%) weighed more than 2.5 kg and 13 (31.8%) were low birth weight 〈 2.5 kg to 1.5 kg, 4 (9.8%) were very low birth weight 〈 1.5 kg to 1 kg. Jaundice was the most common neonatal condition 63%, infection 9.5%, respiratory diseases 9% and birth asphyxia 7.8%.Conclusions: The most sensitive time for morbidity and mortality development is during the newborn period. The main reasons for SNCU hospitalisation include neonatal jaundice, preterm, low birth weight, perinatal asphyxia, and infection. Neonatal Jaundice continues to be the primary reason for both inborn and outborn newborns admission to SNCU, and inborn babies make up the majority of this group.

    Abstract: Background: Diabetes Mellitus (DM) can lead to Diabetic retinopathy. Dry Eye Syndrome is commonly seen in diabetic patients. The purpose of this study was assessment of dry eye in patients with and without diabetic retinopathy.Methods: The study was conducted in the department of Ophthalmology and Rural and Urban centres of Community Medicine, Narayan Medical College and Hospital, Sasaram, Bihar. All cases of Type 2 diabetes mellitus over six months period were considered for the study. The diabetic patients were divided into two groups, one with diabetic retinopathy and others without diabetic retinopathy. All patients were subjected to McMonnie’s questionnaire and clinical evaluation including tear film studies like Schirmer’s test and Tear Film Break Up Time (TBUT).Results: 60 diabetic patients were enrolled for the study, 30 had evidence of diabetic retinopathy while 30 had no evidence. The most common dry eye symptom reported was foreign body sensation (96.7%). All Signs of dry eye disorder were found more commonly in patients with diabetic retinopathy as compared to those without diabetic retinopathy. Mcmonnie’s scores correlates well with both Schirmer’s basic secretion test (BST) and TBUT.Conclusions: Dry eye syndrome must be diagnosed in diabetic patients. Periodic screening of diabetic patients should be carried out to evaluate diabetic retinopathy and its complications.

    Abstract: Few cases of hypoglycaemic peripheral neuropathy due to insulinoma have been reported in literature. We are reporting a case of insulinoma manifesting with chronic progressive encephalopathy, intractable seizures, and neuropathy-a triad of neurological features in insulinoma. The diagnosis was delayed as the repeated investigations including dual phase CECT abdomen was normal. This case report highlights the importance of evaluation of patients for insulinoma with high resolution CECT abdomen and FDG PET scan. A keen suspicion and investigation with appropriate modality can prevent the irreversible neuroglycopenic damage, as reported in this case.

    Abstract: Adhesive Intestinal obstruction is the most common cause of intestinal obstruction in post-operative period. Diagnosis is based on history, clinical examination, plain X-ray abdomen. Authors here report an interesting case of intestinal obstruction after surgery for chronic calcific pancreatitis with pancreatic duct stone with intractable pain. In post-operative period patient developed features of intestinal obstruction, patient was planned for re- exploration and it was found that band was formed by left fallopian tube with transition point at terminal ileum and treated successfully with left Salpingectomy. Very few cases of fallopian tube as cause of intestinal obstruction have been reported in literature and it should be considered as one of the cause of intestinal obstruction in females presenting with acute abdomen.
