    Abstract: Background: Unmet need for contraception is still high in developing countries because of various reasons and poses a great challenge to the success of family welfare programme Assessing the knowledge and filling the gap is essential for successful functioning of the programme and for reducing the unmet need.Methods: A cross-sectional study was conducted to assess the knowledge and practices on contraception among 100 married women in reproductive age group (15-49 years) residing in a Rural Health centre area of Tirunelveli district, Tamil Nadu, South India.Results: Among the 100 participants, common known methods of contraception were IUD (56%), permanent sterilization (38%), Pills (21%) and Condoms (14%). Out of 100 participants, only 38 were using contraception. Among the 62 who are not using any method of contraception, 30 are willing to practice contraception after motivation and among them 27 prefer to use temporary methods. Fear of side effects was most common reason stated for not using contraception.Conclusions: Knowledge and practice related to contraception among the participants were observed to be less. Health education campaigns emphasizing the need of family planning and about the services available in the government health facilities has to be organized regularly.

    Abstract: 〈 p class="abstract" 〉 〈 strong 〉 Background: 〈 /strong 〉 The soft tissue defects around knee joint give a challenging experience to reconstruct. There are various ways to reconstruct soft tissue defects around knee joint. In this study, a reconstructive algorithm is formed for soft tissue defect around knee joint using pedicled flaps. 〈 /p 〉 〈 p class="abstract" 〉 〈 strong 〉 Methods: 〈 /strong 〉 Reconstructive procedures were done for 15 cases of soft tissue defects around knee joints following different etiologies during the tenure of January 2017 to December 2018 in the Department of Burns, Plastic and Reconstructive Surgery, Kilpauk Medical College, Chennai. 〈 strong 〉 〈 /strong 〉 〈 /p 〉 〈 p class="abstract" 〉 〈 strong 〉 Results: 〈 /strong 〉 Soft tissue defects around knee joint need flap to have a durable cover, to protect the underlying structures – bones, vessels, tendon, and to enable ease of free movement of knee joint without producing contracture. There are various etiologies for soft tissue defects around knee joint-post traumatic injuries, post total knee replacement, post electric burn injuries, post burn scar contracture release of knee joint. 15 cases of soft tissue defects around knee joint were reconstructed using various pedicled flaps-medial thigh rotation flap, gastrocnemius muscle and myocutaneous flap, reverse anterolateral thigh flap, propeller flap. In this retrospective study, reconstructive algorithm for soft tissue defects around knee joint is provided. 〈 /p 〉 〈 p class="abstract" 〉 〈 strong 〉 Conclusions: 〈 /strong 〉 The reconstruction of soft tissue defects around knee joint is difficult one. In this study, a simple reconstructive algorithm is provided based on the experience in Department of Burns, Plastic, and Reconstructive Surgery, Kilpauk Medical College, Chennai. 〈 /p 〉

    Abstract: Background: Microalbuminuria among obesity cases reflects specific glomerular damage or is the marker of generalized endothelial cell dysfunction is still debatable. Thus, this study aimed to assess the presence of microalbuminuria among obese persons and non-obese individuals, who were euglycemic and normotensive.Methods: A case control study was conducted among patients attending outpatient department of general medicine in Govt Thiruvarur Medical College, Thiruvarur, for their illnesses from June 2017 to December 2017. A total of hundred participants with fifty obese cases and fifty non obese controls were included in this study. Detailed history and examination were done by the principal investigator and the same was documented in a proforma. Data entry was done using Microsoft excel and the statistical analysis includes odds ratio were calculated using Statistical Package for Social Sciences (SPSS) software version 17.Results: Among the study participants, 32% and 4% had microalbuminuria in obese and non-obese group, respectively. Also, obese participants were 11.29 times at higher chances of having microalbuminuria when compared to the non-obese patients with significant p value (p=0.002).Conclusions: Microalbuminuria can be used to predict the risk of complications in obese subjects in order to bring down the overall morbidity and mortality related to renal function.

    Abstract: Background: Dermatophytosis is considered as a silent epidemic with a global prevalence of 25%. In India, it has been reported in various studies with high prevalence of 27% and it is important to find the modifiable contributing factors for dermatophytosis, so as to prevent the high morbidity affecting the social and occupational health.Methods: A total of 100 patients participated in this cross-sectional study conducted in dermatology outpatient department (OPD) at tertiary care hospital. After obtaining informed written consent, all patients above 18 years of age diagnosed with superficial fungal Skin infections were subjected to pretested, semi-structured questionnaire to collect data regarding the knowledge, attitude and practices towards the dermatophytosis.Results: Out of 100 patients in this study, 45% of patients has knowledge about the spread of fungal infection from person to person and 55% patients not agreeing with it. The study also revealed that 84% of patients reported correctly about the association of sweating with sweat feeling index (SFI) and 76% of patients know about the possible sites of dermatophyte infection. 91% and 96% of patients had no knowledge about the spread of infection through sharing clothes and collective washing clothes of other family members.Conclusions: Despite the patients demonstrating good knowledge about mode of spread of dermatophytosis, still most of the patients are failed to implement the knowledge in maintaining personal hygiene practices. There is a need to improve the gap between the knowledge about the dermatophytosis and their personal hygiene practices towards the infection. It is important to communicate with the patient about the treatment of other family members and changing their attitude towards the infection. This approach will help to improve the adherence of treatment and also prevents the recurrence of dermatophytosis.

    Abstract: Background: Alcohol consumption is a addictive behavior, which eventually leads to personality disorder, stress. However, in recent times it was noted that duration of alcohol consumption is an important factor for the changes in personality profile. Hence this study was conducted to assess the personality disorders among the person who started consuming alcohol in the past 1-5 years with those of non-alcoholics.Methods: This prospective cross sectional study was conducted to assess the personality profile among alcoholics and non-alcoholic individuals in the department of psychiatry in Sri Venkateshwara medical college hospital and research institute, Puducherry during the January 2019 to June 2019. A total of 80 participants with 40 alcohol consuming cases and 40 controls were included in this study. For assessing the personality disorder, 16 personality factor (PF) questionnaires were used.Results: Personality profile like factor F which represents liveliness and Q4 which represents primary factor regarding tension were found to be greatly affected among cases who consume alcohol compared to non-alcoholic controls.Conclusions: Significant changes noted in personality profile among cases who consume alcohol occurred within 1-5 years of starting alcohol consumption.

    Abstract: Background: Appropriate nutrition in early childhood plays an important role in normal growth and development, as well as on the long-term health of individuals. There is increasing recognition that dietary pattern better reflects the overall quality of the diet. The dietary pattern has been shown to be associated with nutrient intakes and linked to chronic diseases including obesity, diabetes and metabolic syndrome in children. The aim of the study was to assess the dietary preferences for food items (vegetables, fruits, meat, milk products, snacks and starch) among children of age 5-10 years in the rural area of Perambalur district, South India.Methods: Six food categories (vegetables, fruits, meat, milk products, snacks and starch) were included in the questionnaire to study the dietary preferences of the child. Data were entered into a Microsoft Excel worksheet and analysed for frequencies and percentages. Mean score and its standard deviation were calculated for individual food items.Results: The study shows that most of the children prefer milk/milk products with a highest mean score of 3.94 followed by a preference for snacks. Most avoided food were vegetables, followed by meat and meat products. The overall score was found to be low for all the categories of food among children who prefer mobile compared to TV and outdoor group. But this difference was not statistically significant. This difference was close to 0.05 only in vegetable group category.Conclusions: The results will help in planning the nutritional counselling programmes for children. These findings have to be further correlated with the nutritional status of the individual children and deficiencies if any.

    Abstract: Background: Enteral nutrition is of paramount importance in management of upper gastrointestinal tract malignancies. Although various access routes for enteral nutrition are available, feeding jejunostomy tube is preferred to other methods. Since minimally invasive techniques have many advantages, we decided to perform it during staging laparoscopy.Methods: The medical records of all patients who underwent laparoscopic feeding jejunostomy between October 2019 and September 2021, in the department of surgical gastroenterology, Madurai medical college were retrospectively reviewed. Patient demographics, ECOG performance status, types of malignancy, indications for feeding jejunostomy, operative time, time to start feeds, length of hospital stay and complications were analysed.Results: 75 patients underwent laparoscopic feeding jejunostomies (51 men and 24 women), with a mean age of 51.6±11.7 years, using this method. ECOG performance status was between 0-2. Mean operating time was 83±8.8 minutes. Mean length of hospital stay was 3.5 days. There was no conversion, however one patient required reoperation for peritubal leak.Conclusions: Feeding jejunostomy can be accomplished by a total laparoscopic technique using intracorporeal suturing. The technique we propose is relatively simple, reproducible, cost effective and can be widely adopted.

    Abstract: Background: Laparotomy forms an important subset of general surgical operations. This study aimed at collecting the baseline information on postoperative analgesia, to detect deficiencies in current management of postoperative pain and to aid as a reference for future endeavours aimed at improving pain management following abdominal surgeries.Methods: It was conducted as a prospective descriptive study in patients undergoing elective laparotomies in the Department of General Surgery of a tertiary care institute, for two years. The patterns of prescription and administration of analgesic drugs for postoperative pain after abdominal surgery, incidence and severity of postoperative pain, adverse effects of drugs and patient satisfaction were assessed.Results: A total of 289 elective laparotomies were performed. Combinations of Non-steroidal anti-inflammatory drugs (NSAID) with opioids, epidural analgesia (EA) with supplemental intramuscular ketorolac and PCEA (Patient controlled EA) provided effective pain relief with high satisfaction score (3/4) and were associated with low sedation (1/3) and the least side effects. Patients receiving NSAIDs had higher pain score with lesser satisfaction score and lower sedation score. IV-PCA with morphine provided effective pain relief with high satisfaction score (3/4), with least side effects, but patients had high sedation score (2/3). Around 55 (55/289; 19.03%) patients experienced postoperative nausea and vomiting, which responded to antiemetic treatment.Conclusions: A combination of NSAIDs with opioids provided effective pain relief, high satisfaction with less sedation and least side effects. Epidural analgesia with supplemental intramuscular ketorolac and PCEA also provided effective pain relief with high satisfaction and less sedation with least side effects.

    Abstract: Achalasia cardia is a motility disorder characterised by failure of relaxation of the lower oesophageal sphincter (LES) and associated with lack of peristalsis in the body of oesophagus. The mainstay of treatment in achalasia cardia is to reduce the LES pressure and improve oesophageal emptying by gravity. LHM is still considered the gold standard against which other methods are compared. The records of all patients who underwent surgery for achalasia cardia between August 2018 and December 2021 in the department of Surgical Gastroenterology, Government Rajaji Hospital and Madurai Medical College, Madurai, Tamil Nadu, India were reviewed retrospectively. Eighteen patients underwent surgery for achalasia cardia with median age of 32.5 years and the median duration of symptoms was 30 months. Seven patients had undergone pneumatic dilatation previously. Two patients who had mucosal injury had undergone pneumatic dilatation previously. The duration of surgery was significantly higher for the patients who underwent previous pneumatic dilatation when compared to those who did not undergo dilatation (200 minutes vs 170 minutes). The Eckardt score was zero in 12 patients in follow-up. One patient had a score of 1 and five patients had a score of 2. Laparoscopic Heller myotomy is the most effective current surgical management for achalasia cardia. It can be safely performed even in patients with prior pneumatic dilatations with lesser reintervention rates.

    Abstract: Background: Undetected and untreated mental disorders can impair a person’s ability to perform at school or work place, cope with daily activities of life and can lead to severe psychiatric disorders and consequences later in their life. Study objective was to determine the prevalence and patterns of depression, anxiety and stress among 400 school going adolescents belonging to classes 10th to 12th of Tirunelveli district, Tamilnadu, India.Methods: Burden of Depression, anxiety and stress was assessed using DASS21 questionnaire. Chi-square test was done using SPSS software version 21 to test for statistical significance.Results: Overall prevalence of depression, anxiety and stress was 73.6%, 86.5% and 24.7% respectively. Depression (p value=0.01), Anxiety (p value = 0.005) and stress (p value = 0.007) were significantly observed more among 10th class students when compared with other classes.Conclusions: The present study has identified a higher prevalence of depression, anxiety and stress among students. This warrants immediate action of creating awareness among teachers and parents in early identification and treatment to prevent serious consequences in later life.
